Is P2/MS score valuable for prediction in HBV-related variceal bleeding?

Objective:To determine the predictive value of P2/MS in patients with chronic HBV-related cirrhosis, and to predict high-risk esophageal varices, and obtain a cut-off ***:A total of 412 patients with HBV-related cirrhosis who were admitted to our hospital between August 2014 and August 2017 were retrospectively evaluated. A diagnosis of cirrhosis was made with standard laboratory, radiological and physical examination findings. According to these evaluations, esophageal varices were classified as small, medium and large. For all obtained data, P2/MS was calculated. Two threshold values (P2/MS25) were considered in predicting the presence of high-risk EVs during recording. And the optimal cut-off value of the P2/MS index was determined for high-risk esophageal varices in patients with chronic viral hepatitis ***:A total of 375 patients who met the inclusion criteria were included in the study. When the P2/MS index was compared with other noninvasive tests, the mean and median P2/MS scores were respectively 54.17 and 33.25. The P2/MS value of the patients without esophageal varices was higher than that of the patients with esophageal varices. When these results were evaluated, the higher the score, the lower the risk of varices. We obtained a positive predictive value of 93.80% [95%CI(80.20-98.70)] when the cut-off value of P2/MS was taken as gative predictive value of 94.30% [95%CI(86.20-98.20%)] when the cut-off value of P2/MS was taken as >***:We could predict the patients with high-risk esophageal varices within this group at a extremely good rate. We also compared the results of this test with other non-invasive tests and achieved successful results. We have shown that P2/MS can be used in order to optimally select patients for endoscopic screening and prevent all of the expensive and unnecessary procedures safely.

The present contactless measurements of magnetoresistance of large area graphene films grown by chemical vapor deposition method on semi-insulating SiC substrates. For this purpose we propose microwave technique using single post dielectric resonator operating at frequency about 13.5 gHz. Comparison of microwave measurements with classic van der Pauw method shows good agreement and proves usefulness of contactless technique in magnetoresistance studies. Experiments have been performed at 4.2 K in magnetic fields up to 7 T. Significant differences in graphene magnetoresistance depending on the orientation and crystal structure of SiC substrates have been observed.

Let F, N、A and N2 denote the properties of being finite, nilpotent, abelian and nilpotent of classes at most 2, respectively. Firstly we consider the class of finitely generated FTV-groups. Wc show that the property FC is closed under finite extensions, and extend this result to finitely generated NF-groups. Secondly we prove that a finitely generated NF-group g is in the class ((FC)F, oo) if and only if g is an FA-group. Finally we prove that a finitely genera ted ATF-group in the class ((FC)F,∞)^* is an FM-group. Moreover, g/Z2(g) is finite.

Evaluation of leukocyte esterase and nitrite strip tests to detect spontaneous bacterial peritonitis in cirrhotic patients

AIM: To investigate the diagnostic eff icacy of leukocyte esterase and nitrite reagent strips for bedside diagnosis of spontaneous bacterial peritonitis (SBP). METHODS: A total of 63 consecutive patients with cirrhotic ascites (38 male, 25 female) tested between April 2005 and July 2006 were included in the study. Bedside reagent strip testing was performed on ascitic ? uid and the results compared to manual cell counting and ascitic fluid culture. SBP was defined as having a polymorphonuclear ascites count of ≥ 250/mm3. RESULTS: Fifteen samples showed SBP. The sensitivity, specificity, positive and negative predictive values of the leukocyte esterase reagent strips were; 93%, 100%, 100%, and 98%, respectively. The sensitivity, specifi city, positive and negative predictive value of the nitrite reagent strips were 13%, 93%, 40%, and 77%, respectively. The combination of leukocyte esterase and nitrite reagents strips did not yield statistically signif icant effects on diagnostic accuracy. CONCLUSION: Leukocyte esterase reagent strips may provide a rapid, bedside diagnostic test for SBP.

