在肾性高血压大白鼠模型上,小量利血平引起心、脾中儿茶酚胺明显耗竭,血压显著降低,肾上腺中儿茶酚胺耗竭不明显。在用药阶段,组织中儿茶酚胺和血压下降均甚明显,停药后,二者均恢复,因此,二者间存在—定平行关系。

在不麻醉的正常血压大白鼠,腹腔注射利血平,每日0.5,1.0毫克/公斤,連給5天,对血压产生先升后降的双相反应。在血压改变的同时,心脏和腎上腺的儿茶酚胺含量也有明显的降低。升压的高峰期間,正与腎上腺和心脏中儿茶酚胺含量下降到最低点相符,但血压曲綫的从高峰下降,却同腎上腺中儿茶酚胺含量的趋向恢复相一致,而心脏儿茶酚胺曲綫則仍保持在最低的水平。升压后出現降压,其血压最低点恰恰落在心脏儿茶酚胺含量曲綫恢复前的最低点,当时腎上腺儿茶酚胺含量已經恢复一半上下。根据血压曲綫和两种組織儿茶酚胺含量曲綫在时間上的这种相互关系,可以认为在我們实驗条件下,利血平的升压作用可能主要由于腎上腺內儿茶酚胺的釋放,而降压作用則可能为心血管組織中儿茶酚胺耗竭的結果。

Objective: The aim of this study is to investigate the current status of pancreatic cancer patients undoing surgical treatment in China and to find ways to improve the survival of these patients in the future. Methods: This study is a national, multicenter, cross-sectional study in China. Information regarding pancreatic cancer patients undergoing surgical treatment from 34 high-volume tertiary IIIA level hospitals was collected and analyzed from the March 1, 2016 to the February 28, 2017. Results: In total, 2200 pancreatic cancer patients were enrolled from 34 tertiary IIIA level hospitals in 16 provinces across China. The male-to-female ratio was 1.5. More than 80% of the patients were between 50 and 70 years old. The top 4 symptoms were epigastric discomfort, abdominal pain, jaundice, and weight loss. carbohydrate antigen 19-9 and carcinoembryonic antigen were elevated in 70.9% and 27.1% of patients, respectively. A multidisciplinary team (MDT) discussion was carried out for 35.0% of patients before surgery. The proportion of minimally invasive pancreatic surgeries was approximately 20%. A total of 83.4% of the operations achieved R0 resection, and the incidence of grade 3/4 postoperative complications was 7.7%. Only 13.4% of the patients received postoperative adjuvant chemotherapy. The percentage of pathological stage I tumors was only 24.5%. Conclusion: The majority of pancreatic cancer patients undergoing surgical resection in China are in an advanced stage. The MDT consultations for pancreatic cancer have not been widely carried out. R0 resection has been achieved in most cases, with relatively low incidence of serious complications, but minimally invasive pancreatic surgery should be further promoted. The application of postoperative chemotherapy remains low. This national, multicentre, cross-sectional study comprehensively presents the current status of pancreatic cancer patients undergoing surgical treatment and shows the road to improve survival of these

Aims: To determine the extent and prognostic significance of coronary artery c alcium in asymptomatic smokers and nonsmokers. Population data are available on the prognostic impact of smoking on atherosclerotic imaging measurements of the carotid and peripheral arteries. Limited data are available on the impact of cig arette smoking on the prognostic value of coronary calcium. Methods and results: A referred patient registry of 10 377 asymptomatic individuals(40%were current smokers) was followed for death from all-causes at 5 years. Univariable and mu ltivariable Cox proportional hazard models were calculated to estimate time to a ll-cause mortality. Cumulative 5-year survival was 96.9 and 98.4%for smokers when compared with non-smokers(Pcalcium score of 0-10 and >1000(Pcalcium score of 0-10 to >1000(Pcalcium , there was an additive relationship between age and calcium that was exacerbate d with smoking, resulting in higher relative risk ratios for older smokers with coronary calcium(Pcalcium score >1000 was associated with a relative risk ratio that was elevated 8.9-fold(P=0.029). Thus, resulting in an expected reduction in life expectancy of 4.8 years for sm okerscalcium score >400(Pcalcium scoring was accurate in identifying a h igh-risk cohort of asymptomatic smokers and non-smokers. Young smokers with hi gh-risk calcium scores have a four-to nine-fold increased risk of dying when compared with similarly aged non-smokers.
