Integrated Control of Meloidogyne Incognita Infesting Tomato Plants Using Callus Powders Derived from Certain Plants Either Alone or in Combination with Oxamyl under Out-Door Conditions

The effect of four dried callus powders derived from certain plants, i.e. tomato, Lycopersicon esculentum; periwinkle, Catharanthus roseus; thorne apple, Datura stramonium and oleander, Nerium oleander applied separately or concomitantly with oxamyl at a half of the recommended dose each on tomato seedlings grown in soil naturally heavy infested with M. incognita under field conditions, indicated that dried callus powder derived from N. oleander either alone or mixed with oxamyl significantly surpassed the other tested treatments in improving percentages of increase in whole plant fresh and shoot dry weights as well as increasing numbers and weights of fruits. Concerning other treatments including oxamyl at a half of recommended dose with each tested callus powder, similar results were evident regarding tomato plant growth parameters. C. roseus plus oxamyl achieved the highest percentage of increase in whole plant fresh weight （141.4%） and shoot dry weight （189.7%） and ranked second to N. oleander ＋ oxamyl in this respect over the untreated plants, respectively. One the other hand, among all tested materials single application of either L. esculentum or oxamyl gave the least values of such criteria which averaged 34.4% and 61.7% or 39.5% and 11.2%, respectively. All tested treatments significantly reduced the total number of galls, egg-masses and eggs in egg-masses especially, dried callus powder of N. oleander which applied singly or combined with oxamyl. The least values of reduction percentages of galls and eggs in egg-masses among all treatments were recorded from tomato plant received dried callus powders of L. esculentum applied alone or concomitant with oxamyl.

Measurement of Gamma Emitting Radionuclides for Assessment, Environmental hazards of Radiation in Rock and Soil Samples of Shabwah and hadramout Regions, Yemen

The natural radioactivity of 226Ra, 232Th and 40K and the fallout of 137Cs in soils and granite rocks of two regions in Yemen (Shabwah and hadramout) were measured by using gamma-ray spectrometry (hPGe) detector. The average values of the radionuclides 226Ra, 232Th and 40K in the soil samples are 14.34, 25.78 and 566.05 Bq/kg respectively. For rock samples, the average activity concentration for 226Ra, 232Th and 40K are 45, 106 and 1235 Bq/kg respectively. Low concentration values of 137Cs in soil and rock samples under investigation, are not radiologically important. The radium equivalent activity (Raeq) and external hazard index (hex) of all samples are less than the limits of 370 Bq·kg-1 and unity, respectively. The average values of total absorbed dose rate due to three primordial radionuclides in soil and rock samples are 46.5 nGy/h and 138.36 nGy/h, respectively, where the absorbed average value of the granite is higher than the permitted limit. So, the local people must avoid using these granite samples as the interior decorative materials of dwelling without radioactivity control.

A Single Institutional Phase II Randomized Trial of Whole Brain Radiation Therapy with or without Irinotecan for the Treatment of Brain Metastases from Solid Tumours

Background: The relatively suboptimal results of whole brain radiation therapy (WBRT) alone in eradication of brain metastases and an attempt to improve outcomes with WBRT have led to studies combining radiotherapy with chemotherapy drugs that could act as radiosensitizers with a rationale of improving local tumor control. Materials and Methods: This randomized phase II study evaluated the use of Irinotecan concomitant with 37.5 Gray (Gy) of WBRT in 2.5 Gy daily fractions × 5 days each week for 3 weeks versus Whole WBRT alone in patients with brain metastasis (BM) from solid tumors. Fifty patients were randomized to receive either WBRT alone or concomitant with three irinotecan IV infusions 80 mg/m2, 2 hrs before RT on days 1, 8, and 15. Results: The objective response rate (ORR) was significantly improved in patients receiving Irinotecan with radiotherapy versus radiotherapy alone (48% vs. 28%;p = 0.048). The median time to progression of brain metastasis was significantly longer in the irinotecan and WBRT arm as compared to the WBRT arm (8 vs. 5 months;p here was no significant difference in survival between treatment arms (p = 0.361). Irinotecan with radiotherapy was generally well tolerated and did not interfere with the delivery of WBRT. Conclusions: Irinotecan concomitant with WBRT was well tolerated and significantly improved local control of BM compared with WBRT alone. These findings require confirmation in a phase III trial with addition of quality of life assessment.

