Introduction: Postoperative nausea and vomiting (PONV) are prevalent symptoms after laparoscopic surgeries with an incidence rate of (54% - 79%) in bariatric procedures. Despite its popularity, limited studies assessed the effect of antiemetics for PONV prophylaxis after laparoscopic sleeve gastrectomy (LSG). The aim of this trail is to compare the effectiveness of a single pre-induction intravenous dose of Palonosetron versus Ondansetron for prophylaxis of PONV, 24 hours after LSG. Subjects and Methods: This prospective randomized controlled double-blind parallel-group study was conducted from May till December 2019. Recruited patients were consented and randomized using a closed envelop method into two groups with fifty patients each. The total number of nausea and vomiting attacks in the 24 hours postoperatively was considered as a primary end point. The secondary end points were the frequency of nausea, retching and vomiting attacks in the 24 hours post-surgery. The severity of nausea was evaluated using a 10 cm visual analogue scale (VAS). Geophysical Evaluation for Evidence of Recharging the Pleistocene Aquifer at El-Nubariya Area, West Nile Delta, Egypt

With ever increasing demands on water resources, Egypt is facing increasing water needs, demanded by a rapidly growing population, by higher standards of living and by an agricultural policys. West Nile Delta represents a new agricultural area where the reclaimed lands are irrigated with groundwater in addition to surface water. The evaluation of groundwater aquifers in West Nile Delta area requires information about the extension, depth and distribution and source of recharging these aquifers. In this work, an integrated study using geoelectric, geomagnetic, hydrogeologic and hydrochemistry investigations has been applied to evaluate groundwater aquifers and to delineate subsurface structures predominant in the basement rocks at El-Nubariya area, West Nile Delta, Egypt. Thirty-five Vertical Electrical Soundings (VES) have been applied to identify the subsurface geoelctric layers predominant in the study area. As well as a detailed land magnetic survey has been carried out along the study area using two automatic proton precession magnetometers with an accuracy of 1 nT. These surveys allowed us to establish the subsurface structures and their relation with the detected aquifers. The geoelectric results are illustrated along seven geoelectric profiles and correlated with the lithologic and hydrogeologic data of drilled wells. The third geoelectric layer represents the water saturated zone (aquifer) with thickness varies from 14 m to 58 m, increasing to east and southeast directions. The hydrchemisty anlysis shows that the salinity of the water aquifer ranges from 600 ppm in the southeast to 3900 ppm in the south. The major dominant dissolved salt is Nacl. In the east and southeast, the water type becomes more NaSO4 which refers to dissolution of gypsum in addition to fertilizers. Background: Non-alcoholic fatty liver disease (NAFLD) has emerged a major challenge and become the leading indication for liver transplantation. We aimed to assess the applicability and performance of real-time elastography (RTE) in diagnosis of liver fibrosis in patients with NAFLD compared with NAFLD fibrosis score (NFS) and FIB-4 index. Patients and Methods: A prospective case-control study was conducted on 260 subjects attended Hepatology, Gastroenterology and Infectious diseases and Internal Medicine departments in Benha University Hospital from Marsh 20, 2018, to September 1, 2019 and divided into group I included 200 cases with NAFLD and group II included 60 healthy control subjects. Results: There was statistically significant increase in FIB-4 scores between two groups (1.39 ± 1.02 and -0.75 ± 0.32 respectively with p Conclusion: Real time elastography could be valuable in diagnosis of fibrosis in NAFLD especially in cases more than F3 score.

Anticipation of Dural Tear in Patients with Blunt Head Trauma

Background: Dural tears are considered among the neurosurgical challenges to be dealt with during management of head trauma patients;it’s important to anticipate such pathology pre-operatively;the purpose of this study is to discuss and analyze various predictors of dural tear as a sequala of blunt head trauma. Methods: Patients with blunt head trauma who underwent surgery during the year 2020 were analyzed;operative data were reviewed;only patients with reported dural repair as a step during surgery were included. Patients with penetrating head injury were excluded. Preoperative radiology, epidemiological and clinical details were analyzed to conclude specific criteria for dural tear. Results: Twenty-three patients were involved in the study;male predominance was evident. The most common primary injury was depressed fracture (73.9%), while the most common mode of trauma was domestic injury (43.5%), especially in pediatric group (76.9%). Two cases of growing skull fracture were involved, and both were in pediatric group. CT findings were conclusive for immediate dural tear findings, however, suggestive for patients with growing skull fractures. Mean depth of depression for depressed fracture group was 15.07 mm. Conclusion: Dural tears are not a serious complication of head trauma, however, their sequalae are life-threatening. Anticipation of dural tear is feasible radiologically pre-operatively for immediate dural repair and craniotomy fashioning;for cases of growing skull fractures, it’s difficult to anticipate the course from time of impact till time of presentation.

