Robotic surgery for rectal cancer: Current immediate clinical and oncological outcomes

Laparoscopic rectal surgery continues to be a challenging operation associated to a steep learning curve. Robotic surgical systems have dramatically changed minimally invasive surgery. Three-dimensional, magnified and stable view, articulated instruments, and reduction of physiologic tremors leading to superior dexterity and ergonomics. Therefore, robotic platforms could potentially address limitations of laparoscopic rectal surgery. It was aimed at reviewing current literature on short-term clinical and oncological(pathological) outcomes after robotic rectal cancer surgery in comparison with laparoscopic surgery. A systematic review was performed for the period 2002 to 2014. A total of 1776 patients with rectal cancer underwent minimally invasive robotic treatment in 32 studies. After robotic and laparoscopic approach to oncologic rectal surgery, respectively, mean operating time varied from 192-385 min, and from 158-297 min; mean estimated blood loss was between 33 and 283 mL, and between 127 and 300 mL; mean length of stay varied from 4-10 d; and from 6-15 d. Conversion after robotic rectal surgery varied from 0% to 9.4%, and from 0 to 22% after laparoscopy. There was no difference between robotic(0%-41.3%) and laparoscopic(5.5%-29.3%) surgery regarding morbidity and anastomotic complications(respectively, 0%-13.5%, and 0%-11.1%). Regarding immediate oncologic outcomes, respectively among robotic and laparoscopic cases, positive circumferential margins varied from 0% to 7.5%, and from 0% to 8.8%; the mean number of retrieved lymph nodes was between 10 and 20, and between 11 and 21; and the mean distal resection margin was from 0.8 to 4.7 cm, and from 1.9 to 4.5 cm. Robotic rectal cancer surgery is being undertaken by experienced surgeons. However, the quality of the assembled evidence does not support definite conclusions about most studies variables. Introduction: At the beginning of the COVID-19 pandemic, a drop in the number of patients treated for cardiac emergencies raised concern about cardiovascular mortality in that period. An increase in care delay for patients with ST-segment elevation myocardial infarction (STEMI) may have affected clinical outcomes. Objectives: To analyze delay times and clinical outcomes of patients with STEMI undergoing primary percutaneous coronary intervention (PPCI), before and during the COVID-19 pandemic. Methods: Retrospective observational study that included patients with STEMI undergoing PPCI from December 2018 to July 2021. The COVID-19 pandemic cases were divided into two groups: pandemic I—from March to August 2020;and pandemic II—from September 2020 to July 2021. Patients were compared according to the period of hospitalization. Primary outcomes were delay times in assistance and clinical outcomes (acute kidney injury [AKI], post-procedural vascular complications and in-hospital mortality). Results: 108 patients were included, 39 (36.1%) in the pre-pandemic period, 13 (12.1%) in pandemic I and 56 (51.8%) in pandemic II. Time from onset of symptoms to arrival at the service and door-to-balloon time did not differ significantly among groups. Vascular complications were more frequent during the pandemic (I and II) than in the pre-pandemic period (2.5% pre-pandemic vs 15.4% pandemic vs 12.5% pandemic II;p = 0.03). AKI incidence was similar in all three periods. There was a non-significant increase in in-hospital mortality during the COVID-19 pandemic. Conclusion: In patients with STEMI, there was an increase in vascular complications and a trend toward increased mortality during the COVID-19 pandemic. Delay times to admission and reperfusion did not differ significantly between before and during the pandemic.

In this study the potential of cloud point extraction formed by a non-ionic surfactant was used in order to separate polyphenols from industrial residues of camu-camu. The effects of operational conditions of the cloud point extraction(CPE) on the polyphenol recovery and volumetric ratio were investigated. The results showed a maximum recovery of 95.71% that was obtained using 7.0 wt% Triton X-114, native pH(3.25), and 80 wt%polyphenol extract at 30 °C. The use of cloud point extraction was successful to recover the polyphenols from agroindustrial residue since it is a simple as well as of low-cost technique.

This study hypothesized that different land-use affect the microbial enzymatic stoichiometry and C-,N-,and P-acquisition in Brazilian semiarid ***,the enzymesβ-glucosidase(C-acquiring enzyme),urease(N-acquiring enzyme),and acid phosphatase(P-acquiring enzyme)were assessed in soil samples collected at 0−5 and 5−10 cm depth from a tropical dry forest,a protected area with Angico,a protected area with Ipê,scrub area,and an agricultural area with *** values of C-,N-,and P-acquiring enzymes were used to calculate the enzymatic C:N,C:P,and N:P *** values of C:P and N:P ratios were higher at 0−5 cm depth,while no significant variation,between soil depth,was observed for C:N *** values of C-and N-acquiring enzymes were higher at 0−5 cm in tropical dry forest areas and Angico forest,*** all land use types,the values of vectors L and A were higher than 1°and 45°,*** study showed that both land-use and soil depth influence the enzymatic stoichiometry,showing higher values of C-and N-acquiring enzymes in native and protected forests at soil surface.

