The“terminal line”:a novel sign for the identification of distal mesorectum end during TME for rectal cancer

Background:Although the clinical importance of complete,intact total mesorectal excision(TME)is the widely accepted standard for decreasing local recurrence of rectal cancer,the residual mesorectum still represents a significant component of resection margin involvement.This study aimed to use a visible intraoperative sign to detect the distal mesorectal end to ensure complete inclusion of the mesorectum and avoid unnecessary over-dissection.Methods:The distal mesorectum end was investigated retrospectively through a review of 124 operative videos at the Union Hospital of Fujian Medical University(Fujian,China)and Cleveland Clinic(Ohio,USA)by two independent surgeons who were blinded to each other.Furthermore,28 cadavers and 44 post-operative specimens were prospectively examined by hematoxylin and eosin(H&E)staining and Masson’s staining to validate and confirm the findings of the retrospective part.Univariate and multivariate analyses were carried out to detect the independent factors that can affect the visualization of the distal mesorectal end.Results:The terminal line(TL)is the distal mesorectal end of the transabdominal and transanal TME(taTME)and appears as a remarkable pearly white fascial structure extending posteriorly from 2 to 10 o’clock.Histopathological examination revealed that the fascia propria of the rectum merges with the presacral fascia at the TL,beyond which the mesorectum ends,with no further downward extension.In the retrospective observation,the TL was seen in 56.6%of transabdominal TME and 56.0%of taTME operations.Surgical approach and tumor distance from the anal verge were the independent variables that directly influenced the detection of the TL(P=0.03 and P=0.01).Conclusion: The TL is a visible sign where the transabdominal TME should end and the taTME should begin. Recognitionof the mesorectal end may impact the certainty of complete mesorectum inclusion. Further clinical trials are needed toconfirm the preliminary findings.

Idiopathic fistula-in-ano

Fistula-in-ano is the most common form of perineal sepsis.Typically,a fistula includes an internal opening,a track,and an external opening.The external opening might acutely appear following infection and/or an abscess,or more insiduously in a chronic manner.Management includes control of infection,assessment of the fistulous track in relation to the anal sphincter muscle,and finally,definitive treatment of the fistula.Fistulotomy was the most commonly used mode of management,but concerns about post-fistulotomy incontinence prompted the use of sphincter preserving techniques such as advancement flaps,fibrin glue,collagen fistula plug,ligation of the intersphincteric fistula track,and stem cells.Many descriptive and comparative studies have evaluated these different techniques with variable outcomes.The lack of consistent results,level I evidence,or long-term follow-up,as well as the heterogeneity of fistula pathology has prevented a definitive treatment algorithm.This article will review the most commonly available modalities and techniques for managing idiopathic fistula-in-ano.

Performance Analysis of an Artificial Intelligence Nanosystem with Biological Internet of Nano Things

Artificial intelligence(AI)has recently been used in nanomedical applications,in which implanted intelligent nanosystems inside the human body were used to diagnose and treat a variety of ailments with the help of the Internet of biological Nano Things(IoBNT).Biological circuit engineering or nanomaterial-based architectures can be used to approach the nanosystem.In nanomedical applications,the blood vascular medium serves as a communication channel,demonstrating a molecular communication system based on flow and diffusion.This paper presents a performance study of the channel capacity for flow-based-diffusive molecular communication nanosystems that takes into account the ligand-receptor binding mechanism.Unlike earlier studies,we take into account the effects of biological physical characteristics such as blood pressure,blood viscosity,and vascular diameter on channel capacity.Furthermore,in terms of drug transmission error probability,the inter-symbol interference(ISI)phenomenon is applied to the proposed system.The numerical results show that the proposed AI nanosystems-based IoBNT technology can be successfully implemented in future nanomedicine.

