Relationship between peripheral arterial disease, carotid intima-media thickness and C-reactive protein in elderly diabetic patients

Purpose: To study the association between carotid intima media thickness (CIMT) and high sensitivity CRP (hs-CRP) level as markers for PAD in elderly patients with DM. Subjects: A case-control study on 90 participants aged 60 years and older divided into 60 cases (30 patients with DM alone and 30 patients with DM and comorbidities) and 30 healthy controls. All were assessed by measuring CIMT, ankle brachial index (AB), and markers for cardiovascular disease such as high-sensitivity CRP (hs-CRP), total cholesterol (TC), triglycerides (TG), high density lipoprotein (HDL), and low density lipoprotein (LDL). Results: hs-CRP levels showed statistically significant difference being highest among patients with DM and comorbidities and lowest among controls (P nificantly higher among cases than controls. ABI was able to detect PAD in many asymptomatic patients. Color changes were present in only 43.30% (n = 26) of positive PAD cases while delayed wound healing, claudication pain, rest pain, cold extremities, and trophic changes were present in 23.30% (n = 14), 16.71% (n = 10), 16.71% (n = 10), 45.00% (n = 27), and 21.7% (n = 13). Using logistic regression analysis revealed that DM, CIMT, and hs-CRP were independent predictors for PAD (OR = 4.194, 7.236, 1.003;P value = 0.044, 0.25, 0.031) after adjustment of other coronary risk factors such as sex, smoking, hypertension, TC, and TG. Conclusion: Diabetic elderly have higher prevalence of asymptomatic PAD thannon-diabetics using solely ABI. DM, CIMT, and hs-CRP are independent predictors for the occurrence of PAD. Hs-CRP levels are highest among diabetics with comorbidities.

Prognostic Value of Serum Uric Acid Level in Patients with ST Elevation Myocardial Infarction Undergoing Primary Percutaneous Coronary Intervention

Background: Serum uric acid (SUA) has been correlated with cardiac morbidity and mortality. However, its prognostic value in acute ST-segment elevation myocardial infarction (STEMI) is still uncertain. The aim of this study was to evaluate the prognostic value of SUA on admission in patients with STEMI undergoing primary percutaneous coronary intervention (PPCI). Methods: We prospectively enrolled 150 STEMI patients underwent PPCI. The mean age of the studied population was 56.1 years, 78%?were males?while 22% were females. Patients were divided into tertiles based on the basal serum uric acid level. Patients with high SUA (n = 72) was defined as a value in the third tertile (>6.4 mg/dl), and a low SUA group (n = 78) was set as a value in the lower two tertiles ( Results: SUA level on admission carried prognostic value in patients with STEMI undergoing PPCI where the low uric acid group had better KILLIP class (P =0.001), better TIMI flow (P = 0.001), higher ejection fraction (49.53 ± 8.75 versus 39.95 ± 7.06;P =0.001), better survival and lower incidence of other major adverse cardiac events (MACE) (P = 0.01) during the hospital stay and three months follow up?than the high uric acid group. Age, SUA > 6 mg/dl, TIMI flow, Killip class and EF 40% were independent predictors for MACE in such patients. Conclusions: High SUA level on admission was associated with higher frequency of in-hospital and three months follow up MACE in patients with acute STEMI undergoing PPCI.

Annexin A2 as a biomarker for hepatocellular carcinoma in Egyptian patients

AIM To investigate the clinical utility of serum annexin A2(AnXA2) as a diagnostic marker for early hepatocellular carcinoma(HCC).METHODS This study was performed in HCC Clinic of Ain Shams University Hospitals, Cairo, Egypt and included: Group 1: Fifty patients with early stage HCC(Barcelona Clinic Liver Cancer stage A); Group 2: Twenty five patients with chronic liver disease; and Control Group: Fifteen healthy, age-and sex-matched subjects who were seronegative for viral hepatitis markers. The followinglaboratory investigations were done: Viral hepatitis markers [hepatitis B surface antigen and hepatitis C virus(HCV) antibodies], HCV RnA in HCV antibody-positive patients, serum alpha fetoprotein(AFP), and serum AnXA2 *** In this study, 88% of HCC patients(n = 44) were HCVpositive, while HBV infection represented only 8% of all HCC patients(n = 4); and two patients were negative for both viral markers. A highly significant difference was found between patients with HCC and chronic liver disease as well as controls with regard to serum AnXA2 levels(130, IQR 15-240; 15, IQR 15-17; and 17, IQR 15-30 ng/m L, respectively). The area under the curve of AnXA2 was 0.865; the cut-off value was established to be 18 ng/mL with a diagnostic sensitivity of 74% and a specificity of 88%, while the sensitivity and specificity of AFP at the cut-off value of 200 ng/dL were 20% and 100%, *** Serum AnXA2 may serve as a biomarker for the early detection of HCC.

