药艾灸对肿瘤化疗骨髓抑制和生活质量的临床研究

目的：通过药艾条灸加化疗与单纯化疗在防治肿瘤化疗患者骨髓抑制、生活质量的比较,观察药艾条灸法对肿瘤化疗患者骨髓抑制及功能状态的改善情况,探讨药艾条灸法是否具有减轻化疗对骨髓抑制、提高患者生活质量的作用,为药艾条灸在肿瘤化疗临床的应用提供依据。方法：病人来自江苏省中医院和江苏省中西医结合医院的肿瘤科。将60例符合纳入标准的肿瘤患者根据随机数字表法分为治疗组和对照组[各30例],剔除脱落病例后,治疗组26例,对照组27例。两组病人均用西医常规化疗方案,治疗组在常规化疗方案基础上加用药艾条灸神阙、关元和足三里,每次、每穴灸15分钟,每周灸6次,从患者化疗开始,至下次化疗为1个疗程[约3周],共治疗3个疗程。记录化疗前、三次化疗后的血象、功能状态及化疗后不良反应,比较两组治疗效果的差异。使用SPSS17.0进行观察数据的统计分析。结果：1.骨髓抑制情况：1.白细胞、中性粒细胞：治疗组三次化疗后的白细胞改变与对照组比较都有统计学意义(P0.05)。3.功能状态：治疗组的ZPS评分与化疗前比较明显升高,有显著统计学意义(P<0.05)。对照组化疗后比化疗前明显下降,有显著统计学意义(P<0.05)。化疗后治疗组患者全身功能状态明显好于对照组,有显著统计学意义(P<0.05)结论：1.药艾条灸能减慢白细胞、中性粒细胞、血小板、血红蛋白下降的速度和幅度,减轻化疗对骨髓的抑制、保护血细胞。2.该研究显示药艾条灸对红细胞的作用不明显。3.药艾条灸能提高患者的全身功能状态,提高生存质量。4.药艾条灸是安全有效的方法[研究中无不良事件发生]。

LactobacillusGG Supplementation on Anti-Helicobacter pyloriTherapy-Related Side Effects and Eradication Rates: A Meta-Analysis

Background: Concerns still exist with respect to unsatisfactory eradication rates and/or therapy-associated side effects for the use of standard triple therapy in the treatment of Helicobacter pylori infection, which prompts considerable interest in new therapy. We systematically reviewed the literature to investigate whether Lactobacillus GG as supplementation to standard triple therapy could improve H. pylori eradication rates and/or reduce therapy-associated side effects. Methods: PubMed, EMBASE, the Cochrane Central Register of Controlled Trials (CENTRAL) were systematically searched from their inception to August 4, 2015 for randomized controlled trials (RCTs). The language was restricted to English only. Results: Four RCTs involving a total of 305 participants (including 83 children) were included. Lactobacillus GG given along with triple therapy significantly reduced the risk of overall H. pylori therapy-related adverse effects (three RCTs, n = 221, RR 0.59, 95% CI 0.45 - 0.78), particularly of diarrhea (four RCTs, n = 285, RR 0.23, 95% CI 0.11 - 0.47), bloating (four RCTs, n = 289, RR 0.61, 95% CI 0.41 - 0.90), and taste disturbance (four RCTs, n = 288, RR 0.38, 95% CI 0.23 - 0.62). There were no significant differences between groups in the risk of other adverse effects. No beneficial effects of Lactobacillus GG were observed for H. pylori eradication rates (four RCTs, n = 284, RR 0.99, 95% CI 0.88 - 1.13). Conclusion: Current evidence indicates that Lactobacillus GG administered along with standard triple therapy is a feasible way to reduce therapy-related side effects, particularly diarrhea, bloating, and taste disturbance. However, Lactobacillus GG shows no effects on eradication rates.

We report a case of left pulmonary artery sling associated with patent ductus arteriosus and atrial septal defect in a 21-month-old child. 256-slice MDCT provides valuable information, such as abnormal origin of the left pulmonary artery, the relationship between pulmonary artery and airway, the diameter of the patent ductus artery and atrial septal defect. The information is helpful in diagnosis, pre-operative evaluation and post-operative follow-up of LPS.