Genotypic characterization of helicobacter pylori isolates among Egyptian patients with upper gastrointestinal diseases

Objective： Over 50% of the world populations are infected with helicobacter pylori （h. pylori）. Most subjects are asymptomatic; however, in 1994, h. pylori has been categorized as group I carcinogen. The aim of the study was to investigate the relationship between h. pylori infection and gastric cancer. Methods： Thirty gastric cancer patients （GCs） and 30 gastritis patients were enrolled in the study. h. pylori was cultured on non-selective and selective medias, infection density was assessed by quantitative culture. Antibiotic sensitivity testing was performed. PCR was done for the h. pylori 16S rRNA gene in addition to cagA, vacA and iceA genes. Results： h. pylori could be cultured from 100% of specimens obtained from all patients. The density of h. pylori was higher in cancer cases than in gastritis patients. The 16S rRNA was detected in all GC patients （100%） while it was only detected in 70% of gastritis patients. The cagA gene was found in 53.3% vs 13.3% of GC and gastritis patients, respectively. The vacA gene was present in all GC patients （by at least one of its alleles） while it was only found in 33.3% of gastritis patients. The vacA slml combination was the most predominant genotype in GC patients, while m2 was the commonest allele in gastritis patients （10%）. The iceA gene was found in 86.7% vs 40% of GC and gastritis patients, respectively. Simultaneous presence of multiple h. pyiori strains was proved, both phenotypically and genotypically. Conclusion： The development of GC is linked to infection with h. pylori harboring certain virulence genes. higher infection density of h. pylori was found in GC patients. Co-existence of more than one strain of h. pylori in the same patient occurs in both malignant and benign lesions.

Association between IL-37 and VEGF Gene Expression in Psoriasis Pathogenesis in Egyptian Population

Background: Psoriasis is considered a common skin disease, marked by the production and elevation of inflammatory plaques that regularly shed scales resulting from extensive skin epithelial cell proliferation. T lymphocytes, neutrophils, and other leukocytes enter the irritated skin, resulting in epidermal keratinocyte hyperplasia, vascular hyperplasia, ectasia, and T lymphocyte, neutrophil other forms of leukocyte infiltration. Aim of the Work: the study aimed to investigate the role of IL-37 and VEGF gene expression in the pathogenesis of psoriasis in Egyptian patients. Methodology: Polymerase chain reaction techniques (PCR) were applied to detect VEGF in the skin homogenates of psoriasis patients. In addition, the ELIZA technique was applied to investigate IL-37 in skin homogenates. One hundred cases have been divided into two groups: 50 healthy volunteers as the group I healthy control;50 psoriasis patients as group II. PCR real-time technology was assessed through extracted DNA samples for VEGF amplification in skin homogenate. Result: Our results revealed that psoriasis patients significantly had a substantial reduction in IL-37 compared to the control group (p Conclusion: There is a significant inverse association between IL-37 and VEGF in psoriasis patients. Study findings revealed that IL-37 gene expression decreases while VEGF gene expression increases in psoriatic individuals. Such a measurement may be beneficial in determining the severity of the condition, as well as taking into consideration of the disease’s diagnosis.

传统土壤图是流域管理、生态水文模型所需土壤空间分布信息的主要数据源。然而,受传统制图技术和基础数据质量所限,传统土壤图的空间详细度和属性精确度并不高。随着地理信息技术的发展,如何利用可获取的高质量空间数据和现代空间分析技术来更新传统土壤图显得十分必要。基于传统土壤图中的土壤多边形与通过模糊聚类所得环境因子组合之间存在着对应关系这一假设,本文提出了一种从传统土壤图中提取土壤-环境关系知识并利用该知识更新传统土壤图的方法。该方法包括四个步骤:对环境数据进行模糊c均值聚类获取环境因子组合;利用传统土壤图建立环境因子组合与土壤类型间的对应关系;提取土壤-环境关系知识;进行土壤推理制图。将该方法应用于加拿大New B runsw ick省的W akefield研究区,以更新该区现有的1∶20 000的传统土壤图。应用结果表明:更新后的数字土壤图显示了更详细的空间分布信息;经野外独立验证点验证,所得土壤图(制图单元为土壤组合-排水等级)精度高出原土壤图约20%。因此,该方法是一种有效的更新传统土壤图的方法,可增加土壤图的空间详细度、提高土壤图的属性精确度。

Effect of Ketamine Instillation on Acute and Chronic Post Mastectomy Pain, a Dose Finding Clinical Study

