Seasonal Performance of Solar Power Plants in the Sahel Region: A Study in Senegal, West Africa

The main objective of this study is to evaluate the seasonal performance of 20 MW solar power plants in Senegal. The analysis revealed notable seasonal variations in the performance of all stations. The most significant yields are recorded in spring, autumn and winter, with values ranging from 5 to 7.51 kWh/kWp/day for the reference yield and 4.02 to 7.58 kWh/kWp/day for the final yield. These fluctuations are associated with intense solar activity during the dry season and clear skies, indicating peak production. Conversely, minimum values are recorded during the rainy season from June to September, with a final yield of 3.86 kWh/kW/day due to dust, clouds and high temperatures. The performance ratio analysis shows seasonal dynamics throughout the year with rates ranging from 77.40% to 95.79%, reinforcing reliability and optimal utilization of installed capacity. The results of the capacity factor vary significantly, with March, April, May, and sometimes October standing out as periods of optimal performance, with 16% for Kahone, 16% for Bokhol, 18% for Malicounda and 23% for Sakal. Total losses from solar power plants show similar seasonal trends standing out for high loss levels from June to July, reaching up to 3.35 kWh/kWp/day in June. However, using solar trackers at Sakal has increased production by up to 25%, demonstrating the operational stability of this innovative technology compared with the plants fixed panel. Finally, comparing these results with international studies confirms the outstanding efficiency of Senegalese solar power plants, other installations around the world.

Treatment of Kidney Stones by Percutaneous Nephrolithotomy: Evaluation of the Results of the First Series of Our Experience in the Urology Department of the Idrissa Pouye General Hospital in Dakar

The principle of PCNL is the extraction through a nephrostomy channel of kidney stones which are defined as the presence of crystalline concretions in the kidneys. Objectives: The objective of this work was to study the epidemiological, clinical and paraclinical aspects of patients with renal lithiasis treated by PCNL and on the other hand the different technical aspects of PCNL. Materials and Methods: This was a retrospective descriptive study, conducted in the urology department of HOGIP, covering the period from January 2015 to January 2019. We studied the demographic and clinical aspects of patients presenting renal lithiasis;we also studied the technical aspects and treatment outcomes. Statistical significance was set for α = 0.05. Results: Our series covered 82 PCNLs performed during the study period. The average age was 45.95 years, the age group [40 - 49 years] was more affected. Our study involved 44 men and 38 women, a sex ratio of 1.15. At the clinic, atypical lumbar pain was more frequent in 45.83% of cases. On URO-CT, the calculations were located more at the pyelic level (31.2%) and lower caliceal (27.2%). The size of stones measured between 15 and 20 mm in 30.52%. The stone density was between 500 and 1000 HU in 47.54% of cases. Intraoperatively, the patients were placed in lateral decubitus in 41.46% of cases. Only one access to the kidney was necessary in 87.8% of cases. The lithoclast was used in 65.85% of cases. The overall success rate (stone free) in our series was 96%. The majority of cases, 41.44%, drainage were done by natural means (totally tubeless). The average duration was 92 minutes. The complications encountered in our experience concerned 10 cases, a rate of 12.19%. Conclusion: Urinary lithiasis is more and more frequent in our regions. The development of Endo-urology offers several therapeutic options. Thus, PCNL occupies an important place in the management of kidney stones.

Surgical Treatment of the Upper Urinary Tract Lithiasis at the Urology Department of the Hopital General Idrissa Pouye

Upper urinary tract lithiasis is a condition characterized by the presence of stones which is a stony concretion of crystallized substances in the kidney and/or ureter. Objective: The objective of this work is to study the epidemiological, clinical and therapeutic aspects of upper urinary tract stones on the one hand, then to compare the different types of surgical treatments and their results. Materials and Methods: This is a retrospective and descriptive study focusing on the surgical management of upper urinary tract stones, between January 2017 and December 2020, at the Urology department of the Hopital General Idrissa Pouye. Results: During the study period, 7.59% of surgical interventions performed were related to the treatment of upper urinary tract stones. Average age was 45.20 ± 16.4 years, the age group [41 - 60] years was more affected with 42.68%. A history of urolithiasis was present in 25.52% of cases. Lower back pain was present in 97.48% of cases. On URO-CT scan, the stone was located more at the level of the ureter (49.57%). The size between 10 to 15 mm was more frequent (30.96%) and the density greater than 1000 HU was more frequent (33.47%). The most used therapeutic procedure was ureteroscopy (52.08%), followed by PCNL (40.83%) and open surgery (7.08%). The success rate (stone free) was 93.68% for the URS, 89.36% for the PCNL. The length of hospital stay for open surgery was 6.76 ± 4.25, for PCNL 2.62 ± 2 days and for URS 1.75 ± 1.62 days. Postoperative complications were present in 23.52% for open surgery, 11.22% for PCNL and 8% for URS. Conclusion: Urolithiasis is constantly growing in our regions. Effective endourological treatment is increasingly replacing open surgery. However, global access to these new techniques in our regions is slow to be effective.

