BACKGROUND Gastroesophageal varices are a rare complication of essential thrombocythemia(ET).ET is a chronic myeloproliferative neoplasm(MPN)characterized by an increased number of blood *** SUMMARY A 46-year-old woman,who denied a history of liver disease,was admitted to our hospital on presentation of *** examination revealed a hemoglobin level of 83 g/L,and a platelet count of 397×109/*** appearance of gastric and esophageal varices with red colored signs as displayed by an urgent endoscopy was followed by endoscopic variceal ligation and endoscopic tissue *** computed tomography revealed cirrhosis,marked splenomegaly,portal vein thrombosis and portal *** addition,bone marrow biopsy and evidence of mutated Janus kinase 2,substantiated the onset of *** patient was asymptomatic with regular routine blood testing during the 6-mo follow-up ***,in this case,gastroesophageal varices were induced by *** MPN should be given considerable attention when performing differential diagnoses in patients with gastroesophageal *** integrated approach such as laboratory tests,radiological examination,and pathological biopsy,should be included to allow optimal decisions and management.

Carbonate deposits in the Ordovivian Majiagou Formation are significant source rocks for natural gas generation in the Ordos Basin, northwestern China. Previous studies mainly focused on the organic matter enrichment mechanism of shales rather than carbonate rocks. The biological sources and paleoenvironment of carbonate source rocks, and the main controlling factors of organic matter enrichment in the carbonate source rocks were studied in this paper in combination with evidence from biomarkers, microfossils and inorganic geochemistry analysis. The results show that four types of microfacies were identified in the Majiagou Formation, respectively as: mud flat microfacies, mud-dolomite flat microfacies, dolomite flat microfacies, and open marine microfacies. The biological sources of organic matter are chiefly planktonic algae, followed by bacteria. The mud flat and mud-dolomite flat contain a high abundance of terrigenous detrital inputs, as indicated by the high content of Al_(2)O_(3), TiO_(2), Th, and Zr. The low Sr/Cu and high Rb/Sr values reveal warm and humid paleoclimate conditions in the mud flat and mud-dolomite flat, whereas the dolomite flat and open marine were likely formed in hot and arid paleoclimate conditions. The mud flat and mud-dolomite flat deposits were characterized by high paleoproductivity of the Majiagou Formation. Low Sr/Ba values were found in the mud flat samples, indicating fresh to brackish water condition, whereas samples of other facies have a relatively high degree of salinity. Based on U_(auth), Mo_(auth), Cr_(auth), Co_(auth), δCe, and δEu values, the mud flat microfacies was formed in a suboxic and anoxic environment, whereas the mud-dolomite flat, dolomite flat and open marine microfacies were within dysoxic to oxic conditions. A model of organic matter enrichment in the Majiagou Formation is thus established. Tectonism is one of the dominant factors affecting the shale pore structure. However, the control of shale pore structure by tectonic movements is still controversial, which limits the research progress of shale gas accumulation mechanism in the complex tectonic region of southern China. In this study, 34 samples were collected from two exploratory wells located in different tectonic locations. Diverse experiments,e.g., organic geochemistry, XRD analysis, FE-SEM, low-pressure gas adsorption, and high-pressure mercury intrusion, were conducted to fully characterize the shale reservoir. The TOC, Ro, and mineral composition of the shale samples between the two wells are similar, which reflects that the shale samples of the two wells have proximate pores-generating capacity and pores-supporting ***, the pore characteristics of shale samples from two wells are significantly different. Compared with the stabilized zone shale, the porosity, pore volume, and specific surface area of the deformed zone shale were reduced by 60.61%, 64.85%, and 27.81%, respectively. Moreover, the macroscopic and fine pores were reduced by 54.01% and 84.95%, respectively. Fault activity and uplift denudation are not conducive to pore preservation, and the rigid basement of Huangling uplift can promote pore preservation. These three factors are important reasons for controlling the difference in pore structure between two wells shales. We established a conceptual model of shale pores evolution under different tectonic preservation conditions. This study is significant to clarify the scale of shale gas formation and enrichment in complex tectonic regions, and helps in the selection of shale sweet spots.

Using an unbalanced panel data covering 75 countries from 1991 to 2019, the authors explore how the political risk impacts on food reserve ratio. The empirical findings show that an increasing political risk negatively affects food reserve ratio, and the same effects hold for both internal risk and external risk. Moreover, the authors find that the increasing external or internal risks both negatively affect food production and food exports, but external risk does not significantly impact food imports and it positively impacts food consumption, while internal risk negatively impacts food imports and food consumption. The results suggest that most governments have difficulty raising subsequent food reserve ratios in face of an increasing political risk, no matter if it is an internal risk or an external risk although the mechanisms behind the impacts are different.

A decline in mucosal vascularity is a histological hallmark of oral submucous fibrosis (OSF), a premalignant disease that is largely induced by betel quid chewing. However, the lack of available models has challenged studies of angiogenesis in OSF. Here, we found that the expression of thrombospondin 1 (THBS1), an endogenous angiostatic protein, was elevated in the stroma of tissues with OSF. Using a fibroblast-attached organoid (FAO) model, the overexpression of THBS1 in OSF was stably recapitulated in vitro. In the FAO model,treatment with arecoline, a major pathogenic component in areca nuts, enhanced the secretion of transforming growth factor (TGF)-β1 by epithelial cells, which then promoted the expression of THBS1 in fibroblasts. Furthermore, human umbilical vein endothelial cells (HUVECs)were incorporated into the FAO to mimic the vascularized component. Overexpression of THBS1 in fibroblasts drastically suppressed the sprouting ability of endothelial cells in vascularized FAOs (v FAOs). Consistently, treatment with arecoline reduced the expression of CD31in v FAOs, and this effect was attenuated when the endothelial cells were preincubated with neutralizing antibody of CD36, a receptor of THBS1. Finally, in an arecoline-induced rat OSF model, THBS1 inhibition alleviated collagen deposition and the decline in vascularity in vivo. Overall, we exploited an assembled organoid model to study OSF pathogenesis and provide a rationale for targeting THBS1.
