Non-alcoholic fatty liver disease (NAFLD) is a common liver disease worldwide. There is no specific biomarker for the diagnosis of NAFLD. Trigly-ceride and glucose index (TyG) may predict the subsequent occurrence of NAFLD in later life. This cross sectional study was aimed to evaluate the effectiveness of triglyceride and glucose index (TyG) as a possible biomarker of NAFLD. The study was conducted at the Department of Laboratory Medicine, Bangabandhu Sheikh Mujib Medical University (BSMMU), Dhaka, Bangladesh from March 2019 to February 2020. A total of 124 subjects were taken as study population following selection criteria. Among them 62 were diagnosed patients of NAFLD and 62 were healthy subjects as control group. Fasting plasma glucose was measured by glucose oxidase method and serum triglyceride was measured by enzymatic-colorimetric method, while TyG index was calculated accordingly. The mean age was 39.5 ± 11.27 years in NAFLD patients and 37.10 ± 12.28 years in control subjects with male female ratio 1:1.7 and 1:1.8 respectively. Major portion of NAFLD patients (62.9%) were overweight (BMI ≥ 25). The mean fasting plasma glucose level was 5.73 ± 1.47 mmol/L in NAFLD patients and 5.27 ± 0.69 mmol/L in control group (p < 0.027). The mean serum triglyceride level was 237.19 ± 96.47 mg/dl in NAFLD patients and 117.32 ± 53.07 mg/dl in control group (p < 0.001). The triglyceride and glucose index (TyG) was 9.36 ± 0.47 in NAFLD group and 8.53 ± 0.42 in control group. TyG index was significantly higher in NAFLD patients in comparison to control group (p < 0.001). In ROC analysis, cut off value of TyG index was 8.85 with sensitivity 93.5% and specificity 79%. As a fast and effective method, TyG index can be used as a diagnostic tool to predict NAFLD.

Non-Diabetic Renal Disease in Patients with Type 2 Diabetes Mellitus with Proteinuria

Background: Diabetes mellitus (DM) is the leading cause of end stage renal disease (ESRD) worldwide. Although DM with proteinuria is the ultimate result of diabetic nephropathy (DN), a wide spectrum of non-diabetic renal diseases (NDRD) can occur in such patients. Objective: To observe the frequency and histological pattern of NDRD in diabetic patients with proteinuria and to explore their association with clinical and laboratory parameters. Methods: This cross-sectional study was conducted in the Department of Nephrology, Bangabandhu Sheikh Mujib Medical University (BSMMU), Dhaka, Bangladesh from April 2016 to September 2017. In this study a total of 38 cases of DM with proteinuria (>1 gm/24-hour) were selected purposively. Renal biopsy was done in all patients. Based on histological findings they were categorized into two groups;Group 1 with NDRD and Group II with DN. Their clinical and laboratory parameters were analyzed and compared. Results: Among the total study subjects, 21 (55.3%) were male and 17 (44.7%) were female, mean (±SD) age 43.45 ± 9.99 years in the NDRD group and 41.57 ± 9.50 years in the DN group. Thirty one cases (81.6%) out of thirty eight had NDRD and seven (18.4%) cases had isolated DN;therefore more than two third cases had NDRD. Duration of DM was found to be significantly shorter (p = 0.004) in the NDRD group. Diabetic retinopathy was present in 12.9% cases in NDRD group vs. 57.1% cases in DN group (p = 0.025). Frequency of microscopic hematuria was significantly higher (90.3%) in NDRD patients (p = 0.002). Conclusion: The frequency of NDRD in type 2 diabetic patients other than diabetic nephropathy is relatively high. Membrano proliferative glomeru-lonephritis and membranous nephropathy are more common in NDRD. Absence of diabetic retinopathy, presence of hematuria and shorter duration of DM are markers associated with NDRD in type 2 DM, which are important indicators for renal biopsy in diabetic patients with proteinuria.

