Apricot Probiotic Drinking Yoghurt Supplied with Inulin and Oat Fiber

In this study, the effects of different amounts of inulin and oat fiber addition on the properties of apricot probiotic drinking yoghurt (APDY) were investigated. Seven different APDY was produced. Six of them were produced by the addition of 0.5%, 1% and 2% inulin (B, C, D) and oat fiber (E, F, G) and one of them was produced as control sample. Pasteurized apricot pureed and sugar (10%) was added to fermented milk beverage. APDYs were analysed 1, 7, 14 and 21 days after production. Addition of fiber to APDYs had significantly affected on the pH, titratable acidity, water holding capacity, S. thermophilus, L. acidophilus Bifidobacterium BB-12 counts, and sensorial properties of the samples (p L. acidophilus and Bifidobacterium BB-12 counts increase by the addition of fiber into samples.

Prognostic Values of PET/CT Findings and Tumor/Patient Characteristics with Non-Small Cell Lung Cancer

Background/Aim: Although numerous prognostic factors have been described for non-small cell lung cancer (NSCLC), there is still a requirement for better and non-invasive markers. FDG-PET is a non invasive diagnostic tool that is being used increasingly in the diagnosis of lung cancer. This study evaluates the prognostic values of PET/CT defined SUV measurements and other patient/tumor characteristics in newly diagnosed stage IIIB and IV NSCLC. Method: This ret- rospective study included 111 patients admitted between 2005 and 2006 with stage IIIB and IV NSCLC, whose diag- noses were verified with biopsy and staging performed with PET/CT. The prognostic values of standart uptake values (SUV) of the primary lesion on PET/CT, and other patient/tumor characteristics were analyzed using survival analysis. Results: SUV was found to be unrelated with survival. Only the presence of distant metastasis, type of metastasis (bone, brain, or the contralateral lung) and the type of radiotherapy used (curative or palliative) were found to be related to survival. SUV values in epidermoid carcinoma were found to be significantly higher compared to adenocarcinoma (16.15 ± 7.18 and 12.32 ± 5.52, respectively, p = 0.021).Conclusion: Our findings do not support that SUV of the pri- mary lesion in inoperable NSCLC has a prognostic value with respect to survival. This condition may be explained by the inclusion of significantly advanced NSCLC patients who are known to have a low survival and a high mortality, and also the relatively small sampling size.

Some Properties of Bio-Yogurt Enriched with Cellulose Fiber

In this study, bio-yogurts, which contain L. acidophilus and Bifidobacterium animalis subsp. lactis, were produced with cellulose fiber (CF) addition at a various level (0%, 0.5%, 1% and 2%) and stored at 4°C ± 1°C for 20 days. Bio-yogurts were analysed 1, 10 and 20 days after production. The storage period significantly influenced overall properties of the samples. Addition of CF has positively affected the physical and textural properties of yoghurt, such as serum separation, water holding capacity, viscosity, firmness, adhesiveness, cohesiveness, springiness, gumminess and chewiness (p < 0.05). It also slightly stimulated the development of probiotic bacteria (p < 0.05). Bio-yogurt, which contains 0.5% CF in sensory aspect, is the most appreciated.

目的：探讨经腹腔镜肾部分切除术治疗局限性肾细胞癌的手术技巧和近期疗效。方法：选择T1期、单发肿瘤,平均肿瘤直径4.0 cm（3.0~7.0 cm）的局限性肾细胞癌27例,术中用内镜bulldog钳阻断肾动脉;用剪刀或超声刀在距离肿瘤边缘0.5~1.0 cm处进行局部切除;肾切除床内置Surgicel薄片卷垫后,间断缝合肾实质,创面边缘喷涂FloSeal明胶基质以防延迟出血。术中冰冻确定切缘阴性。结果：27例手术均获成功,无术中、术后并发症。手术时间平均110 min;热缺血时间20~55 min,平均32 min;术中出血量平均150 mL。术后随访平均6个月,未见肿瘤局部复发,未见转移。结论：腹腔镜肾部分切除术治疗局限性肾细胞癌安全可靠,短期随访效果肯定。