Application of scoring systems with point-of-care ultrasonography for bedside diagnosis of appendicitis

BACKgROUND: Appendicitis is a common disease requiring surgery. Bedside ultrasound(BUS) is a core technique for emergency medicine(EM). The Alvarado score is a well-studied diagnostic tool for appendicitis. This study aimed to investigate the relationship between patients' symptoms, Alvarado score and ultrasound(US) findings, as performed by emergency physicians(EPs) and radiologists, of patients with suspected ***: Three EM specialists underwent the BUS course and core course for appendicitis assessment. Patients suspected of having appendicitis were selected and their Alvarado and modif ied(m) Alvarado scores calculated. The specialists performed the BUS. Then, patients were given a formal US and surgery consultation if necessary. Preliminary diagnoses, admission or discharge from the emergency department(ED) and fi nal diagnosis were documented. The patients were also followed up after discharge from the ***: The determined cut-off value was 2 for Alvarado and 3 for m Alvarado scores. The sensitivities of the two scores were 100%. Each score was used to rule out appendicitis. The results of EP-performed BUS were as follows: accuracy 70%, sensitivity 0.733, specifi city 0.673, + LR 2.24, and – LR 0.40(95%CI). Radiologists were better than EPs at diagnosing appendicitis and radiologists and EPs were equally strong at ruling out appendicitis by US. When US was combined with Alvarado and m Alvarado scores, EP US+Alvarado/m Alvarado scores ≤3 and radiology US+Alvarado/m Alvarado scores ≤4 perfectly ruled out ***: BUS performed by EPs is moderately useful in detecting appendicitis. Combined with scoring systems, BUS may be a perfect tool for ruling out decisions in EDs.

Circulating microRNAs in patients with non-alcoholic fatty liver disease

AIM: To identify novel non-invasive biomarkers for non-alcoholic fatty liver disease(NAFLD). METHODS: Twenty patients with histologically proven NAFLD and 20 controls were included. All NAFLD cases were scored using the NAFLD activity score. The rela-tive expressions of miR-197, miR-146 b, miR-10 b, miR-181d, miR-34 a, miR-122, miR-99 a and miR-29 a were analyzed using real-time polymerase chain reaction. RESULTS: Serum levels of miR-181 d, miR-99 a, miR-197 and miR-146 b were significantly lower in biopsy-proven NAFLD patients than in the healthy controls. Serum lev-els of miR-197 and miR-10 b were inversely correlated with degree of inflammation and miR-181 d and miR-99 a were inversely correlated with serum gamma glu-tamyl transferase levels in non-alcoholic steatohepatitis patients. CONCLUSION: NAFLD is associated with altered se-rum miRNA expression pattern. This study provides clues for defining the non-invasive diagnosis of NAFLD.

目的术后显像有助于对分化型甲状腺癌(DTC)患者进一步治疗方案制定提供依据;然而低剂量131Ⅰ会导致顿抑并对清除残余甲状腺组织(简称清甲)治疗反应欠佳。该回顾性研究目的在于比较DTC患者手术后131Ⅰ显像、99Tcm-高锝酸盐显像、血清甲状腺球蛋白(Tg)和抗甲状腺球蛋白抗体(TgAb)的水平。方法回顾性评估接受甲状腺切除手术的DTC患者。所有患者均在术后行99Tcm-高锝酸盐和清甲治疗后131Ⅰ显像,并测定血清Tg和TgAb水平。盲法评估清甲治疗前的高锝酸盐显像,与清甲后131Ⅰ显像、血清Tg及TgAb水平进行比较。结果该研究共纳入接受甲状腺切除术的DTC患者174例(女146例,男28例),平均年龄为(48.7±13.1)(1284)岁。174例患者中,6例(3%)131Ⅰ和99Tcm-高锝酸盐显像阴性。168例131Ⅰ显像阳性患者中,131例(75%)高锝酸盐显像中至少有1个部位阳性病灶,19例(11%)有可疑摄取,18例(11%)呈阴性。对每个病灶进行分析,如果高锝酸盐显像阳性灶与清甲后131Ⅰ显像中的阳性灶一致,则认为诊断正确。在131Ⅰ显像中356个阳性灶中,273个病灶(77%)高锝酸盐显像明显阳性,41个病灶(11%)为可疑阳性,42个病灶(12%)不能被检测到。高锝酸盐显像的阴性和阳性病灶在Tg水平方面差异有统计学意义(Pg水平是检测DTC患者残留甲状腺组织的重要参数。

