To editor:This study reports a pregnant woman who was naturally conceived with monochorionic triamniotic(MCTA)combined with twin reversed arterial perfusion(TRAP).An ultrasound revealed that one of the fetuses had a cardiac malformation,and a connection between the umbilical arteries was suspected.To reduce the acardiac fetus,radiofrequency ablation was performed at 17+3 weeks of pregnancy.At 36+4 weeks,two healthy infants were delivered via elective cesarean section.Postoperative examination revealed a direct connection between the umbilical arteries,with the placental perfusion acting as a direct pump.It is essential to identify the intrauterine status of the blood-supplying infant and vascular connections in order to achieve successful outcomes in pregnancies with complications,such as edema and heart failure.The studies involving human participants were reviewed and approved by Human Research Ethics Committee of Tianjin Central Hospital of Obstetrics and Gynecology.The patient provided her written informed consent to participate in this study.

Systemic Immune-Inflammation Index (SII) and Neutrophil-Lymphocyte Ratio (NLR) as Systemic Inflammatory Predictors in the Diagnosis of Bullous Pemphigoid and Pemphigus Vulgaris

Introduction: Autoimmune blistering skin disorders such as Bullous Pemphigoid and Pemphigus Vulgaris present diagnostic challenges. The Systemic Immune-Inflammation Index (SII) and Neutrophil-Lymphocyte Ratio (NLR), are inflammatory markers used to assess the body’s immune-inflammatory response. Objectives: The study aims to evaluate the significance of hematologic markers, specifically the Systemic Immune-Inflammation Index (SII) and Neutrophil-Lymphocyte Ratio (NLR), as diagnostic predictors of bullous pemphigoid (BP) and pemphigus vulgaris (PV). Methods: A retrospective study of 64 patients (36 with BP and 28 with PV). Patient clinical data: age, gender, complete blood count, autoimmune antibody levels (Dsg1, 3 and BP180, 230), IgE and C-reactive protein, and history of hypertension, diabetes, brain infarction, and coronary heart disease. The data was analyzed using SPSS. Results: The study involved 36 (56.3%) diagnosed with bullous pemphigoid (BP) and 28 (43.75%) with pemphigus vulgaris (PV). The average age in BP was 71 ± 8 and 52 ± 13 in PV. Laboratory findings showed high levels of Dsg1, Dsg3, neutrophil count, and lymphocyte count in PV, while high levels of eosinophils with a significant increase in C-reactive protein (CRP) in BP. Blood biomarkers, including NLR, PLR, SII, MPV, CRP, and IgE, proved an overall of 84.4% in disease prediction. Dsg1, Dsg3, BP180, and BP230 showed an overall of 88.1%. No significant relationship was noted between NLR, SII, and patients with comorbidities. Conclusion: The study highlights the diagnostic potential of SII and NLR in addition to hematologic markers in BP and PV, emphasizing their role in early diagnosis and therapeutic interventions, requiring further validation in larger patient cohorts.

Objective:To explore the relationship between organizational identification,occupational stress,and insomnia among nurses and to analyze the moderating role of organizational identification between occupational stress and insomnia.Methods:Through stratified sampling,720 nurses working in a tertiary A-level hospital in Shandong Province from March to May 2021 were surveyed using an effort-reward imbalance questionnaire,an organizational identification questionnaire,and the Athens Insomnia scale.Results:Organizational identification was negatively correlated with occupational stress and insomnia(r_(1)=-0.121,P_(1)lieving occupational stress.

Regulation of PM_(2.5)on mitochondrial damage in H9c2 cells through miR-421/SIRT3 pathway and protective effect of miR-421 inhibitor and resveratrol

Fine particulate matter(PM_(2.5))exposure is associated with cardiovascular disease(CVD)morbidity and mortality.Mitochondria are sensitive targets of PM_(2.5),and mitochondrial dysfunction is closely related to the occurrence of CVD.The epigenetic mechanism of PM_(2.5)-triggered mitochondrial injury of cardiomyocytes is unclear.This study focused on the mi R-421/SIRT3 signaling pathway to investigate the regulatory mechanism in cardiac mitochondrial dynamics imbalance in rat H9c2 cells induced by PM_(2.5).Results illustrated that PM_(2.5)impaired mitochondrial function and caused dynamics homeostasis imbalance.Besides,PM_(2.5)up-regulated mi R-421 and down-regulated SIRT3 gene expression,along with decreasing p-FOXO3a(SIRT3 downstream target gene)and p-Parkin expression and triggering abnormal expression of fusion gene OPA1 and fission gene Drp1.Further,mi R-421 inhibitor(mi R-421i)and resveratrol significantly elevated the SIRT3 levels in H9c2 cells after PM_(2.5)exposure and mediated the expression of SOD2,OPA1 and Drp1,restoring the mitochondrial morphology and function.It suggests that mi R-421/SIRT3 pathway plays an epigenetic regulatory role in mitochondrial damage induced by PM_(2.5)and that mi R-421i and resveratrol exert protective effects against PM_(2.5)-incurred cardiotoxicity.