Fabrication of Congo Red/Oxidized Porous Silicon (CR/OPS) pH-Sensors

The fabrication of nano porous silicon, nPSi, using alkali etching process has been studied and carried out. The surface chemistry of anisotropic etching of n-type Si-wafer is reviewed and the anisotropic chemical etching of silicon in alkaline solution using wetting agents is discussed. Transformation of crystallographic plane of n-Si (211) to nPSi (100) has occurred on using n-propanol as wetting agent. The rate of pore formation was 0.02478 - 0.02827 μm/min, which was heavily dependent upon the concentration of the etchant containing wetting agents, allowing patterned porous silicon formation through selective doping of the substrate. A particle size of 15 nm for porous nano-silicon was calculated from the XRD data. Porosity of PS layers is about 10%. Pore diameter and porous layer thickness are 0.0614 nm and 16 μm, respectively. The energy gap of the produced porous silicon is 3.3 eV. Furthermore, the combination of PS with Congo Red, which are nanostructured due to their deposition within the porous matrix is discussed. Such nano compounds offer broad avenue of new and interesting properties depending on the involved materials as well as on their morphology. Chemical route was utilized as the host material to achieve pores filling. They were impregnated with Congo Red, which gave good results for the porous silicon as a promising pH sensor.

Cytokine profile in Egyptian hepatitis C virus genotype-4 in relation to liver disease progression

AIM: To observe the imbalance between T helper cell Th1 and Th2 cytokines in several chronic hepatitis disease at different stages of disease ***: We measured the cytokine levels of Th1 (IL-2 and IL-2R), Th2 (IL-10) and the pro-inflammatory cytokines (IL-6 and IL-6R and TnF and TnF-RⅠ and Ⅱ)by the ELISA technique in the sera of 33 hepatocellular carcinoma (HCC) patients and 20 chronic liver disease (CLD) patients. In addition, 20 asymptomatic hepatitis C virus carriers and 20 healthy subjects negative for hepatitis C virus(HCV) markers served as ***: Anti-HCV antibodies were found to be positive in 94% of HCC cases and 75% of CLD *** the other hand, HCV viremia was detected using RTPCR in 67% of HCC cases and 65% of CLD cases. HBsAg was positive in 9% of HCC cases and 30% of CLD *** bilharzial-Ab was positive in 55% of HCC cases,65% of CLD cases and in 70% of asymptomatic carriers (ASC). HCC patients had significantly higher values of IL-2R, TnF-RⅡ (P＜0.001), and TnF-RⅠ (P＞0.05), but lower TnFα (P＜0.001) and IL-6 (P = 0.032) in comparison to ASC. But, in comparison to non-cancer controls, HCC patients had higher values of IL-2R, IL-6R, TnF-RⅠ and TnF-RⅡ, but lower TnF-α (P＜0.001). CLD patients had higher IL-2R, TnF-RⅠ, and TnF-RⅡ (P＜0.001) than ASC. But, in comparison to non-cancer controls, CLD patients had higher values of IL-2R, TnF-RⅠ and TnF-RⅡ, but lower TnF-α (P＜0.001). IL-10 was higher (though not significantly) in HCC and CLD patients than in symptomatic carriers and non-cancer ***: Liver disease progression from CLD to HCC due to HCV genotype-4 infection is associated with an imbalance between Th1 and Th2 cytokines. IL-2R,TnF-RⅠ, and TnF-RⅡ could be used as potential markers.

Kinetics of Oxidation of 2,6-Dimethylphenol (DMP) Using novel μ-Carbonato [(Pip)_4nCu₄X₄(CO₃)₂] Complexes

