FOCUS: Future of fecal calprotectin utility study in inflammatory bowel disease

AIM To evaluate the perspective of gastroenterologists regarding the impact of fecal calprotectin(FC) on the management of patients with inflammatory bowel disease(IBD).METHODS Patients with known IBD or symptoms suggestive of IBD for whom the physician identified that FC would be clinically useful were recruited. Physicians completed an online "pre survey" outlining their rationale for the test. After receipt of the test results, the physicians completed an online "post survey" to portray their perceived impact of the test result on patient management. Clinical outcomes for a subset of patients with follow-up data available beyond the completion of the "post survey" were collected and *** Of 373 test kits distributed, 290 were returned, resulting in 279 fully completed surveys. One hundred and ninety patients were known to have IBD; 147(77%) with Crohn's Disease, 43(21%) Ulcerative Colitis and 5(2%) IBD unclassified. Indications for FC testing included: 90(32.2%) to differentiate a new diagnosis of IBD from Irritable Bowel Syndrome(IBS), 85(30.5%) to distinguish symptoms of IBS from IBD in those known to have IBD and 104(37.2%) as an objective measure of inflammation. FC levels resulted in a change in management 51.3%(143/279) of the time which included a significant reduction in the number of colonoscopies(118) performed(P < 0.001). Overall, 97.5%(272/279) of the time, the physicians found the test sufficiently useful that they would order it again in similar situations. Follow-up data was available for 172 patients with further support for the clinical utility of FC *** The FC test effected a change in management 51.3% of the time and receipt of the result was associated with a reduction in the number of colonoscopies performed.

To confirm a hybrid swarm population of Pinus densiflora × P. sylvestris in Jilin, China, we used needles and seeds from P. densiflora, P. sylvestris, and P. densiflora × P. sylvestris collected from natural stands or experimental stations to study whether shoot apex morphology of 4-year old seedlings can be correlated with the sequence of a chloroplast DNA simple sequence repeat marker （cpDNA SSRs）. Total genomic DNA was extracted and subjected to sequence analysis of the pine cpDNA SSR marker Pt15169. Results show that morphological characters from 4-year old seedlings did not correlate with sequence variants of this marker. Marker haplotypes from all P. sylvestris trees had a CTAT element that was absent from all sampled P. densiflora trees. However, both haplotype classes involving this insertion/deletion element were found in a P. densiflora × P. sylvestris population and its seedling progeny. It was concluded that the P. densiflora × P. sylvestris accessions sampled from Jilin, China resulted from bi-directional crosses, as evidenced by both species’ cpDNA haplotypes within the hybrid swarm population.

目的:验证血管外周脂肪组织释放血管外膜源性舒张因子(ADRF),并对ADRF的作用机制做初步探讨。方法:检测WKY大鼠带完整血管外周脂肪组织的血管环和不带血管外周脂肪组织的裸血管环的收缩力;并用液体转移方法鉴证血管外周脂肪组织释放ADRF,使用工具药对ADRF作用途径进行观察。结果:与配对裸血管相比,带完整血管外周脂肪组织的胸主动脉在苯肾上腺素(PHE,10-6mol/L)诱发的血管收缩力下降20.5%;把孵育带完整血管外周脂肪组织的胸主动脉的生理池液体转移到配对裸血管,引起裸血管舒张20.8%;在无钙液中,有无血管外周脂肪组织的血管之间PHE诱发的收缩力无区别;钙激活的钾通道(KCa)阻断剂四乙胺和ATP敏感性钾通道(KATP)阻断剂格列苯脲均有效阻断ADRF的血管舒张作用。结论:血管外周脂肪组织释放一种引起血管舒张的ADRF,ADRF通过激活KCa、KATP发挥作用,且需钙离子参与。

Characteristics, Treatment, and Outcomes Associated with Clostridium difficileAssociated Diarrhea in a Veterans Affairs Medical Center

