Objective To compare the pathogenicity of isolates of sequence type 7(ST-7)Neisseria meningitidis(***)belonging to four different serogroups(A,B,C,and X).Methods Four ST-7 *** isolates serogrouped as A,B,C,and X and characterized by different capsule structures,were examined for their adhesion and invasion properties,and their ability to induce cytokine release and apoptosis in the host cell(the A549 cell line).Results Among the four ST-7 *** isolates,the serogroup A isolate possessed the strongest adhesion and invasion *** isolate also induced the release of the highest levels of the proinflammatory mediators interleukin-6,interleukin-1β,and interferon,and the highest apoptosis rate in the host ***,there was no significant difference in interleukin-8 and tumor necrosis factor-αsecretion between the four *** on the findings,the serogroup X *** isolate had the weakest pathogenicity,whereas there was almost no difference in the pathogenicity of the isolates from serogroups B and *** The differences in the capsular structure of the four isolates of ST-7 *** affected their pathogenic *** findings also imply that the hyperinvasive ST-7 *** lineage may include hypoinvasive isolates.

Effects of GLUT4 expression on insulin resistance in patients with advanced liver cirrhosis

Decreased glucose tolerance and diabetes are frequently observed in advanced liver cirrhosis patients and may be related to insulin *** transporter-4 (GLUT4),one of the most important glucose transporters,plays a key role in the development of type 2 *** order to study the mechanism of insulin resistance in liver cirrhosis patients,we measured the insulin sensitivity index and determined the GLUT4 protein and mRNA contents of skeletal muscle by Western blotting and reverse transcription-polymerase chain reaction (RT-PCR),respectively,in normal people and liver cirrhosis *** results showed that the levels of glucose,insulin,and C-peptide in two liver cirrhosis groups were higher and the insulin sensitivity index lower than those of the normal control *** sensitivity of insulin may decrease with the decline of liver ***,the contents of GLUT4 protein and mRNA in patients with advanced liver cirrhosis were similar to those of normal *** conclusion,insulin resistance is observed in patients with advanced liver cirrhosis but may not be correlated with the skeletal contents of GLUT4 protein and mRNA.

Establishment and Modification of Ninety-seven Pneumococcal Serotyping Assays Based on Quantitative Real-time Polymerase Chain Reaction

Objective To establish and modify quantitative real-time polymerase chain reaction(qPCR)-based serotyping assays to distinguish 97 pneumococcal *** A database of capsular polysaccharide(cps)loci sequences was generated,covering 97 pneumococcal *** analyses were performed to identify the cps loci structure and target genes related to different pneumococcal serotypes with specific SNPs.A total of 27 novel qPCR serotyping assay primers and probes were established based on qPCR,while 27 recombinant plasmids containing serotype-specific DNA sequence fragments were constructed as reference target sequences to examine the specificity and sensitivity of the qPCR assay.A panel of pneumococcal reference strains was employed to evaluate the capability of pneumococcal *** A total of 97 pneumococcal serotyping assays based on qPCR were established and modified,which included 64 serotypes previously reported as well as an additional 33 ***-seven novel qPCR serotyping target sequences were implemented in the pneumococcal qPCR serotyping system.A total of 97 pneumococcal serotypes,which included 52 individual serotypes and 45 serotypes belonging to 20 serogroups,could not be identified as individual *** sensitivity of qPCR assays based on 27 target sequences was 1–100 copies/μ*** specificity of the qPCR assays was 100%,which were tested by a panel of 90 serotypes of the pneumococcal reference *** A total of 27 novel qPCR assays were established and modified to analyze 97pneumococcal serotypes.

