Childhood glaucoma profile in Dakahelia, Egypt: a retrospective study

AIm： To analyze childhood glaucoma regarding its demographics, presentations, different causes and surgical modalities used among patients in Dakahelia and to apply the Childhood Glaucoma Research Network（CGRN） classification retrospectively to evaluate its ***： A retrospective study in which the medical files of all glaucoma patients 〈16 years old presented to mansoura Ophthalmic Center, mansoura University from 2014 to 2017, were retrieved and analyzed. Collected data included： age, gender, laterality, visual acuity（VA）, refraction, intraocular pressure（IOP）, corneal diameter, cup-disc ratio, types and number of surgeries and antiglaucomatous drugs（AGD） at the first and last visit. Prevalence of different subtypes was calculated and means of clinical features were ***： A total of 305 eyes of 207 patients were included classified into 6 groups： primary congenital glaucoma（PCG）, juvenile open angle glaucoma（JOAG）, glaucoma associated with systemic disease, glaucoma associated with ocular anomalies, acquired glaucoma and glaucoma following cataract surgery. PCG was the predominant type（55.1%） followed by acquired glaucoma（29.5%）. males represented 63.8% of the whole studied patients. Glaucoma associated with ocular anomaly group showed the youngest age at diagnosis（21.9±30.0 mo）. The shortest corneal diameter was recorded in post cataract group（10.4±0.5 mm）. Highest cup-disc ratio was found in the PCG group（P〈0.0005）. Glaucoma associated with systemic disease presented with the highest baseline IOP（34.5±5.0 mm Hg）. All the cases with PCG were treated surgically with 21.8% required more than one surgery. The majority of the patients（74.2%） in the acquired group were treated medically. Combined trabeculotomy-trabeculectomy was the most frequent operation done, accounting for 34.5% of all primary surgeries. Ahmed valve implantation comprised 87% of the secondary surgeries. Adaptation to climate change in desertified lands of the marginal regions in Egypt through sustainable crop and livestock diversification systems

Environmental degradation resulting from current climate changes, including prolonged drought, land degradation, desertification, and loss of biodiversity, is presenting enormous challenges to achieve food security and eradication of poverty in the marginal regions （about 90% of the total area） of Egypt. In addition to the natural constraints of high temperature, wind erosion, sand dune movement, and recurrent drought, such regions are subjected to improper land and water management. moreover, there is a lack of knowledge, technologies, and experiences to match with the current severe climatic changes. There is a great need for establishing sustainable integrated ecosystem rehabilitation and management programs to overcome such problems in the marginal areas, particularly in the Sinai Peninsula due to its strategic and social importance. A series of research and development programs have been conducted in 2006 to im- prove the livelihoods of smallholders through enhancing the efficient management and utilization of local resources that can cope with the drastic changes of climate in the Sinai Peninsula. An integrated livestock/salt-tolerant fodder crop system was introduced, in 2010 by the project teamwork of Desert Research center, Egypt, to many smallholders in the South Sinai region, where studies were conducted at both the general research and individual farmer levels. The most important results were：（1） adoption of the most salt-tolerant genotypes of three forage crops： pearl millet （Pennisetum glaucum L.）, sorghum （Sorghum bicolor）, and Sudan grass （Sorghum sudanense （Piper） Stapf.）; two cereal crops （triticale and barley）; and two oil crops： safflower （Carthamus tinctorius） and Brassica （mustard）. Land desertification and restoration in middle East and North Africa(mENA) region

The middle East and North Africa （mENA） region is characterized by high population growth, degraded and fragile nat- ural ecosystems, and a limited amount of arable lands. It is one of the most water-scarce regions in the world. The region is heterogeneous in terms of the countries＇ economies, but because it includes some of the richest and some of the poorest countries in the world, regional average economic performance statistics are misleading. The region is mostly semi-arid and arid, with significant areas of extreme aridity. These areas are further challenged by extreme temperatures, frequent drought, land degradation, and desertification. Recent changes in climate patterns, such as prolonged droughts, record temperatures, and increased rainfall irregularity, intensity and distribution, have all further negatively impacted the natural and agro-ecosystems in the region. Such changes have led to increased vulnerability of the people dependent on such re- sources for their livelihood. This article focuses on the impact of land desertification due to climate changes on the prevailing natural resources, and discusses several approaches for mitigating or alleviating desertification. It is clear that water shortage is a problem in many countries of this predominantly arid region, and is unlikely to be reduced and may be exacerbated by climate change. Proposed adaptation strategies might include more efficient organization of water supplies, treatment, and delivery systems, and increased use of groundwater. It is necessary to develop alternative production and management systems appropriate to the socioeconomic and environmental conditions in order to prevent further degradation of the prevailing agro-ecosystems and sustain the livelihoods of farmers living in marginal conditions. Efficient Utilization of 6-Aminouracil to Synthesize Fused and Related Heterocyclic Compounds and Their Evaluation as Prostate Cytotoxic Agents with Cathepsin B Inhibition

