3-Dimensional liver volume assessment in patients with hepatitis B virus-related liver cirrhosis during long-term oral nucleos(t)ide analogues therapy

AIMTo assess the effect of long-term oral nucleos(t)ide analogues (NUCs) therapy on liver volume change in patients with suppress hepatitis B virus (HBV)-related liver *** reviewed the data of naïve patients with HBV-related liver cirrhosis, who had taken oral NUCs therapy, between 2003 and 2007 at Chonbuk University Hospital. We analyzed two consecutive sets of abdominal computerized tomography scans-one at the time of treatment initiation and another at the second-year follow-up. Liver volume was calculated by 3-dimensional liver extraction volumetry *** total of 55 patients (34 males) were included. There was 114.3 mL ± 167.8 mL (12.9% ± 17.9%) of increase in liver volume during the two years of NUCs therapy (993.8 mL ± 242.8 mL at baseline vs 1108.1 mL ± 263.3 mL at two-year follow-up, P < 0.001). The ratio of the measured baseline liver volume to the estimated standard liver volume was improved from 70.8% to 78.0%. An increase in liver volume was shown not only in patients with compensated cirrhosis (P = 0.046) but also in those with decompensated cirrhosis (P < 0.001). Significant factors for volume increases were Child-Turcotte-Pugh grade and model for end-stage liver disease score improvement without virological breakthrough. In multiple linear regression analysis, delta albumin and delta alanine aminotransferase levels showed a significant association with the increase in liver volume (P = 0.002 and 0.005, respectively).CONCLUSIONLong-term oral NUCs therapy in patients with HBV-related liver cirrhosis lead to significant increase in liver volume assessed with 3-dimensional liver extraction volumetry program.

Wrinkle-free graphene with spatially uniform electrical properties grown on hot-pressed copper

The chemical vapor deposition （CVD） of graphene on Cu substrates enables the fabrication of large-area monolayer graphene on desired substrates. However, during the transfer of the synthesized graphene, topographic defects are unavoidably formed along the Cu grain boundaries, degrading the electrical properties of graphene and increasing the device-to-device variability. Here, we introduce a method of hot-pressing as a surface pre-treatment to improve the thermal stability of Cu thin film for the suppression of grain boundary grooving. The flattened Cu thin film maintains its smooth surface even after the subsequent high temperature CVD process necessary for graphene growth, and the formation of graphene without wrinkles is realized. Graphene field effect transistors （FETs） fabricated using the graphene synthesized on hot-pressed Cu thin film exhibit superior field effect mobility and significantly reduced device-to-device variation.

Nanoparticles can be involved in biological activities such as apoptosis, angiogenesis, and oxidative stress by themselves. In particular, inorganic nanoparticles such as gold and silica nanoparticles are known to inhibit vascular endothelial growth factor （VEGF）-mediated pathological angiogenesis. In this study, we show that anti-angiogenic effect of inorganic nanospheres is determined by their sizes. We demonstrate that 20 nm size gold and silica nanospheres suppress VEGF-induced activation of VEGF receptor-2, in vitro angiogenesis, and in vivo pathological angiogenesis more efficiently than their 100 nm size counterparts. Our results suggest that modulation of the size of gold and silica nanospheres determines their inhibitory activity to VEGF-mediated angiogenesis.

Long-term performance of fiber-grid-reinforced asphalt overlay pavements: A case study of Korean national highways

The objective of this study was to verify the effect of fiber grid reinforcement on the longterm performance of asphalt overlay pavements by performing field investigation, laboratory test, and finite element analysis. Crack, rutting, roughness, and other miscellaneous distresses of fiber-grid-reinforced overlay sections and ordinary overlay sections were compared. Pavement conditions of fiber grid overlay sections before and after the overlay were also compared. Cores were obtained from the fiber-grid-reinforced and ordinary sections to measure bonding shear strength between the intermediate and overlay *** energy, displacement after yield, and shear stiffness of the cores were also obtained by analyzing the test results. Finite element analysis was performed by using the test results to validate the effect of the fiber grid reinforcement on the long-term performance of asphalt overlay pavements. Rutting and fatigue cracking were predicted for three cases: 1) fiber grid was not used;2) glass fiber grid was used;and 3) carbon fiber grid was used. The cracking ratio of fiber-grid-reinforced sections was much smaller than that of ordinary sections. The results of field investigation, laboratory test, and finite element analysis showed that the fiber grid reinforcement improved the long-term performance of asphalt overlay pavements.