We report a case of liver cell adenoma （LCA） in a 33-year-old female patient with special respect to its clonality status, pathogenic factors and differential diagnosis. The case was examined by histopathology, immunohistochemistry and a clonality assay based on X-chromosomal inactivation mosaicism in female somatic tissues and polymorphism at androgen receptor focus. The clinicopathological features of the reported cases from China and other countries were compared. The lesion was spherical, sizing 2 cm in its maximal dimension. Histologically, it was composed of cells arranged in cords, most of which were two-cell-thick and separated by sinusoids. Focal fatty change and excessive glycogen storage were observed. The tumor cells were round or polygonal in shape, resembling the surrounding parenchymal cells. Mitosis was not found. No portal tract, central vein or ductule was found within the lesion. The tumor tissue showed a positive reaction for cytokeratin （CK） 18, but not for CK19, vimentin, estrogen and progesterone receptors. Monoclonality was demonstrated for the lesion, confirming the diagnosis of an LCA. Clonality analysis is helpful for its distinction from focal nodular hyperplasia.K

Primary malignant melanoma of the liver is an exceedingly rare tumor. Only 12 cases have been reported in the worldwide literature. We present a case of isolated malignant melanoma of the liver occurring in a 36-year-old Chinese male patient. Comprehensive dermatologic and ophthalmologic examinations revealed no evidence of a cutaneous or ocular primary lesion. Other lesions in brain, respiratory tract, lung, gastrointestinal tract and anus, were not demonstrated by serial position emission tomography (PET). Microscopic examination of the resected specimen revealed a malignant melanoma, which was confi rmed by immunohistochemical staining for HMB-45, S-100 protein, melanoma-pan and vimentin. Moreover, electron microscopy demonstrated melanosomes in tumor cell cytoplasm. Our case shows that primary malignant melanoma may occur in the liver and should be considered when the histopathological appearance is not typical for other hepatic neoplasm.

Two case reports of emergent anesthesia of critical tracheal stenosis are presented. The use of extracorporeal circu-lation may be a lifesaving method for these patients. Two patients both with severe lower tracheal stenosis were admitted with severe inspiratory dyspnea. The first patient had a tracheal tube inserted above the stenosis in the operating room, but ventilation was unsatisfactory, high airway pressure and severe hypercarbia developed, therefore extracorporeal circulation was immediately initiated. For the second patient, we established femoral-femoral cardiopulmonary bypass prior to induction of anaesthesia, and intubated above the tracheal tumor orally under general anesthesia, then adjusted the endotracheal tube to appropriate depth after the tumor had been resected. The patient was gradually weaned from cardiopulmonary bypass. The two patients all recovered very well after surgery. Surgery is lifesaving for patients with critical tracheal stenosis, but how to ensure effective gas exchange is crucial to the anesthetic management. Extracorporeal circulation by the femoral artery and femoral vein cannulation can gain good gas exchange even if the trachea is totally obstructed. Therefore, before the induction of anesthesia, we should assess the site and degree of obstruction carefully and set up cardiopulmonary bypass to avoid exposing the patient to unexpected risks and the anesthesiologist to unexpected challenges.

Objective Campylobacter jejuni NCTC11168 is commonly used as a standard strain for flagellar biosynthesis *** this report,two distinguished phenotypic isolates(CJ1Z,flhA mutant strain,lawn;CJ2S,flhA complemented strain,normal colony)appeared during laboratory passages for *** Phenotypic assessments,including motility plates,transmission electron microscopy,biofilm formation assay,autoagglutination assay,and genome re-sequencing for these two isolates(CJ1Z,flhA mutant strain;CJ2S,flhA complemented strain)were carried out in this *** Transmission electron microscopy revealed that the flagellum was lost in *** assessments and genome sequencing of the two isolates were performed in this *** capacity for biofilm formation,colony auto-agglutination,and isolate motility was reduced in the mutant *** genomic analysis indicated a unique native nucleotide insertion in flhA(nt,2154)that caused the I719Y and I720Y mutations and early truncation in *** FlhA has been found to influence the expression of flagella in *** the best of our knowledge,this is the first study to describe the function of the C-terminal of this protein.
