OBJECTIVE: United Nations Relief and Works Agency for Palestine Refugees in the Near East (UNRWA) provides primary health care services including care for diabetes and hypertension, with limited resources under difficult circumstances in Gaza, West Bank, Jordan, Lebanon and Syria. A total of 114,911 people with diabetes were registered with UNRWA health centres in 2011. The aim of this cross-sectional observational study was to assess the quality of diabetes care in the UNRWA primary health care centres. METHOD: The study population consisted of 1600 people with diabetes attending the 32 largest UNRWA health centres and treated there for at least one year. Between April and Sept. 2012 data from medical records, including results of clinical examinations and laboratory tests performed during the last one year, current management including self-care education and evidence of diabetes complications were collected and recorded in a previously validated data collection form (DCF). Patients were interviewed and clinically examined on the day of the audit and blood collected for HbA1c testing which was done at a central lab using High-performance liquid chromatography (HPLC) method (HLC®-723G8 Tosoh Corporation, Japan). Data was transferred from paper records into a computer and analysed with Epi-info 2000. RESULTS: Type 1 diabetes was present in 4.3% and type 2 diabetes in 95.7%. Co-morbid hypertension was present in 68.5%;90.3% were either obese (64.0%) or overweight (26.3%). Clinical management of diabetes was largely in line with UNRWA’s technical instructions (TI) for diabetes. Records for 2 hour postprandial glucose (2 h PPG), serum cholesterol, serum creatinine, and urine protein analysis were available in 94.7%, 96.4%, 91.4% and 87.5%, cases, respectively. Records of annual fundoscopic eye examination were available in 47.3% cases but foot examinations were less well documented. Ruptured ectopic pregnancy is one of the outcomes of mis or delayed diagnosis and the massive hemorrhage resulting is responsible for more than three quarters of the first trimester maternal death. Simultaneous presence of intrauterine pregnancy adds obstacles for the correct diagnosis to take place. The management of this condition is a real dilemma as life threatening condition could be the only presentation which needs immediate intervention. The high index of suspicion is mandatory to ensure timely diagnosis and management to prevent stormy outcome.

采用粘合节点连接各种结构构件日益增多。只有充分了解粘合节点的性能,才能保证这种节点的功效、安全和可靠性。有些节点的构造,如单、双搭接接头受到了重视,但单盖板的构造并没有得到重视,部分原因是早期的研究显示这种节点效率低。本文的目的之一是论证只要经过合理设计,盖板节点与搭接接头同样有效。这项工作通过详细分析粘合剂中峰值应力的参数影响来完成。另一个目标是提出一个简单的方程,能够使盖板节点的设计便利及有效。为此,对解析表达式进行了简化并给出精确的结果,并对影响边缘力的参数有了进一步的理解。

Drug addiction is one of the current world’s hygienic, mental and social problems. There are different treatments for this illness. MMT (Methadone Maintenance Therapy) is considered as a standard and acceptable therapy for addiction;and there are many affecting factors regarding gaining success in this type of treatment. In this study, we investigated some of these factors. One of the most important issues in MMT is to keep up patient on his/her treatment till complete abstinence;therefore, it is recommended that the patient does not leave his/her therapy without discretion of the therapist. In this study, 230 patients, who began MMT, and admitted to addiction treatment centers of Yazd province were included. In this study, the survival rate in the first, third, and sixth months were evaluated as 93.9%, 63.48%, and 28.36%, respectively. It was found that following parameters are important in rate of survival during treatment: family support, gender, dose of methadone and concomitant use of morphine during treatment. Moreover, factors like level of education, marital status, age and concomitant use of methamphetamine during treatment can be effective in this regard.

