Recurrence of hepatocellular carcinoma following deceased donor liver transplantation:case series

Aim:We aimed to study the clinical and pathological characteristics of liver transplant recipients with hepatocellular carcinoma ***:We reviewed the data for 26 patients who had tumor recurrence after deceased donor liver transplant for hepatocellular carcinoma at the Johns Hopkins Hospital from January 2005 to December ***:In total,88%of recipients were *** mean age was 59 *** explant,poor differentiation was detected in 43%,while 73%had microvascular ***,62%were diagnosed to be outside of Milan *** of these,15%met the criteria for ***(77%)patients had pre-transplant alpha fetoprotein levels≥20 ng/*** 54%of patients,the location of hepatocellular carcinoma(HCC)recurrence was extrahepatic,followed by intrahepatic in 31%and both intra-and extrahepatic in 15%.The post-transplant tumor recurrence was diagnosed at a mean of 427 days(range 34-1502).Fifty percent of HCC recurrences were diagnosed within one year following liver ***(77%)patients received treatment for their recurrent HCC:external radiation(n=10),surgical resections(n=8;brain 4,spine 2,bone 1,and Whipple surgery 1),sorafenib(n=7),locoregional therapy(n=5).Overall,24 out of 26(92%)recipients died within four years after the ***:HCC recurrence after liver transplant is *** than fifty percent of HCC recurrences following liver transplant are *** better recipient selection for liver transplant,the curative options are limited in recurrent cases and associated with extremely poor outcomes.

Improving gastrointestinal scoring systems for predicting short-term mortality in critically ill patients

Shen et al’s retrospective study aims to compare the utility of two separate scoring systems for predicting mortality attributable to gastrointestinal(GI)injury in critically ill patients[the GI Dysfunction Score(GIDS)and the Acute Gastroin-testinal Injury(AGI)grade].The authors note that this study is the first proposal that suggests an equivalence between the ability of both scores to predict mor-tality at 28 days from intensive care unit(ICU)*** et al retrospec-tively analysed an ICU cohort of patients utilising two physicians administering both the AGI grade and GIDS score,using electronic healthcare records and ICU *** these physicians disagreed about the scores,the final decision as to the scores was made by an associate chief physician,or chief *** note that the primary reason for the development of GIDS was to create a clear score for GI dysfunction,with minimal subjectivity or inter-operator *** subjectivity inherent to the older AGI grading system is what ultimately led to the development of GIDS in *** ensuring consensus between physicians administering the AGI,Shen et al have controlled for one of this grading systems biggest *** have concerns,however,that this does not represent the real-world challenges associated with applying the AGI compared to the newer GIDS,and wonder if this arbitration process had not been instituted,would the two scoring systems remain equivalent in terms of predicted mortality?

Inter-and intraspecific variation in flight muscle fibers is associated with migratory timing