The direct dumping of solid wastes into the rivers, discharge of industrial effluents together with direct discharge of domestic sewage have excessively polluted the major rivers Bagmati and Bish-numati. Groundwater along these river corridors is also affected from pollution of these rivers. Two major rivers: Bagmati and Bishnumati and shallow tube wells adjacent to these rivers were monitored for 2 years. Samples were analysed for the stable isotopes of hydrogen and oxygen (δD and δ18O) and selected physico-chemical parameters to investigate the possible interrelationship between river water and shallow groundwater along these river corridors. The physico-chemical values revealed that shallow groundwater and river water along the Bishnumati River corridor were heavily mineralized due to direct discharge of sewage wastes into this river. The isotope compositions of river water and shallow groundwater clustered together revealed possible interrelationship between them. Some of the isotopic compositions of groundwater and river water deviated below the Local Meteoric Water Line (LMWL) indicating that the water has undergone evaporation. The isotopic and chemical results suggested possible interrelationship between river water and groundwater. Fractional contribution of the river water to groundwater was calculated based on isotopic data using mass balance approach. Results showed that shallow groundwater SG1, along the Bagmati River corridor (in September 2013), was composed of approximately 30% - 40% Bagmati River water. Similarly, shallow groundwater SG5 of Bishnumati River corridor (in September 2013), was composed of approximately 45% - 50% river water. This result indicated that high portion of river water mixed-up with adjoining shallow groundwater along the river corridors. Introduction:Mountains occupy 24%of the global land surface and are home to 12%of the world’s population;they host the world’s principal biomes and supply a diverse array of ecosystem *** they are also highly sensitive to both natural and human induced changes,which can affect service provision meant for one third of *** research on impacts of land cover change from mountains at the local scale are limited,especially in developing countries,which poses a barrier to informed planning for sustainable *** being in the forefront as contributor towards global conservation goals,a case study following a systematic research framework considering quantitative,qualitative and geospatial tools was ***:Respondents identified and ranked 24 ecosystem services,among which the provisioning services were the most important,followed by cultural,regulating,and supporting *** was the most important land cover type for services,followed by marsh,scrub,water bodies,and *** area of forest declined by 2%and marsh by 7%over the 32-year period,suggesting a potential decrease in ecosystem services,which was also observed by the local ***:It was observed that 80%of the local people are still directly dependent on the diverse ecosystems for *** subtle,the decrease in land cover has implications for livelihoods of the people and the rich biodiversity of the *** communities for local development needs guidance for identifying trade-offs in land cover types in sustainable *** integrated and holistic approach focusing on both conservation and community development should be used to manage and develop the valley and the region ***:We recommend diversifying the limited livelihood strategies(potato farming and livestock grazing)to reduce the potential vulnerability of the local community and reduce the pressure on forest and *** land use planning

Aim: To evaluate the comparative efficacy of Polymer bur (Smartprep) and Conventional burs (Carbide bur & Diamond Points) in selective Dentin caries removal. Materials and method: 15 freshly extracted carious human premolars and molars were selected for this study. These teeth were split in the center of carious lesion, in order to obtain two corresponding half. Thus 30 samples were prepared in this way. These were randomly divided in to 3 groups of 10 samples each. Group 1: The carious portion of these samples was excavated with the help of polymer bur Smartprep (SS White). Group 3: Prepared with Tungsten carbide round bur (No. 018). Group 3: Prepared with the help of Diamond points (No. 018). The period of time involved in this process is measured. After excavation of all carious lesions, a longitudinal section from mid most part of lesion was obtain with the help of diamond disc at slow speed. After staining with caries detector dye “Seek”, the sections were subjected to histological evaluation under stereomicro-scope. The thickness of remaining caries is measured in more or less than 1 mm. Results: The difference in grading of remaining carious dentin between Polymer bur and Conventional burs were statistically non- significant, but Polymer bur taken more time in comparison to Conventional burs. Conclusion: Smartprep bur is more conservative in selectively dentin caries removal than Conventional burs but same time it is more time consuming than Conventional burs.

目的探讨慢性家族性良性天疱疮(CFBP)的临床、病理学特点,以提高对本病的认识。方法分析经病理学确诊的16例CFBP的临床资料、组织病理及治疗。结果男10例,女6例,平均年龄37.8岁,平均病程10.56年,发病年龄3～52岁;7例有家族史。皮损好发于腹股沟、腋窝、肛周,泛发者1例;表现为在红斑/外观正常皮肤上出现水疱或丘疹伴糜烂渗液,呈扁平柔软、湿润增殖面;1例合并银屑病。组织病理学检查16例均见棘层松解如倒塌砖墙样,11例伴角化过度、松解上方灶性角化不全,13例见角化不良细胞,9例角质层/棘层上部见圆体,5例见谷粒,12例见棘层松解边缘棘层肥厚及表皮突延长;直接免疫荧光检查均阴性。口服小剂量强的松等治疗约2周可以改善临床症状,但易反复。结论本病病程长,43.75%有阳性家族史,临床易误诊为湿疹或增殖型天疱疮,组织病理学检查发现倒塌砖墙样棘层松解及直接免疫荧光检查为阴性有诊断及鉴别诊断价值,但缺乏有效治疗。
