Predictors of pain resolution after varicocelectomy for painful varicocele

Varioocelectomny is a management option for patients with painful varicocele. In this study, we assessed the effectiveness of varicocelectomy for painful varicocele and examined the factors that might be predictive of outcome. All patients who underwent a varicocelectomy for pain between February 2007 and July 2009 were includecL A review of patient medical records was conducted; patient age,body mass index （BMI）, uede, iocatien of the vericocele, tasticular volume, duration and quality of the pain （dull, draaing, throbbing or sharp） and surgical technique （ingulnal versus subinguinal） were documented. All parameters were compared with the rasolution of pain （complete, partial or failure）. We followed up on 53 of 104 patients （51.0%）. Complete postopanntive resolution of pain was reported by 28 patients （52.8%）, whereas 22 （41.5%） reported partial resolution, Only three patients （5.7%） mpaded failure. No relationship was observed between postoperative pain resolution and age, BMI, grade of varicoceio, location of varicocele, ipsilateral testicular, quality of pain or surgical technique. The duration of pain before surllery was the only factor that correlated with postopaative pain resolution （univariate, P=-0.004; multivariate, P=0.002）. Our rasults indicatethat varicocelectomy isan effective treatmentfar painful varicocele in properly selected patients, and that duration of pain before surgery may be predictive of outcome.

Are there any disbenefits to patients in choosing laparoscopic gastrectomy by an expert in open gastrectomy？ Aspects of surgical outcome and radicality of lymphadenectomy

Background There may be concerns over disbenefits to patients who have chosen to undergo laparoscopic gastrectomy by experts in open gastrectomy, considering the disparity between the level of proficiency in open gastrectomy, at which they are already experts, and that in laparoscopic gastrectomy, at which they are beginners. The aim of this study was to compare surgical radicality and outcomes between laparoscopic gastrectomy and open gastrectomy during the learning period of laparoscopic gastrectomy for a senior surgeon who was already an expert in open gastrectomy. Methods Data of short-term surgical outcomes were obtained from patients following laparoscopy assisted distal gastrectomy （LADG） by a surgeon. The initial and following 30 experiences were grouped into LADG-I and LADG-II, respectively. Patients who underwent open distal subtotal gastrectomy （ODSG） and yet could have been candidates for LADG were grouped into ODSG. Known indicators of proficiency levels and the postoperative hospital course were compared. The consequences of extended lymphadenectomy, and the radicality of surgery by completing D2 lymphadenectomy were analyzed. Results The LADG group revealed longer operation time and less bleeding compared to the ODSG group （P 〈0.001）. The number of retrieved lymph nodes and the rate of complications were not significantly different. In the LADG-I group, the DI＋：D2 ratio was 4：1, showing significant differences from those in the LADG-II （0.36：1） and ODSG （0.16：1） groups （P 〈0.001）. The surgeon was able to complete D2 lymphadenectomy during LADG without significant change in the amount of bleeding and the rate of complications, but with a longer operation time （P=0.009）. The number of lymph nodes from the 12a station was not significantly different between the LADG and ODSG groups with D2 lymphadenectomy. Impact of gender and body mass index on surgical outcomes following gastrectomy： an Asia-Pacific perspective

Novel mouse model of Alzheimer's disease exhibits pathology through synergistic interactions among amyloid-β,tau, and reactive astrogliosis

Alzheimer'sdisease(AD)isaprogressive neurodegenerative disorder characterized by cognitive impairment and distinct neuropathological features,including amyloid-β plaques, neurofibrillary tangles, and reactive astrogliosis. Developing effective diagnostic,preventative, and therapeutic strategies for AD necessitates the establishment of animal models that accurately recapitulate the pathophysiological processes of the disease. Existing transgenic mouse models have significantly contributed to understanding AD pathology but often fail to replicate the complexity of human ***, these models are limited in their ability to elucidate the interplay among amyloid-β plaques,neurofibrillary tangles, and reactive astrogliosis due to the absence of spatially and temporally specific genetic manipulation. In this study, we introduce a novel AD mouse model(APP/PS1-TauP301L-Adeno mice) designed to rapidly induce pathological symptoms and enhance understanding of AD mechanisms. Neurofibrillary tangles and severe reactive astrogliosis were induced by injecting AAVDJ-EF1a-hTauP301L-EGFP and Adeno-GFAP-GFP viruses into the hippocampi of 5-month-old APP/PS1 *** months post-injection, these mice exhibited pronounced astrogliosis, substantial amyloid-β plaque accumulation,extensiveneurofibrillarytangles,accelerated neuronal loss, elevated astrocytic GABA levels, and significant spatial memory deficits. Notably,these pathological features were less severe in AAVTauP301L-expressing APP/PS1 mice without augmented reactive astrogliosis. These findings indicate an exacerbating role of severe reactive astrogliosis in amyloid-β plaque and neurofibrillary tangle-associated pathology. The APP/PS1-TauP301L-Adeno mouse model provides a valuable tool for advancing therapeutic research aimed at mitigating the progression of AD.

Gut–brain axis and environmental factors in Parkinson's disease:bidirectional link between disease onset and progression

Parkinson's disease has long been considered a disorder that primarily affects the brain,as it is defined by the dopaminergic neurodegeneration in the substantia nigra and the brain accumulation of Lewy bodies containingα-synuclein *** recent decades,however,accumulating research has revealed that Parkinson's disease also involves the gut and uncovered an intimate and important bidirectional link between the brain and the gut,called the“gut–brain axis.”Numerous clinical studies demonstrate that gut dysfunction frequently precedes motor symptoms in Parkinson's disease patients,with findings including impaired intestinal permeability,heightened inflammation,and distinct gut microbiome profiles and ***,α-synuclein deposition has been consistently observed in the gut of Parkinson's disease patients,suggesting a potential role in disease ***,individuals with vagotomy have a reduced Parkinson's disease *** these observations,researchers have hypothesized thatα-synuclein accumulation may initiate in the gut and subsequently propagate to the central dopaminergic neurons through the gut–brain axis,leading to Parkinson's *** review comprehensively examines the gut's involvement in Parkinson's disease,focusing on the concept of a gut-origin for the *** also examine the interplay between altered gut-related factors and the accumulation of pathologicalα-synuclein in the gut of Parkinson's disease *** the accessibility of the gut to both dietary and pharmacological interventions,targeting gut-localizedα-synuclein represents a promising avenue for developing effective Parkinson's disease therapies.
