Can emergency physicians perform extended compression ultrasound for the diagnosis of lower extremity deep vein thrombosis?

BACKGROUND:Current point-of-care ultrasound protocols in the evaluation of lower extremity deep vein thrombosis (DVT) can miss isolated femoral vein *** compression ultrasound (eCUS) includes evaluation of the femoral vein from the femoral vein/deep femoral vein bifurcation to the adductor *** objective is to determine if emergency physicians (ePs) can learn eCUS for lower extremity DVT evaluation after a focused training ***:Prospective study at an urban academic *** with varied ultrasound experience received instruction in eCUS prior to *** live models with varied levels of difficult sonographic anatomy were intentionally chosen for the *** participant scanned both ***-and post-study surveys were ***:A total of 96 ultrasound examinations were performed by 48 participants (11 attendings and 37 residents).Participants' assessment scores averaged 95.8% (95% CI 93.3%-98.3%) on the easier anatomy live model and averaged 92.3% (95% CI 88.4%-96.2%) on the difficult anatomy *** were no statistically significant differences between attendings and *** the model with easier anatomy,all but 1 participant identified and compressed the proximal femoral vein successfully,and all participants identified and compressed the mid and distal femoral *** the difficult anatomy,97.9% (95% CI93.8%-102%) identified and compressed the proximal femoral vein,whereas 93.8% (95% CI 86.9%-100.6%) identified and compressed the mid femoral vein,and 91.7% (95% CI83.9%-99.5%) identified and compressed the distal femoral ***:ePs at our institution were able to perform eCUS with good reproducibility after a focused training session.

Management of complicated gallstones in the elderly:comparing surgical and non-surgical treatment options

Objective:The aim of this study was to evaluate the differences in clinical outcomes of endoscopic retrograde cholangiopancreatography(eRCP),eRCP followed by cholecystectomy(eC)and percutaneous aspiration(PA)in the elderly population with ***:We included a total of 43338 elderly patients aged 60 years or older and 45295 patients younger than 60 years for comparison in our *** were obtained from the Nationwide Inpatient Sample(Healthcare Utilization Project)for years 2001–14 by identifying patients who were admitted for gallstone complications based on the ICD 9 diagnostic *** logistic regression was used to calculate the odds of in-hospital mortality and to detect statistical differences among the treatment groups,age groups and between male and female *** ordinary linear regression was used to compare the length of hospital stay and readmission frequency among the different age ***:The age of the patient affected mortality and the length of hospital stay after any type of procedure of gallstones *** a manner independent of the patient’s age,PA was associated with the highest risk of death and length of stay,while the eC was characterized by lowest mortality and eRCP by the shortest length of *** age of the patient nor the type of procedure affected the likelihood of *** odds of death and the probability of readmission were not affected by patient ***,in patients aged between 60 and 79 years,the female gender predicted a shorter duration of stay in the ***:A patient’s age negatively affects the treatment outcomes of cholelithiasis with associated *** eC procedure appears to be the method of choice for the management of complicated gallstones in patients of all ages.

Thermoluminescence study of rare earth ion (Dy^(3+)) doped LiF:Mg crystals grown by eFG technique

This work describes investigation of radiation dosimetry characteristics of magnesium （Mg） doped lithium fluoride （LiF） crystals co-doped with different concentrations of dysprosium （Dy）. These crystals were grown by edge defined film fed crystal growth （eFG） tech- nique. Thermoluminescence （TL） measurements were made on as grown （AG） and annealed （AN） crystals after they were irradiated with a gamma dose of 15 Gy. The influence and advantage of optimized Dy^3＋ concentration in enhancing the thermoluminescence （TL） properties of LiF：Mg samples were discussed. The normalized peak height of the annealed crystals was nearly 3 times that of the as grown crystals for the particular concentration samples irradiated with Co6~ gamma source showed linearity up to 10 Gy. Thermoluminescence signal observed over a period of one month showed negligible fading. Thermoluminescence glow curve structure of optimized phosphor remained stable for higher doses of gamma rays of 103 Gy. Glow curve was analyzed using computer glow curve deconvolution （CGCD） method and trapping parame- ters were calculated.

High-throughput phenotyping identifies plant growth differences under well-watered and drought treatments

The ability to screen larger populations with fewer replicates and non-destructive measurements is one advantage of high-throughput phenotyping(HTP)over traditinal phenotyping *** this study,two wheat accessions were grown in a controlled-environment with a moderate drought imposed from stem elongation to ***-green-blue(RGB)imaging was performed on 17 of the 22 d following the start of drought *** measurements from all plants were performed at the conclusion of the *** effect of line was signifcant for shoot dry matter,spike dry matter,root dry matter,and tller number,while the water treatment was significant on shoot dry matter and root dry *** temporal,non-destructive nature of HTP allowed the drought treatment to be significantly differentiated from the well-watered treatment after 6 d in a line from Argentina and 9 d in a line from *** difference of 3 d indicated an increased degree of drought tolerance in the line from ***,HTP from the final day of imaging accurately predicted reference plant height(r=1),shoot dry matter(r=0.95)and tller number(r=0.91).This experiment ilustrates the potential of HTP and its use in modeling plant growth and development.

The doctrine of＇the golden mean＇ of the Confucian certainly applies at the molecular level to cell growth and migration. It is critically important for tissue architecture and homeostasis that cells stop prolifera- tion when reaching appropriate density and halt migration in a direction to avoid collision with others. This ＇red light＇ to hinder cell movement is essential for maintaining contact inhibition of locomotion （CIL）--a phe- nomenon that a cell ceases to continue moving in the same direction when it comes into contact with another cell. The concept of CIL emerged initially from the early work of Abercrombie and Heaysman in the 1950s.

