Filiform needle acupuncture for allergic rhinitis:A systematic review and meta-analysis

Background:Filiform needle acupuncture(FNA),the most classical and widely applied acupuncture method based on traditional Chinese medicine theory,has shown a promising effect in the treatment of allergic rhinitis(AR).Objective:To evaluate the efficacy,safety,cost-effectiveness,and patient preference of FNA in the treatment of AR by comparing FNA with sham acupuncture,no treatment,and conventional *** strategy:Eight electronic databases were systematically searched from inception to October 14,*** studies were acquired from clinical trial registration platforms and reference *** criteria:Randomized controlled trials were included if they compared FNA with either sham acupuncture,no treatment or conventional medication for *** extraction and analysis:Two researchers extracted data independently of each other using a predesigned data acquisition form,and results were cross-checked after *** primary outcome was symptom score(Total Nasal Symptom Score or Visual Analogue Scale),and the secondary outcomes were the AR control questionnaire,quality of life(QoL)score(Different versions of Rhinoconjunctivitis Quality of Life Questionnaire),medication score(use of rescue medication),mental health score,total IgE,adverse event rate,clinical economic indicators,and patient satisfaction *** mean difference(SMD)or mean difference(MD)with 95% confidence interval was used to calculate the effect size for continuous data,while risk ratio with 95%CI was used for dichotomous ***:Thirty studies were included in this *** with sham acupuncture,FNA significantly reduced the symptom score(SMD:-0.29[-0.43,-0.15]),AR’s impact on QoL(SMD:-0.23[-0.37,-0.08])and medication score(SMD:-0.3[-0.49,-0.11]).Compared with no treatment,FNA dramatically reduced the symptom score(SMD:-0.8[-1.2,-0.39])and AR’s impact on QoL(SMD:-0.82[-1.13,-0.52]).There were no increased rates of adverse events with FNA compared to sham

Influence of sintering time on microstructure and properties of hot oscillatory pressing sintered W–Cu refractory alloys

W–Cu refractory alloys are widely used in aerospace,aviation,electronics,power,and other fi ***,because of its large melting point diff erence between alloy elements,the conventional powder metallurgy method required a long time of high temperature sintering is very likely to cause low density and abnormal grain size ***,90W-10Cu refractory alloys were successfully prepared by hot oscillatory pressing(HOP)under diff erent sintering time in this *** the infl uence of sintering time on the microstructure,grain size,density,Vickers hardness,room-tensile property,and electrical conductivity of the W–Cu refractory alloy was systematically *** results showed that during the short sintering time(30–90 min),the density,Vickers hardness,tensile strength,and electrical conductivity of the W–Cu refractory alloy increased signifi cantly to 98.7%,163.29 HV 30,507.3 MPa,and 14.2%International Annealed Copper Standard with the sintering time increasing,respectively,while its grain size showed no obvious *** the sintering time further increased to 120 min,the density was basically consistent with the sintering time of 90 ***,the prolonged high temperature sintering resulted in the grain growth of the samples,which caused the deterioration of the Vickers hardness,room-tensile strength,and conductivity properties of the W–Cu refractory *** the same sintering time,the density,Vickers hardness,and electrical conductivity of the HOP sintered W–Cu refractory alloy were signifi cantly higher than those of the HP sintered *** was indicated that high density,fi ne-grain,and excellent properties W–Cu refractory alloys could be prepared by hot oscillatory pressing under appropriate sintering time.

