A Comparison of Arterial Oxygenation between 60% O₂ CPAP and 100% O₂ CPAP during One-Lung Ventilation: A Prospective Randomized Controlled Study

Background: One-lung ventilation (OLV) is generally adopted for thoracic surgery. The systemic application of a high fraction of inspiratory oxygen (F1O2) and continuous positive airway pressure (CPAP) to the non-ventilated lung is useful for preventing arterial oxygen desaturation. The adverse effects of elevated F1O2 include oxidative lung injury, resorption atelectasis and coronary and peripheral vasoconstriction. It is preferable to avoid hyperoxemia in patients with complications such as chronic obstructive pulmonary disease, idiopathic pneumonia, and bleomycin-treated lungs. We aimed to determine whether the application of 60% O2 CPAP to the non-ventilated lung is sufficient to provide adequate oxygenation with 60% O2 to the ventilated lung. Methods: A total of 70 patients scheduled to receive elective thoracic surgery requiring OLV were recruited. Left double-lumen tubes were applicable in all surgeries. Patients were randomly allocated to one of two groups, to receive either 60% O2 CPAP (60% CPAP group, n = 35), or 100% O2 CPAP (100% CPAP group, n = 35) at a setting of 2 - 3 cmH2O, applied to the non-ventilated lung. Arterial blood gas analyses were obtained at the following stages: RA, spontaneous breathing under room air (RA);TLV, during total lung ventilation (TLV) prior to the initiation of OLV;T5, 5 min after the initiation of OLV;T15, 15 min after the initiation of OLV;T30, 30 min after the initiation of OLV. Results: The PaO2 value in 60% CPAP group vs. 100% CPAP group at each measurement were as follows: RA (mean [standard deviation: SD], 89.7 [8.2] mmHg vs. 85.8 [11.9] mmHg);TLV (277.9 [52.9] mmHg vs. 269.2 [44.0] mmHg);T5 (191.4 [67.9] mmHg vs. 192.3 [66.0] mmHg);T15 (143.2 [67.3] mmHg vs. 154.7 [60.8] mmHg) and T30 (95.6 [32.0] mmHg vs. 112.5 [36.5] mmHg), respectively. Comparison of percutaneous transhepatic portal vein embolization and unilateral portal vein ligation

AIM:To compare the effect of percutaneous transhepatic portal vein embolization(PTPE) and unilateral portal vein ligation(PVL) on hepatic hemodynamics and right hepatic lobe(RHL) ***:Between March 2005 and March 2009,13 cases were selected for PTPE(n = 9) and PVL(n = 4) in the *** PTPE group included hilar bile duct carcinoma(n = 2),intrahepatic cholangiocarcinoma(n = 2),hepatocellular carcinoma(n = 2) and liver metastasis(n = 3).The PVL group included hepatocellular carcinoma(n = 2) and liver metastasis(n = 2).In addition,observation of postoperative hepatic hemodynamics obtained from computed tomography and Doppler ultrasonography was compared between the two ***:Mean ages in the two groups were 58.9 ± 2.9 years(PVL group) vs 69.7 ± 3.2 years(PTPE group),which was a significant difference(P = 0.0002).Among the indicators of liver function,including serum albumin,serum bilirubin,aspartate aminotransferase,alanine aminotransferase,platelets and indocyanine green retention rate at 15 min,no significant differences were observed between the two *** RHL volumes in the PTPE and PVL groups were estimated to be 804.9 ± 181.1 mL and 813.3 ± 129.7 mL,respectively,with volume rates of 68.9% ± 2.8% and 69.2% ± 4.2%,*** were no significant differences in RHL volumes(P = 0.83) and RHL volume rates(P = 0.94),*** 1 mo after PTPE or PVL,postoperative RHL volumes in the PTPE and PVL groups were estimated to be 638.4 ± 153.6 mL and 749.8 ± 121.9 mL,respectively,with no significant difference(P = 0.14).Postoperative RHL volume rates in the PTPE and PVL groups were estimated to be 54.6% ± 4.2% and 63.7% ± 3.9%,respectively,which was a significant difference(P = 0.0056).At 1 mo after the operation,the liver volume atrophy rate was 14.3% ± 2.3% in the PTPE group and 5.4% ± 1.6% in the PVL group,which was a significant difference(P = 0.0061).CONCLUSION:PTPE is a more effective procedure than PVL because PTPE is able to

