Comprehensive molecular characterization to predict immunotherapy response in advanced biliary tract cancer:a phase II trial of pembrolizumab

Background:Immune checkpoint inhibitors(ICIs)are effective in a subset of patients with metastatic solid ***,the patients who would benefit most from ICIs in biliary tract cancer(BTC)are still *** and methods:We molecularly characterized tissues and blood from 32 patients with metastatic BTC treated with the ICI pembrolizumab as second-line ***:All patients had microsatellite stable(MSS)type *** of the 32 patients achieved partial response(PR),with an objective response rate(ORR)of 9.4%(95%confidence interval[CI],2.0–25.2)and nine showed stable disease(SD),exhibiting a disease control rate(DCR)of 37.5%(95%CI,21.1–56.3).For the 31 patients who had access to PD-1 ligand 1(PD-L1)combined positive score(CPS)testing(cut-off value≥1%),the ORR was not different between those who had PD-L1-positive(PD-L1+;1/11,9.1%)and PDL1-(2/20,10.0%)tumors(p=1.000).The tumor mutational burden(TMB)of PD-L1+BTC was comparable to that of PD-L1-BTC(p=0.630).TMB and any exonic somatic mutations were also not predictive of pembrolizumab *** analysis of blood and tumor samples demonstrated a relatively high natural killer(NK)cell proportion in the peripheral blood before pembrolizumab treatment in patients who achieved tumor ***,the tumors of these patients presented high enrichment scores for NK cells,antitumor cytokines,and Th1 signatures,and a low enrichment score for cancer-associated ***:This study shows the molecular characteristics associated with the efficacy of pembrolizumab in BTC of the MSS type.

Developing bifunctional catalysts that can catalyze both oxygen evolution reaction(OER)and hydrogen evolution reaction(HER)is pivotal to commercializing large-scale water ***,a novel hollow nanotriangle composed of NiFe LDH-CoMoS_(x) heterojunction(H-CMSx@NiFe LDH)is proposed as a highly efficient bifunctional electrocatalyst for both OER and *** fabricate a heterojunction system,ultra-thin nickel–iron layered double hydroxide(NiFe LDH)nanosheets are uniformly electrodeposited onto a metal–organic framework-derived hollow CoMoS_(x) *** strong coupling of CoMoS_(x) and NiFe LDH catalysts forms the intimate heterojunction interfaces to facilitate interfacial charge transfer,which is favorable to enhance the bifunctional catalytic ***,the large void of CoMoS_(x) nanotriangles and interconnected ultra-thin NiFe LDH nanosheets result in good electrolyte penetration and gas ***,the as-prepared H-CMSx@NiFe LDH on nickel foam(NF)exhibits an impressive catalytic activity and durability for OER and HER activities,delivering a current density of 100 mA·cm^(−2) at the small overpotentials of 214 and 299 mV in OER and HER,***,H-CMSx@NiFe LDH/NF proves to be an effective electrode for an alkaline electrolyzer,as a voltage of only 1.99 V is enough to achieve a current density voltage of only 1.99 V is enough to achieve a current density of 400 mA·cm^(−2) with no degradation in performance over 50 h.

Potassium-ion batteries(PIBs)have gained significant attention as an alternative to lithium-ion batteries(LIBs)due to the abundance of potassium(K)and low ***,the difficulty in finding appropriate electrode materials that can efficiently store the larger K ions has hindered their practical ***,we report a novel anode material,N-doped multichannel carbon nanofibers embedded with porous CoS nanoprisms(CSP@NMCNF),for high-performance *** CSP@NMCNF was synthesized using a two-step strategy comprising of the electrospinning of Co acetate hydroxide nanoprism/binary polymer blend and a subsequent heat *** porous CoS nanoprisms with an anisotropic morphology were well aligned along the length axis of the N-doped multichannel carbon nanofibers,thus ensuring their structural stability during the repeated charge-discharge *** addition,numerous pores facilitated the transport of electrons and ***,the CSP@NMCNF anode exhibited excellent electrochemical performance,delivering a high specific capacity of 368 mAh·g^(-1)at 0.5 A·g^(-1)after 200 cycles and excellent rate capability(232 mAh·g^(-1)at 2.0 A·g^(-1)).

OBJECTIVE:To examine whether color parameters of tongue inspection(TI)using a digital camera was reliable and valid,and to examine which color parameters serve as predictors of symptom patterns in terms of East Asian medicine(EAM).METHODS:Two hundred female subjects'tongue substances were photographed by a mega-pixel digital *** with the photographs,the subjects were asked to complete Yin deficiency,Phlegm pattern,and Cold-Heat pattern *** three sets of digital imaging software,each digital image was exposure-and white balance-corrected,and finally L*(luminance),a*(red-green balance),and b*(yellow-blue balance)values of the tongues *** examine intra-and inter-rater reliabilities and criterion validity of the color analysis method,three raters were asked to calculate color parameters for 20 digital image ***,four hierarchical regression models were ***:Color parameters showed good or excellent reliability(0.627-0.887 for intra-class correlation coefficients)and significant criterion validity(0.523-0.718 for Spearman's correlation).In the hierarchical regression models,age was a significant predictor of Yin deficiency(β=0.192),and b*value of the tip of the tongue was a determinant predictor of Yin deficiency,Phlegm,and Heat patterns(β=-0.212,-0.172,and-0.163).Luminance(L*)was predictive of Yin deficiency(β=-0.172)and Cold(β=0.173)***:Our results suggest that color analysis of the tongue using the L*a*b*system is reliable and valid,and that color parameters partially serve as symptom pattern predictors in EAM practice.

