Background:Although several prediction models for the occurrence of postoperative pancreatic fistula(POPF)after pancreatoduodenectomy(PD)exist,all were established using Western ***-scale external validation studies in Eastern cohorts that consider demographic variables including lower body mass index(BMI)are *** purpose of this study was to externally validate POPF prediction models using nationwide large-scale Korean ***:Nine tertiary university hospitals in the Republic of Korea ***'preoperative characteristics,intraoperative factors,and pathologic findings were *** grades were determined according to the 2016 International Study Group on Pancreatic Surgery *** POPF risk models(Callery,Roberts,and Mungroop)were selected for external ***:A total of 1,898 PD patients were enrolled.A non-pancreatic disease diagnosis[hazard ratio(HR),1.856;95%confidence interval(CI),1.223–2.817;P=0.004),higher preoperative BMI(HR,1.069;95%CI,1.019–1.121;P=0.006),and soft pancreatic texture(HR,1.859;95%CI,1.264–2.735;P=0.002)were independent risk factors for clinically relevant POPF(CR-POPF).The area under the receiver operating characteristic curve(AUC)values were 0.61,0.64,and 0.63 on the Callery,Roberts,and Mungroop models,respectively;all were lower than those published in each external validation ***:Western POPF prediction models performed less well when applied to Korean ***,a large-scale Eastern-specific and externally validated POPF prediction model is needed.

Inflammatory myofibroblastic tumor （INT） of the stomach in adults is extremely rare, with unpredictable prognosis. We present a 55-year-old woman with a gastric IMT. She experienced sudden abdominal pain 4 d previously. Physical examination showed mild abdominal tenderness in the hypogastrium, but no palpable abnormal abdominal mass. Abdominal CT showed a mass of approximately 8 cm in the gastrocolic ligament. On laparoscopic exploration, unexpected hemoperitoneum of approximately 1.5 L of blood was found, and an exophytic gastric mass of approximately 10 cm, appeared from the anterior wall of the gastric body along the greater curvature. Laparoscopy further showed that non- clotting blood in the abdominal cavity seemed to be from the gastric tumor. After conversion to open surgery for more precise evaluation of the cause of hemoperitoneum and the large friable tumor, gastric wedge resection, including the tumor, was conducted. The final diagnosis was consistent with IMT that originated from the gastric wall.

Seasonal Dynamics of Nutrient Loading and Chlorophyll A in a Northern Prairies Reservoir, Saskatchewan, Canada

Harmful algae blooms have become an increasing concern in context with the safety of water resources around the globe;however, little is known about the dynamics and specific causes of such blooms in the prairie ecozone in North America. The aim of this study was to research the nitrogen (N) and phosphorous (P) content and nutrient limitation (defined as N and P limitation) of growth of cyanobacteria in a northern prairies reservoir (Lake Diefenbaker [LD], SK, Canada). A combination of concentration balance analysis for N and P, controlled bioassays with the natural consortium of phytoplankton or defined monocultures of cyanobacteria, and satellite imagery was applied to address this aim. The current trophic status of Lake Diefenbaker is one of moderate eutrophication. Primary production in the lake is P-limited, and N did not represent a limiting factor for algal production. There was no significant increase in TP con- centrations between the upper and lower portions of the reservoir, indicating that most of the phosphorus in LD comes from upstream sites in Alberta. Anabaena circinalis, a species that has the potential to seriously degrade lake ecosys- tems, was identified as the predominant cyanobacteria in LD. Together with the fact that TP influxes into the reservoir primarily originate from upstream sources, these results suggest the need for remedial measures in the upstream reach of the South Saskatchewan River. Satellite imaging represented a promising approach in support of monitoring for po- tential algal blooms in LD;however, due to limited sensitivity and issues associated with atmosphere interference this methodology should only be used in combination with in situ water quality monitoring. Cognitive Profiles and Subtypes of Patients with Mild Cognitive Impairment: Data from a Clinical Follow-Up Study

Background: Mild cognitive impairment (MCI) is a heterogeneous condition with a variety of clinical outcomes, the presence of which correlates with risk of Alzheimer’s disease as well as pre-clinical stages of other dementia subtypes. The aims of this study were to assess the specific patterns of cognitive profiles and to identify changes from baseline to 24 weeks in patients with MCI using detailed neuropsychological testing. Methods: We consecutively recruited 120 MCI patients at baseline according to the Petersen’s clinical diagnostic criteria, who were admitted to the Dementia and Memory Clinics. We analyzed patients who fulfilled both inclusion and exclusion criteria for MCI and classified them into four subtypes according to deficits in major cognitive domains;amnestic MCI single domain (aMCI-s), amnestic multiple domain MCI (aMCI-m), non-amnestic single domain MCI (naMCI-s) and non-amnestic multiple domain MCI (naMCI-m). Four groups of MCI were evaluated by a detailed neuropsychological battery test. Results: 83 patients with MCI at the 24-week follow-up were classified into four subtypes. The most frequent subtype was amnestic multi-domain MCI, with the frequency of MCI subtypes as follows: aMCI-s (n = 21, 25.3%), aMCI-m (n = 53, 63.9%), naMCI-s (n = 5, 6.0%) and naMCI-m (n = 4, 4.8%). In the major cognitive items of the SNSB-D, there were significant changes between the initial and follow-up tests in the domains of language, memory and the fron-tal/executive function (p < 0.05), except for attention, in all MCI patient subtypes. At 24-weeks follow-up, the conversion rate to Alzheimer’s disease was 2.4% (n = 2) from a subtype of amnestic multi-domain MCI. Conclusions: Our study revealed the most frequent subtype of MCI to be multiple domain amnestic MCI, with this subtype having a higher tendency of conversion to Alzheimer’s disease.