Predicting the Stitch Density of Finished Fabrics Using Weft Blended Grey Knit Fabrics

Stitch density is one of the critical quality parameters of knit fabrics. This parameter is closely related to other physical quality parameters like fabric weight, fabric tightness factor, fiber types, blend ratio, yarn diameter and linear density, and fabric structure. Selecting stitch density (wales per inch, course per inch) is essential to getting the appropriate fabric weight and desired quality. Usually, no rules or assumptions exist to get the desired stitch density in the finished fabric stage. Fifteen types of blended knit fabrics were prepared to conduct the study. The varying percentages of cotton, polyester, and elastane are incorporated in the blends. Regression analysis and regression ANOVA tests were done to predict the stitch density of finished fabrics. A suitable regression equation is established to get the desired results. The study also found that the stitch density value in the finished stage fabric decreases by approximately 15% compared to the stitch density in the grey fabric stage. This study will help the fabric manufacturers get the finished fabric stitch density in advance by utilizing the grey fabric stitch density data set. The author expects this research to benefit the knitting and dyeing industry, new researchers, and advanced researchers.

Background: The robustness is a measurement of an analytical chemical method and its ability to contain unaffected by little with deliberate variation of analytical chemical method parameters. The analytical chemical method variation parameters are based on pH variability of buffer solution of mobile phase, organic ratio composition changes, stationary phase (column) manufacture, brand name and lot number variation;flow rate variation and temperature variation of chromatographic system. The analytical chemical method for assay of Atropine Sulfate conducted for robustness evaluation. The typical variation considered for mobile phase organic ratio change, change of pH, change of temperature, change of flow rate, change of column etc. Purpose: The aim of this study is to develop a cost effective, short run time and robust analytical chemical method for the assay quantification of Atropine in Pharmaceutical Ophthalmic Solution. This will help to make analytical decisions quickly for research and development scientists as well as will help with quality control product release for patient consumption. This analytical method will help to meet the market demand through quick quality control test of Atropine Ophthalmic Solution and it is very easy for maintaining (GDP) good documentation practices within the shortest period of time. Method: HPLC method has been selected for developing superior method to Compendial method. Both the compendial HPLC method and developed HPLC method was run into the same HPLC system to prove the superiority of developed method. Sensitivity, precision, reproducibility, accuracy parameters were considered for superiority of method. Effective Elimination of Hazardous Chromium (VI) Using Periodic Elements and Contemporary Adsorption Methods by Using Magnesium Ferrite Nanoparticle: A Review

A well-known hazardous metal and top contaminant in wastewater is hexavalent chromium. The two forms of most commonly found chromium are chromate ( CrO 4 2− ) and dichromate ( Cr 2 O 7 2− ). Leather tanning, cooling tower blow-down, plating, electroplating, rinse water sources, anodizing baths etc. are the main sources of Cr (VI) contamination. The Cr (VI) is not only non-biodegradable in the environment but also carcinogenic to living population. It is still difficult to treat Cr contaminated waste water effectively, safely, eco-friendly, and economically. As a result, many techniques have been used to treat Cr (VI)-polluted wastewater, including adsorption, chemical precipitation, coagulation, ion-exchange, and filtration. Among these practices, the most practical method is adsorption for the removal of Cr (VI) from aqueous solutions, which has gained widespread acceptance due to the ease of use and affordability of the equipment and adsorbent. It has been revealed that Fe-based adsorbents’ oxides and hydroxides have high adsorptive potential to lower Cr (VI) content below the advised threshold. Fe-based adsorbents were also discovered to be relatively cheap and toxic-free in Cr (VI) treatment. Fe-based adsorbents are commonly utilized in industry. It has been discovered that nanoparticles of Fe-, Ti-, and Cu-based adsorbents have a better capacity to remove Cr (VI). Cr (VI) was effectively removed from contaminated water using mixed element-based adsorbents (Fe-Mn, Fe-Ti, Fe-Cu, Fe-Zr, Fe-Cu-Y, Fe-Mg, etc.). Initial findings suggest that Cr (VI) removal from wastewater may be accomplished by using magnesium ferrite nanomaterials as an efficient adsorbent.

