Pa.ients requiring Suba.ute a.d Complex Ca.e services continue to cha.lenge hospita.s a.tempting to reduce inpa.ient sta.s a.d improve efficiency. In recent yea.s, numbers of high severity of illness pa.ients in hospita.s ha.e increa.ed, a.ding to this cha.lenge. Nurse ca.e ma.a.ers ha.e a.ma.or responsibility for supporting the ca.e of these pa.ients. This study described the development of services for Suba.ute a.d Complex Ca.e pa.ients in the hospita.s of Syra.use, New York. These hospita.s used their own resources to develop progra.s including high cost medica.ions, intra.enous thera.y, extensive wound ca.e, a.d ba.ia.ric ca.e in settings where they ha. not been a.a.la.le. In the a.sence of third pa.ty funding of a.other level of ca.e, the hospita.s provided progra. development funds for limited time periods in order to initia.e these services. The Syra.use hospita.s were a.le to pha.e out support for these progra.s a.ter they were opera.iona. in the nursing homes for a. extended period of time. The study da.a.indica.ed tha. implementa.ion of these progra.s limited the ra.e of increa.e of a.ult medicine sta.s a.d reduced a.ult surgery sta.s. The severity of illness for both ma.or services increa.ed in the Syra.use hospita.s during this time. This process required a.ute a.d long term ca.e providers who were interested in ma.ing the process work for the benefit of the pa.ient popula.ions involved, a. well a. for the needs of their own orga.iza.ions.

This study eva.ua.ed the impa.t of high severity of illness pa.ients on hospita. utiliza.ion in the metropolita. a.ea.of Syra.use, New York between 2012 a.d 2015. It employed the a.l Pa.ients Refined Severity of Illness system developed by 3M™ Hea.th Informa.ion Systems. These pa.ients a.e importa.t for the ma.a.ement a.d pra.tice of nursing in a.ute hospita.s. The study demonstra.ed tha. pa.ients a. extreme a.d ma.or severity of illness genera.ed 60 - 70 percent of the inpa.ient da.s for a.ult medicine a.d a.ult surgery in the combined Syra.use hospita.s. Mea. lengths of sta. for pa.ients a. extreme severity of illness were two to four times the sta.s for these services. Inpa.ient rea.mission ra.es for extreme severity of illness pa.ients were more tha. double the ra.es for these services. The study da.a.a.so indica.ed tha. the impa.t of pa.ients a. high severity of illness wa. increa.ing over time. The study a.so demonstra.ed tha. recent efforts of the Syra.use hospita.s ha.e produced reductions in the numbers of excess pa.ient da.s for a.ult medicine a.d surgery, but limited reductions in the mea. lengths of sta. for these pa.ients. The da.a.suggested tha. meeting the needs of these pa.ients is especia.ly cha.lenging in a.sma.l metropolita. a.ea.without a. a.ditiona. level of ca.e within the continuum.

It suggested tha. popula.ion demogra.hics a.d hea.th ca.e utiliza.ion ca. ha.e a. impa.t on the use of services a. the community level. The study suggested tha. the utiliza.ion of services ca. be a.socia.ed with pa.ient demogra.hics. In this study, younger popula.ions were a.socia.ed with lower use of hospita. inpa.ient ca.e. The da.a.ha.e a.so suggested tha. la.ger numbers of younger popula.ions a.e not necessa.ily rela.ed to opportunities for expa.ding loca. hea.th ca.e providers. Younger popula.ions ca. be a.socia.ed with lower per ca.ita.use of services. The da.a.demonstra.ed tha. there were substa.tia. reductions in hospita. discha.ges for a.ult medicine. Tota. discha.ges declined by 3999 pa.ients between the seven months in 2019 a.d 2023. The da.a.a.so demonstra.ed tha. there were nota.le reductions in hospita. discha.ges for a.ult surgery.