Controllable image generation based on causal representation learning

Artificial intelligence generated content(AIGC)has emerged as an indispensable tool for producing large-scale content in various forms,such as images,thanks to the significant role that AI plays in imitation and production.However,interpretability and controllability remain challenges.Existing AI methods often face challenges in producing images that are both flexible and controllable while considering causal relationships within the images.To address this issue,we have developed a novel method for causal controllable image generation(CCIG)that combines causal representation learning with bi-directional generative adversarial networks(GANs).This approach enables humans to control image attributes while considering the rationality and interpretability of the generated images and also allows for the generation of counterfactual images.The key of our approach,CCIG,lies in the use of a causal structure learning module to learn the causal relationships between image attributes and joint optimization with the encoder,generator,and joint discriminator in the image generation module.By doing so,we can learn causal representations in image’s latent space and use causal intervention operations to control image generation.We conduct extensive experiments on a real-world dataset,CelebA.The experimental results illustrate the effectiveness of CCIG.

Graph Convolutional Networks Embedding Textual Structure Information for Relation Extraction

Deep neural network-based relational extraction research has made significant progress in recent years,andit provides data support for many natural language processing downstream tasks such as building knowledgegraph,sentiment analysis and question-answering systems.However,previous studies ignored much unusedstructural information in sentences that could enhance the performance of the relation extraction task.Moreover,most existing dependency-based models utilize self-attention to distinguish the importance of context,whichhardly deals withmultiple-structure information.To efficiently leverage multiple structure information,this paperproposes a dynamic structure attention mechanism model based on textual structure information,which deeplyintegrates word embedding,named entity recognition labels,part of speech,dependency tree and dependency typeinto a graph convolutional network.Specifically,our model extracts text features of different structures from theinput sentence.Textual Structure information Graph Convolutional Networks employs the dynamic structureattention mechanism to learn multi-structure attention,effectively distinguishing important contextual features invarious structural information.In addition,multi-structure weights are carefully designed as amergingmechanismin the different structure attention to dynamically adjust the final attention.This paper combines these featuresand trains a graph convolutional network for relation extraction.We experiment on supervised relation extractiondatasets including SemEval 2010 Task 8,TACRED,TACREV,and Re-TACED,the result significantly outperformsthe previous.

Border Gateway Protocol(BGP)is a standard inter-domain routing protocol for the Internet that conveys network layer reachability information and establishes routes to different destinations.The BGP protocol exhibits security design defects,such as an unconditional trust mechanism and the default acceptance of BGP route announcements from peers by BGP neighboring nodes,easily triggering prefix hijacking,path forgery,route leakage,and other BGP security threats.Meanwhile,the traditional BGP security mechanism,relying on a public key infrastructure,faces issues like a single point of failure and a single point of trust.The decentralization,anti-tampering,and traceability advantages of blockchain offer new solution ideas for constructing secure and trusted inter-domain routing mechanisms.In this paper,we summarize the characteristics of BGP protocol in detail,sort out the BGP security threats and their causes.Additionally,we analyze the shortcomings of the traditional BGP security mechanism and comprehensively evaluate existing blockchain-based solutions to address the above problems and validate the reliability and effectiveness of blockchain-based BGP security methods in mitigating BGP security threats.Finally,we discuss the challenges posed by BGP security problems and outline prospects for future research.

