Comparison of the diagnostic accuracy of CT scan with oral and intravenous contrast versus CT scan with intravenous contrast alone in the diagnosis of blunt abdominal trauma

Purpose:Abdominal CT scan using oral and intravenous(IV)contrast is helpful in the diagnosis of intra-abdominal ***,the use of oral and IV contrast delays the process of diagnosis and increases the risk of *** has also been shown that CT scan with IV contrast alone is as helpful as CT scan with oral and IV contrast and rectal CT scan in detecting abdominal ***,the present study aims to prospectively compare the diagnostic value of CT scan with oral and IV contrast versus CT scan with IV contrast alone in the diagnosis of blunt abdominal trauma(BAT).Methods:Altogether 123 BAT patients,60(48.8%)women and 63(51.2%)men with the mean age of(40.4±18.7)years who referred to the emergency department of Imam Khomeini Educational and Medical Center in Sari,Iran(a tertiary trauma center in north of Iran)from November 2014 to March 2017 and underwent abdominal CT scans+laparotomy were *** with penetrating trauma or hemodynamically unstable patients were *** participants were randomly allocated to two groups:abdominal CT scan with oral and IV contrast(n=63)and CT scan with IV contrast alone(n=60).No statistically significant difference was found between two groups regarding the hemodynamic parameters,age,gender,injury mechanisms(all p>0.05).The results of CT scan were compared with that of laparotomy *** collected data were recorded in SPSS version 22.0 for *** data were presented as mean and ***:The sensitivity and specificity of CT scan using oral and IV contrast in the diagnosis of BAT were estimated at 96.48(95%CI:90.73-99.92)and 92.67(95% CI:89.65-94.88),respectively;while CT scan with IV contrast alone achieved a comparable sensitivity and specificity of 96.6(95% CI:87.45-99),42 and 92.84(95% CI:89.88-95.00),***:CT scan with IV contrast alone can be used to assess visceral injuries in BAT patients with normal hemodynamics to avoid diagnostic delay.

Disaster relief logistics is a significant element in the management of disaster relief *** this paper,the operational decisions of relief logistics are considered in the distribution of resources to the affected areas to include scheduling,routing,and allocation *** proposed mathematical model simultaneously captures many aspects relevant to real life to face the challenging situation of *** such as multiple uses of vehicles and split delivery allow for better use of vehicles as one of the primary resources of disaster response.A multi-period multi-criteria mixed-integer programming model is introduced to evaluate and address these *** model utilizes a rolling horizon method that provides possibilities to adjust plans as more information becomes *** objectives of efficiency,effectiveness,and equity are jointly *** augmented epsilon constraint method is applied to solve the model,and a case study is presented to illustrate the potential applicability of our *** results show that the model is capable of generating efficient solutions.

Vegetation composition differentiation and species-environment relationships in the northern part of Isfahan Province, Iran

A considerable proportion of Iran＇s territory is covered with arid and semi-arid rangelands and mismanagement and overexploitation of those rangelands have resulted in serious ecological degradation. Thus, the need is pressing to examine the present species composition and the relationships with environmental factors for providing the needed scientific references to species conservation and ecological rehabilitation efforts. The aims of this study were to examine the species composition and to delineate the most important factors influencing the distributions of plant species and groups in the northern rangelands of Isfahan Province （Iran） using two-way indicator species analysis （TWINSPAN）, detrended correspondence analysis （DCA）, principal component analysis （PCA）, and canonical correspondence analysis （CCA）. Field investigations were conducted in the growing season of 2014 using stratified random method in 22 homogeneous sampling units. In total, 75 plant species belonging to 52 genera and 19 families were identified. The most important families were Asteraceae and Papilionaceae, the most important genera were Astragalus, Cousinia, and Acanthophyllum, and the most important species were Artemisia aucheri and Artemisia sieberi. Plant species were classified into 10 groups using TWINSPAN. DCA was used to estimate the magnitude of changes in species composition along the first two ordination axes to provide gradient length estimations for PCA and CCA ordinations. The first three PCA axes and the first three CCA axes demonstrated similar cumulative percentage of variance, indicating that the environmental factors （selected by PCA） used in CCA ordination were acceptable for explaining the species composition and the distributions. Estimation of conservation value of myrtle(Myrtus communis)using a contingent valuation method:a case study in a Dooreh forest area,Lorestan Province,Iran

Background： Around 2000 plant species occur naturally in Lorestan Province of which 250 species are medicinal and myrtle is one of them. Myrtle is a shrub whose leaves and fruits have medicinal value and thus, if managed and harvested properly, could produce sustained economic benefits. In recent years, however, over half of the myrtle site areas was destroyed, due to inappropriate management and excessive harvesting practices. Thus, coming up with a practical harvesting approach along with identifying those factors damaging the sites, seems to be very crucial. Methods： In our investigation, we calculated the conservation value per hectare of myrtle in the Dooreh forest area in Lorestan Province. Using the Contingent Valuation （CV） and Double Bounded Dichotomous Choice （DBDC） methods, we determined the willingness to pay （VVTP） for myrtle conservation. The VVTP was estimated with a Iogit model for which indices were obtained based on a maximum precision criterion. Results： The results showed that 86.67 per cent of people were willing to pay for the conservation of these myrtle sites. Average monthly WTP per family was calculated as $0.79. The annual conservation value in terms of WTP for the preservation of the myrtle sites in Dooreh was estimated as $102,525. Among the variables of the model presented, education had a positive impact, while the amount proposed for payment and family size had a negative impact on the WTP. Conclusions： Our estimate of the value of myrtle conservation should provide justification for policy makers and decision making bodies of natural resources to implement policies in order to conserve the natural sites of this species more effectively.

Standard heat treatment effects on TLP bonded IN-738LC superalloy using BNi-9 filler:An approach to make an ideal joint

The present research is focused on the effects of standard heat treatment on the microstructure and mechanical properties of diffusion brazed IN-738 LC *** distinct heat treatment cycles of full solution annealing,partial solution annealing,and aging treatment were applied to the bonded specimens,*** results reveal that bonding at 1120℃for 5 min leads to incomplete isothermal solidification and formation of eutectic phases including Ni-and Cr-rich borides in the joint *** the holding time to 45 min leads to the full isothermal solidification and formation of a nickel proeutectic solid-solution phase(γ)in the *** standard heat treatment of isothermally solidified and non-isothermally solidified specimens results in the complete elimination of the boride phases in the diffusion-affected zone and also the formation ofγ’precipitates in the isothermally solidified ***,discontinuously re-solidified products are observed in joint district in the non-isothermally solidified *** highest shear strength(~801 MPa)is achieved for isothermally solidified specimen after standard heat treatment;this strength is approximately 99%that of the substrate material.