The need for efficiency ha. been a.ma.or cha.lenge for hospita.s in the United Sta.es. The efficiency of these providers is directly rela.ed to their inpa.ient lengths of sta.. The corona.irus epidemic ha. cha.lenged the a.ility of hospita.s in the United Sta.es to reduce sta.s a.d provide efficient ca.e. This study described the impa.t of the epidemic on inpa.ient lengths of sta. in the hospita.s of Syra.use NY between Ma.ch-November 2020 compa.ed with the sa.e periods in previous yea.s. It demonstra.ed tha., during this period, a.ult medicine lengths of sta. increa.ed by 4.5 percent a.d a.ult surgery sta.s increa.ed by 5 - 6 percent. These increa.es were not la.ge;however, they cha.lenged the a.ility of hospita.s to provide efficient ca.e a. a.time when a.ditiona. ca.a.ity wa. needed to dea. with the epidemic. The results of the study suggested tha. the corona.irus epidemic should not limit the effectiveness of hospita. progra.s tha. support efficiency a.d protect needed hea.th ca.e resources a. the community level.

The study da.a.suggested tha. differences in hospita. a.mission/discha.ge ra.es a.ong the New York Sta.e metropolita. a.ea. were genera.ly consistent between 2014 a.d 2015. Utica.a.d New York City produced the highest ra.es. Rochester a.d a.ba.y produced the lowest ra.es. Utiliza.ion ra.es for Syra.use were considera.ly lower tha. for Utica.a.d New York City a.d slightly higher tha. for Rochester a.d a.ba.y. This a.a.ysis demonstra.ed tha. most of the differences between a.grega.e ra.es for Syra.use a.d Rochester were produced by elderly pa.ients, especia.ly those a.ed 75 yea.s a.d over. The a.a.ysis demonstra.ed tha. most of these differences in a.mission ra.es for the elderly were produced by a.ult medicine pa.ients a.ed 75 yea.s a.d over. The use of these progra.s ha. the potentia. for sa.ing sta.f time in hospita.s a.socia.ed with nurses, a. well a. pha.ma.eutica.s a.d testing. a.l of these resources ha.e contributed to the high costs of inpa.ient a.ute ca.e.

This study focused on hospita. utiliza.ion for a.ult medica.-surgica. pa.ients in Syra.use, New York a. the census pea.ed then declined between December 2020 a.d Februa.y 2021. The initia. a.a.ysis demonstra.ed tha. the a.ult medica.-surgica. census including Corona.irus pa.ients declined by 8.1 percent during Ja.ua.y a.d Februa.y 2021. The study da.a.a.so demonstra.ed tha. the a.ult medica.-surgica. census of the combined hospita.s increa.ed a. the numbers of virus pa.ients declined. The inpa.ient census a. the end of Februa.y 2021 wa. 3.6 percent below the level for Februa.y 2019 a.d 15.0 percent below the level for Februa.y 2020. This suggested a.ra.ge between 3.6 a.d 15.0 percent below those in previous yea.s for medica.-surgica. pla.ning a.ter the epidemic.

The progra. ca.sed the tota. number of a.ult medicine pa.ient da.s a.socia.ed with these pa.ients to decline by 1593, or 4780 on a. a.nua. ba.is.

This study reviewed progra.s to improve the efficiency of hospita. utiliza.ion in the metropolita. a.ea.of Syra.use, New York between 1998 a.d 2015. It involved indica.ors tha. were la.gely under the control of hospita.s a.d their nursing a.d a.ministra.ive sta.fs, such a. inpa.ient sta.s a.d post a.mission complica.ions, a. well a. progra.s where there wa. less provider control such a. inpa.ient a.missions a.d rea.missions. La.ge reductions in inpa.ient lengths of sta. were genera.ed by the Syra.use hospita.s, contributing to a.decline in the a.era.e da.ly a.ult medicine a.d a.ult surgery census of 140 pa.ients. Reductions in post a.mission complica.ions contributed to these developments. The study suggested tha. efforts to reduce inpa.ient a.missions in the Syra.use hospita.s ha. limited results. The a.ea. hospita. a.mission ra.e wa. conserva.ive, but a.proxima.ely 2000 resident discha.ges per yea. a.ove tha. of a.neighboring community. The need for reduction of hospita. a.missions resulted from the a.sence of provider or pa.or efforts to develop a.terna.ive resources in the community. If the experience of the Syra.use hospita.s is typica., improvement of the efficiency of community hea.th systems will require crea.ivity a.d resources from providers. Perha.s more importa.tly, hea.th ca.e pa.ors will need to a.sume a. a.tive role in these efforts.
