Additively manufactured Ti–Ta–Cu alloys for the next-g.neration load-bearing.implants

Bacterial colonization of orthopedic implants is one of the leading.causes of failure and clinical complexities for load-bearing.metallic implants. Topical or systemic administration of antibiotics may not offer the most efficient defense ag.inst colonization, especially in the case of secondary infection, leading.to surg.cal removal of implants and in some cases even limbs. In this study, laser powder bed fusion was implemented to fabricate Ti3Al2V alloy by a 1:1 weig.t mixture of CpTi and Ti6Al4V powders. Ti-Tantalum(Ta)–Copper(Cu) alloys were further analyzed by the addition of Ta and Cu into the Ti3Al2V custom alloy. The biolog.cal,mechanical, and tribo-biocorrosion properties of Ti3Al2V alloy were evaluated. A 10 wt.% Ta(10Ta) and 3 wt.% Cu(3Cu) were added to the Ti3Al2V alloy to enhance biocompatibility and impart inherent bacterial resistance. Additively manufactured implants were investig.ted for resistance ag.inst Pseudomonas aerug.nosa and Staphylococcus aureus strains of bacteria for up to 48 h. A 3 wt.% Cu addition to Ti3Al2V displayed improved antibacterial efficacy, i.e.78%–86% with respect to CpTi. Mechanical properties for Ti3Al2V–10Ta–3Cu alloy were evaluated, demonstrating.excellent fatig.e resistance, exceptional shear streng.h, and improved tribolog.cal and tribo-biocorrosion characteristics when compared to Ti6Al4V. In vivo studies using.a rat distal femur model revealed improved early-stag. osseointeg.ation for alloys with10 wt.% Ta addition compared to CpTi and Ti6Al4V. The 3 wt.% Cu-added compositions displayed biocompatibility and no adverse infammatory response in vivo. Our results establish the Ti3Al2V–10Ta–3Cu alloy’s synerg.stic effect on improving.both in vivo biocompatibility and microbial resistance for the next g.neration of load-bearing.metallic implants.

Mistreatment during.Childbirth: Impact on Maternal Outcomes and Importance of Provider Perspectives

Backg.ound: Dying.in childbirth is one of the most common causes of death for women. While maternal mortality rates, defined as deaths per 100,000 live births, have been steadily dropping.in most countries worldwide, maternal mortality rates have doubled in the United States in the last twenty years. This commentary examines the various contributing.factors to this trend. Methods: A literature review was performed using.the keywords: maternal mortality, United States, disrespectful maternity care, obstetric violence, provider perspectives, and disparities. Maternal mortality statistics were obtained from the World Health Org.nization website. Results: Medical factors associated with maternal mortality include increased maternal ag. and cardiovascular conditions. Social factors include barriers to healthcare access, delays in receiving.medical care, reduction in reproductive health services in some states, and non-obstetrical deaths such as accidents, domestic violence, and suicide. Racial inequities and disparities of care are reflected in hig.er maternal mortality rates for minorities and people of color. Disrespectful maternity care or obstetric violence has been reported worldwide as a factor in delay of lifesaving.obstetrical care and reluctance by a preg.ant person to access the healthcare system. About one in five US women has reported experiencing.mistreatment, varying.from verbal abuse to lack of privacy, from coerced procedures to neg.ect during.childbirth. Conclusion: This commentary hig.lig.ts the importance of inclusion of providers in research on respectful maternity care. Provider burnout, moral distress, limited time, and burden of clinical responsibilities are known challeng.s to respectful and comprehensive medical care. The association of disrespectful care with poor maternal outcomes needs to be studied. Maternal Mortality Rates among.Im/Mig.ant Populations in the United States

Introduction: Maternal mortality rates have more than doubled in the U.S over the last two decades, making.it one of the few places in the world where maternal mortality is increasing. Differences in maternal mortality among.certain races and ethnicities are known but few studies examine maternal mortality among.immig.ants. Since immig.ants represent 13.7% of the U.S. population, it is essential to examine immig.ant subsets to understand maternal mortality among.this vulnerable population. Methods: A literature search identified 318 articles on maternal mortality and immig.ants, with 12 articles from the U.S. The keywords included maternal mortality, United States, mig.ants, asylum seekers, immig.ants, and disparities. Maternal mortality statistics were obtained from the World Health Org.nization and Center for Disease Control. Results: Studies analyzed in this review found an overall lower maternal mortality rate among.immig.ant women compared to U.S.-born women, except for Hispanic immig.ant women. Black women had the hig.est maternal mortality rate, reg.rdless of immig.ation status. Conclusion: Althoug. the literature points to lower maternal mortality among.immig.ants, the data is still somewhat mixed, making.it challeng.ng.to draw comprehensive conclusions. Additional research examining.maternal mortality among.Im/mig.ants in the U.S. is needed to g.ide future training.among.healthcare professionals and policymakers.

Case Study: A Human Pre-Natal Experiment in 1944—“Do No Harm”

Iris du Pré, a professional pianist, wanted a second child, did not conceive quickly and was injected in 1944 by a doctor in Oxford with preg.ant mare serum g.nadotropin (PMSg.. The doctor joked “This child will be a racehorse winner!” In January 1945, Jacqueline du Pré, the remarkable, world-famous cellist was born. In the 1920's and 1930's, animal experimentation and clinical studies had shown that pituitary g.ycoproteins stimulated the ovary (follicle-stimulating.hormone, FSH) and the corpus luteum (luteal-stimulating.hormone, LH) which prepared the human womb for embedding.a fertilized ovum and that preg.ant mare’s blood and urine contained the g.ycoprotein, PMSg.whose orig.n was placental cells, but surprising.y in humans had the actions of both FSH and LH. However, the PMSg.serum alone did not bring.about preg.ancy. The doctor did not know that without subsequent injection of another factor in correct sequence and timing. PMSg.was pointless. In 1947, a placental g.ycoprotein, found in the 1920's in urine of preg.ant women (human chorionic g.nadotropin, hCg., when injected in mice subsequent to PMSg. achieved ovulation but not preg.ancy. Human application of those finding. was extremely risky due to impurities (up to 95%). The Federal Drug.Administration (FDA), established in 1938, requested easily bye-passed marketing.safety. Companies offered material “sufficiently” purified;professional bodies neg.ted clinical use, tempting.to a few. Evidence also sug.ests that, to sustain preg.ancy the doctor also prescribed the new “eostrog.n”, diethyl stilbestrol (DES) of neg.tive fame. In 1947, the Nuremberg.Code of ethics demanded human experiments by qualified personnel and trials preceded by adequate animal studies. It is not the case here. From five, du Pré had a most exceptional musical memory, almost obsessive musicality and a very difficult school-time socially. This paper examines early discussions of stock exchang.s by Max Weber, Liang.Qichao, and Kang.Youwei and considers their contemporaneity. Despite different contexts, the discussions shared a nineteenth-century preoccupation with g.obal competition and Darwinian strug.les for survival. All reveal the attendant anxieties of latecomer nations experiencing.belated modernity. Weber, however, wrote from a position that embraced g.rman colonialism, whereas Liang.and Kang.s advocacy of stock exchang.s was marked by concerns for the Chinese nation that emerg.d as a result of the experience of colonialism and economic imperialism.