Hepatitis B virus infection and alcohol consumption

Hepatocellular carcinoma(HCC) is the most common type of primary liver cancer, and the second most common cause of cancer deaths worldwide. The top three causes of HCC are hepatitis B virus(HBV),hepatitis C virus(HCV), and alcoholic liver disease. Owing to recent advances in direct-acting antiviral agents, HCV can now be eradicated in almost all patients. HBV infection and alcoholic liver disease are expected, therefore, to become the leading causes of HCC in the future. However, the association between alcohol consumption and chronic hepatitis B in the progression of liver disease is less well understood than with chronic hepatitis C. The mechanisms underlying the complex interaction between HBV and alcohol are not fully understood, and enhanced viral replication, increased oxidative stress and a weakened immune response could each play an important role in the development of HCC. It remains controversial whether HBV and alcohol synergistically increase the incidence of HCC. Herein, we review the currently available literature regarding the interaction of HBV infection and alcohol consumption on disease progression.

Simultaneous bile duct and portal venous branch ligation in two-stage hepatectomy

Hepatectomy is an effective surgical treatment for multiple bilobar liver metastases from colon cancer;however,one of the primary obstacles to completing surgical resection for these cases is an insufficient volume of the future remnant liver,which may cause postoperative liver *** induce atrophy of the unilateral lobe and hypertrophy of the future remnant liver,procedures to occlude the portal vein have been conventionally used prior to major *** report a case of a 50-year-old woman in whom two-stage hepatectomy was performed in combination with intraoperative ligation of the portal vein and the bile duct of the right hepatic *** procedure was designed to promote the atrophic effect on the right hepatic lobe more effectively than the conventional technique,and to the best of our knowledge,it was used for the first time in the present *** successful induction of liver volume shift as well as the following procedure,the patient died of subsequent liver failure after developing recurrent *** discuss the first case in which simultaneous ligation of the portal vein and the biliary system was successfully applied as part of the first step of two-stage hepatectomy.

We have isolated a recessive rice mutant, designated as indeterminate growth （ing）, which displays creeping and apparent heterochronic phenotypes in the vegetative period with lanky and winding culms. Rough mapping and subsequent molecular characterization revealed that the ing mutant carries a large deletion, which corresponds to a 103 kb region in the Nipponbare genome, containing nine annotated genes on chromosome 3. Of these annotated genes, the SLR1 gene encoding a DELLA protein is the only one that is well characterized in its function, and its null mutation, which is caused by a single base deletion in the middle of the intronless SLR1 gene, confers a slender phenotype that bears close resemblance to the ing mutant phenotype. The primary cause of the ing mutant phenotype is the deletion of the SLR1 gene, and the ing mutant appears to be the first characterized mutant having the entire SLR1 sequence deleted. Our results also suggest that the deleted region of 103 kb does not contain an indispensable gene, whose dysfunction must result in a lethal phenotype.

Atmospheric lead pollution in fine particulate matter in Shanghai, China

The Pb-monitoring program was extended for 6 years from 2002 to 2007 at 17 representative urban sites （6 traffic, 5 industrial, and 6 residential sites）, and 3 suburban sites to assess the lead pollution in fine particulate matter （PM2.5） after phasing out leaded gasoline in Shanghai. Compared with Pb levels reported in other places, the Pb pollution in Shanghai is still serious after phasing out leaded gasoline, which remains at high concentration range （213-176 ng/m^3） in PM2.5 in winter. Significant spatial variation of Pb concentrations and strong seasonal variation of higher Pb concentration in winter than that in summer were detected. The size distribution of Pb in particulate matter has a unimodat mode that peaks at approximately 0.154-1.59 μm particle diameter, indicating that Pb is mainly concentrated in fine fraction. Lead in the fine fraction is enriched by a factor of 103-104 relative to Pb abundance in crust. Eight categories of Pb pollution sources were identified in the PM2.5 in the winter of 2007 in Shanghai. The important emission sources among them are vehicle exhaust derived from combustion of unleaded gasoline, metaUurgic industry emission, and coal combustion emission.

