Left-sided gallbladder:Its clinical significance and imaging presentations

AIM: To assess the importance of preoperative diagnosis and presentation of left-sided gallbladder using ultrasound (US),CT and angiography. METHODS: Retrospective review of 1482 patients who underwent enhanced CT scanning was performed. Left-sided gallbladder was diagnosed if a right-sided ligamentum teres was present. The image presentations on US,CT and angiography were also reviewed. RESULTS: Left-sided gallbladder was diagnosed in nine patients. The associated abnormalities on CT imaging included portal vein anomalies,absence of umbilical portion of the portal vein in the left lobe of the liver,club-shaped portal vein in the right lobe of the liver,and difficulty in identifying segment Ⅳ. Angiography in six of nine patients demonstrated abnormal portal venous system (trifurcation type in four of six patients). The main hepatic arteries followed the portal veins in all six patients. The segment Ⅳ artery was identified in four of six patients using angiography,although segment Ⅳ was difficult to define on CT imaging. Hepatectomy was performed in three patients with concomitant liver tumor and the diagnosis of left-sided gallbladder was confirmed intraoperatively. CONCLUSION: Left-sided gallbladder is an important clinical entity in hepatectomy due to its associated portal venous and biliary anomalies. It should be considered in US,CT and angiography images that demonstrate no definite segment Ⅳ,absence of umbilical portion of the portal vein in the left lobe,and club-shaped right anterior portal vein.

Photoluminescence and Electroluminescence Studies on Tb-Doped Silicon Rich Oxide Materials and Devices

Photoluminescence （PL） characteristics of Tb-doped silicon rich oxide （SRO） films prepared by DC-sputtering and post-annealing processes were studied. The silicon richness of the SRO film could be controlled by varying the sputtering power and oxygen concentration in the sputtering chamber. PL emission from the as-deposited sample was found to be composed of Th^3 ＋ intra 4f transition-related emission and the silicon nano particle-related broad bandwidth emission. Thermal annealing could significantly improve the material properties as well as the PL signals. PL properties depended strongly upon the annealing scheme and silicon richness. Annealing at high temperatures （900- 1050℃） enhanced Tbrelated emission and suppressed the silicon nano particle-related emission. For samples with different silicon richness, annealing at 950 ℃ was found to produce higher PL signals than at other temperatures. It was attributed more to lifetime quenching than to concentration quenching. Electroluminescent （EL） devices with a capacitor structure were fabricated, the optimized process condition for the EL device was found to be different from that of PL emission. Among the annealing schemes that were used, wet oxidation was found to improve device performance the most, whereas, dry oxidation was found to improve material property the most. Wet oxidation allowed the breakdown electrical field to increase significantly and to reach above 10 mV·cm^-1. The EL spectra showed a typical Th^3＋ emission, agreeing well with the PL spectra. The I-V measurements indicated that for 100 nm thick film, the Fowler-Nordheim tunneling started at an electrical field of around 6 mV·cm^-1 and the light emission became detectable at a current density of around 10-4 A·cm^-2 and higher. Strong electroluminescence light emission was detected when the electrical field was close to 10 mV·cm^-1.

Small-bowel perforation caused by fish bone

A diagnosis of small-bowel perforation, caused by a sharp or pointed foreign body, is rarely made preoperatively because the clinical symptoms are usually nonspecific and can mimic other surgical conditions, such as appendicitis and diverticulitis. We report the case of a 62-year-old woman who experienced severe pain in the right iliac fossa and fever for about five days before arrival at our hospital. The presumptive diagnosis was acute purulent appendicitis and an emergency appendectomy was planned. Swelling and erythema were noted in a segment of the small bowel in the lower right abdomen. A tiny pointed object was found penetrating the inflamed portion of the bowel, which proved to be a sharp fish bone (gray snapper). The bone was removed, followed by segmental resection of the terminal ileum and ascending colon. The postoperative course was uneventful.

Spontaneous chylous peritonitis mimicking acute appendicitis:A case report and review of literature

Acute abdominal pain with signs and symptoms of peritonitis due to sudden extravasation of chyle into the peritoneal cavity is a rare condition that is often mistaken for other disease processes. The diagnosis is rarely suspected preoperatively. We report a case of spontaneous chylous peritonitis that presented with typical symptoms of acute appendicitis such as intermittent fever and epigastric pain radiating to the lower right abdominal quadrant before admission.

Self-certified multi-proxy signature schemes with message recovery

Multi-proxy signature schemes allow the original signer to delegate his/her signing power to n proxy signers such that all proxy signers must corporately generate a valid proxy signature on behalf of the original signer. We first propose a multi-proxy signature scheme based on discrete logarithms and then adapt it to the elliptic curve cryptosystem. With the integration of self-certified public-key systems and the message recovery signature schemes,our proposed schemes have the following advan-tages:(1) They do not require the signing message to be transmitted,since the verifier can recover it from the signature;(2) The authentication of the public keys,verification of the signature,and recovery of the message can be simultaneously carried out in a single logical step;(3) No certificate is needed for validating the public keys. Further,the elliptic curve variant with short key lengths especially suits the cryptographic applications with limited computing power and storage space,e.g.,smart cards. As compared with the previous work that was implemented with the certificate-based public-key systems,the proposed schemes give better performance in terms of communication bandwidth and computation efforts.

