Multiple primary malignancies in a single patient are relatively rare but have increase in frequency in recent decades. This may be a result of medical advancements in diagnostic and therapeutic strategies, a possible effect of new carcinogens in the industrial environment, and longer life span allowing another primary cancer to develop. Among those with multiple primary malignancies, double cancer is commonly seen, while triple cancers occur in 0.5% of patients, and quadruple or quintuple cancers occur in only less than 0.1% of the population.

Water is a limited and valuable *** has four national sources of water supply,one of which is natural *** collected in stormwater runoff are deposited into drainage systems and *** nutrient pollutants found in local stormwater runoff include nitrate and phosphate,which may cause *** systems are efficient in removing these pollutants in the presence of *** paper discusses plant traits that can enhance the phytoremediation of nutrient pollutants in stormwater runoff for application in bioretention *** plant species studied showed variations in chlorophyll florescence,leaf greenness,biomass production,and nitrate and phosphate *** general,dry biomass was moderately correlated to nitrate and phosphate removal(r=0.339–0.501).Root,leaf,and total dry biomass of the native tree species showed a moderate to strong correlation with nitrate removal(r=0.811,0.657,and 0.727,respectively).Leaf dry biomass of fastgrowing plants also showed a moderate to strong relationship with the removal of both pollutants(r=0.707 and 0.609,respectively).Root dry biomass of slow-growing plants showed a strong relationship with phosphate removal(r=0.707),but the correlation was weaker for nitrate removal(r=0.557).These results are valuable for choosing plants for application in bioretention systems.

AIM: To investigate acute transient hepatocellular injury in patients with cholelithiasis and cholecystitis but no evidence of choledocholithiasis. METHODS: The medical records of patients with cholelithiasis who underwent cholecystectomy between July 2003 and June 2007 were retrospectively reviewed. Imaging studies to detect common bile duct (CBD) stones were performed in 186 patients, who constituted the study population. Biochemical liver tests before and after surgery, and with the presence or absence of CBD stones were analyzed. RESULTS: In 96 patients with cholelithiasis and cholecystitis without evidence of CBD stones, 49 (51.0%) had an alanine aminotransferase level elevated to 2-3 times the upper limit of normal, and 40 (41.2%) had an elevated aspartate aminotransferase level. Similar manifestations of hepatocellular injury were, as would be expected, even more obvious in the 90 patients with CBD stones. These markers of hepatocellular injury resolved almost completely within 2 wk to 1 mo after cholecystectomy. Compared to 59 patients with histologically less severe cholecystitis in the group undergoing urgent surgery (total 74 patients), the 15 patients with a gangrenous gallbladder had a higher mean level of total bilirubin (1.14 ± 1.27 mg/dL vs 2.66 ± 1.97 mg/dL, P changes in the gallbladder.

Internal solitary wave propagation over a submarine ridge results in energy dissipation, in which the hydrodynamic interaction between a wave and ridge affects marine environment. This study analyzes the effects of ridge height and potential energy during wave-ridge interaction with a binary and cumulative logistic regression model. In testing the Global Null Hypothesis, all values are p 〈0.001, with three statistical methods, such as Likelihood Ratio, Score, and Wald. While comparing with two kinds of models, tests values obtained by cumulative logistic regression models are better than those by binary logistic regression models. Although this study employed cumulative logistic regression model, three probability functions p^1, p^2 and p^3, are utilized for investigating the weighted influence of factors on wave reflection. Deviance and Pearson tests are applied to cheek the goodness-of-fit of the proposed model. The analytical results demonstrated that both ridge height （X1 ） and potential energy （X2 ） significantly impact （p 〈 0. 0001 ） the amplitude-based refleeted rate; the P-values for the deviance and Pearson are all 〉 0.05 （0.2839, 0.3438, respectively）. That is, the goodness-of-fit between ridge height （ X1 ） and potential energy （X2） can further predict parameters under the scenario of the best parsimonious model. Investigation of 6 predictive powers （ R2, Max-rescaled R^2, Sorners＇ D, Gamma, Tau-a, and c, respectively） indicate that these predictive estimates of the proposed model have better predictive ability than ridge height alone, and are very similar to the interaction of ridge height and potential energy. It can be concluded that the goodness-of-fit and prediction ability of the cumulative logistic regression model are better than that of the binary logistic regression model.