BACKGROUND： Fibromyalgia （FMS） is a syndrome characterized by chronic widespread musculoskeletal pain, whose etiology is not completely understood. Different therapeutic approaches have been used with inconsistent results. This observation does not invalidate the continued search for alternative treatments aimed at improving quality of life （QoL） in FMS. OBJECTIVE： This study compared three classica acupuncture （AC）, electroacupuncture （EAC） and promotion of QoL in FMS patients. traditional Chinese medicine （TCM） therapies： moxibustion （MX） in the management of pain and DESIGN, SETTING, PARTICIPANTS AND INTERVENTIONS： A preliminary, group-assigned, comparative study enrolled 30 women, mean age （46.90_＋9.24） years （range 20-60 years）, who met the 1990 American College of Rheumatology criteria for FMS diagnosis and a pain-pressure threshold （PPT）〈 4 kg/cm2. The study was conducted in a teaching tertiary-care medical institution from May 2010 through April 2012. AC, EAC and MX were delivered for 30 min, once a week, for 8 weeks, bilaterally at Neiguan （PC6）, Hegu （LI4）, Yanglingquan （GB34）, Sanyinjiao （SP6） and Taichong （LR3） acupoints. MAIN OUTCOME MEASURES： Each week, immediately before treatment and after treatment, subjects were tested for PPTs, Wong-Baker Faces Pain Scale （WBFPS; for pain intensity） and Medical Outcomes Study 36-item Short Form Health Survey （SF-36： for QoL）. RESULTS： There was no significant improvement in pain or reduction of tender points in any of the groups studied, at the end of the 8th session. Significant improvement of QoL was perceived in vitality （after AC treatment） and in mental health （after EAC and MX treatments）. CONCLUSION： TCM therapies （AC, EAC and MX） promoted an improvement in the QoL in two areas （vitality and mental health） in FMS women. Further large-scale clinical trials are required to confirm this effect.

Mexico and currently in Veracruz state, there are metropolitan zones (MZ) growing. Therefore, main objective in this paper is to analyze new data and AQ trends during 01.09.2013 to 30.06.2015 of two new AQ monitoring stations installed in Xalapa and Minatitlan MZ in 2013-year. The methodology applied used quality criteria to the datasets, followed by data validation and statistics for further analysis to determine the hourly, weekly and yearly trends of NO2, O3, SO2, PM10 and PM2.5. Indicators were compared with Mexican standards, CAI-LAC report, WHO guidelines, EU and USA standards to evaluate the AQ in both sites. We observed AQ trends from moderate to bad in Xalapa and Minatitlan MZ where the PM10 and PM2.5 surpassed the WHO guidelines and Mexican standards. O3 and SO2 in Xalapa presented a quality from good to moderate and in Minatitlan sometimes were from moderate to bad. NO2 did not exceed the value limits of Mexican standards, only Xalapa has exceeded the WHO guidelines. In Minatitlan, the Mexican limits were not exceeded. Concluding, PM10 and PM2.5 concentrations were the main problem. Others pollutants that influenced the AQ were O3, NO2 and SO2 in Minatitlan MZ due probably to meteorology, site conditions, location and oil and petrochemical industries. In Xalapa, MZ NO2 and SO2 are attributed mainly to road transport.

Removal of vegetal oil from wastewater using comercial resins in batch reactors processes is studied in present study. A traditional and patented type of three different surfactants was used for stabilizing of the emulsion phase. The results for the emulsion made by the synthesized surfactant showed much better stability and performance in the separation process compared to that by conventionally used Span 80. To determine the optimum operation conditions, the effect of several parameters such as initial oil concentration, amount of resin in the solid phase, adding or not a surfactant, resin type, and stabilizer concentration have been investigated. It was found that under the optimum conditions, more than 98% ofoil can be removed in a single batch process.

在汽车的性能中 ,发动机阀座的表面质量是至关重要的 ,表面跳动和圆度等几何形状的误差也同样重要。这些性能影响阀与座之间的密封性 ,阀座是由钢材 (粉末冶金生产 ,维氏硬度为 3GPa)制作成的。在这项研究中 ,作者使用了 PCBN工具加工这些阀座 ,平均切削速度为 16 1m/ min。根据工具侧面的最大磨损和表面形貌参数 ,观察了速度、切深和进给速率等加工参数的影响 ,在这些加工参数中 ,进给速率对阀座的表面粗造度影响最大 ,同时 ,切削速度越高 ,加工的表面质量越好。最高的进给速率和最低的切削速度的结合给磨损提供了一个最大临界值 ,在这种情况下 ,发现了侧面和凹坑磨损的出现。
