Background: The left internal mammary artery (IMA) is widely used as a conduit for coronary revascularization. The incidence of atherosclerosis is known to be lower in the IMA than in the coronary artery. The aim of this study was to evaluate the reliability of the use of the distal section of the IMA as an anastomotic site for bypass grafting and morphometric studies of IMA in patients with proven coronary artery disease and their associated risk factors. Methods: Patients who underwent Coronary Artery Bypass Graft (CABG) from June 2010 to November 2012 were chosen in this retrospective study and the discarded distal segments of the internal mammary artery were analyzed. The potential risk factors for atherosclerosis considered were age, sex, diabetes mellitus, history of cigarette smoking, hypertension and hypercholesterolemia. The samples were analyzed for the degree of intimal thickening and atherosclerosis by calculating the percentage of Luminal Narrowing, Intimal Thickness Index (ITI) and Intima-to-Media Ratio (IMR). Results: There were seven cases of intimal hyperplasia and two cases of focal medial and intimal hyperplasia with fatty streak and no cases of atherosclerosis and medial calcification. ITI was higher in males when compared to females. There was a strong relationship between IMR and smokers when compared to nonsmokers. Conclusion: In our study, when ITI was used as the dependent variable, diabetes was the most important factor. When IMR was used, the strongest predictor was hypercholesterolemia. Pyogenic infections are caused by various pathogens leading to pus formation and that can be attributed due to a wound either through accident or during surgery leading to infection spread. There are pathogenic strains that are not uncommon in hospital settings like Staphylococcus aureus, Klebsiella pneumoniae, Pseudomonas aeruginosa, Acinetobacter etc., that are multidrug resistant (MDR) and are a cause of concern. The bacteriological profile in the pyogenic infections tends to be same but there is a vast difference with the antibiotic resistant patterns in different hospital settings. Hence, the aim was to study the antibiotic susceptibility profiles and Extended spectrum βeta Lactamases (ESβL) production in these pathogens. A prospective study was carried out in Silchar Medical College and Hospital Assam, India, over a four-month period from February to May 2021. The samples were processed using Blood and MacConkey’s agar. Further, these isolated pathogens were identified by standard morphological, cultural and biochemical tests. The antibiotic susceptibility test was conducted by Kirby Bauer disc diffusion method and ESβL production was detected by using combined disk diffusion test. It was observed that the identified pathogens had an incidence rate of 84.2% and further revealed that Gram negative had a higher incidence rate compared to Gram positive with 59.8%. The pathogens isolated from pus samples had a maximum of Klebsiella sps (19.64%) and the lowest was E. coli with 5.36%. Antibiotic susceptibility test (AST) of Gram-negative bacterial isolates showed the highest incidence with aztreonam (40.6%) and the lowest was observed in Piperacillin/Tazobactam with 7.5%. The only Gram positive was observed in our study, Staphylococcus aureus had the highest resistance in amikacin with 80% and interestingly, all the isolates were sensitive to Linezolid with 100%. General radiography leaves enough clues for the ongoing diagnostic evaluation of the patient. The important clues can save a lot of time lost and other unnecessary investigations in the management of the patient illness. Sacrospinous ligament connects the sacrum with the pelvis. This in fact stabilizes the pelvis as it provides the support. This is important as this is helpful in supporting the vaginal vault in cases of prolapsed uterus in females. We report a 50-year-old male who had come for his intravenous pyelography for left ureteric calculus and was found to be having multiple other associated findings like osteophytosis, bilateral ilial horns and bilateral sacrospinous ligament calcifications. The clue was that of calcification and hardening of left sacrospinous ligament which has led to the formation of left side ureteric calculus. This ureteric calculus has caused great progressive damage to the left kidney by causing gross hydrouretero-nephrosis due to complete obstruction.

