Three-dimensional MR and axial CT colonography versus conventional colonoscopy for detection of colon pathologies

AIM： To evaluate the sensitivity and specificity of MR colonography （MRC） and CT performance in detecting colon lesions, and to compare their sensitivity and specificity with that of conventional colonoscopy. METHODS： Forty-two patients suspected of having colonic lesions, because of rectal bleeding, positive fecal occult blood test results or altered bowel habits, underwent the examinations. After insertion of a rectal tube, the colon was filled with 1000-1500 mL of a mixture of 9 g/L NaCI solution, 15-20 mL of 0.5 mmol/L gadopentetate dimeglumine and 100 mL of iodinized contrast material. Once colonic distension was achieved, three-dimensional gradient-echo （3D-GRE） sequences for MR colonography and complementary MR images were taken in all cases. Immediately after MR colonography, abdominal CT images were taken by spiral CT in the axial and supine position. Then all patients were examined by conventional colonoscopy （CC）. RESULTS： The sensitivity and specificity of MRC for colon pathologies were 96.4% and 100%, respectively. The percentage of correct diagnosis by MRC was 97.6%. The sensitivity and specificity of CT for colon pathologies were 92.8%, 100%, respectively. The percentage of correct diagnosis by CT was 95.2%. CONCLUSION： In detecting colon lesions, MRC achieved a diagnostic accuracy similar to CC. However, MRC is minimally invasive, with no need for sedation or analgesics during investigation. There is a lower percentage of perforation risk, and all colon segments can be evaluated due to multi-sectional imaging availability; intramural, extra-intestinal components of colonic lesions, metastasis and any additional lesions can be evaluated easily. MRC and CT colonography are new radiological techniques that promise to be highly sensitive in the detection of colorectal mass and inflammatory bowel lesions.

A standard hepaticojejunostomy technique might be difficult to perform, especially when the bile duct is small and located deep in the liver hilum. Herein we present a new procedure, the Hand-Fan technique, that was used to enhance the exposure and ease the performance of these challenging anastomoses. Thirty-one patients who had had hepaticojejunostomy with this technique for bile duct injury and other benign biliary pathologies from July 2004 to June 2011 were included into the study. Median postoperative hospital stay was 7 days (6-25 days) and median follow-up time was 33 months (2-84 months). Liver function tests revealed that the blood bilirubin levels of the patients were normalized after hepaticojejunostomy. Follow-up showed that there were no signs of clinical recurrence or impaired bile flow. The Hand-Fan technique considerably facilitates challenging hepaticojejunostomies. Surgeon’s comfort is exceptional and the clinical results are satisfactory.

The aim of this study was to investigate the time window, duration and intensity of optimal speech and language therapy applied to aphasic patients with subacute stroke in our hospital. The study consisted of 33 patients being hospitalized for stroke rehabilitation in our hospital with first stroke but without previous history of speech and language therapy. Sixteen sessions of impairment-based speech and language therapy were applied to the patients, 30-60 minutes per day, 2 days a week, for 8 successive weeks. Aphasia assess- ment in stroke patients was performed with Giilhane Aphasia Test-2 before and after treatment. Compared with before treatment, fluency of speech, listening comprehension, reading comprehension, oral motor evaluation, automatic speech, repetition and naming were improved after treatment. This suggests that 16 seesions of speech and language therapy, 30-60 minutes per day, 2 days a week, for 8 successive weeks, are effective in the treatment of aphasic patients with subacute stroke.

Background This study aimed to compare the efficacy of intravenous,intranasal fentanyl and oral sucrose in reducing the pain response during retinopathy of prematurity examinations using premature infant pain profile(PIPP)*** The study included 42 infants who underwent retinopathy screening *** infants were divided into three groups:oral sucrose,intranasal fentanyl,and intravenous *** signs(heart rate,arterial oxygen saturation,and mean arterial pressure)were *** PIPP was used to determine pain *** oxygenation and middle cerebral artery blood flow were evaluated using near-infrared spectroscopy and Doppler ultrasonography,*** data obtained were compared between *** There was no significant difference between the three groups regarding postconceptional and postnatal ages or birth weights and weight at the time of *** babies had moderate pain during the *** correlation was observed between analgesia method and pain scores(P=0.159).In all three groups,heart rate and mean arterial pressure increased,whereas oxygen saturation decreased during the exam compared with pre-examination ***,heart rate(HR),mean arterial pressure(MAP)and arterial oxygen saturation(sPO2)values did not differ between groups(HR,P=0.150;MAP,P=0.245;sPO2,P=0.140).The cerebral oxygenation(rSO2)values between the three groups were found to be similar[rSO2:P=0.545,P=0.247,P=0.803;fractional tissue oxygen extraction(FTOE):P=0.553,P=0.278].Regarding cerebral blood flow values,we also did not find any difference between the three groups[mean blood flow velocity(Vmean):P=0.569,P=0.975;maximum flow velocity(Vmax):P=0.820,P=0.997].Conclusions Intravenous and intranasal fentanyl and oral sucrose were not superior to each other in preventing pain during the examination for retinopathy of prematurity(ROP).Sucrose may be a good alternative for pain control during ROP *** findings suggest