6-aminouracil 1 was utilized to introduce different heterocyclic rings at C-6 position through various synthetic strategies. The synthesized compounds bear rings that are either directly attached to the uracil back bone as in compounds 6, 12a-c and 15, or attached through an amino bridge as compounds 3a-c, 5a, b, 7a, b, 9 and 10, or through an imino bridge as compound 18. Also, compounds 4, 8, 11a-c, 14, 16 and 17 bearing biologically active side chains were synthesized. In addition to, compounds 13, 19, 20, 21 and 22 bear fused rings to the uracil backbone. All synthesized compounds were evaluated for their anticancer activity against prostate PC3 cell line using in-vitro sulforhodamine-B (SRB) method, from which compounds 3a, c, 4, 5a, b, 6, 7a, b, 11a-c, 12a, b, 17 and 20 were the most active. These active compounds were further evaluated for their ability to inhibit cathepsin B enzyme by using enzyme-linked immunosorbent assay, which revealed that compounds 5a, b, 7a, 11a, 12a and 17 exhibited more than 50% inhibition of cathepsin B. Among which the phenyl thiourea derivative 17 was the most active exhibiting 82.3% inhibition, while the reference doxorubicin exerted 18.7% inhibition.

A Sleep Scoring System Using EEG Combined Spectral and Detrended Fluctuation Analysis Features

most of sleep disorders are diagnosed based on the sleep scoring and assessments. The purpose of this study is to combine detrended fluctuation analysis features and spectral features of single electroencephalograph (EEG) channel for the purpose of building an automated sleep staging system based on the hybrid prediction engine model. The testing results of the model were promising as the classification accuracies were 98.85%, 92.26%, 94.4%, 95.16% and 93.68% for the wake, non-rapid eye movement S1, non-rapid eye movement S2, non-rapid eye movement S3 and rapid eye movement sleep stages, respectively. The overall classification accuracy was 85.18%. We concluded that it might be possible to employ this approach to build an industrial sleep assessment system that reduced the number of channels that affected the sleep quality and the effort excreted by sleep specialists through the process of the sleep scoring.

Bacterial infections post-living-donor liver transplantation in Egyptian hepatitis C virus-cirrhotic patients: A singlecenter study

AIm To determine risk factors, causative organisms and antimicrobial resistance of bacterial infections following living-donor liver transplantation(LDLT) in cirrhotic *** This prospective study included 45 patients with hepatitis C virus-related end-stage liver disease who underwent LDLT at Ain Shams Center for Organ Transplant, Cairo, Egypt from January 2014 to November 2015. Patients were followed-up for the first 3 mo after LDLT for detection of bacterial infections. All patients were examined for the possible risk factors suggestive of acquiring infection pre-, intra-and post-operatively. Positive cultures based on clinical suspicion and patterns of antimicrobial resistance were identified. RESULTS Thirty-three patients(73.3%) suffered from bacterial infections; 21 of them had a single infection episode, and 12 had repeated infection episodes. Bile was the most common site for both single and repeated episodes of infection(28.6% and 27.8%, respectively). The most common isolated organisms were gramnegative bacteria. Acinetobacter baumannii was the most common organism isolated from both single and repeated infection episodes(19% and 33.3%, respectively), followed by Escherichia coli for repeated infections(11.1%), and Pseudomonas aeruginosa for single infections(19%). Levofloxacin showed high sensitivity against repeated infection episodes(P = 0.03). Klebsiella, Acinetobacter and Pseudomonas were multi-drug resistant(mDR). Pre-transplant hepatocellular carcinoma(HCC) and duration of drain insertion(in days) were independent risk factors for the occurrence of repeated infection episodes(P = 0.024).CONCLUSION mDR gram-negative bacterial infections are common post-LDLT. Pre-transplant HCC and duration of drain insertion were independent risk factors for the occurrence of repeated infection episodes.

