Surface Preparation is very important in adhesive b on ding of spray coatings to the surface of a work piece. The common practice is gr it-blasting of the surface before subjecting it to the spray coating process. In this study, grit-blasting of an AISI 4130 steel (of different heat treatmen ts) with Al 2O 3 particles was studied. Various grit-blasting parameters such as blasting particle size, the distance between blasting nozzle and the work pi ece (25, 30 and 40 cm.), blasting pressure (3,4,5,6 and 7 bars), blasting time ( 3, 6 and 10 seconds), and the blasting angle (45° and 90°) were examined in or der to find the optimum roughness. The mean roughness (Ra) of the grit-blasted surfaces were measured and the vari ations of the roughness with respect to the above mentioned variables were studi ed. The results show that by increasing blasting time, surface roughness increas es up to a maximum and then slightly decreases it with further duration of t he process. On the other hand a lengthy blasting causes some undesirable results such as an increase in residual particles between surface irregularities. There fore an optimum blasting time is of great importance. Increasing the blasting pr essure also provides a rougher surface, but in grit blasting of harder specimens the surface roughness decreases when the pressure reaches a certain limit. About the blasting angle, it was noticed that an angle of 45° results in less r esidual particles between the surface irregularities, in comparison to the angle of 90°. After grit-blasting, the specimens were plasma spray coated with 80% ZrO 2-20 % Y 2O 3 powder. The adhesive strength of the coating to the substrate was the n measured according to the DIN 50160 standard. The results show that for a certain base metal, the adhesive strength is directl y related to the surface roughness of the base material. We compared the efficacy and safety between once-daily dosing and on-demand use of udenafil for type 2 diabetic patients with erectile dysfunction （ed）. A multi-center, randomized, open-label, parallel-group, 12-week study was conducted. 161 patients who improved with on-demand 200 mg of udenafil according to Sexual Encounter Profile （SEP） diary Question 2 and 3 （Q2 and Q3） were randomized into 200 mg on-demand （n = 80） or 50 mg once-daily （n = 81） dosing groups for 8 weeks. The dosing period was followed by a 4-week treatment-free period. The primary efficacy endpoint was the change of the International Index of Erectile Function （IIEF） erectile function domain （EFD） score. The secondary efficacy endpoints included changes to the SEP diary Q2, Q3, IIEF Q3, Q4, other domains of IIEF, Global Assessment Question, and shift to the normal rate （EFD 〉 26）. Vascular endothelial markers were also assessed. The IIEF-EFD score of both groups improved after 8 weeks of treatment （P 〈 0.0001）. There was no statistically significant difference between two groups. Improvement was not maintained after the treatment-free follow-up period. Similar results were observed in the secondary efficacy endpoints. There was also no significant difference in vascular endothelial markers. Daily udenafil was well-tolerated, and there was no significant difference in the adverse drug reactions and adverse events between the two groups. Flushing and headache were the most frequent adverse events. Both regimens improved ed in diabetic patients and were well-tolerated. Further studies are needed to assess the effect of daily udenafil treatment in diabetic patients.

A sharply transected spinal cord has been shown to be fused under the accelerating influence of membrane fusogens such as polyethylene glycol （PEG）（GEMINI protocol）. Previous work provided evidence that this is in fact possible. Other fusogens might improve current results. In this study, we aimed to assess the effects of PEGylated graphene nanoribons （PEG-GNR, and called ＂TexasPEG＂ when prepared as lwt% dispersion in PEG600） versus placebo （saline） on locomotor function recovery and cellular level in a rat model of spinal cord transection at lumbar segment 1 （L1） level. In vivo and in vitro experiments （n -- 10 per experiment） were designed. In the in vivo experiment, all rats were submitted to full spinal cord transection at L1 level. Five weeks later, behavioral assessment was performed using the Basso Beattie Bresnahan （BBB） locomotor rating scale. Immunohistochemical staining with neuron marker neurofilament 200 （NF200） antibody and astrocyt- ic scar marker glial fibrillary acidic protein （GFAP） was also performed in the injured spinal cord. In the in vitro experiment, the effects of TexasPEG application for 72 hours on the neurite outgrowth of sh-SYSY cells were observed under the inverted microscope. Results of both in vivo and in vitro experiments suggest that TexasPEG reduces the formation of glial scars, promotes the regeneration of neurites, and thereby contributes to the recovery of locomotor function of a rat model of spinal cord transfection.

目的医疗保健管理者鼓励临床医生为患者提供能进入个人健康档案的门户,但实施起来还存有挑战性。尽管大型综合卫生系统一直以来通过昂贵的广告活动推广使用,但对于中小型诊所还需寻找其他可行方法。方法采用混合评估法,评价了8个初级保健诊所提出患者门户(即患者信息门户信息系统)的实施策略〔互动预防性健康档案(IPHR)〕。与实施优胜者进行了一系列的学习合作,且重新设计工作流程以实现医疗门户一体化。对诊所实施策略、患者信息门户的利用及影响因素进行了前瞻性评估。结果由诊所制定的实施策略促使25.6%的患者使用了IPHR,31个月,每月增长率为1.0%。有23.5%的患者就诊当日就签名使用了IPHR。老年及有并存病患者使用IPHR多,黑人和西班牙人使用较少。为了提升利用率,基于临床医生的特性以及诊所采纳改革后的工作流程,诊所实施情况大不相同(从使用率22.1%升高至27.9%,P<0.001)。结论通过促进患者使用信息门户,帮助诊所实现医疗一体化,那么初级保健诊所完全能够达到、甚至超过大型卫生系统的使用率。

目的：探讨伽玛刀对脑深部和功能区海绵状血管瘤（ＣＡ）的治疗效果及有关问题。方法：采用伽玛刀对６７例大脑半球深部、丘脑、基底节及脑干海绵状血管瘤进行治疗。结果：随访３月～８年（平均２．６年），评价其治疗效果，２例死于其他疾病，６例行手术治疗。在ＭＲＩ检查随访５９例中，病变稳定好转率达９８％，并且年再出血发生率大大降低。结论：应用伽玛刀治疗脑深部及险要部位的ＣＡ是一种无创伤、效果比较好的方法。