The viability of some waste as cooling pads for evaporative cooling application in South-Western Nigeria was experimentally *** is to ascertain their effectiveness as a substitute for costly imported pads in a low income *** presented was the feasibility of utilizing standalone evaporative coolers for storage and selling of fruits in South-Western *** ambient air was forced through the various pads at three different fan speeds and constant cooling pad thickness of 30 *** characteristics were considered based on daily analysis using temperature and humidity data measured from morning to evening at location co-ordinates latitude 7°10′N and longitude 5°05′e for 6 *** daily temperature T and humidity h ranged between 26℃≤T≤45℃ and 28%≤h_(2)≤80%.Temperature differenceDT and humidity differenceDh of 0.6℃≤ΔT≤18.3℃ and 1.0%≤Δh≤53%was achieved for the four cooling pad materials tested at three fan *** andDh was recorded at fan speed of 4 m/s with shredded latex foam and jute sack *** cooling efficiency(η)calculated for all the pads under the three speeds ranged from 17.3%≤η≤98.8%.Payback period(PBP)analysis indicated the considered eVC is economically feasible and investors will break even in 1.75 years.

This article contributes to a small but growing body of multi-sited and multi-scalar research on the Belt and Road *** focus on relations at the national,regional and international scales,and present original research from China and Turkey,to show how the Istanbul-Ankara high-speed railway has served as a testing ground for China's Belt and Road Initiative(BRI).Its construction was initially funded by the european Investment Bank,but it is now part of the backbone of the Turkish Government's Middle Corridor plan which enhances west-east connectivity and integration with the Caucasus and Central *** show that in contrast to multinational corporations from the OeCD that seek to remain footloose,Chinese state-owned enterprises(SOes)seek to adapt to,apprehend and ultimately shape local *** the case of Turkey this proved difficult given its institutional alignment with the european ***,while the railway project was completed successfully by a consortium led by a Chinese SOe,Turkey's dynamic and complex regulatory environment discourages Chinese SOes in the infrastructure *** conclude that the Turkish and Chinese governments are currently pursuing complementary territorial visions yet their cooperation is project-based and pragmatic.

Bromide ion mediated modification to digestive ripening process： Preparation of ultra-small Pd, Pt, Rh and Ru nanoparticles

Nanoparticles of catalytically important transition metals, such as Pd, Pt, Rh, and Ru have been prepared by the well-known ＂digestive ripening＂（DR） and ＂modified digestive ripening＂（mDR） methods. In the traditional DR process, a polydisperse colloidal dispersion is refluxed in the presence of a surface-active molecule, such as alkanethiol. The mDR method involved a small modification in the procedure, wherein refluxing was performed with an alkanethiol and a tetra-alkylammonium bromide surfactant. This minor modification led to a dramatic change in the final particle size distributions, giving access to nano- particles in the 〈3 nm size regime; this was not possible with the traditional DR process. Bromide ions, which are present during refluxing, proved to be an important ingredient in the modification process. These bromide ions are revealed to act as etchants, resulting in ultra-small nanoparticles. All transition metal nanoparticles investigated displayed catalytic activity in the reduction reaction of p-nitro phenol. Pd nanoparticles, synthesized by a modified digestive ripening method, exhibited the best catalytic activity among the systems investigated.

Juvenile polyps are relatively common findings in children,while juvenile polyposis syndrome(JPS) is a rare hereditary syndrome entailing an increased risk of colorectal *** in BMPR1A or SMAD4 are found in roughly half of patients diagnosed with *** in PTeN gene are also found in patients with juvenile polyps and in Bannayan-Riley-Ruvalcaba syndrome and Cowden *** previous reports have described microdeletions in chromosome 10q23 encompassing both PTeN and BMPR1A causing aggressive polyposis and malignancy in *** reports have also described extra-intestinal findings in most cases including cardiac anomalies,developmental delay and *** this report we describe a boy with a 5.75 Mb deletion of chromosome 10q23 and a 1.03 Mb deletion within chromosome band 1p31.3who displayed aggressive juvenile polyposis and multiple extra-intestinal anomalies including macrocephaly,developmental delay,short stature,hypothyroidism,atrial septal defect,ventricular septal defect and *** required colectomy at six years of age,and early colectomy was a common outcome in other children with similar *** to the aggressive polyposis and reports of dysplasia and even malignancy at a young age,we propose aggressive gastrointestinal surveillance in children with 10q23 microdeletions encompassing the BMPR1A and PTeN genes to include both the upper and lower gastrointestinal tracts,and also include a flowchart for an effective genetic testing strategy in children with juvenile polyposis.

Hydromagnetic oscillatory Couette flow in rotating system with induced magnetic field

This paper presents a study of hydromagnetic Couette flow of an incompress- ible and electrically conducting fluid between two parallel rotating plates, one of which is oscillating in its own plane. A uniform transverse magnetic field is used, and the induced magnetic field is taken into account. The exact solution to the governing equations is obtained in a closed form. The solution to the problem in the case of vanishing and small finite magnetic Prandtl numbers is also derived from the general solution. The asymp- totic behavior of the solution for large values of the frequency parameter is analyzed to gain some physical insights into the flow pattern. expressions for the shear stress at both the oscillatory and stationary plates due to primary and secondary flows and mass flow rate in the primary and secondary flow directions are also obtained. The results of the fluid velocity and the induced magnetic field are presented. The shear stresses on the plates due to the primary and secondary flows and the corresponding mass flow rates are presented in a tabular form.
