Steel dome structures,with their striking structural forms,take a place among the impressive and aesthetic load bearing systems featuring large internal spaces without internal columns.In this paper,the seismic design optimization of spatial steel dome structures is achieved through three recent metaheuristic algorithms that are water strider(WS),grey wolf(GW),and brain storm optimization(BSO).The structural elements of the domes are treated as design variables collected in member groups.The structural stress and stability limitations are enforced by ASD-AISC provisions.Also,the displacement restrictions are considered in design procedure.The metaheuristic algorithms are encoded in MATLAB interacting with SAP2000 for gathering structural reactions through open application programming interface(OAPI).The optimum spatial steel dome designs achieved by proposed WS,GW,and BSO algorithms are compared with respect to solution accuracy,convergence rates,and reliability,utilizing three real-size design examples for considering both the previously reported optimum design results obtained by classical metaheuristic algorithms and a gradient descent-based hyperband optimization(HBO)algorithm.

TheÇorakyerler fossil locality in Anatolia reveals unique faunal elements.This study introduces Hystrix kayae sp.nov.,a new Late Miocene porcupine initially classified as Hystrix sp.Yavuz et al.,2018.This finding expands our knowledge of Late Miocene Anatolian porcupines,bringing the total number of known species to three.H.kayae sp.***.is larger than Hystrix aryanensis and exhibits greater upper cheek tooth crown height.Its cheek tooth morphology differs from Hystrix depereti,and it possesses distinctive U-shaped choanae unlike Hystrix primigenia.TheÇorakyerler locality may predate sites with H.aryanensis and H.primigenia but aligns temporally with Hystrix parvae localities.This study enhances our understanding of Late Miocene porcupine diversity in Anatolia,emphasizing the importance ofÇorakyerler in unraveling the evolutionary history of these fascinating mammals.

A generalized form of the error function, Gp(x)=pΓ(1/p)∫0xe−tpdt, which is directly associated with the gamma function, is evaluated for arbitrary real values of p>1and 0x≤+∞by employing a fast-converging power series expansion developed in resolving the so-called Grandi’s paradox. Comparisons with accurate tabulated values for well-known cases such as the error function are presented using the expansions truncated at various orders.

BACKGROUND Peripheral nerve injury can result in significant clinical complications that have uncertain prognoses.Currently,there is a lack of effective pharmacological interventions for nerve damage,despite the existence of several small compounds,Despite the objective of achieving full functional restoration by surgical intervention,the persistent challenge of inadequate functional recovery remains a significant concern in the context of peripheral nerve injuries.AIM To examine the impact of exosomes on the process of functional recovery following a complete radial nerve damage.METHODS A male individual,aged 24,who is right-hand dominant and an immigrant,arrived with an injury caused by a knife assault.The cut is located on the left arm,specifically below the elbow.The neurological examination and electrodiagnostic testing reveal evidence of left radial nerve damage.The sural autograft was utilized for repair,followed by the application of 1 mL of mesenchymal stem cell-derived exosome,comprising 5 billion microvesicles.This exosome was split into four equal volumes of 0.25 mL each and delivered microsurgically to both the proximal and distal stumps using the subepineural pathway.The patient was subjected to a period of 180 d during which they had neurological examination and electrodiagnostic testing.RESULTS The duration of the patient’s follow-up period was 180 d.An increasing Tinel’s sign and sensory-motor recovery were detected even at the 10th wk following nerve grafting.Upon the conclusion of the 6-mo post-treatment period,an evaluation was conducted to measure the extent of improvement in motor and sensory functions of the nerve.This assessment was based on the British Medical Research Council scale and the Mackinnon-Dellon scale.The results indicated that the level of improvement in motor function was classified as M5,denoting an excellent outcome.Additionally,the level of improvement in sensory function was classified as S3+,indicating a good outcome.It is noteworthy that these assessments were conducte

Inflammation is a multifaceted cellular and molecular response triggered by injury,infection,or various pathological conditions.Serving as a protective defense mechanism,the inflammatory response involves clinical signs like redness,swelling,pain,and increased body temperature.Immune cells,notably neutrophils and macrophages,play key roles in orchestrating this response.The delicate balance between proinflammatory and anti-inflammatory mediators,including cytokines and chemokines,regulates the inflammatory cascade.While acute inflammation is crucial for tissue repair,chronic inflammation may indicate an imbalance,contributing to conditions like autoimmune diseases.Understanding these mechanisms is vital for developing therapeutic strategies and managing chronic diseases.

