Urgent ERCP for acute cholangitis reduces mortality and hospital stay in elderly and very elderly patients

BACKGROUND： Acute cholangitis in old people is a cause of mortality and prolonged hospital stay. We evaluated the effects of methods and timing of biliary drainage on the outcomes of acute cholangitis in elderly and very elderly patients. METHODS： We analyzed 331 patients who were older than 75 years and were diagnosed with acute calculous cholangitis. They were admitted to our hospital from 2009 to 2014. Patients＇ demographics, severity grading, methods and timing of biliary drainage, mortality, and hospital stay were retrospectively obtained from medical records. Clinical parameters and outcomes were compared between elderly （75-80 years, n= 156） and very elderly （≥81 years, n=175） patients. We analyzed the effects of methods [none, endoscopic retrograde cholangio- pancreatography （ERCP）, percutaneous transhepatic biliary drainage, or failure] and timing （urgent or early） of biliary drainage on mortality and hospital stay in these patients. RESULTS： Acute cholangitis in older patients manifested as atypical symptoms characterized as infrequent Charcot＇s triad （4.2%） and comorbidity in one-third of the patients. Patients were graded as mild, moderate, and severe cholangitis in 104 （31.4%）, 175 （52.9%）, and 52 （15.7%）, respectively. Urgent bili-ary drainage （≤24 hours） was performed for 80.5% （247/307） of patients. Very elderly patients tended to have more severe grades and were treated with sequential procedures of transient hiliary drainage and stone removal at different sessions. Hospital stay was related to methods and timing of biliary drainage. Mortality was very low （1.5%） and not related to patient age but rather to the success or failure of biliary drainage and severity grading of the acute cholangitis. CONCLUSIONS： The methods and timing used for biliary drainage and severity of cholangitis are the major determinants of mortality and hospital stay in elderly and very elderly patients with acute cholangitis. Investigation on static routing and resource assignment of elastic all-optical switched intra-datacenter networks

In this paper, we explore the issue of static routing and spectrum/IT resource assignment(RSIA)of elastic all-optical switched intra-datacenter networks(intra-DCNs) by proposing anycast- and manycastbased integer linear programming(ILP) models. The objective is to jointly optimize the DCN resources, i.e.,network transmission bandwidth and IT resources, under different situations. First, for given service-request matrices with unknown network transmission bandwidth and IT resources, we propose anycast and manycast ILP models to minimize the maximum numbers of required network and IT resources to accommodate all the service requests. For anycast RSIA issue, we proposed two different ILP models that are based on node-arc and link-path methods, respectively. Node-arc based manycast ILP model is also proposed for the first time to our knowledge. Second, for given network transmission bandwidth and IT resources and known servicerequest matrices, we propose node-arc based anycast ILP models to maximize the total number of successfully served service requests. To evaluate the efficiency of anycast and manycast models, all proposed ILP models are evaluated and compared with unicast ILP models. Simulation results show that anycast and manycast ILP models perform much better in efficiently using DCN resources and successfully accommodating more service requests when compared to unicast ILP models under the same network conditions.

Dendrite Spacing Selection during Directional Solidification of Pivalic Acid-Ethanol System

Unidirectional solidification of pivalic acid （PVA）-ethanol （Eth） mixture was performed to examine whether an allowable range of primary dendrite spacing definitely exists at a given growth velocity and how the range is history-dependent. PVA-0.59 wt pct Eth was unidirectionally solidified in the range of growth velocity 0.5-64 μm/s at the temperature gradient of 2.3 K/ram. Sequential change in growth velocity was imposed to determine the upper and lower limits for the allowable range of stable spacing. An allowable range of the steady state primary spacing was observed at a given growth velocity, and the extent of the range seems to be dependent on the degree to which step-increase or step-decrease in growth velocity is accomplished. As the degree of sequential change in growth velocity increases, the history-dependence of the selection for the primary dendrite spacing tends to disappear.

