Development and Validation of the Japanese Version of a Job Stressor Scale for Triage Nurses in Emergency Departments

Background: With an increasing number of patients who visit emergency outpatient units, a shortage of physicians and nurses in emergency units has become an issue in Japan. Triage nurses who interview patients and their families before medical examinations feel stressed to determine the triage level in a limited time, necessitating the measures to alleviate stressors. Objective: To develop a triage nurse job stressor scale (TNJSS) for Japanese triage nurses in emergency outpatient units and to verify the reliability and validity of this scale. Methods: Anonymous, self-administered questionnaires were sent to nursing directors of 180 emergency and critical care centers randomly selected from 251 centers throughout Japan, requesting to distribute the questionnaire to nurses. Results: Based on the responses obtained from 363 nurses, the construct validity, internal consistency, and criterion-related validity were verified. A factor analysis of 44 items yielded five factors: “Lack of triage ability”, “Busy triage work”, “Patients without understanding of explanations”, “Complaints from patients waiting for treatment”, and “Lack of support to improve triage ability”. Cronbach’s α was 0.93 for the full scale and significant correlations were observed between the nurses’ stressor scores and scores for the Stress Response Scale-18 (r = 0.409;p r = 0.410;p Conclusions: The TNJSS with a five-factor structure containing 44 items was determined to be a reliable and valid tool for evaluating Japanese triage nurse job stressors. The findings suggest the necessity to continuously educate and support triage nurses.

Neural-Based Routing Method for Alleviating Congestion in Complex Networks

In this paper, we propose a new packet routing strategy that incorporates memory information for reducing congestion in communication networks. First, we study the conventional routing strategy which selects the paths for transmitting packets to destinations using the distance information and the dynamical information such as the number of accumulating packets at adjacent nodes. Then, we evaluate the effectiveness of this routing strategy for the scale-free networks. From results of numerical simulations, we conclude that this routing strategy is not effective when the density of the packets increases due to the impermeability of the communication network. To avoid this undesirable problem, we incorporate memory information to the routing strategy. By using memory information effectively, packets are spread into the communication networks, achieving a higher performance than conventional routing strategies for various network topologies, such as scale-free networks, small-world networks, and scale-free networks with community

Background/Purpose: Esophageal reconstruction for long gap esophageal atresia (LGEA) is still controversial. We successfully managed 7 cases of patients with LGEA by doing staged elongation of the native esophagus and subsequent end-to-end anastomosis. The technique and efficacy of this procedure are evaluated. Methods: During the last 10 years, 7 patients with LGEA (Gross type A, 5; B, 1; C, 1) underwent multiple extrathoracic esophageal elongations (ETEEs) of the upper esophagus and subsequent esophagoesophagostomy. Medical records were reviewed in regard to the number of ETEE before definitive esophageal reconstruction, interval between each ETEE, operation time, time before initiation of sham feeding, duration of hospital stay, and complications. Results: The definitive esophageal reconstruction was successfully achieved without major complications in all patients after 2 to 4 stages of ETEE. The interval between each ETEE was 72 days on average. The average operation time was 98 minutes. The elongation was 1 to 3.5 cm during each session. Oral sham feeding was recommenced 4.1 days after each ETEE, and the hospital stay was 9.6 days on average. Gastroesophageal reflux occurred in all patients, requiring antireflux surgery. Conclusions: We conclude from our experience (a) that effective esophageal lengthening with preservation of the native esophagus was achieved with multiple ETEE in LGEA and (b) that this procedure allows oral sham feeding at home until esophageal reconstruction.

To assess the peripheral refractive changes that occur when eye growth decelerates in *** We measured peripheral refraction and on-axis vitreous chamber depth on a total of 53 marmosets;36 treated monocularly for 12 weeks 9 hrs/day(26 with-5 D contact lenses,10 with+5 D)and 17 untreated *** the 26 marmosets treated with-5 D,10 wore contact lenses 9 hrs/day without interruptions and were mea-sured at the end of 10 weeks of treatment(T10)and after 4 weeks of recovery(R14).Eight had both contact lenses removed for 30 mins twice/day(mid-morning and mid-afternoon)during the first four weeks of treatment(early interruption group)and eight had the lenses removed during the second four weeks of treatment(late interruption group).The two interruption groups were measured after 4,8 and 12 weeks of treatment only(T4,T8,T12).