Development of an effective vaccine against the leading human bacterial pathogen group A Streptococcus(GAS)is a public health *** species defining group A cell wall carbohydrate(GAC,Lancefield antigen)can be engineered to remove its immunodominant N-acetylglucosamine(GlcNAc)side chain,implicated in provoking autoimmune cross-reactivity in rheumatic heart disease,leaving its polyrhamnose core(GACPR).Here we generate a novel protein conjugate of the GACPR and test the utility of this conjugate antigen in active *** of conjugation to a standard carrier protein,we selected SpyAD,a highly conserved GAS surface protein containing both B-cell and T-cell epitopes relevant to the bacterium that itself shows promise as a vaccine *** was synthesized using the XpressTM cell-free protein expression system,incorporating a non-natural amino acid to which GACpr was conjugated by site-specific click chemistry to yield high molecular mass SpyAD-GACPR conjugates and avoid disruption of important T-cell and B-cell immunological *** conjugated SpyAD-GACPR elicited antibodies that bound the surface of multiple GAS strains of diverse M types and promoted opsonophagocytic killing by human *** immunization of mice with a multivalent vaccine consisting of SpyAD-GACPR,together with candidate vaccine antigens streptolysin O and C5a peptidase,protected against GAS challenge in a systemic infection model and localized skin infection model,without evidence of cross reactivity to human heart or brain tissue *** general approach may allow GAC to be safely and effectively included in future GAS subunit vaccine formulations with the goal of broad protection without autoreactivity.

Perioperative Management of Cleft Palate Repair in a Patient with Williams Syndrome: A Case Report

The Background: Williams syndrome is a neurodevelopmental multisystem genetic disorder characterized by dysmorphic features and a wide range of congenital cardiac, renal, musculoskeletal anomalies. Presence of cleft palate in these patients is an unusual condition. More than 22 cases of sudden cardiac death reported in literature especially during anesthetic management in the patients of Williams syndrome. So these patients present major perioperative challenges to anesthesiologist. The authors report successful anesthetic care for repair of cleft palate in presence of uncorrected patent ductus arteriosus (PDA) and mild supraventricular aortic stenosis (SVAS) in a 3.5-year-old female child diagnosed with Williams syndrome. Case report: Diagnosed case of Williams syndrome with characteristic facial dysmorphic features, SVAS and rare malformation of PDA, was scheduled for cleft palate repair. After keen preoperative assessment, general anesthesia was administered for the procedure with continuous monitoring for vital parameters. Because of dysmorphic features, difficulty was faced during intubation. Otherwise intraoperative procedure was uneventful. Postoperative analgesia was managed with intravenous paracetamol. Conclusion: Being a multisystem disorder, each patient of WS requires meticulous preoperative evaluation and high level of intraoperative and postoperative continuous monitoring regardless of any surgical procedure. Surgical correction of cleft palate in such patients demands very high anesthetic and surgical skills as both share common airway. Present case report highlights the significance of aggressive perioperative management in WS which can result in successful outcome.

Cerebellar Haemangioblastoma Diagnosed as Giant Tuberculoma: Falacies of Magnetic Resonance Spectroscopy—Case Report

Background: Posterior fossa haemangioblastoma tumors are slightly rare and these originate from the brain blood vascular network. Cerebellum and brain stem are the commonest places of occurrence. These are benign and produce symptoms quite late because of their slow growing asymptomatic nature. Case report: We present 24-years old female who was wrongly diagnosed as having giant tuberculomas of the posterior fossa by MR spectroscopy. Contrast enhanced computerized tomography (CECT) and magnetic resonance imaging (MRI) studies had shown the results more in favor of tuberculoma. She was given anti-tubercular therapy (ATT) but without any much improvement. The condition aggravated by causing obstructive hydrocephalus at the level of fourth ventricle. The patient was operated upon for the mass lesion and on histopathology;it was proved to be a case of cerebellar haemangioblastoma. Conclusion: MR spectroscopy can sometimes be misleading and can lead to mis-mangemant. The reliability of spectroscopy is debatable and has to be decided on the merits along with clinical symptomatology.
