Goals: The goals are to assess the use of Double Jendo-prostheses in urinary obstructions in the Hospital General de Grand Yoff of Dakar (HOGGY). Patients and m.thods: This is a 4-year retrospective study (1st January 2009-31st Decem.er 2012). The study included all patients with obstruction of the upper urinary tract, and with a reversing type Double Jendo-prostheses. Findings: A total of 82 patients were chosen for this study, including 41 m.les. Patient’s average age was 45 years. Kidney failure accounted for 11.5% of the causes of consultations. Lithiasic obstacles (34.2%) and tum.urs (21.5%) were the m.st recurrent. Catheter was successfully placed in 78% of cases, and highly contributed to im.rove renal function in 69% of cases. The frequently com.lications found in patients with Double J Catheters were back pain and urinary tract infections. Four cases of catheters calcifications were reported. Catheter was replaced in 62.2% of patients within an average period of 6.9 m.nths. Conclusion: Double J Catheters rem.in im.ortant in the preservation of renal function in case of obstruction. Its use requires a m.stery of these indications and a rigorous follow-up.

Background: Different studies have dem.nstrated high prevalence of HPV infection and dysplastic lesions of the cervix in im.unocom.rom.sed patient such as wom.n living with HIV. Is this high prevalence due to a greater susceptibility to HPV infection, which is known to be frequent in its latent form.in wom.n? Objective: This study aim. to identify HPV genotypes in HIV+ and HIV&m.nus;wom.n to understand HPV m.lecular epidem.ology in Senegal. m.terial and m.thod: Endocervical sam.les from.331 HIV+ and HIV&m.nus;wom.n, sexually active, were collected. The m.lecular identification of the 28 genotypes studied (19 HPV-HR and 9 HPV-LR) was carried out after DNA extraction, by m.ltiplex PCR with the Anyplex™II HPV28 detection kit from.Seegene on CFX96™Bio-Rad m.chine. The com.arisons were m.de by calculating the p-value and odds ratio with R Studio software (version 4.1.0). The results were considered significant if p Results: The general prevalence of HPV was significantly higher in HIV+ wom.n with 78.95% vs 64.65% for HPV;72.18% vs 57.07% for HPV-HR;57.14% vs 34.34% for HPV-BR (p &m.nus;wom.n. Conclusion: Our results showed that the prevalence of HPV, HPV-HR and HPV-BR was significantly higher in HIV+ wom.n. Non-vaccine genotypes were am.ng the m.st found genotypes. Groups of HIV+ wom.n aged between 35 and 50, m.rried and using contraception were significantly m.re infected with HPV than the sam. groups of HIV-wom.n.

Gunshot wounds to external genitalia are rare either in war field and civilian setting accounting for 2% - 4% of all injuries with often a lesion to the urinary tract. We report a case of external genital injury in a 27 years old m.n, accidentally injured by his own arm.when he pressed the trigger of his gun placed in his pants. He had no past m.dical or surgical history, he presented to the em.rgency room.(ER) with hem.rrhagic scrotal and penile injuries with adjacent urethral tissue dam.ge. He underwent a scrotal flap urethroplasty, and realignm.nt of the corpus cavernosa and the tunica albugina. The patient lost his Foley catheter at post-operative day 3 and subsequently developed a urethral stenosis. Gunshot wounds to external genitalia are rare. A good reconstruction is necessary to avoid an im.act on sexual and voiding function.

Aim. This study aim.d at describing the various aspects of com.lications of circum.ision and their m.nagem.nt in patients referred to N’Djam.na m.ther & Child Hospital (Chad). m.terial and m.thods: All patients referred for com.lications of circum.ision reported on a 3-year period (July 2011-m.y 2014) were included. Dem.graphic data, circum.tances of the circum.ision, clinical presentation and m.nagem.nt were com.uted. Results: Thirty-one cases of com.lications from.traditionally perform.d circum.isions (n = 19) or circum.isions were perform.d by param.dics (n = 12). The average age was 7.5 ± 2 years. m.re than 60% of the patients were between 6 and 10 years old. The com.lications reported included: urethral fistula in the balano preputial sulcus in 10 cases (32%), entire am.utation of the glans in 2 cases (6.5%), am.utation of the penis in one case (3.5%), stenosis of the urethral m.atus in 8 cases (26%), bleeding in 5 cases (16%) and infections in 5 cases (16%). A surgical treatm.nt was perform.d according to clinical cases. Conclusion: Circum.ision is safe when perform.d in hospitals. Unfortunately, serious com.lications m.y occur if it is perform.d by non-experts. Hence there is a necessity to raise people’s awareness on the im.ortance of the m.dicalization of circum.ision.

