Validity of QRS Configuration and Myeloperoxidase Level as Determinants of CAD Severity and Prognosis in Patients with STEMI

Background: Terminal QRS distortion and fragmentation (fQRS) with elevated myeloperoxidase (MPO) were linked to poor cardiovascular outcomes in acute coronary syndrome. We aimed to investigate these parameters in early prediction of coronary artery disease severity based on SYNTAX score and in-hospital adverse events in STEMI patients. Methods: A total of 215 patients with first STEMI admitted for primary PCI were included in the study. They were divided according to the admission ECG into group I with QRS distortion or fQRS, group II with combined QRS distortion and fQRS, and group III without QRS distortion or fQRS. Myeloperoxidase and troponin I levels, ST resolution ratio, left ventricular EF%, and severity of coronary artery lesions using SYNTAX risk score were measured. Results: MPO level, SYNTAX score, and in-hospital mortality were higher in group I and II and were higher in group II compared to group I. By regression analysis, QRS distortion, fQRS, and MPO > 412 ng/ml were independent predictors of both CAD severity and in-hospital mortality. DM was an independent predictor of CAD severity (OR: 2.851, P 0.012) while high SYNTAX score was an independent predictor of in-hospital mortality (OR: 6.113, P 0.001). Adding MPO level to any QRS configuration pattern increased predictive value for the detection of CAD severity that was more evident in the combined QRS distortion and fragmentation. Conclusion: Terminal QRS distortion, fragmentation, or combined QRS distortion and fragmentation have a significant value in predicting in-hospital adverse events and CAD severity

The Hidden Value of Assessing Right Ventricular Performance with Exercise in Hypertensive Patients with Left Ventricular Diastolic Dysfunction

Background: Right ventricular (RV) dysfunction could develop during exercise in patients with both hypertension and left ventricular diastolic dysfunction and may contribute to the patient symptoms. The objective is to assess RV function, both at rest and during exercise in patients with hypertension and left ventricular diastolic dysfunction. Methods: We included 30 patients with hypertension and resting LV diastolic dysfunction. The systolic function of the right ventricle was assessed by TAPSE (Tricuspid Annular Plane Systolic Excursion) and S, while E/A ratio, annular lateral E’, E/E’ and E’/A’ were used to measure diastolic function. The global function of the right ventricle was assessed by measuring the right indexed myocardial performance. The dimensions and pulmonary pressures were also measured. Results: The following parameters of RV systolic function were increased significantly with exercise: TAPSE (P = 0.0054), S’ (P = 0.0045). Health Risk Assessment of Some Heavy Metals in Water and Sediment at Marsa-Matrouh, Mediterranean Sea, Egypt

In an attempt to evaluate the environmental quality of Marsa-Matrouh city which covered the most famous beaches in Egypt, an environmental risk assessment was performed, including a screening level ecological risk assessment. The aim of this work was to determine which metals could possibly pose toxic adverse ecological effects to marine organisms and to determine whether hot spots exist or not. To fulfill the goals of study, surfacial sediment and water samples were collected from ten different locations covering Marsa-Matrouh city during four seasons (2010-2011). The average concentrations of Cd, Cu, Fe, Ni, Pb and Zn were 2.381 ± 3.389, 9.307 ± 14.159, 68.969 ± 9.397, 2.642 ± 1.004, 16.712 ± 8.469, 31.168 ± 15.322 μg/l in water and 0.755 ± 0.240, 5.363 ± 1.581, 962.131 ± 975.084, 3.972 ± 2.180, 15.210 ± 4.434 and 24.608 ± 7.706 μg/g dry weight in sediment respectively. The concentrations of the investigated six metals in water were within the acceptable limits except for two stations (Cleopatra and El-Obayed) which exhibited higher values than the permissible level of Cd during autumn 2010. Metal pollution assessment for both water and sediment was studied. Heavy metal pollution index indicates that water is not critically polluted with respect to the investigated metals. For sediment samples, threshold effect concentrations (TEC HQ) were lower than 1 except for Cd which showed higher value than 1, indicating the possibility of occurrence of toxic adverse ecological effects to benthic organisms for Cd, while rare adverse ecological effects are expected to occur with respect to Cu, Ni, Pb and Zn.

