A new technique of monsplasty as an adjunctive procedure in cases of abdominoplasty: A prospective clinical trial

Background: No standard monsplasty technique has existed until now. Although various monsplasty methods have been described, they exhibit high complication rates with sparse postoperative data. Studies that used pubic suspension techniques did not describe exact anatomical landmarks for the mons pexy. This study aimed to improve the aesthetic and functional appearance of the pubic region with long-term results for all grades of pubic ptosis or bulging with abdominoplasty and provide exact anatomical points to anchor the mons to the rectus ***: This non-randomized prospective clinical trial included 30 patients with various degrees of abdominal wall laxity between December 2017 and September 2019. The surgical procedure was performed for female patients with pendulous abdomen, pubic ptosis or bulging, body mass index ≤35 kg/m2, and age 18–60 years. All patients were followed for up to one year; patient satisfaction was assessed before and after surgery regarding aesthetic results and impact on outfits, hygiene, and sexual ***: The marginal homogeneity test revealed a statistically significant patient satisfaction rate regarding outfits, sexual function, hygiene, and aesthetics. Regarding postoperative complications, seroma(one case) was managed by aspiration, and one case of wound dehiscence was managed conservatively with ***: This study recommends a quick and reproducible monsplasty technique with a low complication rate,a technique that helps determine specific anatomical landmarks for anchoring the mons to the rectus *** this technique with abdominal contouring is advisable for optimal aesthetic results and maintained lymphatic drainage in the pubic region.

Fistula-in-ano is the most common form of perineal ***,a fistula includes an internal opening,a track,and an external *** external opening might acutely appear following infection and/or an abscess,or more insiduously in a chronic *** includes control of infection,assessment of the fistulous track in relation to the anal sphincter muscle,and finally,definitive treatment of the *** was the most commonly used mode of management,but concerns about post-fistulotomy incontinence prompted the use of sphincter preserving techniques such as advancement flaps,fibrin glue,collagen fistula plug,ligation of the intersphincteric fistula track,and stem *** descriptive and comparative studies have evaluated these different techniques with variable *** lack of consistent results,level I evidence,or long-term follow-up,as well as the heterogeneity of fistula pathology has prevented a definitive treatment *** article will review the most commonly available modalities and techniques for managing idiopathic fistula-in-ano.

The global incidence of coronavirus disease 2019(COVID-19)continues to increase despite health care *** disease is caused by coronavirus 2 with high transmission and mortality *** is known about the management of COVID-19 in advanced liver *** aim of work was to propose a plan for management of this drastic disease in case of this specific population with review of medications that could be suitable for advanced liver *** the guidelines and medications available for treatment of COVID-19 were reviewed with selection of the less toxic medications that could be used in advanced liver *** suitable to manage COVID-19 in patients with liver disease might include remdesivir intravenously,nitazoxanide+sofosbuvir,ivermectin,tocilizumab,convalescent plasma,and low molecular weight heparin in certain *** liver disease is associated with portal hypertension and splenomegaly with reduction of blood elements and immune dysfunction and impaired T cell ***,when confronted by cytokine storm as an immune response to COVID-19,there may be an increase in the mortality rate of these *** this review,a plan to treat COVID-19 in this special group of patients with advanced cirrhosis is proposed.

An Efficient Ensemble Model for Various Scale Medical Data

Electronic Health Records(EHRs)are the digital form of patients’medical reports or *** facilitate advanced analytics and aid in better decision-making for clinical *** data are very complicated and using one classification algorithm to reach good results is *** this reason,we use a combination of classification techniques to reach an efficient and accurate classification *** model combination is called the Ensemble *** need to predict new medical data with a high accuracy value in a small processing *** propose a new ensemble model MDRL which is efficient with different *** MDRL gives the highest accuracy *** saves the processing time instead of processing four different algorithms sequentially;it executes the four algorithms in *** implement five different algorithms on five variant datasets which are Heart Disease,Health General,Diabetes,Heart Attack,and Covid-19 *** four algorithms are Random Forest(RF),Decision Tree(DT),Logistic Regression(LR),and Multi-layer Perceptron(MLP).In addition to MDRL(our proposed ensemble model)which includes MLP,DT,RF,and LR *** our experiments,we conclude that our ensemble model has the best accuracy value for most *** reach that the combination of the Correlation Feature Selection(CFS)algorithm and our ensemble model is the best for giving the highest accuracy *** accuracy values for our ensemble model based on CFS are 98.86,97.96,100,99.33,and 99.37 for heart disease,health general,Covid-19,heart attack,and diabetes datasets respectively.

Fundamental solutions for axi-symmetric translational motionof a microstretch fluid

The fundamental solution for the axi-symmetric translational motion of a microstretch fluid due to a concen- trated point body force is obtained. A general formula for the drag force exerted by the fluid on an axi-symmetric rigid par- ticle translating in it is then deduced. As an application to the obtained drag formula, this paper has discussed the problem of creeping translational motion of a rigid sphere in a mi- crostretch fluid. The slip boundary condition on the surface of the spherical particle is applied. The drag force and the other physical quantities are obtained and represented graph- ically for various values of the micropolarity and slip param- eters.

