Pattern of Failure and Treatm.nt Results in Triple Negative Breast Cancer Patients

Background: Triple negative breast cancer (TNBC) tends to present aggressively with rapid progression and poor survival. m.thods: We retrospectively reviewed patients’ files to define TNBC patients’ characteristics, predictive and prognostic factors, pattern of recurrence and survival. Results: 965 cases were identified. 147 patients (15.2%) were TNBC. 71.1% patients were prem.nopausal. T2, T3, T4 tum.rs represented 46.1%, 32% and 14.1%, respectively. N0, N1, N2, N3 disease represented 18.5%, 50.9%, 27.8% and 2.8%, respectively. Stages II, III & IV constituted 34.1%, 44.2% and 15.5%, respectively. 31.5% patients received neoadjuvant chem.therapy with 17.7% com.lete pathological response. 19.5%, 35.9%, 44.6% patients had unknown, ≤20 and >20 Ki67, respectively. Am.ng non-m.tastatic patients (n = 108), 21.3% patients developed relapse with m.dian tim. to relapse of 11 m.nths. 78.3% of them.had visceral (88.3% lung) m.tastasis, 13% bone m.tastasis, 21.7% brain m.tastasis and 13% LRR. There is significantly high risk of relapse in patients with large tum.r size [T4: 66.75%, T3: 22.9%, T2: 16.7%, T1: 0% (p = 0.002)], positive LNs [N3: 100%, N2: 37.9%, N1: 15.1%, N0: 4.3% (p 0.001)] and Ki67 [>20: 31.6% versus 10.8% for Ki67 ≤ 20 (P = 0.007)]. m.ltivariate analysis revealed only T4 and N2-3 were significantly associated with high probability for relapse (P = 0.022 & 0.038). The 3-year DFS and OS were 73.2% and 75% respectively. For m.tastatic patients (n = 20), the m.PFS was 7 m.nths and m.OS 1.5 years. Conclusion: Our data confirm. the aggressive nature of TNBC with significant risk of relapse for patients with large tum.r and positive lym.h nodes. m.intenance m.tronom.c capecitabine, neoadjuvant/adjuvant im.unotherapy could be beneficial for non-m.tastatic patients. In nuclear power plants, steam.generators are im.ortant parts. This paper introduces a U-tube steam.generator (UTSG) m.del im.lem.nted com.letely using m.TLAB environm.nt. The UTSG consists of four regions: upward and downward prim.ry regions, upward and downward m.tal tube regions, and secondary regions, which contain heat transfer region, steam.separation region, and subcooled water region. Governing equations are derived by applying energy and m.ss conservation equations in all regions. Accurate functions that describe the relationships between therm.dynam.c properties of the saturated steam.are introduced instead of interpolation m.thod that is widely used. Steady state and one transient case are presented as well.

Restrictive versus Liberal Blood Transfusion Strategies in Egyptian Patients with Esophageal Variceal Bleeding

Background and Study Aim. Esophageal variceal bleeding is a m.jor m.dical em.rgency and one of the m.st im.ortant indications for hospital adm.ssion and for blood transfusion. However, the safest and the effective blood transfusion strategy is controversial. Here, we studied the safety and the effectiveness of the restrictive versus liberal transfusion strategies in patients with esophageal variceal bleeding. Patients and m.thods: The study included 342 patients with esophageal variceal bleeding. Patients were divided into 2 groups: group I (Restrictive strategy) transfusion when the hem.globin level is ≤7 g/dl and group II (Liberal strategy): transfusion when the hem.globin level is ≤9 g/dl. All patients were subjected to com.lete blood counts, liver and kidney profiles, coagulation profile, pelvi-abdom.nal ultrasonography and upper GI endoscopy. Clinical outcom. m.asures include rebleeding, infection, allergic transfusion reactions throm.oem.olic events, and m.rtality. Results: Of all patients adm.tted to hospital with esophageal variceal bleeding, the num.er of transfused RBCs units and hospital stay were m.re in the liberal transfusion strategy. Also, the overall rate of com.lications was higher in the liberal transfusion strategy (49.7% versus 38.5% in the restrictive transfusion strategy). The m.st com.on com.lications were rebleeding (26.9%) and infection (21.6%). As regard the death rate, 13 cases (7.6%) died in the restrictive transfusion strategy versus 25 cases (14.6%) in the liberal transfusion one. Conclusions: For esophageal variceal bleeding, restrictive transfusion strategy is better than the liberal one as regard cost-effectiveness, risk of com.lications and hospital stay with no harm.and less m.rtality as com.ared to liberal strategy.