External Ventricular Drainage Infections Rates: Clinic Experiences

In this study, we aimed to evaluate patients who develop ventriculostomy requirements and to evaluate the external ventricular drainage infection rates retrospectively at our clinic. In the study, dates between January 2012 and 2014, patients who were inserted external ventricular drainage with different indications were examined retrospectively. By using the medical record system, patients’ demographics (age, sex), diagnostic, ventriculostomy indications and ventriculostomy time had been reached. By accessing laboratory data, the patients’ cultures of cerebrospinal fluids biochemical tests were analyzed retrospectively. Within the period of the study, 20 in 117 patients with external ventricular drainage were not included the study because of shunt infection and shunt occlusion of extracted patients. During the treatment period 148 EVD were inserted to 97 patients. The number of male patients was 53;the number of female patients was 44. When the reason of the examined patients’ using external ventricular drainage was analyzed, 55% hemorrhagic cerebrovascular diseases, (subarachnoid hemorrhage, intraventricular hemorrhage, intracerebral hemorrhage, cerebellar hemorrhage, traumatic intracranial injury), 24.7% tumor induced use, 7.4% central nervous system infections (menengitis, apse), and 12.4% occlusive cerebrovascular diseases (hydrocephalus or brain edema that were developed after the infarct) were seen. During the course of our study, 23% of the surveyed 97 patients had leukocytosis. CFT culture of 12 patients found positive. In CFT cultures mostly coagulase-negative staphylococcus growth took place. Eventually, when we compare our infection proportions to the international literature, very large differences were not observed. Except for revisioned patients, no other criteria were found that increased the rate of infection. Effect of double cleavage stage versus sequential cleavage and blastocyst stage embryo transfer on clinical pregnancy rates

Objective:To compare clinical pregnancy rates following sequential day-3 and day-5 embryo transfer with double or sequential cleavage-stage ***:This study enrolled 242 patients undergoing gonadotropin-releasing hormone antagonist protocol and fresh embryo *** follicle stimulating hormone,luteinizing hormone,serum estradiol and anti-Müllerian hormone levels and controlled ovarian stimulation outcomes were *** 242 women,135 underwent double embryo transfer on day 2 or day 3(the double group),54 women underwent sequential embryo transfer on day 2 and day 3(the D2/D3 group),and 53 underwent sequential embryo transfer on day 3 and day 5(the D3/D5 group).Clinical pregnancy rates were compared among the ***:Female age,body mass index,basal follicle stimulating hormone,luteinizing hormone and estradiol levels were similar among the groups(P>0.05).The D3/D5 group had a significantly higher number of metaphaseⅡoocytes,fertilized oocytes and good quality embryos on day 3 compared with the double group and the D2/D3 group(P<0.001).Clinical pregnancy rates in the double,D2/D3 and D3/D5 groups were 26.6%(36/135),16.6%(9/54)and 37.7%(20/53),*** was no significant difference in clinical pregnancy rates between the double group and the D2/D3 group(P=0.204)or the D3/D5 group(P=0.188).The D3/D5 group had significantly higher clinical pregnancy rates compared with the D2/D3 group(P=0.025).Conclusions:Sequential cleavage-stage transfer(D2/D3)or cleavage stage and blastocyst transfer(D3/D5)does not improve clinical pregnancy rates compared with double cleavage-stage embryo *** sequential transfer seems to be an effective option in certain patient populations,routine application of this technique might not be a suitable approach in an unselected population to improve assisted reproductive technology outcomes.