Genotoxic and Histopathological Effects of Water Pollution on <i>Clarias gariepinus</i>Fish at Fayoum Governorate, Egypt

Increasing land reclamation in El-Fayoum governorate has led to increase the amount of drainage water that discharged without prior treatments to two main drains, El-Wadi and El-Bats drainages loaded by salts and heavy metals. So, the present investigation aims to evaluate quality of water samples collected from a branch of the river Nile (El-Lahon canal) and the main drains (El-Bats and El-Wadi) at Fayoum governorate on some biomarkers of the Nile catfish;Clarias gariepinus inhabiting the studied degradable aquatic habitats. Water and nighty fish samples colleted from the different studied sites of collection by the help of fishermen for physicochemical analyes of water and fish biochemical analyses and histopathological alteration. Results revealed highly significant differences at P ≤ 0.01 with the highest water ammonia, nitrite, heavy metals (Cu, Zn, Pb and Cd) and lowest dissolved oxygen content accompanied by metals bioaccumulation in vital organs of fish collected from the main drains (El-Bats and El-Wadi) in comparision with that collected from the studied branch of the river Nile. Higher bioaccumulation of the studied heavy metals in gills, liver, kidney and muscles of Clarias gariepinus collected from the main drains significant increase in serum glucose, liver and kidney functions, disturbance in protein profile of fish collected from the studied main drains than that of fish collected from the river Nile branch, El-Lahon that showed more or less normal values. Influence of Sn Low Doping on the Morphological, Structural and Optical Properties of ZnO Films Deposited by Sol Gel Dip-Coating

In this work, Undoped Zinc Oxide (ZnO) and Sndoped Zinc Oxide (ZnO:Sn) films have been deposited by sol-gel dip coating method, where the Sn/Zn atomic ratio was 3% and 5% in the solution. The effects of Sn incorporation on morphological, structural and optical properties of ZnO films were investigated. The Scanning Electron Microscopy (SEM) showed that the morphological surface of the films was affected by Sn low doping. The X-Ray Diffraction (XRD) patterns showed that all films have polycrystalline structures, and the doping incorporation has not lead to substantial changes in the structural characteristics of ZnO films. The crystallite size was calculated using the well-known Scherrer’s formula and found to be in the range of 23 - 40 nm. The measurements from UV-Visible Spectrophotometer (U-Vis) indicated that the highest average optical transmittance in the visible region was related to Undoped ZnO film, then the optical band gap and Urbach energy values of thin films were calculated. The X-Ray Photoelectron Spectroscopy (XPS) has demonstrated that Sn is incorporated in ZnO lattice.

Effect of closed circuits drip irrigation system and lateral lines length on growth, yield, quality and water use efficiency of soybean crop

Field experiment was conducted for one growing season (2012) in clay loam soil at the Experimental Farm of Faculty of Agriculture, Southern Illinois University at Carbondale (SIUC), USA on soybean crop to study the effect of: 1) the closed drip irrigation system: closed circuits with one and two a manifolds for lateral lines (CM1DIS;CM2DIS) and traditional drip irrigation system (TDIS) as a control;and 2) lateral lines length (LLL): LLL1, LLL2 and LLL3 (40, 60,80 m) on soybean growth, yield, oil, protein content and water use efficiency. Plants were drip irrigated every 4 days. N, K2O and P2O5 fertilizers were added via irrigation water. Data obtained could be outlined as follows: 1) According to the mean values of soybean crop growth (leaf area;plant height), yield (grain and straw), both oil and protein content and water use efficiency, the treatment used could be ranked in the following ascending orders: TDIS < CM1DIS < CM2DIS and LLL3 < LLL2 < LLL1;2) Differences in the means of the studied data among treatments used were significant at the 1% level;3) The effects of the DIC × LLL on the data obtained were significant at the 1 % level;And 4) the highest values of the obtained data and the lowest ones were achieved in the following interactions: CM2DIS × LLL1;TDIS × LLL3, respectively.

Effect of drip irrigation method, nitrogen source, and flushing schedule on emitter clogging

Field experiments were carried out at the National Research Center farm, Nubaria area, Behura Governorate, Egypt, to study the effect of nitrogen source, flushing schedule and irrigation method on emitter clogging. Peanut Giza 5 variety (Arachishy pogaea L.) was planted in sandy soil during two successive growing seasons (2010-2011) in the 1st week of May and harvested after 130 days. Treatments used are: 1) two irrigation methods: surface drip irrigation and sub-surface drip irrigation (SDI;SSDI), 2) nitrogen source (NS):NH4NO3, (NH4)2SO4 and Ca(NO3)2 (NS1, NS2 and NS3) and 3) flushing number (FL) 0, 1 and 4 (FL1, FL2;FL3). The experiments design was split-split plot and three replicates were used. Data obtained were subjected to statistical analysis. The main effects of treatments used on clogging per cent could be written the following ascending orders: SDI 3 2 1, NS1 2 3. Concerning the 1st interaction the following ascending orders denote their effects on clogging percent: SDI × FL3 2 1, SDI × NS1 2 3, SSDI × FL3 2 1, SSDI × NS1 2 3, FL1 × NS1 1 × NS2 1 × NS3, FL2 × NS1 2 × NS2 2 × NS3 and FL3 × NS1 3 × NS2 3 × NS3. The differences between any two treatments and/or any two interactions in clogging percent were significant at the 5% level. The effect of the 2nd interaction on clogging percent was significant at the 5% level. The maximum value of clogging (20.18%) and the lowest one (3.9%) were archived in the interactions: SSDI × FL1 × NS3 and SDI × FL3 × NS1, respectively.

