Background: COVID-19 Pandemic is still circulating within the human population and proving to be a deadlier disease with a mortality rate ranging from 0.5% to 7%. Since COVID-19 is a highly transmissible disease;there is always a probability for its outward spread towards the general public and community from the hospitals and healthcare facilities where they come to seek treatment. Methodology: A prospective cohort study design was used, considering the limited available resources and time—a total of 200 healthcare workers (including doctors, nurses, para-medical staff, janitorial staff, reception staff & pharmacists) working in the OPDs of the two major public sector hospitals of Quetta were made part of this study. The study participants were selected using a simple random sampling technique and selection was made from the daily attendance register. The study participants from “Hospital-A” were first of all educated and trained on various COVID-19 IPC measures later on various COVID-19-IEC materials;written in simple Urdu language, were displayed clearly everywhere in the OPD. Similarly, handwashing stations along with hand sanitizers/soaps and surgical face masks were also made available free of cost for all the study participants of Hospital-A. Moreover the importance and effectiveness of COVID-19 IPC measures were continuously announced in the OPD gallery of Hospital-A, these announcements used simple wording in local languages (i.e., Urdu, Pashto, Balochi and Brahvi). On the other hand, in the OPD of “Hospital-B”, no such interventions were made. The study participants of both the hospitals were followed for one month and observations like which group showed more on-job noncompliance towards various COVID-19 IPC measures were recorded. Transitional cell carcinoma （TCC） of the ovary is a rare and recently recognized subtype of ovarian epithelial cancer. We presented the first case report from our Institute （Sheri Kashmir Instiute of Medical Sciences, Srinagar, India）, which was initially rnisdiagnosed as stromal cell carcinoma （granulosa cell tumour）, and on review of histopathology with immunohistochemistry, the diagnosis of TCC of the ovary was established. The aim of this article was to describe the typical case of primary TCC of the ovary and to review the literature for information on TCC management.

An Investigation into Infection Prevention and Control Practices among Close Contacts of COVID-19 Positive Cases Identified during Trace Test and Quarantine Activities at District Quetta (Unmatched Case-Control Study)

The second wave of COVID-19 pandemic has started globally, right now 220 countries are infected and a total of 71,351,695 confirmed cases and 1,612,372 deaths due to COVID-19 have been reported. Infection Prevention and Control (IPC) measures for COVID-19 all have proved vital in decreasing the transmission rates among the communities. Methodology: Unmatched Case-Control Study was conducted where cases were defined as “every PCR positive contact (symptomatic or asymptomatic) for any index case” similarly controls were defined as “every PCR negative contact (symptomatic or asymptomatic) for any index case who was home quarantined for 14 days based on suspicion by PDSRU team”. A simple random technique was used and 300 individuals were made part of this study. Results: The major findings of this study shows that PCR positive contacts poorly adopted certain COVID-19 IPC measures of interest in their daily life hence got infected. The odds for all the variables of interest were found to be statistically significant among cases as compared to controls like the odds for knowingly and intentionally contacted with a COVID-19 positive case was 13.7 times more among the PCR positive contacts as compare to PCR negative contacts (p = 0.00, C.I = 7.62 - 24.90), similarly, the odds of being a family member of the index COVID-19 case was 7.07 times more among the PCR positive contacts as compared to the PCR negative contacts (p = 0.00, C.I = 3.25 - 15.86). Conclusion: Before the development and availability of a vaccine, the only tools that can help prevent the spread of COVID-19 are IPC measures.

Novel PCR primers to diagnose visceral Leishmaniasis using peripheral blood, spleen or bone marrow aspirates

Objective: To establish a suitable method of diagnosis of visceral Leishmania sis(VL) using peripheral blood, spleen or bone marrow aspirates. Methods: Peripheral blood, bone marrow and spleen aspirate samples were collected from clinically suspected VL patients(n=26). A new PCR primer pair(MK1F/R) was designed targeting kinetoplast mini circle DNA sequences of Leishmania donovani, and Leishmania infantum, and was used to diagnose VL along with some other established primers for VL in polymerase chain reactions. Test was validated by comparing with several other diagnostic methods. Results: The designed primer set showed 100% specificity and 98% sensitivity in detecting VL using blood samples, when compared with more invasive samples: bone marrow or spleen aspirates. Conclusions: The newly designed primer MK1F/R could be a better alternative for PCR based diagnosis of VL using less invasive sample, peripheral blood instead of bone marrow or spleen aspirates.

Drought is one of the critical limitations to agricultural soils and crop *** of water is increasing due to climate change that lead to increasing threats to global food ***,ecofriendly and cost effective strategies are highly desirable for mitigating drought stress along with sustainable and smart agricultural *** aim of the study was to mitigate DS using seed priming and exogenous supplementation ofβ-aminobutyric acid(BABA)in linseed(Linum usitatissimum L.).Different doses(0,50,100 and 150μM)of BABA were used for seed priming agent and foliar spraying under three soil moisture levels viz.,25%(SM25),45%(SM45)and 65%(SM65).The response variables of both experiments included different agro-botanical traits and oxidative stress indicators such as melondialdehyde content,free proline accumulation,and antioxidant defense in *** linseed plants showed water stress at SM25 that reduced plant height,number of branches per plant,time taken to flower initiation and heading,and root and shoot dry ***,the number of capsules and seeds per capsule showed a significant decline at SM25,which led to a drastic reduction in 100-seed weight yield in linseed plants in both ***,seed priming and foliar supplementation with of BABA(50–100μM)significantly improved these morpho-agronomical attributes in linseed plants under *** results revealed that the BABA was fully active in linseed plants at ***,the combination of SM25 with BABA significantly improved the antioxidant enzymes superoxide dismutase(SOD),catalase(CAT),ascorbate peroxidase(APX),and peroxidase(POD)activity,which significantly enhanced DS tolerance in linseed *** findings might be useful to oil seed breeders and farmers linseed for breeding program in linseed plants as well as sustainable agricultural production of oil seed crop plants.

