We describe a 74-year-old man with a colonic metastatic squamous cell carcinoma (SCC) from the lung. His chest X-ray revealed an abnormal shadow in the right upper lobe. Computed tomography (CT) of the chest demonstrated a large lung tumor in the right upper lobe obstructing the right upper bronchus. Bronchoscopy revealed an easy-bleeding tumor in the right upper bronchus that was diagnosed as poorly differentiated squamous cell lung carcinoma. He underwent colonoscopy because he had a positive fecal occult blood test. Colonoscopy revealed a large protruding lesion with central ulceration in the descending colon. Histological examination of the biopsy specimen obtained from the colonic lesion revealed SCC. The lesion was diagnosed as metastatic colonic SCC. He had no abdominal symptoms. He underwent chemotherapy with an infusion of cisplatin 130 mg i.v. day 1, and docetaxel hydrate 100 mg i.v. day 1, repeated every 4 wk, followed by 4 courses of chemotherapy. The primary lesion shrank by less than 10% and was judged to be "Partial Response" (PR) after 3 courses of treatment. The patient still lived 23 wk after the diagnosis of metastatic colonic SCC. Colonic metastasis of primary SCC of the lung is rare.

The clinical course of the Asian variant of intravascular large B-cell lymphoma (AIVL) is generally very aggressive. We describe a case of AIVL demonstrating an unusual clinical course, with spontaneous improvement. An 81-year-old man with high-grade fever and thrombocytopenia was admitted to our hospital. Although we could not confirm the origin of his symptoms, they disappeared completely without intervention within 2 weeks. Three months later, however, thrombocytopenia reappeared and progressed. Finally, he was readmitted due to a subdural hemorrhage with high fever and he finally died of rapidly progressive multiple organ failure. Autopsy findings revealed the presence of B-cell lymphoma cells in microscopic vessels of many organs as well as hemophagocytosis in the bone marrow. He was diagnosed with AIVL with an unusual indolent clinical course with spontaneous improvement.

A 66-year-old Japanese man with pancreatic cancer received eleven courses of gemcitabine *** tumor responded to gemcitabine until metastatic liver tumors ***,he was treated with S-1,an oral fluoropyrimidine anticancer agent,as salvage ***-two days after initiating S-1,he presented with dyspnea and *** computed tomography showed diffuse interstitial lesions with thickening of the alveolar septa and ground glass *** KL-6 level was elevated to 1,230 U/mL and he did not use any other drugs except ***,the development of interstitial lung disease(ILD)was considered to be due to *** blood oxygen pressure was 49.6 Torr in spite of oxygen administration(5 L/min).Steroid therapy improved his symptoms and the interstitial shadows on chest *** S-1-induced ILD has mostly been reported to be mild,clinicians should be aware that S-1 has the potential to cause fatal ILD.

The patient was a 71-year-old man who underwent a right hemicolectomy for ascending colon cancer(pT3,pN1,pM0) and who opted not to receive adjuvant *** months later,multiple liver metastases *** therefore received FOLFOX4(5-fluorouracil/leucovorin and 85 mg/m 2 oxaliplatin) therapy,up to a total of 5 courses,and showed a partial *** receiving the sixth course of FOLFOX4,he complained of chest pain and systemic itching approximately 15 min after the start of *** electrocardiogram revealed typical signs of *** arteriography showed that the coronary arteries were *** the chest pain to be merely coincidental,we continued with the same ***,he again developed the same chest pain during the seventh cycle of FOLFOX4 and treatment was *** concluded that the patient's symptoms were due to acute coronary syndrome(ACS) associated with the FOLFOX4 *** angina as a type of ACS is a rare adverse effect of FOLFOX4. Clinicians should be aware of this potential adverse effect when monitoring patients receiving FOLFOX4.

Tuberculous otitis media(TOM) is a rare manifestation caused by Mycobacterium tuberculosis with low incidence rates among extrapulmonary tuberculosis cases. Diagnosis is often delayed because of the presence of several clinical manifestations and the high prevalence of secondary bacterial infections. Few reports have attributed secondary bacterial infections in patients with TOM to commensal Neisseria. Thus, understanding the pathogenic mechanisms and clinical features of commensal Neisseria is important, considering its recent presentation as an infection-causing pathogen. Neisseria mucosa is a commensal inhabitant in humans and is generally considered non-pathogenic but can cause infection in rare cases. Here, we report an atypical secondary infection caused by Neisseria mucosa in an 81-year-old woman with TOM being treated for pulmonary tuberculosis. Direct purulent otorrhea smear microscopy revealed no acid-fast bacilli using Ziehl-Neelsen staining, whereas the phagocytosis of gram-negative cocci by white blood cells was confirmed using Gram staining. Otorrhea culture revealed the growth of N. mucosa. Subsequently, M. tuberculosis infection in the otorrhea was identified using a culture-based method. Vigilance is critical for the early detection of TOM to prevent further complications. This report raises awareness regarding TOM and provides insight into the pathogenicity of N. mucosa in otitis media.

The existence of excess sodium dodecyl sulfate(SDS)molecules in a single-walled carbon nanotube(SWNT)solution dispersed by hybridization with SDS leads to unstable atomic force microscopy(AFM)*** this study,we demonstrate sequential dialysis against pure water in order to remove excess SDS molecules from an SDS-SWNT hybrid dispersion.A 1:102 volume ratio of SDS-SWNT dispersion to water in the dialysis was found to be effective in realizing stable AFM observations of the SDS-SWNT hybrids despite imperfect filtering of SDS via *** the other hand,the SDS-SWNT hybrids were stable even when this volume ratio was 1:***,the SDS-SWNT hybrids were present in the solution even when the dialyzed samples were stored for 14 *** results reveal that dialysis under optimal conditions enables improved handling of SDS-SWNT hybrids,particularly for stable AFM observations.

Cowden's disease, one of the several hamartoma syndromes, is characterized by hyperplastic lesions and hamartomas distributed in the whole body. About thirty percent of patients with Cowden's disease have been reported to be complicated by malignant tumors. Based on the criteria of the International Cowden Consortium, this disease is mainly diagnosed as trichilemmoma of the face and oral mucosal papillomatosis. However, Cowden's disease patients themselves often do not recognize trichilemmoma of the face and oral mucosal papillomatosis. We report a case of Cowden's disease in a 33-year-old female patient who was diagnosed based on the characteristic findings at gastrointestinal endoscopy. Clinically, the patient was aware of having bloody stools. Multiple polyps found endoscopically in the esophagus, stomach, ileum, colon and rectum showed histopathologically hamartomatous changes and epithelial hyperplasia. Physical examination revealed oral papillomatosis and facial trichilemmomas. A germline mutation in exon 8 of the phosphatase and tensin homolog deleted on chromosome ten (PTEN) gene was found in this case. It was a point mutation of C to T at codon 1003 (CGA→TGA, arginine→stop codon). The characteristic findings on gastrointestinal endoscopy led us to a diagnosis of Cowden's disease. It has been reported that gastrointestinal polyposis with esophageal polyposis is found in about 85.7% of Japanese patients with Cowden's disease. The characteristic findings on gastrointestinal endoscopy can be a useful diagnostic clue to Cowden's disease.