This paper reports the kinetics of the oxidation of 2,6-dimethylphenol (DMP) to get 3,3’,5,5’-tetra- methyl-4,4’-diphenoquinone (DPQ) using novel oxidative coupling complexes [(Pip)4nCu4X4-(CO3)2] (n = 1 or 2, X = Cl or Br, Pip = piperidine). The new prepared tetranuclear complexes were characterized using cryoscopic measurements, electronic spectra, FTIR, EPR and cyclic voltammetry techniques. These complexes are catalytically active. The proposed mechanism of the catalytic oxidative coupling can be illustrated as a pre-equilibrium, K, between the catalyst and DMP to form a complex intermediate which is converted to activated complex through the rate determining step, k2, to form the final product. The inverse of the observed rate constants kobsd versus 1/[DMP]2 gives a straight line with intercept. From the slope and the intercept, both K and k2 are obtained. At different temperatures, thermodynamic and kinetic parameters are evaluated. It is worth to mention that, the dependence of kobsd on [DMP]2 indicates that the coordination number for every copper center in both n = 1 or 2 in [(Pip)4nCu4X4(CO3)2] is equal to six. Therefore, carbonato bridging centers in n = 1 acts as a tridentate ligand, while for n = 2 acts as a bidentate ligand.

Synthesis and Characterization of novel μ-Carbonato Tetranuclear Copper Complexes [(Pip)_4nCu₄X₄(CO₃)₂] in Aprotic Media

In this work, novel oxidative coupling complexes, [(Pip)4nCu4X4(CO3)2] (n = 1 or 2, X = Cl or Br, Pip = piperidine), are synthesized from the reaction of well characterized Lewis base [(Pip)4nCu4X4O2] with carbon dioxide as a Lewis acid in CH2Cl2. These carbonato-derivatives are isolated as stable solids. They are easily soluble in aprotic solvents as CH2Cl2or phnO2. Cryoscopic measurements support tetranuclear structure for all of them. Electronic spectra in the near infrared with high molecular absorptivity may be explained for tetranuclear cuban structure to fulfil 3 halo-ligands for each copper centre in [(Pip)4nCu4X4(CO3)2]. The EPR spectra for [(Pip)4nCu4X4(CO3)2] complexes are axial type of spectra (dx2-y2 G.S) suggesting elongated tetragonal distortion for all of them. Cyclic voltammograms for [(Pip)4nCu4X4(CO3)2] are irreversible in character. These tetranuclear carbonato complexes show catalytical activity. They initiate the oxidation of 2,6-dimethylphenol (DMP) to 3,3’,5,5’-tetramethyl-4,4’-diphenoquinone (DPQ).

Clinical Outcome of Low Dose Contrast during Percutaneous Coronary Intervention in Patients with Moderate to Severe Kidney Impairment

Background: Chronic kidney disease patients are at a greater risk for nephropathy requiring dialysis after percutaneous coronary intervention. Such patients are usually deferred due to fear of “Renalism”.?Objectives This study assesses the outcome of Low dose contrast protocol during PCI in CKD patients whose e-GFR 60 ml/min/1.72 m and investigates a safety margin for contrast use in these high-risk categories.?Methods: Patients were into three groups according to CV/e-GFR ratio: Group (A) low-dose: CV/e-GFR ratio 2.0 Group (B) medium-dose: CV/e-GFR ratio > 2.0 and × bodyweight\***). Group (C) high-dose: CV/e-GFR ratio > MACD. Results: A total of 73 patients were enrolled. Average age was 54 ± 8 years,81.4% were male and 18.6% were females and 52% were diabetic. Mean baseline e-GFR was 40 ± 8.0 ml/min/1.73m2. Contrast Volume used in group A was (58.26 ± 15.05) (n = 24), in group B (109.42 ± 17.11) (n = 26) and in group C (304.5 ± 60.30) (n = 23), respectively. The incidences of CI-AKI in the 3 groups were 0%, 11.5% and 35%, respectively (p = 0.02). All-cause death 0%, 17% and introduction of maintenance hemo dialysis was 0%, 11.5% and 26%, respectively (p Conclusion: Low dose contrast protocol is safe, effective and easily applicable technique without CI-AKI or death.

Utilization of Trimethylolpropane Based Hyperbranched Poly(Amine-Ester) as new Polymeric Admixture

Hyperbranched poly(amine-ester) (HBPAE) was synthesized via pseudo-one-step process between trimethylolpropane as a core molecule and n, n-diethylol-3-amine methylpropionate as the AB2 branched monomer. The prepared polymer was analyzed by IR, GPC, 1H-nMR and thermal analysis (TGA and DSC). The performance of the polymer in cement was tested by measuring the effect of 1, 3 and 5 wt% of HBPAE solutions on the properties of Ordinary Portland Cement. Water of consistency, setting times, bulk density, apparent porosity, compressive strength and combined water content of the polymer/cement pastes were studied. The results showed that water of consistency and apparent porosity decreased while setting times, compressive strength, combined water and bulk density increased with the polymer addition.
