Calcarea carbonica treatment rescues lipopolysaccharide-induced inflammatory response in human mononuclear cells via downregulation of inducible cyclooxygenase pathway

Objective: Prolonged use of nonsteroidal anti-inflammatory drugs is associated with severe side effects and toxicity. Therefore, we studied the anti-inflammatory role of Calcarea carbonica which had minimal toxicity at the low ***: THP-1 human mononuclear cells were treated with C. carbonica to evaluate the 50% cytotoxicity concentration(CC_(50)) and 50% effective concentration(EC_(50)). Cell survival was evaluated in lipopolysaccharide-stimulated C. carbonica-treated cells. Nitric oxide(NO) and tumor necrosis factor-a(TNF-a) were measured to evaluate the anti-inflammatory activity of C. carbonica. Cyclooxygenase-2(COX-2) protein expression was determined by Western blotting analysis, and the interaction of C. carbonica with the COX-2 protein was evaluated using molecular docking ***: The CC_(50) and EC_(50) of C. carbonica were found to be 43.26 and 11.99 mg/mL, respectively. The cell survival assay showed a 1.192-fold(P = 0.0129), 1.443-fold(P = 0.0009) and 1.605-fold(P = 0.0004)increase in cell survival at 24, 48 and 72 h after initiating C. carbonica treatment, respectively. C. carbonica-treated cells showed a reduction in NO levels by 2.355 folds(P = 0.0001), 2.181 folds(P = 0.0001) and 2.071 folds(P = 0.0001) at 24, 48 and 72 h, respectively. The treated cells also showed a reduction in TNFa levels by 1.395 folds(P = 0.0013), 1.541 folds(P = 0.0005) and 1.550 folds(P = 0.0005) at 24, 48 and72 h, respectively. In addition, a 1.193-fold reduction(P = 0.0126) in COX-2 protein expression was found in C. carbonica-treated cells. The molecular docking showed interaction of C. carbonica with the phenylalanine 367 residue present in active site of ***: C. carbonica exhibited anti-inflammatory properties in lipopolysaccharide-stimulated cells by significantly reducing NO production and TNF-a level through downregulation of the COX-2 protein. This effect is probably mediated through interaction of C. carbonica with the phenylalanine

Management and outcomes of gastric volvulus in children: a systematic review

Barrett's esophagus in children: what is the evidence?

Background This study systematically reviewed etiology, prevalence, treatment and outcome of Barrett's esophagus (BE) in the pediatric population. Methods PubMed? was searched for terms 'Barrett's esophagus' and 'children'. End points were age of patients, etiology, association with other syndromes, treatment, incidence of carcinoma and outcome. This review was conducted according to the PRISMA guidelines. Data were collected, entered and analyzed into a Microsoft Excel? spreadsheet database. Results Search revealed 278 articles published between 1984 and 2017, of which 18 met the inclusion criteria. There were 130 patients for analysis with a mean age 10.6 years (0.8–17.2 years). BE was diagnosed in 80 patients with confirmed gastroesophageal reflux (GER) only;further 20 patients were neurologically impaired and had GER, 13 after esophageal atresia (EA) with or without tracheoesophageal fistula (TEF) repair with associated GER, 6 post-chemotherapy, 1 after post caustic burns, 1 after esophageal replacement with stomach, 1 after peptic esophageal stricture, 1 with secretory diarrhea, 1 with Fanconi anemia, 1 tetralogy of Fallot, and 5 healthy children. Regarding treatment, 26 were on medical treatment only, 16 had surgeries combined with medical treatment, 80 patients underwent surgery only, 1 was on diet management, 4 were on surveillance only and 2 were never treated for BE as death occurred because of associated conditions. Fundoplication was the most commonly performed surgery (82.2%). Adenocarcinoma was found in one 23-year-old patient. Mean follow-up was 3.45 years (10 months–13 years) and long-term outcome showed recurrences in 8 and esophago-mediastinal fistula and proximal esophagus ulcer in 1. There were 7 lethal outcomes which were not directly associated with BE. Conclusions Although BE is considered a premalignant condition;incidence of carcinoma in pediatric population is low. Long-term follow-up with endoscopies and biopsies seems to be advisable for

Pigeon chest: comparative analysis of surgical techniques in minimal access repair of pectus carinatum (MARPC)

