Ampullary carcinoma:Effect of preoperative biliary drainage on surgical outcome

AIM: To evaluate the influence of preoperative biliary drainage on morbidity and mortality after surgical resection for ampullary carcinoma. METHODS: We analyzed retrospectively data for 82 patients who underwent potentially curative surgery for ampullary carcinoma between September 1993 and July 2007 at the Singapore General Hospital, a tertiary referral hospital. Diagnosis of ampullary carcinoma was confirmed histologically. Thirty-five patients underwent preoperative biliary drainage (PBD group), and 47 were not drained (non-PBD group). The mode of biliary drainage was endoscopic retrograde cholangiopancreatography (n = 33) or percutaneous biliary drainage (n = 2). The following parameters were analyzed: wound infection, intra-abdominal abscess, intra-abdominal or gastrointestinal bleeding, septicemia, biliary or pancreatic leakage, pancreatitis, gastroparesis, and re-operation rate. Mortality was assessed at 30 d (hospital mortality) and also longterm. The statistical endpoint of this study was patient survival after surgery. RESULTS: The groups were well matched for demographic criteria, clinical presentation and operative characteristics, except for lower hemoglobin in the non-PBD group (10.9 ± 1.6 vs 11.8 ± 1.6 in the PBD group).Of the parameters assessing postoperative morbidity, incidence of wound infection was signif icantly less in the PBD than the non-PBD group [1 (2.9%) vs 12 (25.5%)]. However, the rest of the parameters did not differ signif icantly between the groups, i.e. sepsis [10 (28.6%) vs 14 (29.8%)], intra-abdominal bleeding [1 (2.9%) vs 5 (10.6%)], intra-abdominal abscess [1 (2.9%) vs 8 (17%)], gastrointestinal bleeding [3 (8.6%) vs 5 (10.6%)], pancreatic leakage [2 (5.7%) vs 3 (6.4%)], biliary leakage [2 (5.7%) vs 3 (6.4%)], pancreatitis [2 (5.7%) vs 2 (4.3%)], gastroparesis [6 (17.1%) vs 10 (21.3%)], need for blood transfusion [10 (28.6%) vs 17 (36.2%)] and re-operation rate [1 (2.9%) vs 5 (10.6%)]. There was no early mortality in either group. Median survival was 44 mo (95% CI: 34.2-53.8

目的:研究在马来西亚地区幽门螺杆菌菌株对克拉霉素耐药情况与23SrRNA基因突变的关系。方法:通过E test方法测定幽门螺杆菌菌株对克拉霉素的敏感性,通过对克拉霉素耐药菌株和克拉霉素敏感菌株的23S rRNA基因V区的点突变进行分析聚合酶链反应产物的序列分析扩增。应用限制性内切酶(Mbo II,Bsa I)酶切位点检测相对应的克拉霉素耐药菌株的突变已达到对限制性片段长度多态性分析。结果:120例患者的187个菌株中,有4株对克拉霉素耐药,183株对克拉霉素敏感。该耐药菌株的MIC(最低抑菌浓度)范围从1.5到24ug/ml.两个菌株中一个是A2142G突变,另两个是A2143G突变。4个克拉霉素耐药菌株中2个被检测到T2182C突变,14个克拉霉素敏感菌株中13个被检测到T2182C突变。限制性内切酶(Bsa I、MboII)可检测到突变。结论:在马来西亚地区幽门螺杆菌菌株对克拉霉素耐药的发生比较罕见,A2142G和A2143G突变存在低水平耐药。

The analytical lower bound of Spectral Efficiency(SE)of downlink transmission of the Massive Multiple Input Multiple Output(Ma-MIMO)system is analyzed.In this paper,we derive some novel and approximate mathematical expressions for the lower bound of the SE of a Ma-MIMO with linear precoding schemes,i.e.,Minimum Mean Square Error(MMSE)and Zero-Forcing(ZF).For simulation analysis of the SE,we consider three joint users and antenna scheduling algorithms,namely,the semi-orthogonal,random,and distance-based user scheduling algorithms,whereas the antennas are selected based on Maximum Signal to Noise Ratio(MSNR)with scheduled users.The channel between the user and the transmitter is assumed to have characteristics of Small Scale Fading(SSF)and Large Scale Fading(LSF)with the Rayleigh fading model.We investigate the effect of the variation of transmitting SNR,the number of base station antennas(M),and the radius(R)of the cell area on the SE.We simulate the downlink transmission of Ma-MIMO and compare the simulation and analytical results.It is observed that the trends of variation of both results are similar to the variation of identical factors,and the difference between the simulated and analytical lower bounds of the SE is approximately 1-1.5 bits.The analytical lower bound is smaller than the simulation result.