Corticotropin-releasing factor stimulates colonic motility via muscarinic receptors in the rat

AIM To measure exogenous corticotropin-releasing factor(CRF)-induced motility of the isolated rat colon and to demonstrate the effect of pharmacologic inhibition on CRF-induced motility. METHODS The isolated vascularly-perfused rat colon was used. Luminal pressure was monitored via microtip catheter pressure transducers in the proximal and distal colon. At first, exogenous CRF was administered in a stepwise manner and the concentration of CRF yielding maximal colonic motility was selected. After recording basal colonic motility, hexamethonium, phentolamine, propranolol, atropine and tetrodotoxin were infused into the isolated colon. Initially, only the test drug was infused; then, CRF was added. The motility index was expressed as percentage change over basal level. RESULTS Administration of 1.4, 14.4, 144 and 288 pmol/L CRF progressively increased colonic motility in the proximal and distal colon. Infusion of atropine or tetrodotoxin reduced CRF-induced motility of both the proximal and distal colon, whereas hexamethonium, phentolamine and propranolol had no *** CRF-induced colonic motility appears to be mediated by local cholinergic signaling via muscarinic receptors. Muscarinic receptors are potential targets for counteracting CRF-induced colonic hypermotility.

Efficacy of cap-assisted endoscopy for routine examining the ampulla of Vater

AIM: To determine the efficacy of a cap-assisted endoscopy (CAE) to completely visualize the ampulla of Vater (AV) in patients failed by conventional endoscopy. METHODS: A prospective study was conducted on 120 patients > 20 years of ages who visited the Health Promotion Center of Chungbuk National University Hospital for conscious sedation esophagogastroduodenoscopy (EGD) as a screening test from July to October, 2011. First, forward-viewing endoscopy was performed with reasonable effort using a push and pull method. We considered complete visualization of the AV when we could observe the entire AV including the orifice clearly, and reported the observation as complete or incomplete (partial or not found at all). Second, in cases of complete failure of the observation, an additional AV examination was conducted by attaching a short cap (D-201-10704, Olympus Medical Systems, Tokyo, Japan) to the tip of a forward-viewing endoscope. Third, if the second method failed, we replaced the short cap with a long cap (MH-593, Olympus Medical Systems) and performed a re-examination of the AV. RESULTS: Conventional endoscopy achieved complete visualization of the AV in 97 of the 120 patients (80.8%) but was not achieved in 23 patients (19.2%). Age (mean ± SD) and gender [male (%)] were not significantly different between the complete observation and the incomplete observation groups. Additional short CAE was performed in patients in whom we could not completely visualize the AV. This group included 13 patients (10.9%) with partial observation of the AV and 10 (8.3%) in which the AV was not found. Short CAE permitted a complete observation of the AV in 21 of the 23 patients (91.3%). Patients in whom visualization of the AV failed with short CAE had satisfactory outcomes by replacing the short cap with a long cap. The additional time for CAE took an average of 141 ± 88 s. Lymphangioma usually occurs in children and usually involves the skin. Mesenteric lymphangioma is extremely rare in adults. Typically, lymphangioma appears on computed tomography (CT) as a lower attenuation of a cystic mass, however, some cases appear to be a solid mass. We describe the CT and 18F-FDG positron emission tomography/CT appearance in a case of jejunal and mesenteric cavernous lymphangi-omatosis mimicking metastasis in an adult patient with rectal cancer.

Computer vision(CV)-based techniques have been widely used in the field of structural health monitoring(SHM)owing to ease of installation and cost-effectiveness for displacement *** paper introduces computer vision based method for robust displacement measurement under occlusion by incorporating random sample consensus(RANSAC).The proposed method uses the Kanade-Lucas-Tomasi(KLT)tracker to extract feature points for tracking,and these feature points are filtered through RANSAC to remove points that are noisy or *** the filtered feature points,the proposed method incorporates Kalman filter to estimate acceleration from velocity and displacement extracted by the *** validation,numerical simulation and experimental validation are *** the simulation,performance of the proposed RANSAC filtering was validated to extract correct displacement out of group of displacements that includes dummy displacement with noise or *** the experiment,both RANSAC filtering and acceleration measurement were validated by partially occluding the target for tracking attached on the *** results demonstrated that the proposed method successfully measures displacement and estimates acceleration as compared to a reference displacement sensor and accelerometer,even under occluded conditions.

