Background and Objectives: The main benefits of laparoscopic surgery in comparison to open surgery involve the rapid discharge from recovery room, decreased postoperative hospital stays, reduced postoperative discomfort, easier getting back to work and faster return to ordinary daily life as well as cosmetic surgical wounds. The anesthesia type has an essential role in attenuation of the surgical stress and achievement of these advantages. We aimed to determine the outcome of giving general anesthesia in conjunction with thoracic epidural analgesia (TEA) compared to general anesthesia alone on stress response to surgery and anesthesia by investigating cytokine reaction (interleukin 6 and 8 levels), hemodynamic changes (BP, HR, RR, SPO2), and Visual Analogue Scale (VAS) scores postoperatively in patients subjected for laparoscopic cholecystectomy. Methods: This study included 40 patients aged 20 - 60 years old with American Society of Anesthesiologists physical status (ASA) I and II. They were planned for laparoscopic cholecystectomy at Aswan University Hospital from April 2017 to March 2018. They were randomly allocated into two groups. Group A (n. 20) received general anesthesia only and Group B (n. 20) received general anesthesia in conjunction with thoracic epidural analgesia using fentanyl and bupivacaine in the epidural catheter. AIM：To evaluate the efficacy of femtosecond laser assisted MyoRing intrastromal corneal implant and deep anterior lamellar keratoplasty （DALK） for management of moderate to advanced keratoconus regarding the degree of changes in visual acuity, refraction, corneal asphericity and ***：A prospective non comparative interventional case study was conducted in Ophthalmology Department,Ain shams University Hospital in the period from January 2015 to February 2017. The study included 30 eyes of moderate to advanced keratoconus. MyoRing was implanted in one eye （Group Ⅰ） and DALK operation was performed in the contralateral eye of the same patient （Group Ⅱ）.Preoperative and 6mo post-operative uncorrected visual acuity（UCVA）, corrected distance visual acuity （CDVA）, spherical equivalent （SE）, corneal and refractive astigmatisms,keratometry and Q-value using topography images were acquired. Quality of vision was assessed in all eyes including total corneal, anterior corneal high order aberrations analysis at 5 mm pupil size and the Strehl ratio of point spread function （PSF） as an objective measure of ***：Mean postoperative UCVA, CDVA, SE, refractive astigmatism,keratometryreadingsandasphericity were statistically improved compared to preoperative parameters in both groups （P〈0.05）. Significant reduction of all corneal aberrations following both techniques（P〈0.05） was achieved except mean trefoil and mean PSF in Group Ⅰ （P〉0.05）. Postoperative corneal aberrations were significantly lower in Group Ⅱ compared to Group Ⅰ.A statistically significant negative correlation was found in Group Ⅰ between the mean change in CDVA （logMAR）and the mean preoperative and mean postoperative total corneal aberrations root mean square （RMS; r=-0.78,P=0.04）. Also a statistically significant negative correlation was found between mean preoperative coma RMS and mean post PSF （r=-0.86, P=0.01）. Populus alba is a large woody deciduous *** plant has been introduced to shooting,then multiplication of rooting on Murashige and Skoog(MS)*** work was designed to estimate the effect of two factors(low levels of 1-Naphthaleneacetic acid NAA and sucrose)on *** response resulting in 6 treatments compared to the control,with twelve measured *** was a significant difference in some measurements in morphology,like plantlets fresh-weight,shoot-,root-length,and leaf *** the physiological measurements,there were significant differences in all the measured *** low concentrations of sucrose and media composition/power(MS grams/L)led to starvation in plants;however,these conditions led to enhancement in some morphological and physiological parameters to overcome the starvation effect,compared to the *** RAPD-PCR molecular marker(four decamers)was used to evaluate the new individuals’genetic variation(instability),resulting in a total polymorphism percentage of 50.83%.It was formerly known that the plantlets were identical to each other and to the mother *** this study,however,the use of distinct media power,hormonal and sucrose levels resulted in molecular variation reflected in ***’s morphological and physiological responses.

AIM： To find out the role of bacteria as a possible etiological factor in lymphocytic colitis. METHODS： Twenty patients with histopathological diagnosis of lymphocytic colitis and 10 normal controls were included in this study. Colonoscopic biopsies were obtained from three sites （hepatic and splenic flexures and rectosigmoid region）. Each biopsy was divided into two parts. A fresh part was incubated on special cultures for bacterial growth. The other part was used for the preparation of histologic tissue sections that were examined for the presence of bacteria with the help of Giemsa stain. RESULTS： Culture of tissue biopsies revealed bacterial growth in 18 out of 20 patients with lymphocytic colitis mostly Escherichia coil （14/18）, which was found in all rectosigmoid specimens （14/14）, but only in 8/14 and 6/14 of splenic and hepatic flexure specimens respectively. In two of these cases, E coli was associated with proteus. Proteus was found only in one case, Klebsiella in two cases, and Staphylococcus aureus in one case. In the control group, only 2 out of 10 controls showed the growth of E coli in their biopsy cultures. Histopathology showed rod-shaped bacilli in the tissue sections of 12 out of 14 cases with positive E coli in their specimen＇s culture. None of the controls showed these bacteria in histopathological sections. CONCLUSION： This preliminary study reports an association between E coli and lymphocytic colitis, based on histological and culture observations. Serotyping and molecular studies are in process to assess the role of E coli in the pathogenesis of lymphocytic colitis.

Molecular mechanisms of the anti-obesity potential effect of Moringa oleifera in the experimental model

Objective: To elucidate the molecular mechanisms of the potent anti-obesity effect of Moringa oleifera Lam.(M. oleifera) ethanolic extract and to clarify the link between these mechanisms and the associated metabolic and vascular risks in the experimental model of visceral ***: M. oleifera ethanolic extract was orally administered at 600 mg/kg body weight in obese female rats daily for 12 weeks. At the end of treatment, body weight was determined, and the atherogenic index, coronary artery index, glucose level, insulin resistance status, liver and kidney functions were assessed. Also, the m RNA of leptin, adiponectin and resistin in visceral adipose tissue was determined by quantitative real ***: The results showed that M. oleifera extract down-regulated m RNA expression of leptin and resistin, while it up-regulated adiponectin gene expression in obese rats relative to untreated obese control counterparts. This amelioration of genes expression was paralleled by a reduction in body weight and improvement of the atherogenic index and coronary artery index, as well as glucose level and insulin resistance value without adverse effects on liver or kidney functions, versus the untreated obese control ***: It is reasonable to assume that the anti-obesity, anti-atherogenic and antidiabetic properties of M. oleifera are mechanistically achieved via working directly on the adipokines of the visceral adipose tissue. Therefore, M. oleifera may be a good therapeutic candidate for the symptoms of metabolic syndrome.