Interannual variability of boreal summer intraseasonal oscillation over the northwestern Pacific influenced by the Pacific Meridional Mode

During the boreal summer,intraseasonal oscillations exhibit significant interannual variations in intensity over two key regions:the central-western equatorial Pacific(5°S-5°N,150°E-150°W)and the subtropical Northwestern Pacific(10°-20°N,130°E-175°W).The former is well-documented and considered to be influenced by the ENSO,while the latter has received comparatively less attention and is likely influenced by the Pacific Meridional Mode(PMM),as suggested by partial correlation analysis *** elucidate the physical processes responsible for the enhanced(weakened)intraseasonal convection over the subtropical northwestern Pacific during warm(cold)PMM years,the authors employed a moisture budget *** findings reveal that during warm PMM years,there is an increase in summer-mean moisture over the subtropical northwestern *** increase interacts with intensified vertical motion perturbations in the region,leading to greater vertical moisture advection in the lower troposphere and consequently resulting in convective *** a process is pivotal in amplifying intraseasonal convection *** observational findings were further verified by model experiments forced by PMM-like sea surface temperature patterns.

A three-level authenticated conference key establishment protocol for UMTS networks

A conference key establishment protocol allows a group of conferees to agree on a secret key shared among them for secure group *** paper proposes a three-level conference key establishment protocol based on the Universal Mobile Telecommunications System (UMTS) framework to establish a group-level key,home location register (HLR) level keys,and visitor location register (VLR) level keys simultaneously for a group of *** group-level key is used to secure the communications for all conferees,the HLR-level key is for those within the same HLR domain,and the VLR-level key is for those within the same VLR *** group-level key can be used for securing inter-domain group-oriented applications such as commercial remote conferencing *** HLRand VLR-level keys can be used for securing intra-domain subgroup applications (e.g.,location-based or context-aware services) and dynamic key *** our proposed protocol exploits existing UMTS security functions and the exclusive-or operation,it is compatible with UMTS *** means that it is fast and easy to implement on the existing UMTS ***,the proposed protocol has low computational complexities and can provide cost effectiveness,load-amortization,scalability,user authentication,key establishment,key confirmation,key updating,and lawful interception.

Splenic tumors are rare. Differentiation of the tumors before operation is of great value regarding the outcome. A case of a 32-year-old man with a splenic inflammatory pseudotumor (IPT) mimicking splenic angiosarcoma is described. The tumor was highly suspected of being splenic angiosarcoma based on radiological findings preoperatively. However, after splenectomy, histopathological examinations revealed splenic IPT. Splenic IPT and angiosarcoma are rare and often pose diagnostic difficulties because the clinical and radiological fi ndings are obscure. Due to large differences in prognosis, we briefl y reviewed the clinical, radiological, and pathological features of both of the tumors.

Robotic-assisted laparoscopic resection of ectopic pancreas in the posterior wall of gastric high body: Case report and review of the literature

Minimally invasive surgery has revolutionized the treatment of gastrointestinal tumors. Submucosal tumors of the stomach can be resected using laparoscopic techniques. We report here a case of ectopic pancreas tissue in the gastric wall that was removed using robotic-assisted laparoscopic resection. The patient was a 15-year-old female who presented with abdominal discomfort and tarry stools. Laboratory analysis showed iron deficiency anemia. Preoperative endoscopy revealed a submucosal lesion in the posterior wall of the gastric high body. Intraoperative upper endoscopy clearly located the lesion. A robotic-assisted laparoscopic wedge resection of the putative gastric submucosal tumor was performed. The pathology results showed an ectopic pancreas. The patient had an uneventful recovery and we believe that this is a valid treatment option for this benign condition.

Bubble Size Distribution for Waves Propagating over A Submerged Breakwater

Experiments are carried out to study the characteristics of active bubbles entrained by breaking waves as these propagate over an abruptly topographical change or a submerged breakwater. Underwater sounds generated by the entrained air bubbles are detected by a hydrophone connected to a charge amplifier and a data acquisition system. The size distribution of the bubbles is then determined inversely from the received sound frequencies. The sound signals are converted from time domain to time-frequency domain by applying Gabor transform. The number of bubbles with different sizes are counted from the signal peaks in the time-frequency domain. The characteristics of the bubbles are in terms of bubble size spectra, which account for the variation in bubble probability density related to the bubble radius r. The experimental data demonstrate that the bubble probability density function shows a - 2.39 power-law sealing with radius for r 〉 0. 8 mm, and a- 1.11 power law for r 〈0.8 mm.