目的在儿童霍奇金淋巴瘤（HL）的治疗中,采取反应适应性治疗策略来减少不良反应非常重要,然而关于充分或不充分治疗反应的定义仍存在争议。为确定18F-脱氧葡萄糖（FDg）PET/CT（PET）预测疾病结局的最佳方法,拟对3种不同方法进行比较。方法根据修订的国际协调项目标准（IHP）和Deauville标准对72例HL患儿治疗前基线、治疗中期、治疗结束后的PET显像结果进行解读。用受试者工作特征（ROC）曲线获得治疗中期和治疗结束后FDg最大标准摄取值的变化值(△SUVmax),用于预测无进展生存（PFS）。比较定量数据与可视化图像数据预测PFS的效能。结果原发病灶治疗中期和治疗结束后平均△SUVmax分别为（77.4±19.5）%、（68.8±30.4）%,截断值为82%和73%。只有治疗结束后△SUVmax对3年PFS的预测有统计学意义（P=0.043）。根据文献报道的截断值（66%和77%）进一步分析治疗中期△SUVmax,结果仍不具统计学意义（P=0.604和0.431）。对于治疗中期评价,IHP标准与Deauville标准（P=0.002和0.001）和△SUVmax（P=0.03）相关;而对于治疗后评价,IHP标准与△SUVmax明显相关（P=0.04）,但与Deauville标准的相关性不明显（P=0.055和0.058）。1年、3年和5年PFS分别为（95.7±0.2）%、（89.6±0.4）%和（80.8±0.7）%。3种方法在预测3年PFS方面表现相当,但是治疗中期使用Deauville标准和治疗结束后使用IHP标准进行评估更具意义。3种方法都显示出较高的阴性预测价值和较低的阳性预测价值。结论在儿童HL预后评估中,治疗中期用Deauville标准解读PET显像结果优于其他方法,而在治疗结束后可以使用IHP标准进行定量分析和视觉评估。研究得出的SUVmax降低的最佳截断值是73%。

Egypt, Possible Conflict and Cooperation Areas in the Foreign Policy of the Islamic Republic of Iran

A country’s foreign policy process is also a decision-making process. Foreign policy is a rational, multifaceted, and balanced area that needs to be thought and executed. States operate within the framework of the decisions of governments. The decision-making process can change its direction with reflections and influences from national, regional, and international politics. While some theories think that governments/decision-makers are rational actors, there are other theories. Decision-makers/governments are able to make decisions in accordance with national interests. Who or which institution will determine what a country’s national interest is and whether its foreign policy is in the national interest. Claiming that a foreign policy carried out by political will is in line with national interest and gives it a certain degree of legitimacy and authority. The main problem here is the identification of national interest with the discourse/policies of those officially responsible for the conduct of foreign policy. National interest can best be determined if it has become a verifiable expression of the priorities of a nation. Heads of state trust their advisors in their immediate surroundings when making foreign policy decisions, so it is important to recognize the composition of the immediate environment. In authoritarian and totalitarian regimes, such as Iran, ideology and leader are vital in foreign policy. The Iranian geopolitics, the ideological structure of the political regime, and the constitutional structure, which is constitutionally effective and superior, should be analyzed in order to examine Iran’s foreign policy. This comprehensive assay will naturally exceed the purpose and scope of this study.
