Review of the optimal timing and technique for extensor tendon reconstruction in composite dorsal hand wounds

Aim: The management of complex dorsal hand wounds with extensor tendon loss is controversial. Treatment has focused on soft tissue coverage, but there is limited evidence comparing immediate vs. staged tendon reconstruction. This review evaluates existing literature to determine the optimal management of composite hand defects. Methods: A MEDLINE database review was performed including objective measurements such as number of operations, total active motion, grip strength, days to maximum range of motion (ROM), and return to work. Data extraction included demographics, surgical techniques, complications, and relative outcome. We compared primary and secondary staged reconstruction to correlate any significant differences in outcome and determine optimal timing and technique for extensor tendon reconstruction. We extracted information on flap types including regional and free tissue transfer with tendinous components vs. staged tendon grafts. Results: Comparison of outcomes showed that patients with immediate reconstruction had fewer operations, faster return to maximum ROM, and greater chance of returning to work. The most successful single stage flaps include the radial forearm, suitable for reconstructing one to three tendons and the dorsalis pedis for three or four tendons;however, there were significantly more complications in immediate reconstruction particularly regarding donor site morbidity. Pedicled flaps had better total active motion. The two-stage approach resulted in acceptable functional outcomes without significant complications. ;Conclusion: Immediate cutaneous tendinous flaps have clear advantages over staged approaches for reconstruction ofcomposite dorsal hand wounds. Benefits include less operations, faster time to maximum ROM, and higher percent of patients returning to work;however, significantly more flap related complications were seen. Lesion-based Patterns of Morbidity and Mortality in Hospitalized Adolescents with Congenital Heart Disease

Objective:The objective of this analysis is to describe the characteristics and morbidity during hospitalizations among adolescents with congenital heart disease(AdoCHD)from the Pediatric Health Information System(PHIS)***:The PHIS database was queried for all AdoCHD admissions aged 12–18 years(1/1/2004–12/31/2013).Major forms of CHD were identified by their International Classification of Diseases,ninth revision codes,further verified based on their secondary diagnosis and/or procedure *** characteristics,diagnoses,procedures and vital status were ***:In total,there were 4,267 adolescents admitted to 42 Children’s Hospitals,58.3%were males,24.6%single ventricle(SV)patients,64.1%bi-ventricle(BV),and 11.3%could not be *** accounted for 8,512 hospitalizations(41,240 total hospital days),of which 31.6%were intensive care unit(ICU)*** stay was similar for the SVand BV patients with similar duration of mechanical ventilation between the two ***,the most common CHD among in-patients was tetralogy of Fallot(TOF,36.4%).Larger proportion of the BVAdoCHD admissions were for elective surgical and electrophysiological *** were 109(2.5%)heart transplantations(1.3%SV vs.0.6%BV)and 120 in-hospital deaths(2.8%)(1.1%SV vs.1.3%BV).Hypoplastic left heart syndrome was the most common diagnosis in transplanted patients(46%)and those who died(28%);TOF(29%)was frequent in 91(2.1%)patients who had cardiac ***:Different hospitalization patterns exist for BV and SV *** this risk may encourage directing resources toward optimizing long-term care of CHD patients.

Identification of C9-C11 unsaturated aldehydes as prediction markers of growth and feed intake for non-ruminant animals fed oxidized soybean oil

Background: The benefits of using the oxidized oils from rendering and recycling as an economic source of lipids and energy in animal feed always coexist with the concerns that diverse degradation products in these oxidized oils can negatively affect animal health and performance. Therefore, the quality markers that predict growth performance could be useful when feeding oxidized oils to non-ruminants. However, the correlations between growth performance and chemical profiles of oxidized oils have not been well examined. In this study, six thermally oxidized soybean oils(OSOs) with a wide range of quality measures were prepared under different processing temperatures and processing durations, including 45 °C-336 h;67.5 °C-168 h;90 °C-84 h;135 °C-42 h;180 °C-21 h;and225 °C-10.5 h. Broilers and nursery pigs were randomly assigned to diets containing either unheated control soybean oil or one of six OSOs. Animal performance was determined by measuring body weight gain, feed intake,and gain to feed ratio. The chemical profiles of OSOs were first evaluated by common indicative tests, including peroxide value, thiobarbituric acid reactive substances, p-anisidine value, free fatty acids, oxidized fatty acids,unsaponifiable matter, insoluble impurities, and moisture, and then analyzed by the liquid chromatography-mass spectrometry-based chemometric ***: Among common quality indicators, p-anisidine value(An V), which reflects the level of carbonyl compounds,had the greatest inverse correlation with the growth performance of both broilers and pigs, followed by free fatty acids and oxidized fatty acids. Gastric cancer in Alaska Native people: A cancer health disparity

AIM To evaluate recent trends in gastric cancer incidence, response to treatment, and overall survival among Alaska Native(AN) people. METHODS A retrospective analysis of the Alaska Native Medical Center patient database was performed. Patient history, clinical, pathological, response to treatment and patient outcomes were collected from one-hundred and thirty-two AN gastric cancer patients. The Surveillance, Epidemiology and End Result database 18 was used to collect comparison United States non-Hispanic White(NHW) and AN gastric cancer patient data between *** AN gastric cancer patients have a higher incidence rate, a poorer overall survival, and are diagnosed at a significantly younger age compared to NHW patients. AN patients differ from NHW patients in greater prevalence of non-cardia, diffuse subtype, and signet ring cell carcinomas. AN females were more likely to be diagnosed with later stage cancer, stage IV, compared to AN males. Diminished overall survival was observed among AN patients with increasing stage, O+ blood type, < 15 lymph nodes examined at resection, and no treatment. This study is the first report detailing the clinicopathologic features of gastric cancer in AN people with outcome *** Our findings confirm the importance of early detection, treatment, and surgical resection for optimizing AN patient outcomes. Further research on early detection markers are warranted.