The capacity of a massive MIMO cellular network depends on user and antenna selection algorithms,and also on the acquisition of perfect Channel State Information(CSD).Low computational cost algorithms for user and an-tenna selection significantly may enhance the system capacity,as it would consume a smaller bandwidth out of the total bandwidth for downlink transmission.The objective of this paper is to maximize the system sum-rate capacity with efficient user and antenna selection algorithms and linear precoding.We consider in this paper,a slowly fading Rayleigh channel with perfect acquisition of CSI to explore the system sum-rate capacity of a.massive MIMO network.For user selection,we apply three algorithms,namely Semi orthogonal user selection(SUS),Descending Order of SNR-based User Scheduling(DOSUS),and Random User Selection(RUS)algorithm.In all the user selection algorithms,the selection of Base Station(BS)antenna is based on the maximum Signal-to-Noise Ratio(SNR)to the selected users.Hence users are characterized by having both Small Scale Fading(SSF)due to slowly fading Rayleigh channel and Large.Scale Fading(ISF)due to distances from the base station.Further,we use linear precoding techniques,such as Zero Forcing(ZF),Minimum Mean Square Error(MMSE),.and Maximum Ratio Transmission(MRT)to reduce interferences,thereby improving average system sum-rate capacity.Results using SUS,DOSUS,and RUS user selection algorithms with ZF,MMSE,and MRT precoding techniques are compared.We also analyzed and compared the computational complexity of all the three user selection algorithms.The computational complexities of the three algorithms that we achieved in this paper are 0(1)for RUS and DOSUS,and 0(M^2N)for suS which are less than the other conventional user selection methods.

Agricultural wastes are environmental hazards,as these wastes can catch fire,resulting in the loss of human and animal lives and properties.Alternatively,the wastes are dumped in large spaces,which are already limited.Cementitious composites are quasi-brittle and develop cracks at the micro and nano level,which affect their strength,durability,and esthetics.Transforming agricultural wastes to biochar and using it as fibers in cementitious materials for crack arresting and enhancing fracture toughness is an environment-friendly approach.In this research,nano to microscale carbonaceous inert fibers(biochar)of millet and maize were prepared through pyrolysis followed by ball milling.The X-ray spectroscopy(EDX)revealed that 82.08%and 86.89%of the carbon content was retained in millet and maize,respectively.The scanning electron microscope(SEM)confirmed the presence of angular,flaky,and needle-like particles in the carbonaceous inerts,which may enhance the strength and the fracture response of the cementitious materials.These inerts were added individually to mortar specimens at dosage levels of 0,0.025%,0.05%,0.08%,0.2%and 1%by mass of cement.The dispersion of the synthesized nano inerts was ensured by UV–VIS spectroscopy.The compressive strength,flexural strength,porosity,and fracture toughness of cement mortar were evaluated.The carbonized nano intrusions reduced the porosity and density of the mortar specimens.The minimum porosity was noted with 1%and 0.08%dosages of millet and maize,respectively,whereas the minimum density was observed at 1%dosage for both.An increase in compressive and flexural strengths was also noticed.The compressive strength increased by 32%and 28%with 0.2%and 0.5%millet and maize,respectively.An increase of 168%and 114%in fracture toughness was noticed at optimized dosages of 0.5%and 1%of maize and millet,respectively.It is concluded that the addition of carbonaceous inert fibers of millet and maize resulted in light-weight porous mortars with enhanced strength and fracture toughness.The fr

Text extraction from images using the traditional techniques of image collecting,and pattern recognition using machine learning consume time due to the amount of extracted features from the images.Deep Neural Networks introduce effective solutions to extract text features from images using a few techniques and the ability to train large datasets of images with significant results.This study proposes using Dual Maxpooling and concatenating convolution Neural Networks(CNN)layers with the activation functions Relu and the Optimized Leaky Relu(OLRelu).The proposed method works by dividing the word image into slices that contain characters.Then pass them to deep learning layers to extract feature maps and reform the predicted words.Bidirectional Short Memory(BiLSTM)layers extractmore compelling features and link the time sequence fromforward and backward directions during the training phase.The Connectionist Temporal Classification(CTC)function calcifies the training and validation loss rates.In addition to decoding the extracted feature to reform characters again and linking them according to their time sequence.The proposed model performance is evaluated using training and validation loss errors on the Mjsynth and Integrated Argument Mining Tasks(IAM)datasets.The result of IAM was 2.09%for the average loss errors with the proposed dualMaxpooling and OLRelu.In the Mjsynth dataset,the best validation loss rate shrunk to 2.2%by applying concatenating CNN layers,and Relu.

Presently,the global search for alternative renewable energy sources is rising due to the depletion of fossil fuel and rising greenhouse gas(GHG)emissions.Among alternatives,hydrogen(H2)produced from biomass gasification is considered a green energy sector,due to its environmentally friendly,sustainable,and renewable characteristics.However,tar formation along with syngas is a severe impediment to biomass conversion efficiency,which results in process-related problems.Typically,tar consists of various hydrocarbons(HCs),which are also sources for syngas.Hence,catalytic steam reforming is an effective technique to address tar formation and improve H2 production from biomass gasification.Of the various classes in existence,supported metal catalysts are considered the most promising.This paper focuses on the current researching status,prospects,and challenges of steam reforming of gasified biomass tar.Besides,it includes recent developments in tar compositional analysis,supported metal catalysts,along with the reactions and process conditions for catalytic steam reforming.Moreover,it discusses alternatives such as dry and autothermal reforming of tar.