Urinary incontinence (UI) is a common problem *** has a major impact on physical and social activities and interpersonal *** is common in women,but is under-reported and *** affects the quality of life of female patients *** and moxibustion have been proposed as potentially effective interventions for female ***,for the benefit of acupuncture practitioners around the world,the World Federation of Acupuncture-moxibustion Societies initiated a project to develop a clinical practice guideline (CPG) for the use of acupuncture and moxibustion to treat female *** CPG was developed according to the Grading of Recommendations Assessment,Development,and Evaluation (GRADE) methodology,referring to the principles of the World Health Organization Handbook for Guideline *** the development of the CPG,the guideline development group (GDG) played an important *** clinical questions,recommendations and therapeutic protocols were all formulated by GDG using the modified Delphi *** CPG contains ten recommendations about the use of acupuncture and moxibustion interventions for ten clinical questions,which include nine conditional recommendations for the intervention and one conditional recommendation for either the intervention or the *** CPG also provides one protocol for conventional filiform needle therapy,two therapy protocols for deep needling stimulation on lumbosacral acupoints,and four moxibustion therapy protocols,based on the protocols presented in randomized controlled trials reviewed by the GDG.

Acupuncture is one of the most effective complementary therapies for allergic rhinitis(AR)and has been recommended by several clinical practice guidelines(CPGs)for ***,these CPGs mentioned acupuncture without making recommendations for clinical implementation and therapeutic protocols,therefore limiting the applicability of acupuncture therapies for ***,for the benefit of acupuncture practitioners around the world,the World Federation of Acupuncture-moxibustion Societies have initiated a project to develop the CPG for the use of acupuncture and moxibustion to treat *** CPG was developed according to the Grading of Recommendations Assessment,Development,and Evaluation(GRADE)methodology,referring to the principles of the World Health Organization Handbook for Guideline *** the development of the CPG,the guideline development group(GDG)played an important *** clinical questions,recommendations and therapeutic protocols were all formulated by the GDG using the modified Delphi *** CPG contains recommendations for 15 clinical questions about the use of acupuncture and moxibustion *** include one strong recommendation for the intervention based on high-quality evidence,three conditional recommendations for either the intervention or standard care,and 11 conditional recommendations for the intervention based on very low quality of *** CPG also provides one filiform needle acupuncture protocol and five moxibustion protocols extracted based on the protocols presented in randomized controlled trials reviewed by the GDG.

Key questions and outcomes for the clinical practice guideline on acupuncture and moxibustion for allergic rhinitis

Objective:Formulating critical clinical questions and outcomes is essential for developing clinical practice guidelines(CPGs).This study aims to formulate the key clinical questions and outcomes for the CPG on acupuncture and moxibustion for allergic rhinitis(AR).Methods:PICO(patient,intervention,comparator,and outcome)question survey,Delphi survey,and consensus conference of the guideline development group(GDG)were *** questionnaires were used to gather and analyze the target users’key demands and *** GDG then selected the critical clinical questions and outcomes via a mixed method of Delphi survey and consensus ***:Fifteen potential clinical questions and ten types of outcomes were formulated based on the analysis of 123 responses from seventeen countries on five *** two rounds of the Delphi survey and complete discussions,a consensus was reached by the GDG that all potential questions were *** three rounds of the Delphi survey and complete debate,the consensus was reached that nine outcomes were considered ***:Fifteen key clinical questions and nine important outcomes were formulated for the *** covered issues elicited the most attention from global users in acupuncture therapy and *** results will contribute to the CPGs development and future clinical studies.

A Mn-porphyrinic metal–organic framework immobilizing glucose oxidase for combined photodynamic/chemodynamic/starvation therapy