To increase the payload,reduce energy consumption,improve work efficiency and therefore must accordingly reduce the total hull weight of the submersible.This paper introduces a design optimization process for the pressurehull of submarines under uniform external hydrostatic pressure using bothfinite element analysis(FEA)and optimization tools.A comprehensive study about the optimum design of the pressure hull,to minimize the weight and increase the volume,to reach minimum buoyancy factor and maximum operating depth minimizing the buoyancy factor(B.F)is taken as an objective function with constraints of plate and frame yielding,general instability and deflection.The optimization process contains many design variables such as pressure-hull plate thickness,unsupported spacing,dimensions of long and ring beams andfinally the elliptical submersible pressure-hull diameters.The optimization process was conducted using ANSYS parametric design language(APDL)and ISIGHT.The Multi-Island Genetic Algorithm(G.A)is considered to conduct the optimization process.Additionally,parametric analysis is done on the pressure hull to examine the effect of different design variables on the pressure-hull design.As a result,the B.F of the proposed optimal model is reduced by an average of 31.78%compared with Reference Model(RM).Maximum von Mises stress is reduced by 27%as well.These results can be helpful for submarine pressure-hull designers.

With the emergence of the COVID-19 pandemic,the World Health Organization(WHO)has urged scientists and industrialists to exploremodern information and communication technology(ICT)as a means to reduce or even eliminate it.The World Health Organization recently reported that the virus may infect the organism through any organ in the living body,such as the respiratory,the immunity,the nervous,the digestive,or the cardiovascular system.Targeting the abovementioned goal,we envision an implanted nanosystem embedded in the intra living-body network.The main function of the nanosystem is either to perform diagnosis and mitigation of infectious diseases or to implement a targeted drug delivery system(i.e.,delivery of the therapeutic drug to the diseased tissue or targeted cell).The communication among the nanomachines is accomplished via communication-based molecular diffusion.The control/interconnection of the nanosystem is accomplished through the utilization of Internet of bio-nano things(IoBNT).The proposed nanosystem is designed to employ a coded relay nanomachine disciplined by the decode and forward(DF)principle to ensure reliable drug delivery to the targeted cell.Notably,both the sensitivity of the drug dose and the phenomenon of drug molecules loss before delivery to the target cell site in long-distance due to the molecules diffusion process are taken into account.In this paper,a coded relay NM with conventional coding techniques such as RS and Turbo codes is selected to achieve minimum bit error rate(BER)performance and high signal-to-noise ratio(SNR),while the detection process is based on maximum likelihood(ML)probability and minimum error probability(MEP).The performance analysis of the proposed scheme is evaluated in terms of channel capacity and bit error rate by varying system parameters such as relay position,number of released molecules,relay and receiver size.Analysis results are validated through simulation and demonstrate that the proposed scheme can significantly improve delivery performance of t

Incidental findings of Cysticercus tenuicollis metacestodes in five oryx species

Objective:To determine the prevalence of Cysticercus tenuicollis(C.tenuicollis) metacestodes in five oryx species kept in A1 Ain Zoo animal collection.Methods:This study was based on a retrospective analysis of post-mortem records covering a four year period(July 2010 to July 2014).Results:A total of 213 individual animals were recorded dead during the four year period(July 2010 to July 2014).Out of this,12(5.6%) were recorded with C.tenuicollis.More females(8) than males(4) were recorded to have C.tenuicollis,although this was not statistically significant(P= 0.3737).Conclusions:This study shows that,Arabian oryx,beisa oryx,fringe-eared oryx,gemsbok and scimitar-horned oryx are susceptible to C.tenuicollis.Based on the epidemiology and the life cycle of this parasite,it is possible that these captive animals ingested the parasite through contaminated feed which could have happened in the pasture land or stray dogs and wild canidae(e.***.fox) visited the zoo contaminating the oryx feed.Stray dogs and wild canidae should be prevented from visiting pasture land and a captive animal facility.

