BACKGROUND:There is a constant and global shortage of deceased-donor organs for *** to identify areas for securing potential deceased-donor organs may improve the supply and hence benefit more patients in need of transplantation. METHODS:We looked into the disparity of the number of deceased-donor liver transplantation(DDLT)performed at our hospital on different days of the weeks from January 2000 to the end of December 2009(237 DDLTs).The number of DDLT performed on each day was compared with the other days of the week. RESULTS:It was apparent that there were fewer DDLTs on Mondays,as shown by the numbers of DDLT performed on different days of the week in an ascending order:Monday 18(7.6%),Sunday 30(12.7%),Thursday 34(14.3%),Friday 36(15.2%),Wednesday 38(16.0%),Tuesday 40(16.9%), and Saturday 41(17.3%).The difference reached statistical significance when Monday was compared with Tuesday (P=0.019),Wednesday(P=0.010),Friday(P=0.021),and Saturday(P=0.007).It was twice as unlikely a DDLT would be performed on Monday as compared with other *** a trend did not change even with an increase in the number of deceased-donor liver grafts in the last *** consent to donation was obtained from the donor family the day before DDLT,fewer consents were thus obtained on ***:These findings suggested that deceased-donor organ donation activities were less active on Sundays and could be *** further raises the concern of possible wastage of potential cases of organ donation.

Leptin, a protein hormone produced by the adipocytes, has long been recognized to regulate metabolism, neuroendorine and other physiological functions. Early findings of increased leptin production during infection and inflammation and dysregulated immune response in leptin signaling-deficient mice provide strong evidence for the involvement of leptin in the immune responses. Recent data have established the regulatory function for leptin in immunity similar to the function of a pro-inflammatory cytokine, while gene-targeting studies also demonstrated an essential role of leptin in regulating hematopoiesis and lymphopoiesis. Moreover, there has been increasing evidence that leptin is involved in the pathogenesis of various autoimmune diseases. This review discusses recent advances in understanding the role of leptin in immunity and leptin-signaling pathways involved in modulating immune homeostasis and autoimmune pathogenesis. BACKGROUND: Chemoembolization with doxorubucin-eluting beads (DEB) has been used to treat hepatocellular carcinoma (HCC) since 2007. This study compared the efifcacy and sur-vival between transarterial chemoembolization (TACE) with DEB and conventional approach (cTACE) in HCC treatment. METHODS: This retrospective case-control study compared the overall survival and tumor response of HCC patients to cTACE (n=190) and DEB (n=143) by the reassessment of com-puted tomography and serum alpha-fetoprotein (AFP). Mul-tivariate analysis was used to determine the factors affecting tumor response. RESULTS: The median post-treatment to pre-treatment AFP level was 0.8 for a DEB session (n=258) and 1.0 for a cTACE session (n=452), showing a signiifcantly greater decrease in AFP after DEB (P80 months after the ifrst TACE session in the DEB group. CONCLUSION: DEB is a safe alternative to cTACE in HCC pa-tients with better therapeutic efifcacy.

Association between the elderly frequent attender to the emergency department and 30-day mortality： A retrospective study over 10 years

BACKGROUND: To determine if elderly frequent attenders are associated with increased 30-day mortality, assess resource utilization by the elderly frequent attenders and identify associated characteristics that contribute to mortality. METHODS: Retrospective observational study of electronic clinical records of all emergency department(ED) visits over a 10-year period to an urban tertiary general hospital in Singapore. Patients aged 65 years and older, with 3 or more visits within a calendar year were identified. Outcomes measured include 30-day mortality, admission rate, admission diagnosis and duration spent at ED. Chi-square-tests were used to assess categorical factors and Student t-test was used to assess continuous variables on their association with being a frequent attender. Univariate and multivariate logistic regressions were conducted on all significant independent factors on to the outcome variable(30-day mortality), to determine factor independent odds ratios of being a frequent ***: 1.381 million attendance records were analyzed. Elderly patients accounted for 25.5% of all attendances, of which 31.3% are frequent attenders. Their 30-day mortality rate increased from 4.0% in the first visit, to 8.8% in the third visit, peaking at 10.2% in the sixth visit. Factors associated with mortality include patients with neoplasms, ambulance utilization, male gender and having attended the ED the previous ***: Elderly attenders have a higher 30-day mortality risk compared to the overall ED population, with mortality risk more marked for frequent attenders. This study illustrates the importance and need for interventions to address frequent ED visits by the elderly, especially in an aging society.

A high-performance concrete （HPC） is required to have superior performance in various aspects such as workability,strength, durability, dimensional stability, segregation stability, and passing ability. The mix design of HPC is rather complicatedbecause the number of ingredients in HPC is usually more than those in conventional concrete and some of the required propertiesare conflicting with each other in the sense that improvement in one property would at the same time cause impairment of anotherproperty. However, there is still lack of understanding regarding how the various mix parameters should be optimised forachieving best overall performance. Most practitioners are still conducting mix design primarily through trial concrete mixing,which is laborious, ineffective, and often unable to timely respond to fluctuations in the properties of raw materials. To addressthese issues, the authors have been developing the packing and film thickness theories of concrete materials, in order to revamp themix design philosophy of HPC in terms of the water film thickness （WFT）, paste film thickness （PFT）, and mortar film thickness（MFT） in the concrete. Based on the findings from an extensive experimental programme, suitable ranges ofWFT, PFT, and MFThave been recommended.