A Novel LSB Steganography Technique for Enhancing Cloud Security

Steganography is a technique that is frequently used to hide hidden information in multimedia artifacts including music, video, and images. In order to protect data saved in the cloud, this paper presents a steganography method for encrypting sound utilizing LSB-based computation. By using the least significant bit (LSB) of a byte to represent a message and then substituting each LSB bit with a binary message and encrypting a significant quantity of data. The proposed system uses the LSB technique of picture steganography, Multi-Level Encryption Algorithm (MLEA) and Two-Level Encryption Algorithm (TLEA) data encryption to give the highest level of cloud security. Compared to other current schemes, the performance of the suggested method is 1.732125% better on average.

Assessment of Physico-Chemical and Microbial Quality of Sugarcane Juice Sold by Street Vendors in Three Regions of Bangladesh

Sugar cane juice is a popular refreshing drink in most part of Bangladesh. It has great taste and health benefits;also it is available most of the public places at reasonable prices which consumed by road side customers including general public, shopping personals, tourists, students. In our country, street vendors crushing sugarcane between roller crusher and sold without any heat treatment or preservative, also served with or without added ice and lemon juice. The study aimed to identify and compare the physico-chemical and microbial quality of sugarcane juice. The chemical qualities of juices including moisture, PH, ash, total soluble solid, total sugar, reducing sugar and titrable acidity were found slightly different in Mymensingh, Gazipur and Narayangonj areas. The highest and lowest value of moisture, ash, PH, total soluble solids, total sugar, reducing sugar and titrable acidity were found in 84.33% - 79.26% (Gazipur-Mymensingh), 0.57% - 0.04% (Mymensingh-Gazipur), 5.9 - 2.9 (Gazipur-Narayangonj), 17.48% - 4.98% (Mymensingh-Narayangonj), 21.9% - 6.56% (Mymensingh-Gazipur), 3.7 - 2.1 (Gazipur-Mymensingh) and 0.523% - 0.007% (Narayangonj-Gazipur) respectively. For microbiological analysis, the total viable count of sugar cane juice in Mymensingh, Gazipur and Narayangonj were ranged from 0.6 × 105 - 43.6 × 105 cfu/ml, 4.6 × 105 - 21.6 × 105 cfu/ml and 3.6 × 105 - 36.6 × 105 cfu/ml respectively, where the permitted value is 1.0 × 104 cfu/ml, whereas the total coliform count was ranged from 0.4 × 105 - 6.4 × 105 cfu/ml, 0.6 × 105 - 8.4 × 105 cfu/ml and 0.00 - 8.4 × 105 cfu/ml, where the permitted value is 100 cfu/ml and total fungal count was ranged from 5.5 × 105 - 56.5 × 105 cfu/ml, 21.5 × 105 - 54.5 × 105 cfu/ml and 32.5 × 105 - 68.5 × 105 cfu/

Acute Lymphoblastic Leukemia Presenting as Systemic Juvenile Idiopathic Arthritis: Experience from Bangladesh

Background: Acute lymphoblastic leukemia (ALL), the most common paediatric malignancy, is a heterogeneous hematologic disease. ALL patients may present with isolated and persistent osteo-articular complaints, lower incidence of hepatomegaly, splenomegaly or lymphadenopathy without clear laboratory features, and misdiagnosed as systemic juvenile idiopathic arthritis (sJIA). Methods: This was a single center cross sectional study over a period of 4 years. Clinic laboratory profiles of 39 ALL children were compared with 39 age and sex-matched sJIA cases. Result: Among 39 ALL patients 89.7% were initially misdiagnosed as sJIA upon clinical presentation. Majority (66.7%) of ALL patients had oligo-articular joint involvement. In sJIA, small joints of the hands were most commonly involved. The total WBC count was significantly higher in ALL patients (p-value 0.0065). CRP and LDH values between the two groups showed significant differences (p-value 0.00006 and 0.00001 respectively). Conclusion: The presentation of leukemia with arthralgia or arthritis makes the diagnosis difficult for the physicians. The diagnosis of sJIA must be made with caution keeping the possibility of haematological malignancy in mind.