Potential Insecticidal Effect of a Wasp (Polistes dominulus) and a Bee (Apis mellifera) Venoms in Controlling Spodoptera littoralis

Cotton leaf worm, Spodoptera littoralis, is considered one of the most common arthropod pests that contribute to crop devastation of cotton. Previously, agricultural pests were controlled through the application of chemical insecticides. However, insecticide-resistant insect populations emergence, as well as increasing concerns about the environmental and human health risks. Venomous animals introduce valuable sources of bioactive compounds which are employed for defence. Some of these toxins have high phyletic specificity, making them appropriate for use in IPm programs. This study aims to test the insecticidal effects of Polistes dominulus and Apis mellifera venoms. Crude venoms were examined for their insecticidal effect against cotton leaf worms by four different application methods including: leaf dipping, integument dropping, spraying, and injection. The data demonstrated a strong response to purified (Polistes dominulus) venom at an initial time than that of honey bee (Apis mellifera) which increases response with increasing the dose and the time interval. A dosage of 0.015 - 0.16 ml of both venoms had notably varied in LD50 values on Spodoptera littoralis that showed higher wasp venom toxicity. Cotton leaf worms showed more susceptibility and mortality to the Polistes sp. venom than that of honey bees.

HLA-DQB1~* alleles and genetic susceptibility to type 1 diabetes mellitus

AIm: To determine human leukocyte antigen (HLA)-DQB1 allele association with susceptibility to type 1 diabetes (T1D) and to clinical and laboratory findings. mETHODS: This study was conducted on 85 unrelated Egyptian children with T1D recruited consecutively from the Pediatric Diabetes Endocrinology outpatients Clinic; mansoura University Children’s Hospital, Egypt. Patient mean follow up period was 2.5 years. Patients were subdivided according to level of HbA1c (optimal/suboptimal control elated age- and sex-matched healthy subjects without T1D or other autoimmune diseases. Genomic DNA extraction was done for all subjects using a DNA isolation kit. HLA-Class II-DQB1 allele typing was carried out with a polymerase chain reaction-sequence-specific oligonucleotide probe using a INNO-LiPA HLA-DQB1 update kit. RESULTS: Significant differences were detected between Egyptian patients with T1D and control groups in the frequencies of DQB1*02 [44.4% vs 18.6%, corrected P value (Pc) multiple comparisons, the significance was retained for HLA-DQB1*06 (Pc ely associated with T1D (Pc = 0.014, Pc ely), while HLA-DQB1*060101 was negatively associated (Pc eles 030101 and 050101 were significantly higher in controls (P = 0.016, P = 0.025 respectively), both of them lost statistical significance after correction of P value. The frequency of the HLA-DQB1 genotypes 02/02, 02/03, and 03/03 was higher in T1D patients, and the frequency of the genotypes 03/06, 05/06, and 06/06 was higher in controls,

Comparison between FIB-4 Index and Fibroscan as marker of Fibrosis in Chronic HCV Infection in Egyptian Patients

The FIB-4 index is a simple and noninvasive algorithm consisting to evaluate liver fibrosis in chronic HCV infection. Aim: To evaluate the utility of FIB-4 index as a noninvasive marker to assess liver fibrosis in chronic HCV infection in comparison to transient elastography. Patients and methods: We studied 30 patients having chronic HCV infection based on clinical features, laboratory tests, diagnostics images, Fibroscan and FIB-4 score. According to the results of Fibroscan, the 30 patients were classified into two groups in order to obtain a cutoff value to exclude patient with significant fibrosis: group Ia: 7 patients with no or mild liver fibrosis (F0-F1) and group Ib: 23 pa-tients with significant fibrosis or cirrhosis (F2-F3-F4). Group IIa: 17 patients with no or significant fibrosis (F0-F1-F2-F3) and group IIb (F4): 13 patients with cirrhosis (F4). Results: The mean of FIB-4 index increased with the increase of the fibrosis score. FIB-4 index proved to be sensitive and specific in differentiation between patients with no or mild fibrosis (F0-F1) and patients with significant fibrosis or cirrhosis (F2-F3-F4) with the best cutoff value at 1.61. It also proved to be sensitive and specific in differentiation between patients with no or significant fibrosis (F0-F1-F2-F3) and patients with cirrhosis (F4) with cutoff value at 1.88. Conclusion: The FIB-4 index enabled the correct identification of extreme types of fibrosis. Using these cutoffs (1.61 - 1.88), 87% of patients fell outside these ranges and could thus avoid liverbiopsy with an overall accuracy of 70%.
