Clinical application of liver stiffness measurement using transient elastography in chronic liver disease from longitudinal perspectives

Accurate determination of the presence and degree of fibrosis in liver is of great importance, because the prognosis and management strategies for chronic liver disease depend mainly on these factors. To date, liver biopsy (LB) remains the "gold standard" for assessing the severity of liver fibrosis; however, LB is often limited by its invasiveness, sampling error, and intra/ inter-observer variability in histological interpretation. Furthermore, repeated LB examinations within a short time interval are indeed ineligible in a real clinical practice. Thus, due to the pressing need for non-invasive surrogates for liver fibrosis, transient elastography (TE),as a novel ultrasound based technology, has allowed a noninvasive measurement of liver stiffness and has gained in popularity over recent years. In the past few years, additional roles for transient TE beyond the initial purpose of a non-invasive surrogate for LB have included the prediction of the most two critical consequences of fibrosis progression: the development of portal hypertension-related complications and hepatocellular carcinoma. This indicates that the role of transient TE is not merely limited to reducing the need for LB, but transient TE can enable the establishment of tailored management strategies by providing more detailed prognostic information. In particular, under the concept in which the clinical course of liver fibrosis is dynamic and bidirectional, especially when appropriate intervention is commenced, transient TE can be used to track the dynamic changes in fibrotic burden during antiviral or antifibrotic treatment. This review discussed extended applications of transient TE in prediction of the development of real clinical endpoints from a longitudinal perspective.

Impact of old age on clinical and angiographic characteristics of coronary artery spasm as assessed by acetylcholine provocation test

Background Smoking and other risk factors have been well known as important factors of variant angina or coronary artery spasm （CAS）. However, clinical features related to age on coronary artery spasm have been rarely evaluated. Methods We evaluated 3155 consecutive patients with insignificant coronary artery lesion. Patients underwent Acetylcholine （Ach） provocation test for induction of CAS. CAS was defined as 〉 70% luminal narrowing of coronary arteries during Ach provocation test. The results of Ach provocation test were compared among age groups; 〈 45 years （Group 1）, 45-54 years （Group 2）, 55-64 years （Group 3）, and 〉_ 65 years （Group 4）. Results Older patients had higher incidence of hypertension, diabetes, but lower incidence rate of current smoking, male sex compared with younger patients. Positive Ach provocation test finding was frequently showed with aging （47.36% vs. 58.3% vs. 62.6% vs. 61.5%; P 〈 0.001）. Multivariate logistic analysis showed that age, male, and myocardial bridge were independent predictors of CAS induced by Ach provocation test. Conclusion Our present study showed that old age was independent predictor for Ach-induced significant coronary artery spasm.

锥束图象重建算法正在快速发展,并用于重要的生物医学和工业应用中。在本文中,主要讨论有效的精确的、可能用于动态研究的算法,特别是近年发展起来的Grangeat类和Katsevich类的算法。这一选择是基于CT和显微CT的定量的和功能的应用需求。2002年,Lee和 Wang提出了圆周和螺旋情形Grangeat类的半扫描锥束算法,解决了短物体重建问题。原理是利用在Grangeat类重建公式中的Radon 空间信息,在阴影区域进行适当的数据插值,从而抑制Feldkamp类重建算法的亮度减低伪影。在圆形和螺旋半扫描情形,首先我们确定位于不同采样冗余区域之间的边界。然后我们导出相应的加权函数以用于特征点的计算。就亮度减低伪影而论,Grangeat类半扫描重建算法优于Feldkamp类半扫描重建算法。2001年, Katsevich推导了第一个理论上精确的螺旋锥束滤波反投影型重建公式。其局限性是探测器窗口较大和待重建物体的半径较小。2002年 Katsevich改进了他的第一个公式。新的公式对病人的尺寸没有多少限制,而且相对旧公式假设了较小的探测器阵列。最近,Katsevich将他的方法推广到一般的扫描轨迹,证明了早期的两个公式是他的一般结果的特例。针对长物体的动态体成像,我们极其需要改进现有的Grangeat类和Katsevich类的算法。

We demonstrate integrated photonic circuits for quantum devices using sputtered polycrystalline aluminum nitride(Al N)on ***-chip Al N waveguide directional couplers,which are one of the most important components in quantum photonics,are fabricated and show the output power splitting ratios from 50:50 to 99:*** beam splitters with an extinction ratio of more than 10 d B are also realized from the Al N directional *** the fabricated Al N waveguide beam splitters,we observe hong–Ou–Mandel interference with a visibility of 91.7%±5.66%.