如今,经济增长已经成为反映一个国家发展水平的最重要因素之一,但是要提高一个国家的经济增长速度发展,就必须提供一些稳定的增长发展因素,其中有一个重要的增长因素就是对外投资。这对埃及来说没有什么不同,在过去的20年里,埃及在外国投资方面已经成为一个非常有吸引力的国家,但是中国成为了为埃及变成最重要的投资伙伴之一。中埃经济关系发展迅速,近年来,中国成为埃及非常重要的贸易伙伴,也是不同领域的有效投资者。然而,没有足够的研究可以给我们展示中埃两国之间这种合作的重要性,也无法使我们充分了解这种合作的方式,以及这种合作将如何成为两国的双赢局面。本文的主要目的是简要概述中国直接投资对埃及贸易和和经济增长的影响,以及这种投资将如何有益于埃及改善经济状况,并且埃及如何做出与中国的这种合作成为强大而高效的合作,还将检验中国企业在埃及进行投资的时候会面临什么问题,并提供一些解决方案,为了解决这些问题,以使两国中国和埃及从该合作中获得最大利益。为了实现本文的主要目的,按照两国之间的贸易和投资数据要分析中国投资对埃及经济增长在短期间和长期间会有什么影响,并试图找出中国投资与埃及经济增长之间的关系。用了回归模型以检验中国投资怎么影响埃及经济增长以后,发现中国投资对埃及年比一年有不断和稳定的增长,再加上中国投资对埃及经济增开始越来越很重要。研究清楚地表明,在过去的20年里许多国际中国公司在埃及投资了在各种个领域中开始进行投资活动,从汽车、电信、建筑、电子和金融服务到再生能源、石油和天然气,以及中国投资对埃及经济增长有很大的影响,中国投资在埃及越多埃及经济越发展,但是还发现中国投资在埃及尚未达到其全部产能,因为双方中国与埃及还面临一些障碍首先需要克服,以使中国对埃及的投资能够实现两国中国和埃及都希望的预期潜力,并双方中国和埃及从该经济合作中得到最大的收获。

Continental Flood Basalts and Rifting: Geochemistry of Cenozoic Yemen Volcanic Province

Rift formation is a crucial topic in global tectonics. The Yemen rift-related area is one of the most important provinces, being connected to the rifting processes of the Gulf of Aden, the Red Sea and Afar Triangle. In this paper, a review of the Yemen volcanic province and its relations with the Red Sea rifting are presented. Tertiary continental extension in Yemen resulted in the extrusion of large volumes of effusive rocks. This magmatism is divided in the Oligo-Miocene Yemen Trap Series (YTS) separated by an unconformity from the Miocene-Recent Yemen Volcanic Series (YVS). Magmas of the YTS were erupted during the synrift phase and correlate with the first stage of sea-floor spreading of the Red Sea and the Gulf of Aden (30 - 15 Ma), whereas the magmas of the YVS were emplaced during the post rift phase (10 - 0 Ma). A continental within plate character is recognized for both the YTS and YVS basalts. The YTS volcanic rocks are contemporaneous with, and geochemically similar to, the Ethiopian rift volcanism, just as the volcanic fields of the YVS are geochemically alike to most of the Saudi Arabian volcanics. YTS and YVS have analogous SiO2 ranges, but YVS tend to have, on average, higher alkalis and MgO contents than YTS. Fractional crystallization processes dominate geochemical variations of both series. Primitive magmas (MgO > 7.0%) are enriched in incompatible elements and LREEs with respect to primitive mantle, but YVS are more enriched than YTS. To first order, the different geochemical patterns agree with different degrees of partial melting of an astenospheric mantle source: 25% - 30% of partial melting for YTS and 10% - 3% for YVS. Secondly, the higher degree of enrichment in incompatible elements of YVS reflects also greater contribution of a lithospheric mantle component in their source region.

本研究的目的是想通过增加岸线点作为绝对约束的试验，把约束引入独模法空三的基本线性化数学模型中。独模法空三包含七个参数，三个角元素（Ω、φ、π），三个线元素（X0、Y0、Z0）和一个比例因子（λ）。被运用的这条航线有8张象片，近似摄影比例尺为1／7500，位于阿拉伯海湾科威特城的一部分，构成7个立体模型。航线中包含5个全野外地面控制点，5个平面检查点，10个高程检查点，8个岸线点。用带有约束和不带约束两次完成航带网平差，并对结果进行比较。在平羞过程中保持检查点一致以保证晗当的精度评估。象片坐标量测在Kevn（克恩）DSR11解析测图仪上完成，并对影象主要误差进行预处理。在构成统一航带网之前解析地构成立体模型。结果表明，当增加岸线点作为绝对约束时，高程中误差和平面中误差分别改善了约19．5％和8．5％。对于岸线覆盖的许多立体模型，增加这些约束的结果，仅使的法方程系数矩阵的带宽增加。