Digital infrared thermography is suitable for monitoring the planar two-dimensional temperature distribution of curved surfaces of objects by sensing their infrared radiation. Cardiac infrared thermography also has a thermal coronary angiography alias. This study proposes a digital image processing methodology for locating blood clot blockage. This methodology contains four consequent processes. The two-dimensional gray scale infrared thermograph pixels are first binarized and classified as background or coronary arteries using multi-thresh adaptive *** coronary artery contours are extracted from segmented raw pixels using continuous pepper-like pixel removal,erosion,subtraction,recursive neighborhood visiting,contour point-list construction and short edge *** the third process one coronary artery branch is selected by physicians for calculating the longest curved central axis using morphological thinning and neighborhood analysis. In the last process the nearest left and right distances from each pixel along the directional central axis to its corresponding boundary contour are added as the coronary artery variable diameter at the current pixel's position. A variable diameter versus straighten length diagram along this axial curved path is plotted to provide useful physiological information to the physician. An obstruction rate equation is then defined to calculate the possible vascular blockage positions with the local minimal rates. Finally,preoperative cases are tested to prove the predictive positions are correct in comparison to individual patient myocardial perfusion imaging.

BACKGROUND Cystic artery pseudoaneurysm is a condition rarely encountered by clinicians;this,its etiology and presentation as well as appropriate treatments are not well *** it is treated by removal of the diseased gallbladder and cystic artery,such surgery can be difficult and risky if acute inflammation with bleeding occurs,and not every patient can tolerate the *** SUMMARY An 81-year-old man complained of epigastric pain and tarry stool passage that lasted for 3 *** had a medical history of poor cardiopulmonary *** computed tomographic scan of abdomen showed cystic artery pseudoaneurysm and dilatation of *** of high adverse outcomes related to general anesthesia,the patient was successfully managed with endovascular embolization for this cystic artery pseudoaneurysm and percutaneous drainage for the distended *** A patient with cystic artery pseudoaneurysm may quickly deteriorate with the occurrence of concurrent arterial bleeding and *** report presents the case of a patient who did not undergo surgery due to multiple cardiopulmonary comorbidities and whose condition was managed successfully with embolization and biliary *** embolization and biliary drainage may provide an alternative option to manage this complicated condition.

在人文医学领域,疾病叙事是患者与医者生命书写的一部分和建构自我主体性的叙事方法;文学史上的疾病叙事具有清晰的发展脉络,并逐步获得作为文学主题的独立地位。叙事医学的视角下,文学文本中的疾病叙事言说疾病的“不可见”维度,弥合“可见”与“不可见”的裂痕。以白先勇的两部短篇小说为样本,对文学文本中的疾病叙事做了具体的叙事医学阐释,说明其从对“不可见”的具身体验的言说走向肉灵跨越的身心救赎的丰富意蕴;同时指出过往疾病叙事中医者角色的缺席,提出利用文学文本疾病叙事建立医患主体间性。

A new species of the elaterid genus Cryptocoelus Dolin and Nel, 2002 from the Late Jurassic to Early Cretaceous Yixian Formation in the western Liaoning, China is described. Diagnosis of the genus is revised, and the systematic position of the genus is briefly discussed. Because the elaterids originated in the Early to Mid-Jurassic, this new material from the Late Jurassic-Early Cretaceous will enhance our understanding of the transition between ancient elaterids and extant ones, bridge the gap of cryptic relationships between the Mesozoic cupedids and elaterids, and expand our knowledge of their evolutionary history.

In this paper, three approachs were developed to find indicator species from the habitat created by coastal structures. These approachs consist of a model of species co-occurrence probability, a model of k-environmental factor probability and a composite model. Simultaneously, a case study was conducted in Hsinchu Fishing Port of north-western Taiwan. Based on the aforementioned models, three primary producer species, Ahnfeltiopsis flabelliformis, Chondrus ocellatus and Sarcodia montagneana, were chosen as the indicator species which had the highest co-occurrence probabilities and showed greater tolerance to more critical environment. It is imperative to understand how the three species under particular co-occurring conditions and environmental factors influence the composition of sessile assemblages in coastal water. The results indicate that for the purpose of increasing biodiversity, these models are feasible to find indicator species of artificial structures, and to help make it possible to design coastal structures based on biological considerations. This study provides an innovative approach for further advanced application in the artificial habitat of coast management.