Nephrotoxicity Evaluation in Outpatients Treated with Cisplatin-Based Chemotherapy Using a Short Hydration Method

Background: To evaluate cisplatin-induced nephrotoxicity in outpatients receiving chemotherapy with cisplatin alone or in combination with other agents using a short hydration method. Methods: Forty-nine patients enrolled in the study were monitored during 3 cycles of chemotherapy. Cisplatin was given in 1000 mL of 0.9% NaCl solution for 90 min as an intravenous infusion. Renal parameters were evaluated before and after each chemotherapy cycle, and 6 weeks after the completion of treatment. Results: Blood urea nitrogen, creatinine, and cystatin C levels increased significantly during the 3 cycles of chemotherapy, whereas sodium and potassium levels decreased significantly. Magnesium and calcium levels decreased only during the second cycle of chemotherapy. Significant increases in uric acid level were observed during the 1st and 3rd cycles, and 6 weeks after the completion of treatment. Conclusions: The method used in our study shows minimal changes in renal functions. To effectively monitor nephrotoxicity, renal parameters and electrolyte levels should be measured before and after each cisplatin based chemotherapy cycle. More investigations are required to evaluate this method with higher doses of cisplatin.

Hypersexuality after Modafinil Treatment： A Case Report

Modafinil is employed for the treatment of narcolepsy and other sleep disorders like idiopathic hypersornnia. Modafinil is involved in the modulation of orexin, a hypothalamic neuropeptide that regulates wakefulness. It also indicated that the drug interferes with the activity of additional neurotransmitters like hypocretin, histamine, gamma-aminobutyricacid, glutamate and norepinephrine. Modafinil can also block the dopamine transporter by increasing brain dopamine levels. Common side effects of modafinil include headache, nausea, nervousness, anxiety and trouble sleeping. Other side effects are affective lability, hyposexuality, personality change, aggression, abnormal dreams, irritability, suicidal ideas, mania, hypomania and psychotic exacerbation. In this case report, we present a 45 year old woman with idiopathic hypersomnia which is characterized primarily excessive day time sleepiness. Before modafinil treatment ESS （Epworth Sleepiness Scale） score of patient was 13. Treatment with Modafmil 200 mg daily within two weeks produced a subjective improvement in her daytime sleepiness （ESS score after treatment is 4）. But one week after the start of modafmil treatment, her sexual desire increased. Before this treatment, her sexual intercourse frequency was 1-2 times per week and after this treatment she wanted coitus every day. She has continual thoughts about sexuality. She didn＇t have any additional medication use or medical condition that increase sexual desire. Modafinil dose was decreased 100 mg/daythen to 50 mg/day. Her ESS score was 7 and daytime sleepiness had no effect on her life. After decreasing modafinil treatment her sexual desire returned to normal. Hypersexuality has not been reported as a side effect of modafinil yet. The pathophysiological mechanism of modafinil induced hypersexuality is not clear. In conclusion, clinicians should be aware of this rare significant side effect of modafinil and should maintain treatment with the lowest possible dos

Thromboprophylaxis for Hip and Knee Arthroplasty: Current Managements and Review of the Literature

Total Hip Arthroplasty (THA) and Total Knee Arthroplasty (TKA) are major surgical procedures which can cause high morbidity and even mortality. Among these complications is venous thrombo embolism (VTE) comprising deep vien thrombosis (DVT) and pulmonary embolism (PE). Therefore, after these operations, thromboprophylaxis is routinely used. However, it has some complications such as bleeding, adverse effect of chemical agents for using prevention of DVT. Anti-thrombotic prophylaxis includes: low molecular weight heparin (LMWH), fondaparinux, apixaban, dabigatran, rivaroxaban, low dose unfractionated heparin (LDUH), adjusted dose vitamin K antagonist (VKA), aspirin, or mechanical thromboprophylaxis devices. All over the World, orthopaedic surgeons consider a balance between thromboprophylaxis and bleeding. However, it has been still controversy about optimum prophylaxis for DVT. In this current paper, we aimed to review the literature under light of the current studies.