The combination of photodynamic therapy（PDT） and chemodynamic therapy（CDT） in tumor treatment has attracted considerable ***,tumor hypoxia and glutathione（GSH） overproduction in the tumor tissue restricted the progress of their ***,a Mn-porphyrinic metal-organic framework（Mn-TCPP） was constructed by the one-pot method and further used for immobilizing glucose oxidase（GOx） to obtain GOx@*** would rapidly exhaust endogenous glucose into hydrogen peroxide（H2O2） and gluconic acid,thus shutting off the energy supply of tumor cells for starvation ***-TCPP catalyzed H2O2to produce oxygen,regulating the hypoxic tumor microenvironment and in turn improving1O2generation under laser ***,Mn-TCPP can reduce reactive oxygen species consumption owing to the redox reaction between Mn3+and GSH,thus greatly enhancing ***,benefiting from GOx-mediated starvation therapy,Mn2+produced by Mn3+reduction can react with sufficient intracellular H2O2to generate ·OH with high cytotoxicity through a Fenton-like *** treatment by GOx@Mn-TCPP under laser irradiation in vitro,the cell viability of 4T1 and A549 tumor cells reached to 20%,reflecting GOx@Mn-TCPP could give full play to the advantages of PDT/CDT/starvation *** results in vivo demonstrated that GOx@Mn-TCPP mediated synergistic cascade therapy could significantly inhibit tumor growth and improve the therapeutic effect.

Cobalamin(Cbl)-dependent radical S-adenosyl-L-methionine(SAM) proteins constitute the largest collection of the radical SAM superfamily that has hundreds of thousands of individual members. Many of these proteins are involved in the biosynthesis of pharmaceutically important natural products to catalyze chemically demanding reactions. In the biosynthetic pathway of chuangxinmycin(CXM), a unique indole alkaloid antibiotic with potent anti-infective activity, functionalization of the characteristic thiopyrano[4,3,2-cd]indole scaffold by regio-and stereoselective C3-methylation is believed to rely on a Cbl-dependent radical process, which, however, remained to be reconstituted biochemically. We here report the dissection of this enzymatic process, which requires the incorporation of Cxm8, a Cbl-dependent radical SAM protein, with Cxm9, a duF5825family protein that shares no homology to any proteins of known functions. Cxm8 and Cxm9 function together by forming an unexpected heterodimeric complex that selectively catalyzes C3-methylation of the tricyclic indole-S-hetero ring system in a successive manner, achieving CXM and a recently identified, C3-dimethylated congener. Detailed biochemical characterization,isotope labeling, structural simulation and bioinformatics analysis rationalized the catalysis of the Cxm8/Cxm9 complex and particularly the necessity of the duF5825 protein for C3-methylase activity. This is the first example that a Cbl-dependent protein acts with a partner to exhibit radical SAM activity.

Reported here is an iridium-catalyzed enantio-divergent C（sp3）–H borylation of α-gem-dimethyl groups of trifluoromethyl ketone-derived hydrazones to construct a series of acyclic all-carbon quaternary stereocenters with a high level of chiral induction. Two different chiral bidentate boryl ligands from a common chiral source not only can switch the enantioselectivity but also are complementary in terms of the substrate scope.

The formulation of recommendations is a crucial step in the development of clinical practice guidelines,which requires tools provided by the GRADE *** tools include the evidence quality assessment tool,the evidence summary tool,and the evidence-to-decision(EtD)*** there is a methodologically guided standard pathway for formulating recommendations in clinical practice guidelines,it remains unclear how to translate this theoretical pathway into the practical process for formulating recommendations and what the specific steps in this process ***,this paper takes the World Federation of Acupuncture-Moxibustion Societies Clinical Practice Guidelines on Acupuncture and Moxibustion:Allergic Rhinitis as an example to detail the full process of formulating recommendations and summaries it into a three-step *** one:Collect original materials from different sources to create EtD *** two:Distribute and collect EtD table,and preliminarily gather the recommendation voting *** three:Organize"salami-slicing pattern"guideline development group consensus meeting to formulate *** three-step method for formulating recommendations in clinical practice guidelines fully utilizes the GRADE EtD table to provide guideline development group with decision-making foundation and reference from multiple dimensions,including the importance of clinical questions,balance of benefits and harms,quality of evidence,resource required,values and preferences,health equity,acceptability,and *** approach enhances the scientific and practical nature of guideline *** paper provides a standard operating procedure to formulate recommendations for the incoming clinical practice guidelines,which aims to establish a standard pathway for the formulation of acupuncture clinical practice guideline recommendations.