Test Accuracy of CT-Scan for the Detection of Malignant Liver Mass

Background: Detection of malignant liver mass is very important for the treatment modalities. Objective: The purpose of the present study was to establish the usefulness of CT scan in the diagnosis of malignant hepatic mass. Methodology: This cross sectional study was carried out in the Department of Radiology and Imaging at Mymensingh Medical College Hospital (MMCH), Mymensingh, Banghabandhu sheikh Mujib Medical University (BSMMU), Dhaka and Dhaka Medical College Hospital (DMCH), Dhaka during the period of 1st January 2006 to 31st December 2007. Patients admitted in the Department of Medicine and Department of Hepatobiliary of MMCH, BSMMU, and DMCH with the clinical diagnosis of fever, abdominal pain, anorexia, nausea/vomiting, loss of appetite, jaundice, weight loss and ascites were selected as study population. CT scan and histopathology were performed to all the patients. Result: A total number of 50 patients were recruited for this study. Mean age of all patients was 51.28 ± 14 years with a range of 17 year to 78 years. Among all patients 28 had multiple lesion, of them 71.4% was malignant and 28.6% was benign. On the other side 22 patients had solitary lesion, of them 36.4% was malignant and 63.6% was benign

CT-Scan Findings of Hepatic Mass Patients Attending at a Tertiary Care Hospital in Bangladesh

Background: CT-scan is a very useful diagnostic tool for the detection of hepatic mass. Objective: The present study was undertaken to determine the CT-scan findings of benign and malignant hepatic mass patients. Methodology: This was a cross sectional study conducted in Radiology and Imaging Department at Mymensingh Medical College Hospital (MMCH), Mymensingh;Dhaka Medical College Hospital (DMCH), Dhaka and Banghabandhu sheikh Mujib Medical University (BSMMU), Dhaka with the collaboration of Pathology Department of the same institute for histopathological confirmation. This study was carried out from January 2006 to December 2007 for a period of 2 years. The patients who were clinically suspected of having hepatic mass attended in the Radiology and Imaging Department in the above mentioned institutes were included as study population. All the CT-scan findings were recorded. Result: A total number of 50 patients were enrolled for this study. CT-scan was done among 40 males and 10 females with a mean age of 51.28 years old. Hypodensity was found in 17 (60.7%) and 18 (81.8%) cases in malignant and benign hepatic lesions respectively. Ill-defined margin was detected in 12 (42.9%) and 6 (27.3%) cases respectively. Calcification was present on 11 (39.3%) malignant lesion and 6 (27.3%) benign lesions. Pressure effect on biliary apparatus was found in 11 (39.3%) malignant lesions and 1 (4.5%) benign lesions (p < 0.05). Lymphadenopathy was found in 10 (35.7%) malignant lesions and 1 (4.5%) benign lesions (p < 0.05). Conclusion: In conclusion, CT-scan findings of malignant and benign hepatic mass show hypodensity with more contrast enhancement in malignant lesions with more calcification in malignant lesion;however, significant difference is detected in pressure effect on biliary apparatus and lymphadenopathy.

Patients Returning to Dialysis after Failed Kidney Transplant: How Do They Fare? A Gulf Perspective

An increasing number of failed transplant patients returning to dialysis (FTRD) have been observed with reported worse survival compared to transplant-naive dialysis (TxN) patients. This study aimed to assess outcomes of FTRD vs. matched TxN controls in a Gulf region multi-center trial of 800 HD patients. Similar mortality was seen, likely due to earlier start and better HD adequacy in FTRD. Younger age, less diabetes and living donor transplantation in majority with 27% graft nephrectomy (Nx) might also confer benefits. Subgroup analysis of Nx patients showed more hospitalizations and prior rejection episodes with lower graft survival. The deaths, however, oc-curred only in nonNx group and are likely explained by older age, longer duration on HD, more prevalence of diabetes and CAD. FTRD showed similar survival to TxN. Early intensive HD might account for the benefit. Whether Nx confers advantage is unclear because of the small sample size.

Experience with Post Transplant Parathyroidectomy in Gulf Region and Literature Review

Sustained elevation of parathyroid hormone (PTH) levels is not uncommon post renal transplantation. Though in the majority of patients, it gradually normalizes, on average 5% of transplanted patients require parathyroidectomy (PTX). However, PTX itself has been associated with deterioration in allograft function and even completes graft loss seen with both total and subtotal PTX as well as an increased rate of acute rejection. The aim of this study was to determine the effect of post transplant partial PTX on allograft function in our patients as well as the incidence of acute rejection. Our results show that post transplantation, subtotal PTX, was successful in achieving metabolic control while preserving graft function without an increased incidence of acute rejection. Retention of sufficient residual parathyroid tissue with partial PTX might account for favorable outcome in our study. Despite this, surgery for advanced hyperparathyroidism should optimally take place in the pre-transplant period.