Response of Rice Cultivars to Elevated Air Temperature and Soil Amendments: Implications towards Climate Change Adaptations and Mitigating Global Warming Potentials

Global mean surface air temperature is expected to increase 1.1˚C - 6.4˚C by the end of 21st century which may affect rice productivity and methane emissions in the future climate. This experiment was conducted to investigate the response of rice cultivars to elevated air temperature (+1.5˚C higher than ambient) and soil amendments in regards to rice yield, yield scaled methane emissions and global warming potentials. The experimental findings revealed that replacement of inorganic fertilizers (20% - 40% of recommended NPKS) with Vermicompost, Azolla biofertilizer, enriched sugarcane pressmud, rice husk biochar and silicate fertilization increased rice yield 13.0% - 23.0%, and 11.0% - 19.0% during wet aman and dry boro season, respectively. However, seasonal cumulative CH4 fluxes were decreased by 9.0% - 25.0% and 5.0% - 19.0% during rainfed wet aman and irrigated dry boro rice cultivation, respectively with selected soil amendments. The maximum reduction in seasonal cumulative CH4 flux (19.0% - 25.0%) was recorded with silicate fertilization and azolla biofertilizer amendments (9.0% - 13.0%), whereas maximum grain yield increment 10.0 % - 14.0% was found with Vermicompost and Sugarcane pressmud amendments compared to chemical fertilization (100% NPKS) treated soils at ambient air temperature. However, rice grain yield decreased drastically 43.0% - 50.0% at elevated air temperature (3˚C higher than ambient air temperature), eventhough accelerated the total cumulative CH4 flux as well as GWPs in all treatments. Maximum seasonal mean GWPs were calculated at 391.0 kg CO2 eq·ha−1 in rice husk biochar followed by sugarcane pressmud (mean GWP 387.0 kg CO2 eq·ha−1), while least GWPs were calculated at 285 - 305 kg CO2 eq·ha−1 with silicate fertilizer and Azolla biofertilizer amendments. Background: Emergence agitation (EA) is a common phenomenon observed in pediatric patients following general anesthesia. This study aimed to assess the efficacy of propofol and fentanyl in preventing EA and to compare their associated complications or side effects. Methods: This prospective randomized observational comparative study was conducted at Dhaka Medical College Hospital from July 2013 to June 2014. The study aimed to evaluate the effects of propofol and fentanyl on EA in children aged 18 to 72 months undergoing circumcision, herniotomy, and polypectomy operations. Ninety children were included in the study, with 45 in each group. Patients with psychological or neurological disorders were excluded. Various parameters including age, sex, weight, American Society of Anesthesiologists (ASA) class, duration of anesthesia, Saturation of Peripheral Oxygen (SPO2), heart rate (HR), respiratory rate (RR), Pediatric Anesthesia Emergence Delirium (PAED) score, duration of post-anesthesia care unit (PACU) stay, incidence of laryngospasm, nausea, vomiting, and rescue drug requirement were compared between the two groups. Results: Age, sex, weight, ASA class, and duration of anesthesia were comparable between the two groups. Perioperative SpO2 and HR were similar in both groups. However, the PAED score was significantly higher in the fentanyl group during all follow-ups except at 30 minutes postoperatively. The mean duration of PACU stay was significantly longer in the fentanyl group. Although the incidence of laryngospasm was higher in the fentanyl group, it was not statistically significant. Conversely, nausea or vomiting was significantly higher in the fentanyl group. The requirement for rescue drugs was significantly higher in the fentanyl group compared to the propofol group. Conclusion: Both propofol and fentanyl were effective in preventing emergence agitation in pediatric patients undergoing various surgical procedures under sevoflurane anesthesia. However, propo