Clay minerals and geochemistry of the bottom sediments in the northwestern East China Sea

Clay minerals of 34 sediments collected from the northwestern continental shelf of the East China Sea have been determined by X-ray diffraction analysis. The clay mineral distribution is mainly controlled by the sediment source and the dominant circulation pattern. The predominant clay mineral in our study area is illite comprising more than 67% of the whole clay fraction. The highest concentration of illite （〉68%） is found in the southeastern offshore parts beyond the reach of terrigenous input from the Jeju Island. It means that these illites are largely transported by the Kuroshio Current from the South China Sea （SCS）. Smectite is highly concentrated in the northwest middle part and in the outer-shelf mud patch. It seems to be due to the high supply of smectite transported from China where fine-grained sediments are discharged from modern and ancient Huanghe （Yellow） River. The relatively high abundant kaolinite is likely derived from the Changjiang （Yangtze） River via the Taiwan Warm Current. In contrast, large amounts of chlorite and high chlorite/kaolinite ratios occur in the northwestern area, reflecting the transportation by the Yellow Sea Coastal Current from the southern Yellow Sea. The discrimination diagrams clearly show that the sediments in the northwestern East China Sea are ultimately sourced from Chinese rivers, especially from the Huanghe River, whereas the sediment in the northeast part might come from the Jeju Island. The muddy sediments of the Changjiang River＇s submerged delta have much lower 87Sr/S6Sr ratios （0.716 2-0.718 0） than those of the Shandong Peninsular mud wedge （0.721 6-0.724 9）, which are supposed to be originated from the Huanghe River, suggesting the distribution pattern of 87Sr/86Sr ratios as a new tracer to discriminate the provenance of shelf sediments in the study area. The 87Sr/86Sr ratios of the outer-shelf muddy sediments ranged from 0.7169 to 0.7216 in a wide range and was between those of the Huanghe River and Changjia

Effect of Doubling the Ensemble Size on the Performance of Ensemble Prediction in the Warm Season Using MOGREPS Implemented at the KMA

Using the Met Office Global and Regional Ensemble Prediction System （MOGREPS） implemented at the Korea Meteorological Administration （KMA）, the effect of doubling the ensemble size on the performance of ensemble prediction in the warm season was evaluated. Because a finite ensemble size causes sampling error in the full forecast probability distribution function （PDF）, ensemble size is closely related to the efficiency of the ensemble prediction system. Prediction capability according to doubling the ensemble size was evaluated by increasing the number of ensembles from 24 to 48 in MOGREPS implemented at the KMA. The initial analysis perturbations generated by the Ensemble Transform Kalman Filter （ETKF） were integrated for 10 days from 22 May to 23 June 2009. Several statistical verification scores were used to measure the accuracy, reliability, and resolution of ensemble probabilistic forecasts for 24 and 48 ensemble member forecasts. Even though the results were not significant, the accuracy of ensemble prediction improved slightly as ensemble size increased, especially for longer forecast times in the Northern Hemisphere. While increasing the number of ensemble members resulted in a slight improvement in resolution as forecast time increased, inconsistent results were obtained for the scores assessing the reliability of ensemble prediction. The overall performance of ensemble prediction in terms of accuracy, resolution, and reliability increased slightly with ensemble size, especially for longer forecast times.

Secondary endoscopic submucosal dissection for locally recurrent or incompletely resected gastric neoplasms

AIM To investigate the feasibility and safety of secondary endoscopic submucosal dissection(ESD) for residual or locally recurrent gastric tumors. METHODS Between 2010 and 2017, 1623 consecutive patients underwent ESD for gastric neoplasms at a single tertiary referral center. Among these, 28 patients underwent secondary ESD for a residual or locally recurrent tumor. Our analysis compared clinicopathologic factors between primary ESD and secondary ESD groups. RESULTS The en bloc resection and curative rate of resection of secondary ESD were 92.9% and 89.3%, respectively. The average procedure time of secondary ESD was significantly longer than primary ESD(78.2 min vs 55.1 min, P = 0.004), and the adverse events rate was not significantly different but trended slightly higher in the secondary ESD group compared to the primary ESD group(10.7% vs 3.8%, P = 0.095). Patients who received secondary ESD had favorable outcomes without severe adverse events. During a mean follow-up period, no local recurrence occurred in patients who received secondary ESD. CONCLUSION Secondary ESD of residual or locally recurrent gastric tumors appears to be a feasible and curative treatment though it requires greater technical efficiency and longer procedure time.