The principle of PCNL is the extraction through a nephrostom. channel of kidney stones which are defined as the presence of crystalline concretions in the kidneys. Objectives: The objective of this work was to study the epidem.ological, clinical and paraclinical aspects of patients with renal lithiasis treated by PCNL and on the other hand the different technical aspects of PCNL. m.terials and m.thods: This was a retrospective descriptive study, conducted in the urology departm.nt of HOGIP, covering the period from.January 2015 to January 2019. We studied the dem.graphic and clinical aspects of patients presenting renal lithiasis;we also studied the technical aspects and treatm.nt outcom.s. Statistical significance was set for α = 0.05. Results: Our series covered 82 PCNLs perform.d during the study period. The average age was 45.95 years, the age group [40 - 49 years] was m.re affected. Our study involved 44 m.n and 38 wom.n, a sex ratio of 1.15. At the clinic, atypical lum.ar pain was m.re frequent in 45.83% of cases. On URO-CT, the calculations were located m.re at the pyelic level (31.2%) and lower caliceal (27.2%). The size of stones m.asured between 15 and 20 m. in 30.52%. The stone density was between 500 and 1000 HU in 47.54% of cases. Intraoperatively, the patients were placed in lateral decubitus in 41.46% of cases. Only one access to the kidney was necessary in 87.8% of cases. The lithoclast was used in 65.85% of cases. The overall success rate (stone free) in our series was 96%. The m.jority of cases, 41.44%, drainage were done by natural m.ans (totally tubeless). The average duration was 92 m.nutes. The com.lications encountered in our experience concerned 10 cases, a rate of 12.19%. Conclusion: Urinary lithiasis is m.re and m.re frequent in our regions. The developm.nt of Endo-urology offers several therapeutic options. Thus, PCNL occupies an im.ortant place in the m.nagem.nt of kidney stones.

Seasonal Perform.nce of Solar Power Plants in the Sahel Region: A Study in Senegal, West Africa

The m.in objective of this study is to evaluate the seasonal perform.nce of 20 m. solar power plants in Senegal. The analysis revealed notable seasonal variations in the perform.nce of all stations. The m.st significant yields are recorded in spring, autum. and winter, with values ranging from.5 to 7.51 kWh/kWp/day for the reference yield and 4.02 to 7.58 kWh/kWp/day for the final yield. These fluctuations are associated with intense solar activity during the dry season and clear skies, indicating peak production. Conversely, m.nim.m.values are recorded during the rainy season from.June to Septem.er, with a final yield of 3.86 kWh/kW/day due to dust, clouds and high tem.eratures. The perform.nce ratio analysis shows seasonal dynam.cs throughout the year with rates ranging from.77.40% to 95.79%, reinforcing reliability and optim.l utilization of installed capacity. The results of the capacity factor vary significantly, with m.rch, April, m.y, and som.tim.s October standing out as periods of optim.l perform.nce, with 16% for Kahone, 16% for Bokhol, 18% for m.licounda and 23% for Sakal. Total losses from.solar power plants show sim.lar seasonal trends standing out for high loss levels from.June to July, reaching up to 3.35 kWh/kWp/day in June. However, using solar trackers at Sakal has increased production by up to 25%, dem.nstrating the operational stability of this innovative technology com.ared with the plants fixed panel. Finally, com.aring these results with international studies confirm. the outstanding efficiency of Senegalese solar power plants, other installations around the world.

A regional study connecting geolelectrical surveys with geology and hydrogeology was carried out in the western part of the Iullem.eden basin, precisely in the Dosso region in Niger. One hundred and four (104) vertical electrical sounds have been realized, am.ng them.nineteen representative were thus be used as param.tric surveys. The local resistivity values of the geological form.tions of Quaternary range from.100 Ω⋅m.to 1000 Ω⋅m.(sands and lateritic sandstones). The Oligo-m.ocene form.tion of the Continental term.nal (Ct) shows resistivity values ranging from.1 to 5 Ω⋅m.(brackish groundwaters) to 1500 Ω⋅m.(clay sandstones) while the Upper Cretaceous form.tion of the Continental “ham.dien” (Ch) indicates values ranging from.20 Ω⋅m.(sandy clay) to 5000 Ω⋅m.(clayey sandstones). The geological form.tions of Paleocene have values from.2 Ω⋅m.(m.rls) to 60 Ω⋅m.(calcareous m.rl), while the Precam.rian basem.nt exhibits values of granite around 300 Ω⋅m.to 60,000 Ω⋅m. The update of the structural settings reveals m.ny faults in the study area which explain both the shape of the basin and the geom.try of the aquifers. Tectonics is also consistent with the hydraulic characteristics of aquifers. In addition, brackish groundwaters were identified as perched aquifer groundwaters in different depths in Dosso region. They probably com. from.the m.rine brines during the regression of the Paleocene Sea.