Monoclinic yttrium tantalate (M'-YTaO4,M'-YTO),and two different kinds of yttrium niobium-tantalate (M'-YTao.85Nbo.15O4 (M'-YTNO) and Eu3+ doped M'-YTao.85Nbo.15O4 (M'-YTNO∶Eu3+)) were produced by sol-gel method and grown on single crystalline Si (100) substrate by spin coating *** properties and thermal behaviours of the films were characterized by means of X-ray diffraction (XRD),atomic force microscopy (AFM),scanning electron microscopy (SEM),and thermogravimetry and differential thermal analysis (TG-DTA).Systematic Steady-state photoluminescence and lifetime measurements in a series of yttrium niobium-tantalate with varying amounts of Eu3+ were *** photoluminescence spectra of the films exhibited strong blue (380-400 nm) and red (614 nm) emissions upon ultraviolet *** intensities were strongly dependent on the host lattice composition and film morphology.1.5％ Eu3+ doped films exhibited the brightest luminescence and long lifetime extending to 1.22 ms when excited at 254 *** the best of our knowledge,this is the first attempt in the production of M'-YTO,M'-YTNO,and M'-YTNO∶Eu3+ films on single crystalline Si (100) substrate via sol-gel spin coating.

AIM： To investigate the role of mangafodipir trisodium （MnDPDP） in focal pancreatic masses and mass-like lesions by evaluating contrast uptake features of the lesions and pancreatic parenchyma after contrast medium injection. METHODS： A total of 37 patients with pancreatic mass or mass-like lesions were examined by unenhanced and MnDPDP-enhanced magnetic resonance imaging （MRI）. RESULTS： MRI was obtained 20-40 min after infusion of MnDPDP and homogeneous contrast enhancement was observed in normal pancreas parenchyma. In patients with atrophic pancreas there was no enhancement in pancreatic parenchyma on MnDPDP-enhanced MRI. In 37 patients with 41 pancreatic masses and mass-like lesions, contrast enhancement was observed at 5 lesions on MnDPDP enhanced MRI. Three of these 5 lesions were focal pancreatitis and the other 2 were adenocarcinoma. No contrast enhancement was determined in 36 pancreatic masses and mass-like lesions in 32 patients. CONCLUSION： MnDPDP contrast-enhanced MRI, especially in cases with no parenchyma atrophy, can distinguish focal pancreatic lesion margins. Information about the function of pancreatic parenchyma can be obtained out of tumor. MnDPDP facilitates staging of pancreatic tumors by detection of metastatic lesions in the liver. In addition, diminished heteregenous uptake of MnDPDP in patients with pancreatitis may be helpful in differential diagnosis.

Objective:To compare laparoscopic Anderson-Hynes pyeloplasty(LAHP)and retroperitoneal laparoscopic YV-pyeloplasty(LRYVP)in ureteropelvic junction obstruction(UPJ)in presence of a crossing vessels(CV).Methods:Our database showed 380 UPJO-cases,who underwent laparoscopic retroperitoneal surgery during the last 2 decades including 206 non-dismembered LRYVP,157 dismembered pyeloplasties LAHP,and 17 cases of laparoscopic *** them 198 cases were suitable for a matched-pair(2:1)analysis comparing laparoscopic retroperitoneal non-dismembered LRYVP(Group 1,n Z 131)and dismembered LAHP(Group 2,n Z 67)in presence of a crossing *** were matched according to age,gender,kidney functions,and obstruction *** were graded according to modified ***:Comparative data were similar between both groups(LRYVP ***)including mean operating time(112 min vs.114 min),complication rates(4.2%vs.7.3%)mainly Grade 1e2 according to Clavien classification,and success rates(90%vs.89%).These results reflected in the reviewed literature indicate that LRYVP provides the advantage of minimal dissection in case of CV with similar ***,redundant pelvis and anteriorly crossing vessels still require a dismembered pyeloplasty ***:LRYVP has achieved similar results compared with the previous golden standard of open surgery,especially in case of crossing vessels apart from presence of a redundant pelvis or anteriorly crossing *** can be further improved when using the small access retroperitoneoscopic technique respectively mini-laparoscopy.

以安纳托利亚东南部的天然沥青岩为原料制备泡沫炭。所制泡沫炭的平均孔径和密度分别为150μm和800 kg/m3。主要研究了压力、温度、泄压时间及在最高温度时的停留时间对泡沫形成的影响,同时还评价了炭化过程对泡沫结构的影响。主要研究了压力、温度、泄压时间及在最高温度时的停留时间对泡沫炭进行了表征,并进行了密度及抗压强度分析。结果表明,1 323 K炭化后,泡沫炭压缩强度由10 MPa增加到18.7 MPa。沥青的灰分含量(41.76%)对泡沫炭的密度和压缩强度起重要作用。