Tumor-derived exosomes induce initial activation by exosomal CD19 antigen but impair the function of CD19-specific CAR T-cells via TGF-βsignaling

Tumor-derived exosomes (TEXs) enriched in immune suppressive molecules predominantly drive T-cell dysfunction and impair antitumor immunity. Chimeric antigen receptor (CAR) T-cell therapy has emerged as a promising treatment for refractory and relapsed hematological malignancies, but whether lymphoma TEXs have the same impact on CAR T-cell remains unclear. Here, we demonstrated that B-cell lymphoma-derived exosomes induce the initial activation of CD19-CAR T-cells upon stimulation with exosomal CD19. However, lymphoma TEXs might subsequently induce CAR T-cell apoptosis and impair the tumor cytotoxicity of the cells because of the upregulated expression of the inhibitory receptors PD-1, TIM3, and LAG3 upon prolonged exposure. Similar results were observed in the CAR T-cells exposed to plasma exosomes from patients with lymphoma. More importantly, single-cell RNA sequencing revealed that CAR T-cells typically showed differentiated phenotypes and regulatory T-cell (Treg) phenotype conversion. By blocking transforming growth factor β (TGF-β)-Smad3 signaling with TGF-β inhibitor LY2109761, the negative effects of TEXs on Treg conversion, terminal differentiation, and immune checkpoint expression were rescued. Collectively, although TEXs lead to the initial activation of CAR T-cells, the effect of TEXs suppressed CAR T-cells, which can be rescued by LY2109761. A treatment regimen combining CAR T-cell therapy and TGF-β inhibitors might be a novel therapeutic strategy for refractory and relapsed B-cell lymphoma.

Fast thermal responsive separators toward long-life and safe lithium metal batteries

lithium metal batteries(LMBs)have been extensively investigated during the past decades because of their ultrahigh energy densities.With the increasing demand for energy density,however,the safety issue of LMBs has become a significant challenge.In particular,localized areas of increased temperature(namely,hotspots)may be induced and even exacerbated within LMBs by uneven current distribution,internal short circuits,or inadequate heat dissipation,which significantly sacrifices battery safety and cycle life.Here,we report the rational design and fabrication of a fast thermal responsive separator capable of inhibiting the growth of lithium dendrites and mitigating thermal propagation,thereby reducing the risk of thermal runaway.The as-achieved separator comprises both an electrospun membrane using a phase change material with superior thermal-storage ability and a thermally conductive modification layer of hexagonal boron nitride nanosheets with a fast heat-transfer feature.It is demonstrated that such a unique integration of heat conduction and heat storage enables the functional separator with attractive abilities to mitigate hotspots and inhibit the growth of lithium dendrites upon the cycling of LMBs.Moreover,pouch cells with the thermal-responsive separator,as well as numerical simulations,verify much enhanced safety and cycle life of LMBs.This work may offer a new conceptual design of intelligent separators that acts as a functional unit encapsulated within a single cell to boost in-situ thermal management,which will help to develop high-safety and energy-dense LMBs.

Resolving co- and early post-seismic slip variations of the 2021 MW 7.4 Madoi earthquake in east Bayan Har block with a block-wide distributed deformation mode from satellite synthetic aperture radar data

On 21 May 2021(UTC),an MW 7.4 earthquake jolted the east Bayan Har block in the Tibetan Plateau.The earthquake received widespread attention as it is the largest event in the Tibetan Plateau and its surroundings since the 2008 Wenchuan earthquake,and especially in proximity to the seismic gaps on the east Kunlun fault.Here we use satellite interferometric synthetic aperture radar data and subpixel offset observations along the range directions to characterize the coseismic deformation of the earthquake.Range offset displacements depict clear surface ruptures with a total length of~170 km involving two possible activated fault segments in the earthquake.Coseismic modeling results indicate that the earthquake was dominated by left-lateral strike-slip motions of up to 7 m within the top 12 km of the crust.The well-resolved slip variations are characterized by five major slip patches along strike and 64%of shallow slip deficit,suggesting a young seismogenic structure.Spatial-temporal changes of the postseismic deformation are mapped from early 6-day and 24-day InSAR observations,and are well explained by time-dependent afterslip models.Analysis of Global Navigation Satellite System(GNSS)velocity profiles and strain rates suggests that the eastward extrusion of plateau is diffusely distributed across the east Bayan Har block,but exhibits significant lateral heterogeneities,as evidenced by magnetotelluric observations.The block-wide distributed deformation of the east Bayan Har block along with the significant co-and post-seismic stress loadings from the Madoi earthquake imply high seismic risks along regional faults,especially the Tuosuo Lake and Maqên-Maqu segments of the Kunlun fault that are known as seismic gaps.