Optimization of rotor shape for constant torque improvement and radial magnetic force minimization

The design of notch and barrier was optimized in order to improve the characteristics of constant torque while minimizing the cogging torque that occurs as a result of teeth and slot structure. The barrier was installed in order to minimize the cogging torque and torque ripple by finite element method (FEM) with a reduced barrier width toward the center of magnetic pole. The position and width of notch, which can offset cogging torque, can be calculated with energy distribution of air-gap using Fourier series. The optimized model demonstrates a 60% decrease in the cogging torque, a 75.3% decrease in the torque ripple and a 3% increase in the operating torque when compared with the basic model.

Loss of CDX2 expression is associated with poor prognosis in colorectal cancer patients

AIM:To investigate the clinicopathologic characteristics and prognostic implications associated with loss of CDX2 expression in colorectal cancers(CRCs).METHODS:We immunohistochemically evaluated CDX2 expression in 713 CRCs and paired our findings to clinicopathologic and molecular characteristics of each *** included cytokeratin 7 and CK20 expression,microsatellite instability,Cp G island methylator phenotype,and KRAS and BRAF mutation *** and multivariate survival analysis was performed to reveal the prognostic value of CDX2 ***:CDX2 expression was lost in 42(5.9%) ***,loss of CDX2 expression was associated with proximal location,infiltrative growth,advanced T,N,M and overall *** microscopic examination,loss of CDX2 expression was associated with poor differentiation,increased number of tumor-infiltrating lymphocytes,luminal serration and mucin *** of CDX2 expression was also associated with increased CK7 expression,decreased CK20 expression,Cp G island methylator phenotype,microsatellite instability and BRAF *** a univariate survival analysis,patients with loss of CDX2 expression showed worse overall survival(P < 0.001) and progression-free survival(P < 0.001).In a multivariate survival analysis,loss of CDX2 expression was an independent poor prognostic factor of overall survival [hazard ratio(HR) = 1.72,95%CI:1.04-2.85,P = 0.034] and progression-free survival(HR = 1.94,95%CI:1.22-3.07,P = 0.005).CONCLUSION:Loss of CDX2 expression is associated with aggressive clinical behavior and can be used as a prognostic marker in CRCs.

Unexpected positive cultures including isolation of Propionibacterium acnes in revision shoulder arthroplasty

Objective Little information is available to guide treatment strategies regarding patients with unexpected positive cultures （UPC）,including Propionibacterium acnes （PA）,without overt signs of infection in revision shoulder arthroplasty （RSA）.The purpose of our study was to analyze the prevalence,clinical meaning,treatment and prognosis of UPC in *** sources and study selection We performed a systematic review of the literature between 1950 and 2013 for all studies reporting on UPC for *** with the prevalence and prognosis of patients with UPC in RSA were *** Six studies meeting our inclusion criteria yielded data for 1 402 patients who underwent a total of 1405 *** on the available data,following RSA 235 shoulders had UPCs with a pooled percentage of 16.7％（235/1405）.The most commonly isolated bacteria from shoulders following RSA with UPCs was PA with pooled percentages of 63.4％（149/235）.Occurrence of true infection from UPCs after RSA was seen in 24 shoulders （24/235,10.2％）.Antibiotic use did not influence the rate of the occurrence of true infection from UPCs （P=0.498）.Conclusions Our study showed a low risk of having a true infection from UPCs after RSA without clinical signs of preoperative infection at the time of the ***,prolonged antibiotic therapy may not be necessary in these patients.