Background After minimally invasive repair for pectus excavatum (MIRPE), similar procedures for pectus carinatum were developed. This study aimed to analyse the various published techniques of minimal access repair for pectus carinatum (MARPC) and compare the outcomes. Data sources Literature was reviewed on PubMed with the terms 'pectus carinatum', 'minimal access repair', 'thoracos-copy' and 'children'. Results Twelve MARPC techniques that included 13 articles and 140 patients with mean age 15.46 years met the inclusion criteria. Success rate of corrections wasn = 125, about 89% in cumulative reports, with seven articles reporting 100%. The complication rate was 39.28%. Since the pectus bar is placed over the sternum and has a large contact area, skin irritation was the most frequent morbidity (n = 20, 14.28%). However, within the complication group (n = 55), wire breakage (n = 21, 38.18%) and bar displacement (n = 10, 18.18%) were the most frequent complications. Twenty-two (15.71%) patients required a second procedure. Recurrences have been reported in four of twelve techniques. There were no lethal outcomes. Conclusions MARPC techniques are not standardized, as MIRPE are, so comparative analysis is difficult as the only common denominator is minimal access. Surgical morbidity is high in MARPC and affects > 2/3rd patients with about 15% requiring surgery for complication management.

Treatment of wheat straw using tannase and white-rot fungus to improve feed utilization by ruminants

Background： Current research to enrich cattle feed has primarily focused on treatment using white rot fungi, while there are scarce reports using the enzyme tannase, which is discussed only in reviews or in the form of a hypothesis. In this context, the aim of the present study was to evaluate the effect of tannase on wheat straw （WS） and also the effect of lyophilized tannase at concentrations of 0.1%, 0.2%, and 0.3% （w/w） on WS followed by fermentation with Ganodermo sp. for 10 d and compared in relation to biochemical parameters, crude protein （CP） content, and nutritional value by calculating the C/N ratio in order to improve the nutritional value of cattle feed. Results： Penicillium charlesii, a tannase-producing microorganism, produced 61.4 IU/mL of tannase in 54 h when 2% （w/v） tannic acid （TA） was initially used as a substrate in medium containing （% w/v） sucrose （1.0）, NaNO3 （1.0）, and MgSO4 （0.08 pH, 5.0） in a 300-L fermentor （working volume 220 L）, and concomitantly fed with 1.0% （w/v） TA after 24 h. The yield of partially purified and lyophilized tannase was 5.8 IU/mg. The tannin-free myco-straw at 0.1% （w/w） tannase showed 37.8% （w/w） lignin degradation with only a 20.4% （w/w） decrease in cellulose content and the in vitro feed digestibility was 32.2%. An increase in CP content （up to 1.28-fold） along with a lower C/N ratio of 25.0%, as compared to myco-straw, was obtained. Conclusions： The use of tannin-free myco-straw has potential to improve the nutritional content of cattle feed. This biological treatment process was safe, eco-friendly, easy to perform, and was less expensive as compared to other treatment methods.

The excitation function of the 58Ni(n,p)58Co reaction was measured using the well-established neutron activation technique andγ-ray *** in the energy range of 1.7 to 2.7 MeV were generated using the 7Li(p,n)*** neutron flux was measured using the standard 115In(n,n’)115mIn monitor *** results of the neutron spectrum averaged cross-section of 58Ni(n,p)58Co reactions were compared with existing cross-section data available in the EXFOR data library as well as with various evaluated data libraries such as ENDF/B-VIII.0,JEFF-3.3,JENDL-4.0,and *** calculations were performed using the nuclear reaction code *** nuclear level density(NLD)models were tested,and their results were compared with the present *** NLDs were also obtained through the spectral distribution method(SDM).The cross-section results,along with the absolute errors,were obtained by investigating the uncertainty propagation and using the covariance *** forγ-ray true coincidence summing,low-energy background neutrons,andγ-ray self attenuation were *** experimental cross-section obtained in the present study is consistent with previously published experimental data,evaluated libraries,and theoretical calculations carried out using the TALYS code.

Triptolide and management of systemic malignancies besides pancreatic carcinomas

The recent article by Zhou et al was highly interesting and thought provoking. The authors have clearly shown that triptolide administration is associated with upregulation of the Bax gene, resulting in an attenuating effect on cell growth in gastrointestinal malignancies such as pancreatic carcinomas. The article by Zhou et al is all the more important because it highlights the rapidly increasing role of triplodide in the management of systemic malignancies. For instance, triptolide acts on the PI3k/Akt/NF-κB pathway, thereby enhancing apoptosis secondary to the administration of bortezomib in multiple myeloma cells. Similar synergisms are seen when triptolide is administered along with 5-fluoruracil for the management of colonic carcinomas. Similarly, triptolide causes down-regulation of the Bcl-2 gene, resulting in control of cell growth in tumors, such as glioblastoma multiformes.
