Software-Defined Networking(SDN)is a new network technology that uses programming to complement the data plane with a control *** enable safe connection,however,numerous security challenges must be *** attacks have been one of the most prominent risks on the internet for decades,and they are now becoming challenging difficulties in SDN *** solve these challenges,we proposed a unique firewall application built on multiple levels of packet filtering to provide a flooding attack prevention system and a layer-based packet detection *** study offers a systematic strategy for wrapping up the examination of SDN *** Mininet simulator examines the effectiveness of SDN-based firewalls at various network *** fundamental network characteristics that specify how SDN should *** three main analytical measures of the network are jitter,response time,and *** regular operations,their behavior evaluates in the standard SDN conditions of Transmission Control Protocol(TCP)flooding and User Datagram Protocol(UDP)flooding with no SDN *** Orbit Ion Cannon(LOIC)is applied to launch attacks on the transmission by the allocated *** and MATLAB are used for the behavioral study to determine how sensitive the parameters are used in the SDN network and monitor the fluctuations of those parameters for different simulated scenarios.

A consensus meeting of national experts from all major national hepatobiliary centres in the country was held on May 26,2023,at the Pakistan Kidney and Liver Institute&Research Centre(PKLI&RC)after initial consultations with the *** Pakistan Society for the Study of Liver Diseases(PSSLD)and PKLI&RC jointly organised this *** effort was based on a comprehensive literature review to establish national practice guidelines for hilar cholangiocarcinoma(hCCA).The consensus was that hCCA is a complex disease and requires a multidisciplinary team approach to best manage these *** coordinated effort can minimise delays and give patients a chance for curative treatment and effective *** diagnostic and staging workup includes high-quality computed tomography,magnetic resonance imaging,and magnetic resonance *** cytology or biopsy utilizing endoscopic retrograde cholangiopancreatography is a mainstay for ***,histopathologic confirmation is not always required before *** ultrasound with fine needle aspiration of regional lymph nodes and positron emission tomography scan are valuable adjuncts for *** only curative treatment is the surgical resection of the biliary tree based on the Bismuth-Corlette *** patients with unresectable hCCA can be considered for liver *** chemotherapy should be offered to patients with a high risk of *** use of preoperative biliary drainage and the need for portal vein embolisation should be based on local multidisciplinary *** with acute cholangitis can be drained with endoscopic or percutaneous biliary *** chemotherapy with cisplatin and gemcitabine has shown improved survival in patients with irresectable and recurrent hCCA.

A novel tetrapolymer(TP) consisting of carboxylate, sulphonate, phosphonate and sulfur dioxide based comonomers was synthesized using Butler cyclopoymerization technique. The synthesized tetrapolymer was characterized using FTIR,1H-NMR,13CNMR and elemental analysis. The performance of the tetrapolymer as a corrosion inhibitor for St37 carbon steel in 15% HCl and 15% H2SO4 acid media was assessed using electrochemical impedance spectroscopy(EIS), linear polarization resistance(LPR), potentiodynamic polarization(PDP) and electrochemical frequency modulation(EFM) techniques. The influence of addition of a small amount of KI on the corrosion inhibition efficiency of TP was also assessed. Results obtained showed that the tetrapolymer moderately inhibited the corrosion of St37 steel in the acid media with protection efficiency of 79.5% and 61.1% at the optimum concentration of 1000 mg·L^-1 studied in HCl and H2SO4 media respectively. On addition of 5 mmol·L^-1 KI to the optimum tetrapolymer concentration, the protection efficiency was upgraded to 90.6% and 93.5% in HCl and H2SO4 environment, respectively. The enhanced performance of the polymer in the presence of KI is due to synergistic action deduced from synergism parameter(S1) which was found to be greater than *** tetrapolymer afforded the corrosion inhibition of St37 steel in the acid media by virtue of adsorption of the polymer molecules on the steel surface which was confirmed by ATR-FTIR analysis of the adsorbed film extracted from the steel surface. TP + KI formed complex with St37 steel surface in H2SO4 solution but not in HCl solution.

Dear Dr. Martin Hovland, We learned from the literature that premixed insulins are short-acting insulin or rapid-acting insulin analogue mixed with intermediate-acting insulin in a fixed ratio, addressing FBG and PPBG in one injection. There are two categories;high-mix and low-mix premixed insulins. We, a Saudi task force, gathered to develop an explicit, evidence-based consensus for the use of the low-mix premixed insulin for better glycemic control. The treatment with premixed aspart 30 was non-inferior to treatment with premixed insulin lispro 25. In addition, Self-monitored blood glucose levels were comparable. Safety profiles were similar between both treatments, as was the incidence of hypoglycemic episodes. The switch between both products of the low-mix family can be carried out without any problem. Both products of the low-mix premixed insulin analogues aspart 30/70 and premixed insulin lispro 25/75 have comparable efficacy and safety as shown from the medical literature. Therefore, we can change from one to another safely as demonstrated by the US FDA statement. In addition, the ergonomic features of KwikPen’s design and function may offer important advantages for the user during insulin administration.

Stroke as a Presenting Feature of COVID-19

Objective: To analyze the coronavirus disease-2019 (COVID-19) patients presenting with acute stroke by determining their clinical characteristics, hospitalization course and prognosis. The common and conventional stroke risk factors in these patients were assessed with the aim of determining the role and contribution of the COVID-19 infection to stroke pathogenesis. Methods: Retrospective observational study involving 24 patients from a single tertiary care center over a time period of three months. Risk factors such as Age, Hypertension, Diabetes Mellitus, smoking status and underlying cardiac history were analyzed. COVID-19 relevant laboratory and radiological data were documented. Results: 87.5% of patients had ischemic stroke, with 58.3% of total patients being younger than 55 years. An equal incidence of both Diabetes Mellitus and Hypertension (37.5%) was identified. 29.2% were completely asymptomatic for COVID-19, of which 85.7% had no chest X-ray changes on admission. Eight patients (61.5%) developed pneumonia during admission despite an initially normal chest X-ray. Conclusion: Patients without COVID-19 symptoms and with normal chest radiography presenting with stroke does not rule out a possible underlying COVID-19 infection. Such patients may be positive for the virus and may go on to develop pneumonia shortly after suffering from strokes. This could suggest that stroke in COVID-19 patients is a possible initial presenting feature and consequence of the inflammatory state triggered by the Severe Acute Respiratory Syndrome Coronavirus-2 (SARS-CoV-2) infection. It is imperative to analyze the association of COVID-19 and stroke, and to maintain a high index of suspicion of COVID-19 infection in stroke patients, to enhance early detection and reduce transmission.

Early Hyperglycemia in Diabetics as a Predictor of Disease Severity in COVID-19

Background: Hyperglycemia is an important predictor of prolonged hospitalization and mortality. The objective of this study was to analyze the role of early hyperglycemia in predicting the severity of COVID-19 illness and mortality. Method: Retrospective study on 259 patients with COVID-19 with measurement of average glucose during 24 hours of admission. Association of Early Hyperglycemia categorized as 180 mg/dl;was studied with mortality, requirement of ICU stay and prognostic markers. Results: Early hyperglycemia (>180 mg/dl) revealed a higher median value for CRP (109 mg/l [IQR;48 - 199]) in comparison to 180 mg/dl group;24 (15.9%) vs 14 patients (13%) [p = 0.51] in the 180 mg/dl was 8 (5.3%) while Conclusion: Hyperglycemia in the first 24 hours may anticipate an increased risk of ICU stay and a higher mortality rate. It may be a possible predictor of disease severity.