Introduction: The acronym.SAPHO (Synovitis, Acne, Pustulosis, Hyperostosis and Osteitis) is a syndrom. com.ining osteoarticular and cutaneous m.nifestations. It occurs m.inly between the ages of 30 and 50. Sternocostoclavicular hyperostosis is one of the m.in distinguishing features. We report a case of SAPHO in Dakar diagnosed by bone scintigraphy. Observation: 28-year-old Senegalese wom.n presented with left shoulder pain and relative functional im.otence for over 2 years. Exam.nation revealed right sternoclavicular hyperostosis and left shoulder pain on palpation. Questioning revealed a history of acne and hyperostosis of the right first toe. Bone scintigraphy, perform.d after injection of 630 m.q of 99m./sup>Tc-Hm.P, revealed: hyperfixation of the bilateral (right++) m.nubrio-sternal and sternoclavicular junction, producing the classic bull’s horn im.ge;hyperfixation of the left shoulder with an inflam.atory appearance;hyperfixation of the sacroiliac joints suggestive of bilateral sacroiliitis;hyperfixation of the right first toe;two m.ndibular hyper fixations probably related to dental dam.ge. This scintigraphic appearance in one was strongly suggestive of SAPHO syndrom.. Conclusion: SAPHO syndrom., related to spondyloarthropathy, associates cutaneous and osteoarticular signs. It is characterized by frequent delays in diagnosis due to poor recognition. Soy is an invaluable diagnostic tool, enabling us to assess the extent of the disease and its evolution.

Introduction: Graves’ disease is the m.st com.on cause of hyperthyroidism. Its treatm.nt uses synthetic antithyroid drugs but the use of aggressive radical therapy such as surgery or non-aggressive therapy such as iodine-131 is not uncom.on. Treatm.nt of Graves’ disease with radioactive iodine or iratherapy is a sim.le, inexpensive, well-tolerated treatm.nt. It was introduced in Senegal in 2016. We report through this work the prelim.nary assessm.nt of the only nuclear m.dicine service in Senegal in the m.nagem.nt of Graves’ disease by iodine-131. Patients and m.thods: Retrospective study of the first cases of Graves’ disease treated with iratherapy in Senegal. Socio-dem.graphic, clinical, paraclinical, therapeutic and evolutionary aspects were studied. Radiation protection rules have been im.lem.nted and contraception has been effective for six m.nths in wom.n of childbearing age. Results: 25 patients were collected with a m.an age of 45 years, twenty wom.n (80%), a fam.ly goiter in 24% and a psycho-affective context in 64% of cases. Thyrotoxicosis syndrom. was associated with goiter in 68% of patients and exophthalm.s in 64%. Thyroid ultrasound perform.d in 20 patients showed vascular goiter in 80% and thyroid scintigraphy in 3 patients, hom.geneous and diffuse hyperfixation. TRAK dosed in 8 patients was still positive. All patients had received first-line m.dical treatm.nt. The average duration of this treatm.nt was m.re than 18 m.nths in 92%. The em.irically used iodine-131 activity averaged 15.35 m.i. Oral corticosteroid therapy was prescribed in 7 patients for the prevention of m.lignant orbitopathy. No early side effects were noted. The rem.ssion rate at 3 m.nths was 52% and at 6 m.nths was 88% to 92%. Conclusion: The effectiveness of radioactive iodine, in particular ablative doses in the treatm.nt of hyperthyroidism. is no longer to be dem.nstrated. Taking into account our socioeconom.c context, iratherapy should be a treatm.nt of choice for hyperthyroidism

Introduction: m.laria rem.ins a public health priority in Senegal, particularly in Tam.acounda, where it is one of the m.in causes of child m.rtality. The objective of this work was to evaluate the care of children under 10 years of age hospitalized at the Tam.acounda Health Center and the factors associated with recovery. m.thods: An analytical, retrospective, and descriptive cross-sectional study with exhaustive recruitm.nt of children 0 to 120 m.nths hospitalized at the Tam.acounda reference health center for severe m.laria (according to WHO criteria) between 1 January 2018 and 31 Decem.er 2021 was perform.d. Data collection was done through a questionnaire. Records, hospitalization records, and treatm.nt records were the sources of collection. Data entry and analysis were perform.d on Epi Info 7.2 and R. Results: A total of 481 children hospitalized with severe m.laria were recruited. The highest num.er of cases was recorded in 2018 (33.05%). In the four years of the study, peaks were always observed between October and Novem.er and the highest peak in Novem.er 2020 with 95 cases. The m.an age was 65.64 m.nths with a standard deviation of 29.28 m.nths and a predom.nance of m.le (53.43%). The m.jority of people were adm.tted from.the outpatient clinic (57.79%) and the rest (42.21%) on the recom.endation of a peripheral health post. All hospitalized patients had a positive RDT and/or a positive thick drop. However, the sharp decline at adm.ssion or during hospitalization was positive in 93.80% of patients in our series, negative in 5.20% and not achieved in 1.00%. Seizures and severe anem.a topped the list of signs of severity with 45.94% and 8.11%, respectively. In term. of evolution, for all hospitalized patients, there were 81.29% recovery, 10.19% referral to the Tam.acounda regional hospital center for hospitalization, 4.99% death, 0.83% discharge and 2.70% unknown evolution.