Comparative Study of Available Solar Potential for Six Stations of Sahel

Sahel is an African area with high solar potential. However, this potential is not uniform across the region. This paper examines the spatial distribution of the available solar potential by using six stations across the Sahel area. This comparative study was based on the analysis of in situ measurements in Dakar in Senegal, Niamey in Niger, Ouagadougou, Gaoua, Dori in Burkina Faso and N’Djamena in Chad. The results showed the presence of a good global solar potential with an average value of about 5.43 kWh/m2/day. The maxima of global potential are noted in the northern part in Niamey with a value of 6.24 kWh/m2/day while the minima are recorded in the south-eastern part in N’Djamena with an irradiation close to 4.71 kWh/m2/day. Then, the monthly evolution of this potential shows similar trends for all stations. Indeed, two maximums are observed during the year in Spring (March) and Autumn (October). However, for most of these stations, the minima of global potential are recorded in Winter (November, February) and during the rainy season (July, October). Moreover, the direct normal potential also shows seasonal trends for the two stations (Dakar, Niamey) where it was measured. The maxima of direct normal irradiation (DNI) are observed between February and May with a value of 5.5 kWh/m2/day in Dakar and in Niamey with a value around 5.32 kWh/m2/day between February and November.

Abrikossof’s Tumor: Report of a Case

The granular cell tumor of the breast is a rare tumor and a usually benign disease that appears clinically and radiologically like a malign tumor. We report a case of GCT in a woman of 41 years, who present a clinically and radiologically suspect later. The anatomopathological examination showed the benign tumor proliferation and a large tumorectomy was realized. The immunostaining with PS100 and NSE protein confirm the nature of the tumor.

Sex Differences in Clinical and Angiographic Characteristics in Acute Coronary Syndrome Patients in Dakar, Senegal

Background: Coronary artery disease (CAD) is in relentless progression, along with the adoption of western lifestyle in sub-Saharan Africa. In Senegal, the incidence of acute coronary syndrome (ACS) rose from 5% to 12% in Dakar during the last 20 years. That increase was observed in both women and men, and according to current data, the former paid the heaviest price in terms of mortality compared to men. In this study, we aim to retrospectively assess the clinical characteristics and angiographic profile of ACS in women compared to men in two tertiary care centers in Dakar. Methods: In this study, we retrospectively analyzed and compared data between men and women among 133 patients who underwent coronary angiogram for ACS in 2 tertiary centers from February 2019 to January 2020. Results: A total of 133 patients were included in our study of whom 97 (72.9%) were men. The mean age was 58 ± 13.6 years. Women were older than men (61.4 ± 14.3 years vs 56.6 ± 13.5 years (p = 0.07). Hypertension, sedentary lifestyle and obesity were significantly more frequent in women (69.4%, 52.7% and 19.4%) when compared to men (38.8%, 25.7% and 6.19%) respectively, (p = 0.001;0.002 and 0.03). Smoking was less frequent in women (2.8%) than in men (44.3%), (p Conclusion: In our study, women with ACS were older, had more cardiovascular risk factors but less angiographic extent of disease than men.

MINOCA Resulting from Coronary Spasm Confirmed Angiographically without Ergonovine Stimulation: A Case Report

Introduction: Myocardial infarction with nonobstructive coronary arteries disease (MINOCA) encompasses a heterogeneous group of disorders. Multimodality imaging is crucial to figure out the underlying mechanism. Coronary artery spasm is a common cause of MINOCA and might be difficult to identify. Case Presentation: A 43-year-old male with no cardiovascular risk factor presented with prolonged acute chest pain at 5 am that resolved spontaneously. On admission, he was asymptomatic and clinical findings were unremarkable. The electrocardiogram showed an anterolateral negative T wave. The troponin was positive. Coronary angiography (CA) revealed a moderate lesion of the proximal left anterior descending (LAD) coronary artery. He was treated with dual antiplatelet therapy, statin, and beta-blocker with a favourable hospital course. One week later, he presented chest pain recurrences with anterior ST-segment elevation. CA, without intra-radial nitrate, owing to a vasospasm suspicion, showed a sub-occlusion of the proximal LAD which resolved after intracoronary nitrates. Chest pain and ST-segment elevation then disappeared. He remained asymptomatic and was discharged on CCB (calcium channel blocker), long-acting nitrates dual antiplatelet and statin. Cardiac magnetic resonance (CMR) demonstrated an ejection fraction (EF) of 45% with anterior ischemia. Conclusion: Our case is about a common cause of MINOCA, namely coronary spasm, diagnosed by coronary angiogram without provocative test.