The life-history strategies of organisms are shaped by trade-offs among traits that influence survival, development, reproduction, and ultimately fitness. However, the specific physiological and environmental mechanisms driving population-level variation in phenotypic traits, particularly in relation to migratory adaptations, remain poorly understood. Elevational migration represents an adaptive behavior for mountain birds that offers a unique model to investigate adaptive phenotypes because of the diverse migratory strategies that have been observed within and between species. Flight muscles, as multifunctional organs, exhibit phenotypic variation that aligns with locomotor and thermoregulatory demands. Linking flight muscle phenotypes to migratory strategies can provide insights into inter-and intraspecific variation in migratory adaptations. In this study, we examined spring arrival dates at high-altitude breeding sites for three closely related high-altitude breeding birds in the mountains of Asia: Blue-fronted Redstart(Phoenicurus frontalis), Himalayan Bluetail(Tarsiger rufilatus), and Rufous-gorgeted Flycatcher(Ficedula strophiata). We analyzed relationships between arrival dates and phenotypic traits, including flight muscle physiology, morphology, and internal organ mass, alongside environmental effects on these traits. Our findings indicate that species arriving earlier encountered lower temperatures and exhibited reduced fiber cross-sectional areas coupled with higher fast oxidative glycolytic(FOG) fibers densities. Males displayed pronounced protandry and higher fast glycolytic(FG) fibers densities. Ambient temperature significantly influenced flight muscle phenotypes, highlighting intraspecific variation and the potential plasticity of flight muscle fibers as crucial mechanisms for adapting to migration strategies and environmental conditions. Grapevine(Vitis vinifera L.)is one of the most economically and culturally important perennial fruit crops in the world for fresh consumption and wine *** disease is one of the most important diseases in *** is difficult to control and causes great harm to grapevine physiology and significant economic losses in grape production(Endeshaw et al.,2014;Montero et al.,2016b;Wang et al.,2023;Yang et al.,2024).More than 80viruses have been reported to infect grapevines worldwide,and more than 20 grapevine viruses have been reported in China(Umer et al.,2019;Fan et al.,2020).Grapevine leafroll disease(GLD)is one of the most widespread and economically damaging viral diseases in grapevine around the world,and it can cause an30%-40%yield reduction per hectare without any control measures(Atallah et al.,2012).GLD is caused by a complex of virus species named grapevine leafroll-associated viruses(GLRaVs),among which GLRaV-3 is the most prevalent and economically important virus(Ling et al.,2004;Naidu et al.,2014;Martelli,2017).GLRaV-3 infection can impact vine growth,yield,berry weight,and fruit quality of some grapevine cultivars(Vega et al.,2011;Endeshaw et al.,2014;Alabi et al.,2016;Montero et al.,2016a,2016b;Cabaleiro et al.,2021;Lee et al.,2023).In addition to GLRaVs,other viruses have also been found to adversely affect grape growth and fruit *** example,Grapevine red blotchassociated virus(GRBV)can compromise crop yield and berry chemical composition,delay ripening,and damage the flavor and aroma properties of must and wine(Blanco-Ulate et al.,2017).

Aryloxypropanolamine is an essential structural scaffold for a variety of b-adrenergic receptor antagonists such as *** with such a structural motif tend to degrade into α,β ehydroxypropanolamine impurities via a radicaleinitiated oxidation *** impurities are typically polar and nonchromophoric,and are thus often overlooked using traditional reversed phase chromatography and UV *** this work,stress testing of metoprolol confirmed the generation of 3-isopropylamino-1,2-propanediol as a degradation product,which is a specified impurity of metoprolol in the European Pharmacopoeia(impurity N).To ensure the safety and quality of metoprolol drug products,hydrophilic interaction chromatography(HILIC)methods using Halo Penta HILIC column(150mm×4.6 mm,5 μm)coupled with charged aerosol detection(CAD)were developed and optimized for the separation and quantitation of metoprolol impurity N in metoprolol drug products including metoprolol tartrate injection,metoprolol tartrate tablets,and metoprolol succinate extended-release *** HILIC-CAD methods were validated per USP validation guidelines with respect to specificity,linearity,accuracy,and precision,and have been successfully applied to determine impurity N in metoprolol drug products.

Unsupervised learning of load signatures to estimate energy-related building features using surrogate modelling techniques

Characterization of an existing building’s energy-related features is critical to inform maintenance and retrofit ***,existing field-scale characterization methods tend to be labour intensive,invasive,and require high fidelity longitudinal data gathered through tightly regulated *** highlights the need for a low cost,scalable,and efficient screening *** paper puts forward a surrogate model-based approach to rapidly estimate energy-related building *** this end,EnergyPlus models for 12 midrise office archetypes,all with a rectangular footprint,are *** thousand variants of each archetype are generated by altering envelope,causal heat gain,and heating,ventilation,and air conditioning operation features.A unique load signature is derived for each variant’s heating and cooling energy *** parameters of the load signatures are clustered,then each cluster is associated with a set of plausible energy-related *** accuracy of the results was evaluated using five test buildings not seen by the *** method could effectively identify building features with reasonable accuracy and no significant degradation in performance across all 12 archetypes.