Welding is a vital com.onent of several industries such as autom.tive,aerospace,robotics,and *** welding,these industries utilize alum.num.alloys for the m.nufacturing of m.ny com.onents or ***,fusion welding of alum.num.alloys is challenging due to several factors,including the presence of non-heat-treatable alloys,porosity,solidification,and liquation of *** m.nufacturers adopt conventional in-air friction stir welding(FSW)to weld m.tallic alloys and dissim.lar *** researchers reported the drawbacks of this traditional in-air FSW technique in welding m.tallic and polym.ric m.terials in general and alum.num.alloys and alum.num.m.trix com.osites in specific.A num.er of FSW techniques were developed recently,such as underwater friction stir welding(UFSW),vibrational friction-stir welding(VFSW),and others,for welding of alum.num.alloy joints to overcom. the issues of welding using conventional ***,the m.in objective of this review is to sum.arize the recent trends in FSW process of alum.num.alloys and alum.num.m.tal m.trix com.osites(Al m.Cs).Also,it discusses the effect of welding param.ters of the traditional and state-of-the-art developed FSW techniques on the welding quality and strength of alum.num.alloys and Al *** am.ng the techniques and advantages and lim.tations of each are *** review suggests that VFSW is a viable option for welding alum.num.joints due to its energy efficiency,econom.c cost,and versatile m.difications that can be em.loyed based on the *** review also illustrated that significantly less attention has been paid to FSW of Al-m.Cs and considerable attention is dem.nded to produce qualified joint.

Petrological and Geochem.cal Constraints on the Evolution of EI-Kahfa Alkaline Ring Com.lex, South Eastern Desert, Egypt

El-Kahfa com.lex locates at the intersection of Lat. 24°8′18″ N and Long. 34038′55″ E, South Eastern Desert of Egypt. It com.rises an outer ring com.osed of alkaline syenites, ranging from.barrento quartz-bearing syenites, an inner ring which varies in com.osition from.leucocratic to m.lanocratic syenites, and a central stock form.d essentially of alkali gabbros. These alkali gabbros are cut by sm.ll bodies of alkaline syenites, which occasionally grade into nepheline syenites. The country rocks are represented m.inly by m.tavolcanics and epidiorites. El-Kahfa syenites lack any prim.ry hydrous m.fic silicates indicating that their m.gm. was anhydrous. m.jor and trace elem.nts geochem.stry of EI-Kahfa rocks reflects the crystallization of early plagioclase followed by Fe-Ti oxides. Salic m.m.ers in the com.lex are co-m.gm.tic and related to crystal liquid fractionation. The REE （rare earth elem.nts）＇s tetrad is obvious on the prim.tive m.ntle norm.lized pattern of El-Kahfa rocks. The m.type tetrads clearly appear in the m.re evolved rocks. These tetrad effects are either a feature of m.gm.-fluid system.before crystallization or inherited from.external fluid during or after the em.lacem.nt of the m.gm.. Generally, El-Kahfa rocks parental m.gm. was probably derived from.Nb-enriched or at least under-plate m.ntle source within the continental East-Africa rift zone, where it m.y be subjected to m.nor contam.nation through their fractional crystallization.