We conducted a study to assess the characteristics, treatment and outcomes associated with Clostridium difficile associated diarrhea in a Veterans Affairs Medical Center. Fifty-eight consecutive individual cases of C. difficile infection in 2013 were observed within the Veterans Affairs New Jersey Health Care System (VA NJHCS). We molecularly typed all 58 individual strains and identified the associated characteristics, treatment and outcomes. Forty-four out of 58 specimens (76%) which were probed had characteristics of the epidemic strain bi/NAP1/027 making this virulent strain to be the predominate strain at the VA NJHCS. All C. difficile bi/NAP1/027 strains were resistant to fluoroquinolones and sensitive to fidaxomicin, metronidazole and vancomycin. Fidaxomicin had the most potent in vitro activity (MIC90 = 0.5 μg/ml) against the bi/NAP1/027 strain. Twenty-six of 44 patients (59%) with the virulent strain were from a long-term care facility (LTCF). Patients possessing the virulent strain from the LTCF had a mean APACHE II score of 14.1 and a predicted death rate of 21.9%. Two-third of patients were treated with metronidazole alone (mean APACHE II scores 9.6), and one-third required oral vancomycin and metronidazole (mean APACHE II scores 14.1). There were no C. difficile infection related deaths. C. difficile bi/NAP1/027, an epidemic strain, is the endemic strain at the VA NJHCS, but no increased mortality was seen with infection with this strain.

IoTFLiP： IoT-based flipped learning platform for medical education

Case-Based Learning （CBL） has become an effective pedagogy for student-centered learning in medical education, which is founded on persistent patient cases. Flippped learning and Internet of Things （IoTs） concepts have gained significant attention in recent years. Using these concepts in conjunction with CBL can improve learning ability by providing real evolutionary medical eases. It also enables students to build confidence in their decision making, and efficiently enhances teamwork in the learning environment. We propose an IoT-based Flip Learning Platform, called IoTFLiP, where an IoT infrastrneture is exploited to support flipped case-based learning in a cloud environment with state of the art security and privacy measures for personalized medical data. It also provides support for application delivery in private, public, and hybrid approaches. The proposed platform is an extension of our Interactive Case-Based Flipped Learning Tool （ICBFLT）, which has been developed based on current CBL practices. ICBFLT formulates summaries of CBL cases through synergy between students＇ and medical expert knowledge. The low cost and reduced size of sensor device, support of IoTs, and recent flipped learning advancements can enhance medical students＇ academic and practical experiences. In order to demonstrate a working scenario for the proposed IoTFLiP platform, real-time data from IoTs gadgets is collected to generate a real-world case for a medical student using ICBFLT.

Balanced Topology NEMO Construction for the Internet-Based MANET

A mobile ad hoc network (MANET) is a wireless network without any fixed infrastructure. All nodes must communicate with each other by a predefined routing protocol. Most of routing protocols don’t consider binding internet addresses to mobile nodes. However, in network mobility (NEMO), all mobile nodes can’t only communicate with Internet using bi-directional tunneling but also can be allocated an Internet address. In this paper, we propose two algorithms with the nested NEMO topology to reconstruct the Internet-based MANET. Additionally, a novel load balancing solution is proposed. The Mobile Router (MR) which acts as a central point of internet attachment for the nodes, and it is likely to be a potential bottleneck because of its limited wireless link capacity. We proposed a load-information in the route advertisement (RA) message. The simulation results show that the proposed solution has significantly improved the connection throughput.

The effects of microwave sintering on the sintering behaviour, microstructure and dielectric properties of bi2O3-doped (Ba0.6Sr0.4)(Ti0.94Cu0.06)O3 (BSTC) ceramics were investigated. The microstructure and dielectric properties of a BSTC ceramic were also studied given different amounts of bi2O3 doping. Microwave heating with sintering temperatures below 1000°C significantly improves the densification of bi2O3-doped BSTC ceramics. The BSTC ceramic with 1 wt% bi2O3 addition sintered at 950°C in air for 30 min exhibited dielectric properties of er = 3756, dielectric loss of tanδ = 7 × 10-3 and bulk density > 96% of theoretical density.

钼和镧是好的镍硼触媒的促进剂, 对于对氯硝基苯氢化反应有好的效果, 因此同时加入钼和镧在镍硼触媒上应该是有趣的研究题目. 本研究利用化学还原法制备了一系列含有不同量镧的镧钼镍触媒, 醋酸镍、钼酸胺及硝酸镧同时加入含有甲醇的水溶液中. 以X光绕射, 穿透式电子显微镜、高分辨率电子显微镜及X光光电能谱仪鉴定了这些触媒. 对氯硝基苯氢化反应是在批次反应器中于333 K、1.2 MPa氢气压力及500 r/min转速下操作. 即使在镍钼硼中加入很少量的镧, 对提高活性及对氯苯胺的选择性都有显著的效果, 加入适量的镧可以抑制镍钼硼的长范围的结晶排列, 并保持镍钼硼在非结晶状态. 镧以氧化态存在, 其功效为阻隔物, 避免镍钼硼聚集, 但是过量的镧会有相反的效果, 镧和镍的适当比例是0.2.