Oral Health Management of Three Head and Neck Cancer Patients Performed by Dental Hygienists

Introduction: Oral health management is often difficult in patients after surgical tumor resection and/or radiotherapy due to defects, bulky flaps, limitation of tongue movement, and trismus. In addition, patients who have undergone radiotherapy have serious disabilities such as oral mucositis, mucosal weakening, soft tissue fibrosis, salivary gland disorder, and osteoradionecrosis. Dental hygienists must understand the characteristics of patients after surgical tumor resection and/or radiotherapy. In this report, the oral health management of three patients after maxillectomy, mandibulectomy, and radiotherapy was shown. Case Description: Case 1: A 53-year-old male patient visited our clinic after chemotherapy, radiotherapy, and partial maxillectomy for cancer of the right maxillary gingiva. Case 2: A 65-year-old male patient visited our clinic after radiotherapy for a right-sided tongue cancer, partial mandibulectomy of the right tongue to the oropharynx, a right-sided segmental mandibulectomy and a speech aid and a mandibular prosthesis were provided. Case 3: A 36-year-old female visited our clinic for radiotherapy for left-sided tongue cancer. Dental hygienists provided oral health management to the patients. In oral health management, it was suggested that intervention before cancer treatment, oral hygiene instructions tailored to patients’ symptoms, and frequent professional care are important. Conclusion: Oral health management by dental hygienists is extremely important for patients undergoing maxillofacial cancer treatment to maintain their quality of life in the long term.

Legionella dumoffii Tex-KL Mutated in an Operon Homologous to traC-traD is Defective in Epithelial Cell Invasion

Objective To understand the mechanism of invasion by Legionella dumoffii. Methods The L. dumoffii strain Tex-KL was mutated using the Tn903 derivative, Tn903 d IIlac Z. After screening 799 transposon insertion mutants, we isolated one defective mutant. We then constructed the gene-disrupted mutant, KL16, and studied its invasion of and intracellular growth in He La and A549 cells, and in A/J mice survival experiments. The structure of traC-traD operon was analyzed by RT-PCR. Results The transposon insertion was in a gene homologous to Salmonella typhi tra C, which is required for the assembly of F pilin into the mature F pilus structure and for conjugal DNA transmission. Results from RT-PCR suggested that the traC-traD region formed an operon. We found that when the tra C gene was disrupted, invasion and intracellular growth of L. dumoffii Tex-KL were impaired in human epithelial cells. When mice were infected by intranasal inoculation with a tra C deficient mutant, their survival significantly increased when compared to mice infected with the wild-type strain. Conclusion Our results indicated that the traC-traD operon is required for the invasion and intracellular growth abilities of L. dumoffii Tex-KL in epithelial cells.

Characterization and source identification of fine particulate matter in the atmosphere of downtown Shanghai using μ-SXRF and ICP-MS

To investigate the characteristic and sources of the fine particulate matter(PM2.5),and the major sources affecting particulate air pollution in Shanghai,China,the individual aerosol particles were analyzed using the synchrotron radiation micro-beam X-ray fluorescence analysis(?-SXRF),and the PM2.5 multi-elements were determined by the inductively coupled plasma mass spectrometer(ICP-MS).The results show that the mass concentration and chemical elements have seasonal variation,and the enrichment factors show that the chemical elements in the inhalable particles could come from the earth crust and anthropogenic *** extent of metal pollution was assessed by comparing the measured concentrations with those reported in the *** categories of PM2.5 pollution sources identified at the center of Shanghai show that the vehicle exhaust,emission of metallurgic industry and coal combustion are important.