Anterolateral Approach for Unstable Lumbar Burst Fracture with Anterior Compression

Background: Lumbar burst fractures are common spinal injuries that cause severe instability with kyphotic deformities and neurological complications requiring surgical decompression and reconstruction with spinal instrumentation for unstable burst fracture, but there is controversy about the optimal surgical approach anterior, posterior or combined approach. Objectives: To assess the efficacy & safety of anterolateral approach in decompression and reconstruction with spinal instrumentation for lumbar burst fractures. Subjects & Methods: A retrospective study including 16 patients, 10 males and 6 females with lumbar burst fractures and anterior compression treated operatively by anterolateral approach for corpectomy and single level fusion by using expandable cage or mesh cage loaded with bone graft and plat with screws. The clinical and radiological follow up after discharge from the hospital ranged from 12 to 24 months. Results: All patients improved regarding the Frankel score more than one grade after surgery, except 2 cases of grade A didn’t improve. Mean preoperative visual analogue scale was 7.4 improving to 0.9 postoperatively. The mean Local kyphosis improved from 8.8° before surgery to -1° after surgery. Conclusions: Anterolateral approach is feasible, effective, and safe approach for unstable lumbar burst fractures. Angular deformity is successfully corrected when the anterior approach is used.

BACKGROUND The resulting tissue hypoxia and increased inflammation secondary to severe coronavirus disease 2019(COVID-19)combined with viral load,and other baseline risk factors contribute to an increased risk of severe sepsis or co-existed septic condition *** To describe the clinical,radiological,and laboratory characteristics of a small cohort of patients infected by severe acute respiratory syndrome coronavirus 2 who underwent percutaneous drainage for septic complications and their postprocedural *** This retrospective study consisted of 11 patients who were confirmed to have COVID-19 by RT-PCR test and required drain placement for septic *** mean age±SD of the patients was 48.5±14 years(range 30-72 years).Three patients underwent cholecystostomy for acute acalculous *** drainage was performed in seven patients;two peripancreatic collections;two infected leaks after hepatic resection;one recurrent hepatic abscess,one psoas abscess and one lumbar *** patient underwent a percutaneous nephrostomy for acute *** Technical success was achieved in 100%of patients,while clinical success was achieved in 4 out of 11 patients(36.3%).Six patients(54.5%)died despite proper percutaneous drainage and adequate antibiotic *** patient(9%)needed operative *** patients(18.2%)had two drainage procedures to drain multiple fluid *** patients(18.2%)had repeat drainage procedures due to recurrent fluid *** average volume of the drained fluid immediately after tube insertion was 85 ***-up scans show a reduction of the retained content and associated inflammatory changes after tube insertion in all *** was no significant statistical difference(P=0.6 and 0.4)between the mean of WBCs and neutrophils count before drainage and seven days after *** lymphocyte count shows significant increased seven days after drainage(P=0.03).CONCLUS

Purpose: The aim of this study is early detection of increased cup to disc ratio and a survey study of cup-disc ratio in Egyptian children age group less than 15 years. Methods: Three hundred and eighty-two (382) children were enrolled in this study. They were classified into three age groups;infants’ Group (1) from 0-2 years, preschool Group (2) from 2-6 years and school children Group (3) from 6-15 years old. C/D ratio was measured for each child. Results: In Group (1), normal C/D ratio ranged from 0 to 0.1. The C/D ratio > 0.3 or a binocular difference of >0.2 could be regarded as one of the criteria of newborn glaucoma. Group (2): the normal ratio ranged from 0.1-0.2. Group (3): the normal ratio ranged from 0.2-0.3. The highest ratio 0.7 was found in two eyes. An asymmetry of 0.2 or more in the C/D ratios of the eyes was found in 4.8% of the children. Conclusion: C/D ratio > 0.3 and asymmetry of 0.2 is suggestive of glaucoma especially in infants of lower age group.

Volumetric Assessment of Corticospinal Tract Infiltration by Astrocytoma Using Diffusion Tensor Tractography

Objectives: The aim of the study is to assess volumetric Corticospinal Tract infiltration by Astro-cytoma using Diffusion Tensor Tractography. Material and Methods: Preoperative, anatomic (T1- and T2-weighted) and diffusion tensor MRI were performed in 9 patients with supratentorial gliomas (WHO grade II and III). The tumors were manually segmented from the T1- and T2-weighted MRI, and their volume calculated. A three-dimensional tractography was performed in each case. A second segmentation and volume measurement was performed on the tumor regions intersecting adjacent white matter fiber tracts. Results: We identified that white matter tracts were infiltrated by 6 Astrocytoma tumors. The median tumor volume (±standard deviation) in our patient population was 33 ± 26.82 ml. The median tumor volume (±standard deviation) infiltrating white matter fiber tracts was 4.15 ± 9.23 ml. The median fraction of tumor volume infiltrating white matter fiber tracts was 26.3% ± 10.1%. Conclusions: Diffusion tensor MR Tractography is a reliable preoperative assessment tool since it detects extensive white matter infiltration by Astrocytoma irrespective of brain tumors volume. Recommendations: Prospective large population studies are required to clarify how infiltration relates to tumor location.