Cationic polymerizations of vinyl ethers have gained recent attention due to their ability to produce robust,biorenewable *** developments have been largely enabled by the advent of cationic reversible addition−fragmentation chain-transfer(RAFT)polymerization;however,the scalability and sustainability of this method are hindered by current chain-transfer agents(CTAs),which exist as viscous,colored oils requiring complex syntheses and solvent-intensive ***,we produce a solid,colorless CTA through a green synthetic route in 83%yield on a 50-gram *** investigate the utility of this CTA in chemical,electrochemical,photochemical,and acid-initiated methods,revealing that it achieves efficient polymerization with excellent control over molecular weight,low dispersity values,high chain end fidelity,and temporal control in cationic RAFT polymerizations.

Amino acid digestibility in low-fat distillers dried grains with solubles fed to growing pigs

The objective of this experiment was to determine the standardized ileal digestibility（SID） of amino acids（AA） in 3sources of distillers dried grains with solubles（DDGS） with different concentrations of *** growing barrows（initial body weight：76.1 ± 6.2 kg） were randomly allotted to a replicated 6×4 Youden square design with 6 diets and 4 *** fat content of the 3 sources of DDGS were 11.5,7.5,and 6.9%*** contained 60%DDGS and fat concentration of the diets were 7.5,52,and 5.2%,*** additional diets containing the 2sources of DDGS with 75 and 6.9%fat were also formulated,and corn oil was added to these diets to increase the concentration of fat in the diets to levels that were calculated to be similar to the diet containing conventional DDGS with 11.5%fat.A N-free diet was also formulated to calculate endogenous losses of crude protein（CP） and AA from the *** were fed experimental diets during four 7-d *** first 5 d of each period were an adaptation period and ileal digesta were collected on d 6 and 7 of each *** apparent ileal digestibililty（AID） and SID of CP and all indispensable AA,except AID Pro and SID of Trp,were greater（P 〈 0.01） in conventional DDGS than in the 2 sources of DDGS with reduced *** oil to the diets containing the 2 sources of DDGS with reduced fat did not consistently increase SID of *** conclusion,conventional DDGS has greater SID values for most AA compared with DDGS that contains less fat and inclusion of additional oil to diets containing low-fat DDGS does not increase AID or SID of *** lower AA digestibility in low-fat DDGS could not be overcome by the inclusion of additional fat to the diets.

Effects of Foot Rotation on ACL Injury Risk Variables During Drop Landing

Purpose Landing is considered a"high-risk"movement for anterior cruciate ligament(ACL)*** progression angle(FPA)during landing has been shown to influence hip,knee,and ankle *** purpose of this study was to compare five FPA conditions during drop landings between males and *** Twenty males and females were tested using five FPA conditions:self-selected,toe-in 15°,toe-in 30°,toe-out 15°,and toe-out 30°.Right hip,knee,and ankle joint kinematics and kinetics were collected using a 12-camera motion capture system and two force *** successful trials were collected and compared between FPA *** The main effect for FPA condition was statistically different for initial contact hip flexion,hip abduction,hip internal rotation,knee flexion,knee abduction,knee external rotation,and ankle inversion angles compared to self-selected FPA at P<*** hip extension,hip abduction,knee adduction,knee internal rotation,ankle plantar flexion,and ankle inversion moments were also found to be statistically significant(P<0.05).Overall,females experienced greater initial knee abduc-tion angle(P=0.028),hip abduction moment(P=0.006),knee extension moment(P=0.033),and knee internal rotation moment(P=0.044),regardless of FPA *** experienced greater initial contact hip abduction angle(P=0.0017),regardless of FPA *** The results suggest that landing with large toe-in FPA will increase the magnitude of ACL injury risk variables,and females demonstrated a greater magnitude in these variables compared to *** prevention programs may seek to target FPA as part of training activities to avoid large FPA toe-in landings.

Naturopathic Cancer Treatment： Associated with Increased Quality of Life

The purpose of this investigation was to assess the effects of an individualized naturopathic treatment plan on the overall quality of life （QOL） of cancer patients as pertaining to fatigue, pain and overall mood. Twenty-two male （n = 10） and female （12） cancer patients ranging in age from 32 to 82 years-of-age volunteered from a cancer rehabilitation center in Kalispell, MT. Subjects were given the option to utilize a naturopathic treatment option via their physician. Each subject completed a FACT-G questionnaire at the beginning and the end of the study. This short survey asked the subjects a range of questions pertaining to subscales including personal well-being （PWB）, social well-being （SWB）, emotional well-being （EWB）, functional well-being （FWB） and total score. A Dependent t-test was used to examine whether difference （s） existed within the cancer patients from pre to post survey scores （P 〈 0.05） Significant differences were found in PWB, SWB, EWB and total score （P 〈 0.05）. In all categories, the QOL subscales favorably increased from the pre to post values. These findings demonstrate that naturopathic rehabilitation can be a positive alternative treatment for cancer patients undergoing therapy.