Synchronous Chem.radiotherapy in Patients with Stage III and IV Head and Neck Cancer: Com.aring Cisplatin with Capecitabine

Purpose: To evaluate the efficacy of concurrent intravenous cisplatin versus oral capecitabine with radical radiotherapy in locally advanced squam.us cell carcinom. of the head and neck. m.terials and m.thods: Between January 2007 and Decem.er 2009, 60 patients with stage III/IV head and neck squam.us cell carcinom. (0 to 1 perform.nce status) were enrolled into this study. Thirty cases are given cisplatin 30 m./m. IV infusion weekly for 6 weeks with conventional radiotherapy. The rem.ining thirty cases are given oral capecitabine 500 m./m. twice daily, continuously for 28 - 35 days with conventional radiotherapy also. The radiotherapy dose was 4600 cGy in 20 fractions over 4 weeks to prim.ry and neck nodes followed by boost to prim.ry site and any residual disease 1500 - 2000 cGy in 6 to 8 fractions. Results: The m.dian age was 53 (range 25 - 71) years;10 cases had stage III disease, 36 cases IVa disease and 14 cases IVb disease. Seventy-three percent of patients com.leted the course of capecitabine and 80% com.leted prescribed cisplatin. There were no treatm.nt-related deaths, grade 4 haem.tological toxicity or grade 3 renal toxicity in either arm. The com.lete response rate at 3 m.nths was 77% (23/30 patients) in the capecitabine group and 60% (18/30) in the cisplatin group. Relapse occurred in 10/30 (33%) patients by 2 years in the capecitabine group and in 12/30 (40%) in the cisplatin group. On analysis of survival data, the m.dian follow-up period was 35 ± 15 m.nths for overall survival and 33 ± 10 m.nths for disease free survival. The overall survival, and disease-free survival rates at 2 years were 67%, and 85%, respectively for the capecitabine group versus 60% and 73% for the cisplatin group. Conclusion: Synchronous chem.-radiotherapy with capecitabine was found to be very effective, with excellent response, local control and 3-year cancer-specific survival rates.

Effect of m.tform.n on Endom.trial Thickness and Subendom.trial Flow Patterns in Anovulatory Patients with Polycystic Ovarian Syndrom.

Background: Polycystic ovarian syndrom. (PCOS) is considered the com.onest endocrinological disorder affecting reproductive aged wom.n. PCOS com.rom.ses fertility through various pathways. These pathways include hyperandrogenism. insulin resistance and im.edance of the uterine and endom.trial blood flow. m.tform.n im.roves the blood flow to the endom.trium. It acts by reducing androgen level and correction of insulin resistance. Endom.trial vascular indices were evaluated in this study to evaluate endom.trial receptivity in anovulatory patients with PCOS. Aim.of the Work: To evaluate the outcom. of m.tform.n adm.nistration in anovulatory patients with PCOS and its effect on the endom.trium. This included its role in ovulation and im.rovem.nt of pregnancy rates. Patients and m.thods: This study included 85 patients from.Ain Sham. University outpatient infertility clinics from.1st of January, 2018 till 30th of June, 2018. We investigated these patients before treatm.nt with ultrasound on day 14, 21. We evaluated endom.trial thickness, uterine artery flow pattern, endom.trial and subendom.trial flow patterns. The patients received m.tform.n 500 m. three tim.s per day for three m.nths. After this duration, we reevaluated them.by ultrasound at days 14, 21. Results: m.tform.n therapy resulted in a significant increase of endom.trial thickness and had a significant decrease on uterine, endom.trial and subendom.trial resistance index (R.I) and pulsatility index (P.I) at day 14, 21 com.ared to pre-treatm.nt values indicating better blood flow. Conclusion: m.tform.n therapy resulted in im.rovem.nt of endom.trial flow patterns. Also, it resulted in increase in endom.trial thickness and im.rovem.nt of uterine artery flow.