BACKGROUND Cerebral palsy(CP)describes a group of disorders affecting movement,balance,and posture.Disturbances in motor functions constitute the main body of CP symptoms.These symptoms surface in early childhood and patients are affected for the rest of their lives.Currently,treatment involves various pharmacotherapies for different types of CP,including antiepileptics for epilepsy and Botox A for focal spasticity.However,none of these methods can provide full symptom relief.This has prompted researchers to look for new treatment modalities,one of which is mesenchymal stem cell therapy(MSCT).Despite being a promising tool and offering a wide array of possibilities,mesenchymal stem cells(MSCs)still need to be investigated for their efficacy and safety.AIM To analyze the efficacy and safety of MSCT in CP patients.METHODS Our sample consists of four CP patients who cannot stand or walk without external support.All of these cases received allogeneic MSCT six times as 1×106/kg intrathecally,intravenously,and intramuscularly using umbilical cord-derived MSCs(UC-MSC).We monitored and assessed the patients pre-and post-treatment using the Wee Functional Independence Measure(WeeFIM),Gross Motor Function Classification System(GMFCS),and Manual Ability Classification Scale(MACS)instruments.We utilized the Modified Ashworth Scale(MAS)to measure spasticity.RESULTS We found significant improvements in MAS scores after the intervention on both sides.Two months:Rightχ^(2)=4000,P=0.046,leftχ^(2)=4000,P=0.046;four months:Rightχ^(2)=4000,P=0.046,leftχ^(2)=4000,P=0.046;12 months:Rightχ^(2)=4000,P=0.046,leftχ^(2)=4000,P=0.046.However,there was no significant difference in motor functions based on WeeFIM results(P>0.05).GMFCS and MACS scores differed significantly at 12 months after the intervention(P=0.046,P=0.046).Finally,there was no significant change in cognitive functions(P>0.05).CONCLUSION In light of our findings,we believe that UC-MSC therapy has a positive effect on spasticity,and it partially improves motor functions.

Median Nerve Somatosensory Evoked Potentials in Patients with Chiari Malformation

Abnormal SEP reflects dysfunction of the medial lemniscus and posterior cervical cord. These structures are likely to be affected in Chiari malformation. Therefore, SEP abnormalities may provide valuable information in patients with CM. However, the consistency of SEP abnormality or normality with the damage is a matter of research. Knowing whether median nerve somatosensory evoked potential (SEP) is useful in revealing subclinical damage in patients with Chiari malformation is important in the treatment and follow-up plan of the disease. The aim of this study was to investigate the relationship between median nerve SEP values and the severity of cerebellar ectopia in patients with Chiari type 1 malformation. Median nerve SEP values were obtained from 30 healthy individuals and 146 individuals with Chiari malformation. The cerebellar ectopia degree and McRae line length were measured. SEP values were not significantly different between groups. The McRae line was found to be significantly shorter in the control group than in the Chiari malformation group (p = 0.031). There was no correlation between the degree of cerebellar ectopia and the length of the McRae line (r = 0.002, p = 0.979). Neither cerebellar ectopy degree nor McRae line length had a relationship with SEP values (r = -0.153, p = 0.066;r = -0.056, p = 0.500, respectively). There was no difference in cerebellar ectopy degree or SEP values between the groups with cerebellar ectopy with and without a syrinx (p = 0.899;p = 0.080, respectively). Likewise, McRae line length was not found to be related to the presence of a syrinx (p = 0.139). Median nerve SEP examination was not beneficial for diagnosing asymptomatic-oligosymptomatic Chiari malformation as a subclinical injury, whether accompanied by syringomyelia or not.