Serum Bilirubin as a Predictor of Coronary Artery Disease Severity in Patients Undergoing Primary Percutaneous Coronary Intervention

Introduction: Coronary artery disease (CAD) is one of the most common causes of death worldwide. In 2010, about 7 out of total 53 million deaths were due to ischemic heart disease. The aim of this study is to evaluate the relationship of serum bilirubin level with the severity and complexity of coronary artery disease (CAD) in the patients undergoing primary percutaneous coronary intervention (PCI). Materials and Methods: 70 patients with STEMI who were undergoing primary PCI were included in the study. All the patients included in the study were subjected to full routine investigations and standard coronary angiographic projections. Total bilirubin level was measured and the patients were divided into two groups. Group 1 was with serum TB (and Group 2 was with serum TB (>1 mg/dl). Severity and complexity of coronary artery lesions will be assessed using Gensini score. Results: After PCI, the two studied groups were compared regarding the number of vessels affected by one and more than one vessel disease. Single vessel disease was frequent in Group 2 (71%) with significant p value (0.003). Cardiac enzymes (troponin I) was more in Group 1 (S. Bil value (0.02). Also (ALT, AST) were more in Group 1 (p value = 0.01). By comparing the 2 groups, there was a significant difference regarding (EF) between both which was less in Group 1 (*** 1), p value significance (0, 0001). Also GENSENI was more in Group 1 (*** 1) with mean (80.35 vs 34.71) and significant pvalue (0.0001). There was a highly significant negative correlation between serum bilirubin & GENSENI score (r = -0.762, p value 0.0001). Regarding the incidence of complications, incidence was more in Group 1 (*** 1), which means a significant difference between both groups with significant p value (0.0001). There was a significant negative correlation between serum bilirubin & incidence of complications (R = -0.38, pvalue 0.001). Vaginal Trachelectomy for Retained Cervical Stump after Supracervical Hysterectomy: Technical Tips and Outcomes

Background and Objectives: The supracervical hysterectomy (SCH) has resulted in a group of patients with a retained cervix at risk of persistent symptoms, who may require a trachelectomy in the future. This study was to evaluate the efficacy of vaginal trachelectomy (VT) after a previous SCH. Methods: This was a prospective study that includes 13 cases with different ages and different complaints, sharing the same primary operation supracervical hysterectomy. They have different pathologies of the SCH specimen but they share the same completion surgery. The surgical outcome was analyzed. Results: Thirteen patients underwent vaginal trachelectomy for recurrent symptoms. The ages of patients were ranged from 37 years to 68 years (Mean ± SD, 56.4 ± 10.7). SCH was most commonly performed for abnormal uterine bleeding AUB (7/13, 53.8%), pelvic mass (5/1, 38.5%), and pelvic pain (1/13, 7.7%), the symptoms leading to vaginal trachelectomy were the same as those leading to supracervical hysterectomy. The median interval time from SCH to seeking medical help for the persistence or recurrence of symptoms and to VT was 2 weeks (1 to 96 weeks). Concomitant procedures were laparoscopic removal of both ovaries in 2 cases and pelvic lymphadenectomy in 1 case. The median length of operation was 45 minutes. In all cases, symptoms leading to trachelectomy resolved completely after surgery, and patients reported a significant improvement. Conclusions: The cervix, left behind at subtotal hysterectomy, requires removal, the vaginal route is probably the safest, and least traumatic. Vaginal radical trachelectomy appears to be feasible and safe for the treatment of endometrial malignancy discovered after supracervical hysterectomy.

2-(4-Oxo-4,5-dihydro-thiazol-2-yl) acetamide compound (1), was condensed with aromatic aldehydes either (1:1 molar ratio) or (1:2 molar ratio), and gave the newly 4,5-dihydro- 4-oxo-thiazole derivatives (2a-d), and (3a-c), respectively. Compounds (4a-d) were obtained via cyclocondensation of compound (1), malononitrile, and aromatic aldehydes (1:1:2 molar ratio). Cyclization of compound (2a) with α- substituted cinnamonitriles (6a-d), and (10a-c) yielded the newly thiazolo [3,2-a] pyridine derivatives (9a-d), and (13a-c), respectively.