Recently, many surgeons have been using intraoperative neurophysiological monitoring(IOM) in spinal surgery to reduce the incidence of postoperative neurological complications, including level of the spinal cord, cauda equina and nerve root. Several established technologies are available and combined motor and somatosensory evoked potentials are considered mandatory for practical and successful IOM. Spinal cord evoked potentials are elicited compound potentials recorded over the spinal cord. Electrical stimulation is provoked on the dorsal spinal cord from an epidural electrode. Somatosensory evoked potentials assess the functional integrity of sensory pathways from the peripheral nerve through the dorsal column and to the sensory cortex. For identification of the physiological midline, the dorsal column mapping technique can be used. It is helpful for reducing the postoperative morbidity associated with dorsal column dysfunction when distortion of the normal spinal cord anatomy caused by an intramedullary cord lesion results in confusion in localizing the midline for the myelotomy. Motor evoked potentials(MEPs) consist of spinal, neurogenic and muscle MEPs. MEPs allow selective and specific assessment of the functional integrity of descending motor pathways, from the motor cortex to peripheral muscles. Spinal surgeons should understand the concept of the monitoring techniques and interpret monitoring records adequately to use IOM for the decision making during the surgery for safe surgery and a favorable surgical outcome.

We study the large-time dynamics of Cucker-Smale(C-S)flocking particles interacting with nonNewtonian incompressible *** of particles and fluids were modeled using the kinetic Cucker-Smale equation for particles and non-Newtonian Navier-Stokes system for fluids,respectively and these two systems are coupled via the drag force,which is the main flocking(alignment)mechanism between particles and *** present a global existence theory for weak solutions to the coupled Cucker-Smale-Navier-Stokes system with shear *** also use a Lyapunov functional approach to show that sufficiently regular solutions approach flocking states exponentially fast in time.

Two-dimensional(2D)MXenes are promising as electrode materials for energy storage,owing to their high electronic conductivity and low diffusion ***,similar to most 2D materials,MXene nanosheets easily restack during the electrode preparation,which degrades the electrochemical performance of MXene-based materials.A novel synthetic strategy is proposed for converting MXene into restacking-inhibited three-dimensional(3D)balls coated with iron selenides and *** strategy involves the preparation of Fe_(2)O_(3)@carbon/MXene microspheres via a facile ultrasonic spray pyrolysis and subsequent selenization *** 3D structuring effectively prevents interlayer restacking,increases the surface area,and accelerates ion transport,while maintaining the attractive properties of ***,combining iron selenides and carbon with 3D MXene balls offers many more sites for ion storage and enhances the structural robustness of the composite *** resultant 3D structured microspheres exhibit a high reversible capacity of 410 mAh g^(−1) after 200 cycles at 0.1 A g^(−1) in potassium-ion batteries,corresponding to the capacity retention of 97% as calculated based on 100 *** at a high current density of 5.0 A g^(−1),the composite exhibits a discharge capacity of 169 mAh g^(−1).

The study of photonic sintering has gained interest based on the advantages of fast processing at room ***,printed electronics made from photonic sintering with an intensive pulsed light(IPL)energy source exhibit more mechanical instability than those made from conventional thermal sintering *** solve the mechanical instability problems,we fabricated Ag flake hybrid pastes with a variety of concentrations of Ag flake(0,25,50,75,and 100 wt.%).All of the screen-printed hybrid Ag circuits were fabricated on polyimide substrates and were sintered at 3.5 *** porosity was analyzed using the Brunauer-Emmett-Teller *** IPC(Packaging Electronic Circuits)sliding test was performed to analyze the flexibility of the screen-printed Ag flake hybrid *** adhesion strength of the hybrid circuits was evaluated with a roll-type 90°peel *** hybrid Ag printed circuit showed improvements in both the flexibility and adhesion strength with the addition of Ag flake.

Once-daily extended-release tacrolimus （Tac-OD） has been introduced as a useful therapeutic option to increase patient adherence to immunosuppressive therapy. This study aimed to evaluate the safety, efficacy and immunosuppressant adherence of conversion from twice-daily tacrolimus （Tac-BID） to Tac-OD in stable adult living donor liver transplant （LDLT） recipients in a single institution. METHODS： Between February and May 2013, Tac-BID was converted to Tac-OD in recipients followed up for at least 12 months after transplantation and without previous rejection episodes. The switching policy was based on a dose ratio of 1：1 with dose adjustment target trough levels at 3-5 ng/mL. Tacro- limus trough levels, laboratory parameters, metabolic disor- ders, and adverse events were assessed. RESULTS： A total of 229 patients were enrolled in the study. The median age at conversion was 53 years （range 31-73）. The median transplant duration was 35.3 months （range 12.0-95.4）. During a median follow-up of 13.5 months after conversion, 9 patients returned to Tac-BID because of adverse events. No acute rejection episodes were observed. Of 214 patients still on Tac-OD at 12 months, 12 （5.6%） received a reduced dose and 95 （44.4%） required an increased dose over baseline. Overall adherence was 82.2% at the end of follow-up. CONCLUSION： The conversion from Tac-BID to Tac-OD with similar target trough levels after conversion is safe and effec- tive for long-term stable LDLT patients.