Background: Previous research has dem.nstrated that specific radiographic criteria, including the presence of calcifications and the enhancem.nt pattern on com.uted tom.graphy (CT) im.ging, correlates with clinicopathologic features and outcom.s of patients with gastroenteropancreatic neuroendocrine tum.rs (NET). We sought to investigate whether these radiographic characteristics were prognostic am.ng patients with neuroendocrine liver m.tastases (NELm. undergoing surgical resection. m.thods: The preoperative contrast-enhanced CT scans of all patients who underwent resection of NELm.at a single institution between 2000-2015 were retrospectively reviewed. The presence of calcifications was determ.ned on non-contrast phase im.ging. Enhancem.nt on the arterial phase scan was categorized as hyperenhancing, hypoenhancing, or m.xed. Relevant clinicopathologic characteristics as well as recurrence-free survival (RFS) and overall survival (OS) were com.ared between groups. Results: Am.ng 82 patients who underwent resection of NELm. 57 had available data on calcifications while 51 had data available on arterial enhancem.nt patterns. Am.ng all patients, m.dian age was 58 (IQR:47-63) and the m.jority were fem.le (N=48, 59.5%). The m.st com.on prim.ry tum.r locations were pancreas (N=25, 30.5%) and sm.ll bowel (N=27, 32.9%). The m.st com.only perform.d operations were right hepatectom. (N=29, 35.4%), bisegm.ntectom. (N=15, 18.3%), and segm.ntectom. (N=14, 17.1%). m.dian tum.r num.er was 4 (IQR: 2-9), m.dian Ki-67 was 5% (IQR: 2-10%), and m.dian size of the largest liver m.tastasis was 4.5 (IQR: 2.8-7.7) cm. Twelve (21%) patients had tum.r calcifications. Am.ng patients with and without calcifications there were no differences in dem.graphics, clinicopathologic characteristics, RFS (P=0.772) or OS (P=0.095). Arterial enhancem.nt was hypoenhancing in 23 (45.1%), hyperenhancing in 10 (19.6%), and m.xed in 18 (35.3%). Sim.larly, there were no differences between arterial enhancem.n

Power Source for Wireless Sensors in Pipes

In this paper, we present investigations on energy harvesters for wireless sensors inside pipes. The harvesters are of flexible piezoelectric PVDF （Poly-Vinylidene-Di-Fluoride） and alum.num.foils as electrodes. The layers were stacked alternating on each other and wound to a spool. An LDPE （low-density polyethylene）-film.wraps the spool and prevents the inflow of liquids. A ring shaped bluff body was placed inside the pipe to induce turbulence in the fluid stream. As the harvesters have been arranged downstream.of the bluffbody, they were forced to oscillate independent of the m.dia. This led to a polarization and a separation of electrical charges. Experim.nts were carried out in a wind channel as well as in a water pipe. In air, the spool oscillates with a frequency of about 30 Hz, at a wind speed of about 7 m.s. A voltage of about 4 V （peak-peak） was m.asured. This delivers in case of im.edance adjustm.nt power values of about 0.54 p.W. In water, oscillation starts at a speed above 0.6 m.s. The average oscillation frequency is about 18 Hz. At a velocity of 0.74 m.s, a peak-peak-voltage up to about 2.3 V was found. In case of im.edance adjustm.nt, the power was about 0.33 μW. This power is stored in a capacitor. Assum.ng a data transm.ssion unit consum.s about 0.2 m.s during one operational period of I s, the duty cycle can be calculated to about 6.2 m.n for air harvesting and 10.1 m.n for harvesting in water.

E-shaped wideband plasm.nic nantennas with linear and dual-linear polarizations

In this paper we present a novel nanoantenna(nantenna) design for energy harvesting. The nantenna has an 'E'shape and is placed on a Si O2 substrate. Its operation is based on the excitation of surface plasm.n polaritons through the gold arm. of the E shape. By varying the lengths and widths of the arm., two overlapping working bandwidths can be achieved. This results in a wideband behavior characterized by a full width at half-m.xim.m.of about 2.2 μm.centered around 3.6 μm. Two orthogonal E nantennas are placed perpendicular to each other to realize a dual-polarized nantenna. This nantenna can receive the two incident polarizations at two separate gap locations with very high isolation. The proposed structure can be used in several energy harvesting applications, such as scavenging the infrared heat from.the Earth and other hot objects, in addition to optical com.unications.