Exact solution for the unsteady flow of a semi-infinite micropolar fluid

The unsteady motion of an incompressible micropolar fluid filling a half-space bounded by a horizontal infinite plate that started to move suddenly is considered. Laplace transform techniques are used. The solution in the Laplace transform domain is obtained by using a direct approach. The inverse Laplace transforms are obtained in an exact manner using the complex inversion formula of the transform together with contour integration techniques. The solution in the case of classical viscous fluids is recovered as a special case of this work when the micropolarity coecient is assumed to be zero. Numerical computations are carried out and represented graphically.

Plasma adrenomedullin level in children with obesity: relationship to left ventricular function

Background Obese children are at increased risk for abnormal cardiac structure and function. Little is known about adre-nomedullin (AM), a cytokine produced in various organs and tissues, as a biomarker of cardiac hypertrophy in obese children. This study aimed to assess the plasma AM levels in a cohort of obese children and its relationship to left ventricular (LV) functions. Methods The study included 60 obese children and 60 non-obese children matched for age and gender as control group. Blood pressure, serum lipid profile, fasting glucose, insulin and plasma AM and the homeostatic model assessment of insulin resistance (HOMA-IR) were measured. Cardiac dimensions and LV functions were assessed using conventional echocardiography. Results Compared to control subjects, obese children had higher blood pressure (P = 0.01), insulin (P = 0.001), HOMA-IR (P = 0.001), and AM (P = 0.001). Moreover, obese children had higher LV mass index (LVMI) (P = 0.001), indicating LV hypertrophy;prolonged isovolumic relaxation times (P = 0.01), prolonged mitral deceleration time (DcT) (P = 0.01) and reduced ratio of mitralE-to-mitralA-wave peak velocity (P = 0.01), indicating LV diastolic dysfunction. Laboratory abnor-malities were only present in children with LV hypertrophy. In multivariate analysis in obese children with LV hypertrophy, AM levels were positively correlated with LVMI [odds ratio (OR) 1.14, 95% confidence interval (Cl) 1.08–1.13,P = 0.0001] and mitral DcT (OR 2.25, 95% CI 1.15–2.05,P = 0.01) in the presence of higher blood pressure and HOMA-IR. A cut-offvalue of AM at 52 pg/mL could differentiate obese children with and without left ventricular hypertrophy at a sensitivity of 94.32% and specificity of 92.45%. Conclusions Plasma AM levels may be elevated in obese children particularly those with LV hypertrophy and is correlated with higher blood pressure and insulin resistance. The communication in the Millimeter-wave(mmWave)band,i.e.,30~300 GHz,is characterized by short-range transmissions and the use of antenna beamforming(BF).Thus,multiple mmWave access points(APs)should be installed to fully cover a target environment with gigabits per second(Gbps)***,inter-beam interference prevents maximizing the sum rates of the established concurrent *** this paper,a reinforcement learning(RL)approach is proposed for enabling mmWave concurrent transmissions by finding out beam directions that maximize the long-term average sum rates of the concurrent ***,the problem is formulated as a multiplayer multiarmed bandit(MAB),where mmWave APs act as the players aiming to maximize their achievable rewards,i.e.,data rates,and the arms to play are the available beam *** this setup,a selfish concurrent multiplayer MAB strategy is *** different MAB algorithms,namely,ϵ-greedy,upper confidence bound(UCB),Thompson sampling(TS),and exponential weight algorithm for exploration and exploitation(EXP3)are examined by employing them in each AP to selfishly enhance its beam selection based only on its previous *** a few rounds of interactions,mmWave APs learn how to select concurrent beams that enhance the overall system *** proposed MAB based mmWave concurrent BF shows comparable performance to the optimal solution.

Experimental evaluation of Odonata nymph in the biocontrol of schistosomiasis intermediate hosts

Objective:To evaluate the predatory potential of the Odonata nymph on freshwater snails that serve as intermediate hosts for Schistosoma species(Bulinus truncatus and Biomphalaria alexandrina).Methods:Observations on the searching,attacking and devouring of the two snail types with series of laboratory-based predation experiments,whose aims were to determine daily predation rate,differential predation,prey preference considering small-,mediumand large-sized snails were ***:Laboratory evaluation revealed that,the Odonata nymph could kill and consume the two intermediate *** number of snails consumed differed according to the snail type,size and *** times taken for searching and handling times were dependent on the snail size,type and satiation of the *** predation rate varied also with respect to snail type,size and *** study also evaluated that Odonata nymphs consumed more Bulinus truncatus than Biomphalaria alexandrina per unit time,and that there may be a preference for smaller than larger ***:According to our observation,the predator,Hemianax ephippiger nymph may be a suitable biocontrol agent in connection with Schistosoma intermediate hosts.