A probabilistic seismic loss assessment of RC high-rise(RCHR)buildings designed according to Eurocode 8 and located in the Southern Euro-Mediterranean zone is presented herein.The loss assessment methodology is based on a comprehensive simulation approach which takes into account ground motion(GM)uncertainty,and the random effects in seismic demand,as well as in predicting the damage states(DSs).The methodology is implemented on three RCHR buildings of 20-story,30-story and 40-story with a core wall structural system.The loss functions described by a cumulative lognormal probability distribution are obtained for two intensity levels for a large set of simulations(NLTHAs)based on 60 GM records with a wide range of magnitude(M),distance to source(R)and different site soil conditions(SS).The losses expressed in percent of building replacement cost for RCHR buildings are obtained.In the estimation of losses,both structural(S)and nonstructural(NS)damage for four DSs are considered.The effect of different GM characteristics(M,R and SS)on the obtained losses are investigated.Finally,the estimated performance of the RCHR buildings are checked to ensure that they fulfill limit state requirements according to Eurocode 8.

BACKGROUND Celiac disease(CD)is a multifactorial disease,but genetic factors play a major role in its etiology.It has been known that human leucocyte antigen(HLA)-DQ2/DQ8 haplotypes are one of the most important predisposing genetic factors.The risk of developing CD in first-degree relatives and especially siblings of celiac patients is quite high because of having the same HLA haplotypes.AIM To evaluate the frequency of CD and the distribution of the HLA-DQ2/DQ8 haplotypes in siblings of celiac patients.METHODS Patients with biopsy-proven CD and their siblings were included in the study;those who did not have HLA genotyping were excluded from the study.All siblings were on a gluten-containing diet.The HLA genotyping,tissue transglutaminase antibody IgA antibody test,and total IgA test were performed in all participants.RESULTS A total of 57 celiac patients and their 112 siblings were included in the study.The mean age of celiac patients and siblings were 10.30±3.87 years and 9.90±6.11 years,respectively.HLA-DQ2/DQ8 alleles were detected in 98.2%of patients with CD and 90.2%of siblings of celiac patients.HLA-DQ genotypes were present in all siblings diagnosed with CD.Tissue transglutaminase antibody IgA test was found to be positive in 16 siblings.CD was diagnosed in 12 siblings(10.7%)by intestinal biopsy.CONCLUSION The prevalence of CD was found to be 10.7%in siblings of celiac patients in our study.One-third of the siblings diagnosed with CD were asymptomatic.We detected HLA-DQ alleles in 98.2%of celiac patients and 100%in siblings diagnosed with CD.In addition,1 of the 2 siblings was diagnosed with CD 1 year later and the other 4 years later.Therefore,we suggest that siblings of celiac patients should be followed up with clinical findings as well as HLA analysis and serological examination.Since the risk of developing CD is much higher in asymptomatic siblings,we recommend that siblings should be screened for CD even if they are asymptomatic.

In this work,we propose a real proportional-integral-derivative plus second-order derivative(PIDD2)controller as an efficient controller for vehicle cruise control systems to address the challenging issues related to efficient operation.In this regard,this paper is the first report in the literature demonstrating the implementation of a real PIDD2 controller for controlling the respective system.We construct a novel and efficient metaheuristic algorithm by improving the performance of the Aquila Optimizer via chaotic local search and modified opposition-based learning strategies and use it as an excellently performing tuning mechanism.We also propose a simple yet effective objective function to increase the performance of the proposed algorithm(CmOBL-AO)to adjust the real PIDD2 controller's parameters effectively.We show the CmOBL-AO algorithm to perform better than the differential evolution algorithm,gravitational search algorithm,African vultures optimization,and the Aquila Optimizer using well-known unimodal,multimodal benchmark functions.CEC2019 test suite is also used to perform ablation experiments to reveal the separate contributions of chaotic local search and modified opposition-based learning strategies to the CmOBL-AO algorithm.For the vehicle cruise control system,we confirm the more excellent performance of the proposed method against particle swarm,gray wolf,salp swarm,and original Aquila optimizers using statistical,Wilcoxon signed-rank,time response,robustness,and disturbance rejection analyses.We also use fourteen reported methods in the literature for the vehicle cruise control system to further verify the more promising performance of the CmOBL-AO-based real PIDD2 controller from a wider perspective.The excellent performance of the proposed method is also illustrated through different quality indicators and different operating speeds.Lastly,we also demonstrate the good performing capability of the CmOBL-AO algorithm for real traffic cases.We show the CmOBL-AO-based real PIDD2 controller as the