Background: Uncontrolled acute postoperative pain is considered a risk factor for the development of chronic pain afterward. Objectives: To explore the most effective dose of ketamine instillation (1 of 3 doses: 1, 2, or 3 mg/kg) for acute and chronic post mastectomy pain (PMP). Methods: Ninety female patients with cancer breast, aged (18 - 60 yrs), weighted (50 - 90 kg), scheduled for modified radical mastectomy, randomly allocated into 3 groups to receive ketamine instillation after surgical homeostasis before wound closure (1 of 3 doses;1, 2, or 3 mg/kg as A, B or C groups respectively) patients were followed up for 48 h for acute pain (total morphine consumption, the first request of analgesia and visual analog scale at rest and movement (VASR/M), chronic pain by Leeds assessment of neuropathic signs and symptoms (LANSS) for six-months, hemodynamics, and side effects. Results: Median total dose of morphine consumption was 8 mg (5 - 10) versus 6 mg (6 - 7) in A and B groups respectively in the first 48 h postoperatively. Lowest VASR/M was recorded in C then B and lastly A group (P = 0.037). No patients in the C group requested analgesia versus thirty (100%) and nine (30%) patients in the A and B groups respectively with the first request of analgesia was 12 h (5 - 36) in the A group versus 30 h (12 - 36) in the B group respectively (P Conclusion: Ketamine instillation effectively controlled acute post mastectomy pain (PMP) in a dose-dependent manner and reduced the incidence and severity of chronic pain in patients who undergoing a modified radical mastectomy.

Five Years Follow-Up of Adjustable Continence Therapy (ACT) in the Treatment of Recurrent Female SUI

Purpose/Objective: The primary objective of this FDA study is to evaluate the safety and efficacy of the ACT device (Adjustable Continence Therapy) in the treatment of female recurrent Stress Urinary Incontinence (SUI). The secondary objective is to evaluate the difficulty of placement and aspects of device adjustability. Materials and Methods: The Adjustable Continence Therapy (ACT) system (Uromedica, Inc., Plymouth, MN) consists of two silicone balloons providing urethral coaption and bladder neck support. Each balloon is attached to a titanium port buried in the labia allowing for post-operative adjustment of volume. Females with recurrent SUI with or without urethral hypermobility were evaluated at baseline and follow-up periods of 6 weeks, 3, 6, 9, 12 months and annually thereafter including urinalysis, a 3-day voiding diary, provocative pad weight test, direct visual stress test, Stamey score and validated questionnaires to assess severity of incontinence, voiding dysfunction, sexual function and quality of life. Results: During a 5 years period (2002-2007), 162 patients were implanted (mean age 67.6, range 31 - 94 years). Of these 162 patients 142, 90, 80, 56 and 31 patients completed the 1, 2, 3, 4 and 5 years follow-up respectively. One hundred and thirty five (83%) had failed at least one surgery for incontinence and 44% had failed 2 or more procedures prior to ACT implantation. Difficulty of ACT placement was rated mild in 62%, moderate in 30% and severe in 9%. Improvement in Stamey score of >1 grade was achieved in 75% (107/142) at 1 year, 76% (68/90) at 2 years, 86% (62/78) at 3 years, 93% (50/54) at 4 years and 83% (25/30) at 5 years. Dry rate (provocative pad weight 2 gms) was 51%, 62% , 76%, 76% and 76% at 1 through 5 years, and >50% improvement was achieved in 83%, 86%, 86%, 90% and 93%,respectively. Background: Ketamine or dexmedetomidine as an adjuvant to bupivacaine in local wound infiltration attenuated postoperative stress response, especially with ketamine in patients undergoing total abdominal hysterectomy. Objectives: Compare effect of local wound infiltration with ketamine or dexmedetomidine added to bupivacaine to bupivacaine alone on inflammatory cytokine response after total abdominal hysterectomy. Methods: Sixty female patients with endometrial carcinoma underwent total abdominal hysterectomy and scheduled to receive local wound infiltration before wound closure either with one of three;40 ml of 0.25% bupivacaine alone (C Group) or with the addition of 2 mg/kg ketamine (K Group) or 2 μg/kg dexmedetomidine (D Group). After extubation, they were followed up for postoperative interleukin 6 (IL6), IL1β, IL10, and TNF-α levels were assessed at baseline, pre-infiltration, 6, and 24 h by blood samples obtained from each patient, hemodynamic variables, analgesic profile and side effects. Results: Inflammatory cytokines response was attenuated in K and D groups, evidenced by decreased mean pro-inflammatory cytokines IL6, TNF-α, and increased anti-inflammatory IL10 at 6 and 24 h postoperatively compared to pre-infiltration levels (p ≤ 0.01) with preservation of IL1β at its preoperative level (p > 0.05). Attenuation was more in K and D groups than in the C group and was highest in the K group with decreased 1st request, total morphine consumption without serious side effect. Conclusion: Local wound infiltration with ketamine or dexmedetomidine added to bupivacaine has a good postoperative analgesic profile and attenuated cytokines inflammatory response more than bupivacaine alone after total abdominal hysterectomy, with highest attenuation in ketamine group.