This research investigated the water qualities and distribution of heavy metals concentration in rain, river, ground and supply waters, providing a baseline in Dhaka Urban, Bangladesh, against which future changes can be assessed. The water sample was collected in the Dhaka central zone, from a site located in the center of the Bengal Delta plain between 23䓨'N - 23䓶'N latitude and 90䓔'E - 90䓟'E longitude which is affected by environmental pollution for high traffic, industrialization and because of its rapid urbanization. The water samples were analyzed for heavy metals by Inductively Coupled Plasma Mass-Spectrometry (ICP-MS), major ions by Ion-Chromatography (IC), water tracer (δ18O and δD) by Mass-Spectrometry (MS), following the standard techniques. Elevated EC and chloride (Cl-) are the markers of wastewater, providing an indication of impacts in the Buriganga River water. The relative concentrations of major ions were presented in the form of Piper and Stiff Diagram. The color maps were constructed to present ions distribution over the areas. The stream reaches directly in Buriganga River from leather industries which may reflect the naturally low dissolved oxygen (DO) 0.3 - 3.5 (mg/L), where aquatic life can’t survive. Uranium contents U238 (0.1 - 2.7 μg/L) are not exceeding the WHO guideline level of 30 μg·L-1. Significant portion of Cs133 (0.05 - 0.32 μg/L), Se (0.03 - 0.12 μg/L), B (50 - 2986 μg/L) and Cr (0.55 - 18.59 μg/L), could be increased in Buriganga River areas by the effect of local sources like leather industry. Zn (10 - 1466 μg/L) in the Dhaka Rain and Pb (4 - 6 μg/L) in Dhaka Water occur mainly in industrial and the high traffic areas, respectively. The complexation of gallium with 2-hydroxy-5-T-butylphenol-4’-methoxy-azobenzene (HR) has been studied by atomic absorption and spectrophotometric methods. The optimal conditions for the formation and extraction of the complex were found. The maximum light absorption of the complex in n-butanol is in the range of 450 - 470 nm. The molar absorption coefficient is (3.3 - 4.2)⋅104. The stability constant of the gallium coordination compound in n-butanol is βl = 4.2⋅1010. The developed technique allows to determine the gallium content within n × 10−1 - n × 10−4%. The selective and sensitive technique for the extraction-atomic absorption determination of gallium in soils has been developed.

The complex of molybdenum (VI) with stilbene-2,2’-disulfonic acid-4,4’’bis (azo-1’’)-3’’, 4’’ dioxybenzene (stillbazo) in the presence of a cationic surfactant cetylpyridinium chloride (CPCl) was studied spectrophotometrically. It was found that in the presence of CPCl, a mixed-ligand complex with a component ratio of 1:2:4 is formed. The effect of pH, time, temperature, the concentration of reagents, and CPCl in the formation of homogeneous ligand and mixed-ligand complexes were studied. The stability constants (lgβk = 86 × 108) and the molar absorption coefficient of the mixed-ligand complex (εк = 11,835) were determined. It was found that Beer’s law is observed at a concentration of 0.05 - 17.00 μg/ml. A method has been developed for the spectrophotometric determination of molybdenum (VI) in complex industrial facilities.

1 IntroductionCulture systems capable of expanding and/or maintaining hematopoietic stem cells will not only facilitate our understanding of stem cell biology, but also broaden clinical applications. Among various in vitro hematopoietic culture systems, co-cultures of marrow or CD34+ cells with an adherent stromal layer that can produce cytokines and extracellular matrix components most effectively supports long-term hematopoiesis ( LTC ), mimicking the bone marrow micro-environment.

Hydroxycitric acid ameliorates inflammation and oxidative stress in mouse models of multiple sclerosis

Hydroxycitric acid （HCA） is derived primarily from the Garcinia plant and is widely used for its anti-in- flammatory effects. Multiple sclerosis can cause an inflammatory demyelination and axonal damage. In this study, to validate the hypothesis that HCA exhibits therapeutic effects on multiple sclerosis, we established female C57BL/6 mouse models of multiple sclerosis, i.e., experimental autoimmune encephalomyelitis, using Complete Freund＇s Adjuvant （CFA） emulsion containing myelin oligodendrocyte glycoprotein （35-55）. Treatment with HCA at 2 g/kg/d for 3 weeks obviously improved the symptoms of nerve injury of experimental autoimmune encephalomyelitis mice, decreased serum interleulin-6, tumor necrosis factor alpha, nitric oxide, and malondialdehyde levels, and increased superoxide dismutase and glutathione reduc- tase activities. These findings suggest that HCA exhibits neuroprotective effects on multiple sclerosis-caused nerve injury through ameliorating inflammation and oxidative stress.