在连铸过程中，结晶器保护渣在通过控制润滑来实现较好的可浇性和铸坯质量方面起到重要作用。在缺乏结晶器保护渣不同热物性的情况下，黏度是判断结晶器保护渣润滑性能的必要参数。许多研究人员（包括笔者）证实了熔融的结晶器保护渣的流变性能。实际操作中，在剪切速度约为10-40s^-1的结晶器上表面，为减少卷渣，需要高黏度保护渣；但是在剪切速度为100～1000s^-1的结晶器壁，为了保证最大的润滑性能，保护渣黏度应该低。因此，这个特定的属性可能有利于实际连铸过程。目前的研究试图通过用旋转黏度计测量CaO—CaF2-SiO2、CaO-CaF2-B2O3-SiO2和CaO-CaF2-SiO2-Si3N4基保护渣体系的黏度来表征该体系的黏弹性（名为流变性能）。旋转黏度计配备一个1623K温度下受控氧逸度的熔炉。而且，通过拉曼光谱和XPS分析来了解结晶器保护渣黏弹性和结构之间的关系。根据结果，所有经检测的结晶器保护渣都具有流变性能，当剪切速率增大时，黏度降低。应用Os—wald-De Waele指数模型来定量每个结晶器保护渣的剪切变稀度，得出每个结晶器保护渣的聚合度与剪切变稀行为的强度大体上成比正。

Risk factors affecting pancreatic fistulas after pancreaticoduodenectomy

AIM: To analyze the risk factors of pancreatic leakage after ***: We retrospectively reviewed 172 consecutive patients who had undergone pancreatico-duodenectomy at Inha University Hospital between April 1996 and March 2006. We analyzed the pancreatic fistula rate according to the clinical characteristics, the pathologic and laboratory findings, and the anastomotic ***: The incidence of developing pancreatic fistulas in patients older than 60 years of age was 21.7% (25/115), while the incidence was 8.8% (5/57) for younger patients; the difference was significant (P = 0.03). Patients with a dilated pancreatic duct had a lower rate of post-operative pancreatic fistulas than patients with a non-dilated duct (P = 0.001). Other factors, including clinical features, anastomotic methods, and pathologic diagnosis, did not show any statistical difference. CONCLUSION: Our study demonstrated that pancreatic fistulas are related to age and a dilated pancreatic duct. The surgeon must take these risk factors into consideration when performing a pancre-aticoduodenectomy.

BACKGROUND Pancreatic ductal adenocarcinoma(PDAC)is a serious disease with a poor *** a minority of patients undergo surgery due to the advanced stage of the disease,and patients with early-stage disease,who are expected to have a better prognosis,often experience ***,it is important to identify the risk factors for early recurrence and to develop an adequate treatment *** To evaluate the predictive factors associated with the early recurrence of earlystage *** This study enrolled 407 patients with stage I PDAC undergoing upfront surgical resection between January 2000 and April *** recurrence was defined as a diagnosis of recurrence within 6 mo of *** optimal cutoff values were determined by receiver operating characteristic(ROC)*** and multivariate analyses were performed to identify the risk factors for early *** Of the 407 patients,98 patients(24.1%)experienced early disease recurrence:26(26.5%)local and 72(73.5%)distant *** total,253(62.2%)patients received adjuvant *** ROC curve analysis,the optimal cutoff values for early recurrence were 70 U/mL and 2.85 cm for carbohydrate antigen 19-9(CA 19-9)levels and tumor size,*** the 181 patients with CA 19-9 level>70 U/mL,59(32.6%)had early recurrence,compared to 39(17.4%)of 226 patients with CA 19-9 level≤70 U/mL(P70 U/mL(P=0.006),tumor size>2.85 cm(P=0.004),poor differentiation(P=0.008),and non-adjuvant chemotherapy(P=0.025)were significant risk factors for early recurrence in early-stage *** Elevated CA 19-9 level(cutoff value>70 U/mL)can be a reliable predictive factor for early recurrence in early-stage *** adjuvant chemotherapy can prevent early recurrence,it should be recommended for patients susceptible to early recurrence.

Broadband omnidirectional light detection in flexible and hierarchical ZnO/Si heterojunction photodiodes

The development of flexible photodetectors has received great attention for future optoelectronic applications including flexible image sensors, biomedical imaging, and smart, wearable systems. Previously omnidirectional photodetectors were only achievable by integration of a hemispherical microlens assembly on multiple photodetectors. Herein, a hierarchical photodiode design of ZnO nanowires （NWs） on honeycomb-structured Si （H-Si） membranes is demonstrated to exhibit excellent omnidirectional light-absorption ability and thus maintain high photocurrents over broad spectral ranges （365 to 1,100 nm） for wide incident angles （0° to 70°）, which enabled broadband omnidirectional light detection in flexible photodetectors. Furthermore, the stress-relieving honeycomb pattern within the photodiode micromembranes provided photodetectors with excellent mechanical flexibility （10% decrease in photocurrent at a bending radius of 3 mm） and durability （minimal change in photocurrent over 10,000 bending cycles）. When employed in semiconductor thin films, the hierarchical NW/honeycomb heterostructure design acts as an efficient platform for various optoelectronic devices requiring mechanical flexibility and broadband omnidirectional light detection.