KML001, an arsenic compound, as salvage chemotherapy in refractory biliary tract cancers: A prospective study

Background: Sodium meta-arsenite(NaAsO_2, KML001) is a potential oral anticancer agent acting on telomerase and telomere length. This prospective study evaluated its safety, tolerability, and effectiveness as salvage chemotherapy in patients with advanced biliary tract cancer(BTC) resistant to gemcitabinebased chemotherapy. Methods: Forty-four patients(21 women and 23 men) with advanced BTC and failure history of gemcitabine-based chemotherapy, performance status(PS) 0–2, normal cardiac, hepatic, and renal function were enrolled. Daily dose of KML001(7.5 mg. p.o.) was administered to eligible subjects for 24 weeks divided into six treatment cycles. Response was evaluated bimonthly using CT. Results: After an average of 1.5 months of treatment(range: 0.5–10.0), 3 patients(6.8%) obtained progression-free status, 23 patients(52.3%) had disease progression, and 18 patients(40.9%) dropped out before evaluation. One patient(2.3%) completed six treatment cycles without progression. During the treatment, morphine dosage kept the same or decreased in 20 patients(47.6%). Nine patients(20.5%) experienced grade-3 adverse events(AEs), while no patient experienced grade-4 AEs. The most common AEs were liver enzyme elevation(11/44, 25%) and anemia(10/44, 22.7%). KML001 was discontinued in six patients(13.6%) due to AEs, including liver toxicity( n = 3), QTc prolongation( n = 2), and abdominal pain( n = 1). Conclusions: KML001 did not have enough anticancer effect on patients with advanced BTC resistant to gemcitabine. However, KML001 was safe and well-tolerable in terms of AEs and pain control when used as salvage therapy. Further studies are needed to establish arsenic agents as a reliable treatment option in patients with BTC.

Development of a skill assessment tool for the Korea operational oceanographic system

A standard skill assessment （SA） tool was developed and implemented to evaluate the performance of op- erational forecast models in the Korea operational oceanographic system. The SA tool provided a robust way to assess model skill in the system by comparing predictions and observations, and involved the com- putation of multiple skill metrics including correlation and error skills. User- and system-based acceptance criteria of skill metrics were applied to determine whether predictions were acceptable for the system. To achieve this, the tool produced a time series comparison plot, a skill score table, and an advanced sum- marized diagram to effectively demonstrate the multiple skill scores. Moreover, the SA was conducted to evaluate both atmospheric and hydrodynamic forecast variables. For the atmospheric variables, acceptable error criteria were preferable to acceptable correlation criteria over short timescales, since the mean square error overwhelmed the observation variance. Conversely, for the hydrodynamic variables, acceptable root mean square percentage error （e.g., perms） criteria were preferable to acceptable error （e.g., erms） criteria owing to the spatially variable tidal intensity around the Korean Peninsula. Furthermore, the SA indicated that predetermined acceptance error criteria were appropriate to satisfy a target central frequency （fc） for which errors fell within the specified limits （i.e., the .fc equals 70%）.

Stacking plates of CFRP composite materials are increasingly used because of their unique characteristics. However, unlike other materials used in metallurgy they have a disadvantage of uneven quality and anisotropy when combined with other composites. Hence, specimens of CFRP stacking plates are manufactured by changing orientation angles throughout three quasiisotropic plies （0°/45°/90°/-45°）68, （03°/453°/903°/-453°）2s, and （06°/456°/906°/-456°）s and throughout three cross plies （0°/90°/0°/90°）68, （03 °/903 °/03°/903°）2s, and （06 °/906 °/06°/906°）8 In this study 3-point bending tests and transverse bending tests have been carried out in order to find out mechanical characteristics according to orientation angles by stacking in 6 different types along with the change of stacking composition method of a CFRP composite.