Cardiac Resynchronization Therapy in Heart Failure in Sub-Saharan Africa Environment: Experience of the Principal Hospital of Dakar (Senegal)

Background: Heart failure is a major public health challenge in sub-Saharan Africa. In patients with chronic Heart Failure and cardiac desynchrony, studies have suggested that cardiac resynchronization, can improve cardiac function and the quality of life of patients. However, in Sub-Saharan Africa, very few studies have been done on cardiac resynchronization which is in its infancy. The aim of this study is to report the local data from our hospital. Method: It was a transversal, descriptive and analytical study conducted from November 2019 to September 2022 at the Cardiology Department of the Principal Hospital of Dakar. Results: Twelve patients were implanted for Cardiac Resynchronization Therapy (CRT). The sex ratio was 8 males/4 females. The average age was 67 ± 11 years. Ten patients had non-ischemic heart disease and the two others had ischemic one. All of them had NYHA III or IV scores before CRT. The Quality of Life (QOL) was judged as poor by all of the patients. The average duration of QRS was 156 ± 9 ms. 27.9% ± 5% was the mean Left Ventricular ejection fraction (LVEF). Complications occur in 3/12 patients (25%). It was one CS vein dissection, one micro LV lead dislodgement and one phrenic nerve stimulation. Nine patients, who were considered as responders, had an improvement of QOL and NYHA, the LVEF increased and the end-diastolic dimension, and the duration of the QRS interval all decreased. Two patients do not respond and one (1) who had permanent atrial fibrillation, was a secondary responder after an atrioventricular junction ablation. Conclusion: Cardiac resynchronization is a therapy that improves the QOL of patients, the LVEF and reduces the duration of the QRS interval. However, this procedure is not without risk of complications. In sub-Saharan Africa, the major challenge is to improve the financial accessibility of this therapy for the population.

Inadvertent Lead Malposition in the Left Ventricle during Permanent Ventricular Pacing about One Case

Inadvertent Lead Malposition in Left Ventricle is a rare and underdiagnosed incident, which may occur during implantation of cardiac electronic devices and may remain asymptomatic. We reported the case of a 71-year-old man who was implanted with a ventricular single-chamber pacemaker for a slow atrial fibrillation with syncope and whose routine transthoracic echocardiography 23 months after implantation displayed a malposition of the pacemaker lead into the Left Ventricle through a patent foramen oval. The patient was asymptomatic. The electrocardiogram showed right bundle branch block QRS-paced morphology with a positive QRS pattern in V1, a median paced QRS axis on the frontal plane at -120°, a Precordial transition on V5. At the lateral Chest X-ray the lead curved backwards to the spine. Given the age of this old patient who already received oral anticoagulant for Atrial Fibrillation and the Lead malposition discovered 23 months after pacemaker’s implantation, we decided to maintain the lead in LV and continue anticoagulation.

Influence of Dust Deposition on the Electrical Parameters of Silicon-Based Solar Panels Installed in Senegal (Dakar Region)

In recent years, photovoltaic (PV) modules are widely used in many applications around the world. However, this renewable energy is plagued by dust, airborne particles, humidity, and high ambient temperatures. This paper studies the effect of dust soiling on silicon-based photovoltaic panel performance in a mini-solar power plant located in Dakar (Senegal, 14°42'N latitude, 17°28'W longitude). Results of the current-voltage (I - V) characteristics of photovoltaic panels tested under real conditions. We modeled a silicon-based PV cell in a dusty environment as a stack of thin layers of dust, glass and silicon. The silicon layer is modeled as a P-N junction. The study performed under standard laboratory conditions with input data of irradiation at 1000 W/m2, cell temperature at 25°C and solar spectrum with Air Mass (AM) at 1.5 for the monocrystalline silicon PV cell (m-Si). The analysis with an ellipsometer of dust samples collected on photovoltaic panels allowed to obtain the refraction indices (real and imaginary) of these particles which will complete the input parameters of the model.
