BACKGROUND The distinction between foot and ankle wound healing complications as opposed to infection is crucial for the appropriate and efficacious allocation of antibiotic *** reports have focused on the diagnostic accuracy of different inflammatory markers,however,mainly in the diabetic *** To evaluate the diagnostic accuracy of white cell count(WCC)and C-reactive protein(CRP)as diagnostic tools for this distinction in the non-diabetic *** Data was reviewed from a prospectively maintained Infectious Diseases Unit database of 216 patients admitted at Leicester University Hospitals–United Kingdom with musculoskeletal infections over the period between July 2014 and February 2020(68 mo).All patients with confirmed diagnosis of diabetes were excluded while only those with confirmed microbiological or clinical diagnosis of foot or ankle infection were included in our *** the included patients,we retrospectively retrieved the inflammatory markers(WCCs and CRP)at the time of *** of CRP 0-10 mg/L and WCC 4.0-11.0×109/L were considered *** After exclusion of patients with confirmed diabetes,25 patients with confirmed foot or ankle infections were *** infections were confirmed microbiolo-gically with positive intra-operative culture results.7(28%)patients with osteomyelitis(OM)of the foot,11(44%)with OM of the ankle,5(20%)with ankle septic arthritis and 2(8%)patients with post-surgical wound infection were *** bony surgery was identified in 13(52%)patients,either a corrective osteotomy or an open reduction and internal fixation for a foot or ankle fracture with the infection developing on top of the existing metalwork.21(84%)patients did have raised inflammatory markers while 4(16%)patients failed to mount an inflammatory response even with subsequent debridement and removal of metal *** sensitivity was 84%,while WCC sensitivity was only 28%.CONCLUSION CRP has a relatively goo

Vibration Analysis of an Infinite Poroelastic Circular Cylindrical Shell Immersed in Fluid

The purpose of this paper is to study the effect of presence of fluid within and around a poroelastic circular cylindrical shell of infinite extent on axially symmetric vibrations. The frequency equation each for a pervious and an impervious surface is obtained employing Biot’s theory. Radial vibrations and axially symmetric shear vibrations are uncoupled when the wavenumber is vanished. The propagation of axially symmetric shear vibrations is independent of presence of fluid within and around the poroelastic cylindrical shell while the radial vibrations are affected by the presence of fluid. The frequencies of radial vibrations and axially symmetric shear vibrations are the cut-off frequencies for the coupled motion of axially symmetric vibrations. The non-dimensional phase velocity as a function of ratio of thickness to wavelength is computed and presented graphically for two different types of poroelastic materials for thin poroelastic shell, thick poroelastic shell and poroelastic solid cylinder.

On Axially Symmetric Vibrations of Fluid Filled Poroelastic Spherical Shells

Employing Biot’s theory of wave propagation in liquid saturated porous media, waves propagating in a hollow poroelastic closed spherical shell filled with fluid are studied. The frequency equation of axially symmetric vibrations for a pervious and an impervious surface is obtained. Free vibrations of a closed spherical shell are studied as a particular case when the fluid is vanished. Frequency as a function of ratio of thickness to inner radius is computed in absence of dissipation for two types of poroelastic materials each for a pervious and an impervious surface. Results of previous works are obtained as a particular case of the present study.

Voltage Mode Universal Filter Using Current Differencing Buffered Amplifier as an Active Device

Integrated filter circuit design resulted in desire for replacement of Inductors by active elements like Operational Amplifiers which led to the introduction of active filters. Active filter design has evolved over a period of time. Starting with OP AMPs, we have witnessed phenomenal growth of active component usage in filter design and development catering to varying requirements. This has contributed greatly in emergence of circuits with minimal limitations and advantages in terms of wide Bandwidth and High slew rates. Current Differencing Buffered Amplifier (CDBA) based active filter design has resulted in introduction of many novel circuits .This paper proposes a new Voltage-mode three input and Single output (TISO) multifunction filter based on single CDBA, four resistors and two capacitors. This second order filter circuit is capable of realizing various filter functions by choosing values of the three inputs variably. The natural frequency (ω0) can be tuned with passive components and the Q of the circuit is independent of (ω0) .The higher cascading capability of the circuit is ensured because of its low - output impedance. Further PSPICE-simulated results are in conformity with theoretical values.

A Study on Propagation of Waves in a Transversely Isotropic Poroelastic Layer Bounded between Two Viscous Liquids

Propagation of Love waves in a transversely isotropic poroelastic layer bounded between two compressible viscous liquids is presented. The equations of motion in a transversely isotropic poroelastic solid are formulated in the framework of Biot’s theory. A closed-form solution for the propagation of Love waves is obtained in a transversely isotropic poroelastic layer. The complex frequency equation for phase velocity and attenuation of Love waves is derived for a transversely isotropic poroelastic layer when it is bounded between two viscous liquids and the results are compared with that of the poroelastic layer. The effect of viscous liquids on the propagation of Love waves is discussed. It is observed that the presence of viscous liquids decreases phase velocity in both transversely isotropic poroelastic layer and poroelastic layer. Results related to the case without viscous liquids have been compared with some of the earlier results and comparison shows good agreement.

Torsional Vibrations of Coated Hollow Poroelastic Spheres

Torsional vibrations of coated hollow poroelastic spheres are studied employing Biot’s theory of wave propagation in poroelastic solid. The dilatations of solid and liquid media are zero, therefore the frequency equation of torsional vibrations is same both for a permeable and an impermeable surface. The coated poroelastic sphere consists of an inner hollow poroelastic sphere bounded by and bonded to a sphere made of distinct poroelastic material. The inner sphere is designated as core and outer sphere as casing. Core and casing are bonded at the curved surfaces. The inner and outer boundaries of the coated hollow poroelastic sphere are free from stress and at the interface of core and casing the displacement and stresses are continuous. It is assumed that the each material of coated sphere is homogeneous and isotropic. The frequency equation of torsional vibrations of a coated poroelastic hollow sphere is obtained when the material of the core vanishes. Also a coated poroelastic solid sphere is obtained as the limiting case of the frequency equation of coated hollow poroelastic sphere when the inner radius of core approaches to zero. Non-dimensional frequency as a function of ratio of thickness of core to that of inner radius of core is determined and analyzed. It is observed that the frequency and dispersion increase with the increase of the thickness of the coating.
